1.Vascular endothelial cell injury and repair mechanisms

The vascular endothelial cells (VECs) are not only a mechanical barrier of blood cycle cells and vascular smooth muscle cells, but also largest and most important type of endocrine organs.Because of its mechanical barrier action It is highly susceptible to in vitro and in vivo a variety of physical and chemical factors in the injury; damaged VECsvascular endothelial cells, especially the disordered endocrine function, the inevitable imbalance to a variety of active substances secreted or related active substances, finally result in dysfunction of cardiovascular system.This article reviews many mechanisms underlying VECsvascular endothelial cell injury, investigates VECsvascular endothelial cell repair mechanisms, and study ways to protect and repair the injured VECsvascular endothelial cells, and improve the prognosis of vascular disease.
2.A Study on Correlation between Bone Mineral Density and Biochemical Parameters of Bone Metabolism in Female Patients with Systemic Lupus Erythematosus

Objective To determine bone mineral density (BMD) and biochemical parameters of bone metabolism in premenopausal female patients with systemic lupus erythematosus(SLE), and investigate the correlation between them. Methods Lumbar and femoral BMD in 30 female patients with SLE was determined by dual energy X ray absorptiometry. In 30 female patients and 39 controls, the corrected serum levels of calcium, phosphorus, alkaline phosphatase, creatinine and albumin were measured by routine methods, and bone specific isoform of alkaline phophatase, propeptide of type 1 procollagen, deoxypyridinoline excretion, telopeptide of type 1 collagen, osteocalcin, parathyroid hormone, 25-OH vitamin D, testosterone, progesterone, estradiol, follicle stimulating hormone and luteinotropic hormone were measured by ELISA or RIA. Results According to the WHO criteria, 39% patients with SLE had normal BMD, 46% had osteopenia and 15% had osteoporosis at the lumbar spine; at the femoral neck 38.5% had normal BMD, 38.5% had osteopenia and 23% suffered form osteoporosis. All other bone resorption and formation markers measured were not significantly different except significantly higher corrected serum levels of albumin calcium (P=0.0001)and significantly lower osteocalcin level and phosphorus value(P=0.03,P=0.002) were found in the SLE patients. Of all sex hormones tested lower testosterone and higher follicle stimulating hormone concentration were seen in patients with SLE(P=0.001,P=0.42). Conclusion A high incidence of osteopenia and osteoporosis was found in premenopausal patients with SLE, which seems to be attributable, at least in part , to decreased bone formation in SLE patients.
3.Treatment of flexion deformity of the hallux after fibular fracture

Objective To discuss the etiology,clinical findings,anatomical characters,diagnosis and treatment of the painful flexion deformity of the hallux after open reduction and internal fixation of the fibular fractures.Methods From October 1996 to March 2004,we treated 33 cases with flexion deformity of the hallux after fibular fracture,including 19 males,14 females,with a mean age of 33.2 years old(ranged from 22 to 49 years).24 cases were ankle fractures,according to Lauge-Hansen clssification,pronation-external rotation for 18 cases;supination-exteral rotation for 4 cases;pronation-eversion for 2 cases.9 cases were distal third tibial and fibular fractures.19 cases were simple hallux flexion deformity,8 cases were combined with the second and third toes deformity,6 cases were combined with not only the second and third toes but also the fourth and fifth toes deformity.The deformities were corrected by flexor hallucis longus tendolysis or lengthening alone or flexor digitorum longus lengthening in combination.Results 19 cases were simple hallux flexion deformity,the deformities were corrected simply by flexor hallucis longus tendolysis(7 cases)and lengthening(12 cases).14 cases were combined with the lesser toes deformity,the flexor hallucis longus tendon were lengthened alone in 6 cases,and both the flexor hallucis longus and the flexor digitorum longus were lengthened in 8 cases.During an average duration of follow-up of 6 months(2 to 10 months),there were no postoperative complications.All patients had full correction of all deformed toes and reported relief from pain,ease of shoe-fitting and subjective improvement of gait.There was no recurrence of the deformity.Conclusion Adhesions of the flexor hallucis longus tendon to the fibular fracture site should be considered when a flexion deformity of the hallux occurs after open reduction and internal fixation of the fibular fractures.Variations in the interconnections between flexor hallucis longus and flexor digitorum longus in the region of the knot of Henry also have special clinical significance.
4.Investigation on Interpersonal Communication of Preparatory Students in One Xinjiang Medical College

Objects:To investigate the current situation of interpersonal communication inpreparatory students in one Xinjiang medical college from the exchanges between students.Methods:Using self-developed questionnaire,a survey was conducted among 200 preparatory students and depth interviews were conducted for typical cases.Results:Preppies showed a strong willing of interpersonal communication.In the process of students' interaction,some features were presented as followings:pure motives but dissimilarity,narrow range of interactions,lack of interaction ability and the single form of social activities.Conclusion:Based on the above current situation of interpersonal communication in preparatory students,it should enhance the language application ability of preparatory stage,change the idea and attach importance to the education of humanistic quality in preparatory education and train preppy's expression and listening art.
5.Effect of Weici Acupuncture on Shoulder-hand Syndrome after Stroke

Objective To observe the effect of Weici acupuncture on shoulder-hand syndrome after acute stroke.Methods 90 cases were divided into two groups:the test group(49 cases) who were treated with Weici(a kind of acupuncture) and the control group(41 cases) who were treated with the normal acupuncture.Results The total effective ratio of the test group was 93.88%,and that of the control group was 73.17%(χ2=7.2863,P<0.01).Conclusion Weici acupuncture is more affective on shoulder-hand syndrome after acute stroke than the normal acupuncture.
6.Rehabilitation effect of functional exercises on nerve root cervical spondylosis

