1.Study of diagnostic value of neutrophil CD64 and sTREM-1 in elderly patients with community-acquired pneumonia

Objective:To explore the neutrophil CD64 and sTREM-1 in elderly patients with community-acquired pneumonia ( CAP) in the diagnosis of clinical value. Methods: 76 elderly CAP hospitalized patients were divided into severe pneumonia group (n=23) (live in 15 cases,8 deaths)and ordinary pneumonia group(n=53) according to severity;45 patients in the control group compared with healthy older persons. Peripheral blood neutrophil CD64 was measured by automatic flow cytometry,sTREM-1 levels was measured by double-antibody sandwich enzyme-linked immunosorbent assay. The receiver operating characteristic curve ( ROC curve) was used to check diagnostic value of the detection. Results: Median concentrations of CD64 and sTREM-1 in the severe pneumonia group,general pneumonia group and control group were 37. 49,18. 82 and 10. 63 MFI,and 75. 39,65. 31 and 43. 96 pg/ml, respective-ly. Although there was significant differences among the three groups ( P<0. 05 ); CD64 and sTREM-1 in Severe pneumonia survival group reduced to a normal level as his condition improved gradually,death group continued to rise as the disease,and peaked at the time of death;the area under the ROC curve of CD64,sTREM-1were respectively 0. 876,0. 843,which was 0. 917 by combination of CD64 and sTREM-1. Conclusion: Both CD64 and sTREM-1 are good markers in the diagnosis value of CAP, the dynamic changes of two may reflect the condition and prognosis of CAP.
2.Correlation between RDW,CysC,and LP(a) with lesion severity of coronary heart disease

Objective To study the correlation between the red cell distribution width (RDW) ,cystatin C (Cys c) and lipopro‐tein (a) [LP (a)]with the lesion severity of coronary heart disease .Methods 130 cases of coronary heart disease(CHD) definitely diagnosed by coronary arteriography in the cardiology department of our hospital from March 2012 to September 2014 were select‐ed .The subjects were divided into the single‐vessel lesion group ,double‐vessel lesion group and multiple‐vessel lesion group accord‐ing to the coronary arteriographic results;furthermore the subjects were divided into the 1-30 points group ,31 -60 points group and the >60 points group according to the Gensini scores ;other contemporaneous 45 individuals of normal coronary arteriography were selected as the control group .RDW ,Cys C and LP (a) levels were detected in each group and the detection results were per‐formed the comparative analysis .Results The RDW ,CysC and LP (a) levels in the CHD group were significantly higher than those in the control group ,the differences were statistically significant (P< 0 .05) .With the increase of coronary lesion vessels number and Gensini score ,the RDW ,CysC and LP (a) levels showed roughly increasing trend ,the correlation analysis showed that the RDW ,CysC and LP (a) were positively correlated with Gensini scores .Conclusion The RDW ,CysC and LP (a) levels in the patients with CHD are higher than those in the normal control group ,which are closely related with the lesion severity of CHD and may be the new predictors of coronary lesion severity .
3.Effect of moxibustion plus Duhuo Jisheng decoction on hs-CRP, IL-1β and TNF-α levels in middle-aged and elderly patients with knee osteoarthritis

Objective: To observe the clinical effect of moxibustion combined with Duhuo Jisheng decoction for middle-aged and elderly patients with knee osteoarthritis (KOA) and its impact on serum high sensitive C-reactive protein (hs-CRP), interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) levels. Methods: A total of 90 eligible KOA patients were randomized into an observation group (n=45) and a control group (n=45). Cases in the observation group received moxibustion plus oral administration Duhuo Jisheng decoction, while cases in the control group received Duhuo Jisheng decoction. Then the hs-CRP, IL-1β and TNF-α levels were examined and the clinical effect was evaluated. Results: Before treatment, the hs-CRP, IL-1β and TNF-α levels showed no between-group statistical differences (all P>0.05). After 8 weeks of treatment, the hs-CRP, IL-1β and TNF-α levels all dropped significantly in both groups (P<0.01 or P<0.05), and the treatment group showed more substantial changes than the control group (all P<0.05). The total effective rate was 97.8% in the treatment group, versus 86.7% in the control group, showing a statistical difference (P<0.05). Conclusion: Moxibustion plus Duhuo Jisheng decoction has a good clinical effect for middle-aged and elderly KOA patients, and the effect may relate to the decreased hs-CRP, IL-1β and TNF-α levels.