目的观察功能锻炼对神经根型颈椎病恢复期的作用和效果。方法患者随机分为试验组49例和对照组48例。试验组采用体育保健学的锻炼方法结合常规手法整复,对照组只采用与试验组相同的手法整复。结果两组间基线资料无显著性差异(P>0.05)。试验组的总有效率为89.8%,对照组的总有效率为56.3%(P<0.01),试验组治疗前后症状积分明显优于对照组(P<0.01)。结论主动锻炼有助于改善神经根型颈椎病疗效。
7.Effects of Partner Rehabilitation Gymnastics on Periarthritis of Shoulder in Later Stage

ObjectiveTo observe the effects of the Partner Rehabilitation Gymnastics on periarthritis of shoulder in later stage. MethodsPatients were randomly divided into 2 groups: 60 in treatment group accepted the Partner Rehabilitation Gymnastics, and the other 60 accepted the forcing active and passive physiotherapy.Results76.66% of the treatment group recovered, 96.66% improved; while 45% of the control group recovered, 73.33% improved. The scores of visual analogous scale and activity of daily living improved after treatment in both groups, and the differences pre- and post-treatment were more in treatment group than in control group. ConclusionThe Partner Rehabilitation Gymnastics is more effective than the routine forcing active and passive physiotherapy on periarthritis of shoulder.
8.Therapy of AF system inside and outside after joint vertebral bone fusion treatment on thoracolum-bar burst fracture

Objective To explore the clinical curative effect of AF system combined vertebra re-duction and posterolateral bone fusion treatment thoracolumbar burst fracture .Methods Seventeen pa-tients with thoracolumbar burst fracture using period stage , according to the system of AF clinical symptoms and image data line pressure decreased , and the pressure or not to hurt inside the vertebral pedicle autoge-nous bone grafts are grain filling and vertebral posterolateral line between the axon , lamina around small joints and bone , and postoperative follow-up X-ray inspection regularly , observe internal fixation and verte-bral vertebral height and adjacent intervertebral space changes , or after fixation and reset lost .Results following up 13 months, vertebral body height restore and maintain good , bolt looseness and broken and ver-tebral body height complications such as lost were no found in 17 cases.Conclusion Treatments of Poste-rior fixation combined short stage AF vertebral body and posterolateral bone fusion on thoracolumbar burst fracture are not only rebuild the vertebral height , but also make the vertebral obtained after three pillars in stable, reduce the internal fixation and compressed vertebral again such complications .
9.Treatment of degenerative Achilles'tendon rupture using V-Y tendinous flap and transfer of flexor hallucis longus tendon

Objective To introduce the technique.indication and outcome of treatment of degenerative Achilles'rupture with transfer of flexor hallucis longus tendon and V-Y tendinous flap. Methods From October 2003 to May 2006,21 cases of degenerative Achilles'tendon rupture were treated in our department using transfer of flexor hallucis longus tendon and V-Y tendinous flap.Rivets were used in 17 cases and surface screws in 4.They were followed up for 12 to 18 months(average,14). Results According to the Arner-Lindholm criteria for curative results,19 of the 21 cases were excellent and 2 good.Follow-ups revealed no Achilles'tendon re-rupture,infection,skin necrosis,stiff ankle,or injury to nerves and vessels.Walking gaits nearly recovered to normal.The repair strength allowed early functional exercise of the Achilles'tendon. Conclusion Transfer of flexor hallucis longus tendon combined with V-Y tendinous flap can cure the degenerative Achilles'tendon ruptures and defects of Achilles'tendon within 3 cm.
10.Reconstruction of the lateral ankle ligaments with autograft of semitendinosus & bio-interference screws in chronic lateral instability of the ankle

ObjectiveTo observer the effect of autograft of semitendinosus and interference screws for reconstruction of lateral ankle ligaments. MethodsA total of 12 cases of injured lateral ankle ligaments were selected, including 7 males and 5 females with an average age was 29.4 years, with a course of disease from 3 to 6 months. All lateral ligaments were reconstructed by autograft of semitendinosus & bio-interference screws. A 4.5 mm diameter tunnel was drilled on the lateral ankle, where the tendon of semitendinosus was permeated through, both ends of semitendinosus tendons were fixed with bio-interference screws on talus of at the end of anterior talofibular ligament and calcaneus of at the end of calcaneofibular ligament, maintaining ankle neutral and moderate valgus position. Following operation, the ankle was fixed in neutral position and slightly valgus position with plaster cast. The wound healing, the outcomes of American Orthopaedic Foot and Ankle Society (AOFAS) score, Foot and Ankle Outcome Score (FAOS), and stress radiographic evaluation were assessed at the follow-up. ResultsAll 12 patients showed no infection and the wound healed well at the primary stage. At 3, 6, 12, 24 months after operation, all patients were followed-up. According to AOFAS and FAOS of preoperation and final follow-up postoperation, function of ankle in all patients was significantly improved (P<0.01), the scores of Chrisman-Snook group were more lower than this approach. No patients complained of instability of the ankle, and stress radiograph confirmed this improvement. ConclusionReconstruction of lateral ligaments of the ankle with autograft of semilendinosus & biointerference screws is a practical and reliable treatment, which restores stability and function of the ankle.
