Chinese Journal of Orthopaedic Trauma 2004;0(10):-
Objective To investigate the operative treatment of displaced intra-articular fractures of the calcaneus using a small lateral incision approach. Methods Thirty-one patients with 32 intra-articular calcaneal fractures, were treated with open reduction and internal fixation through a small lateral incision approach from October 2004 to April 2006.The skin incision was made from the distal tip of the fibula to the base of the fourth metatarsal.According to Sanders classification, there were 21 typeⅡfractures and 11 typeⅢfractures. The first change of dressing was done on the second day after operation. The residual blood clots in the wounds were cleaned as much as possible when the drainage sheet was removed. The operated foot was bandaged with pressure dressing of cotton pad. Results After an average follow-up of 10.5 months(range,3 to 18 months),no soft tissue com- plications were found, and all had acceptable reduction. The preoperative X-ray film showed that B(?)hler angle was 6.84??9.36?,Gissane angle was 106.04??12.03?.The postoperative X-ray film demonstrated that B(?)hler angle was 32.06?+6.87?,Gissane angle was 115.81??9.48?,and the difference was statistically significant(P<0.01).According to the AOFAS evaluation system for Ankle-Hindfoot,two feet scored 60-70 points, nine feet 70-80 points,16 feet 80-90 points and five feet 90-100 points.Condusion The small lateral incision approach is a good option for management of calcaneus fractures, because it causes minimal soft tissue damage, provides excellent exposure, and leads to convenience for later removal of internal fixators and subtalar arthrodesis.

Surface electromyogram (sEMG) may have low signal to noise ratios. An adaptive wavelet thresholding technique was developed in this study to remove noise contamination from sEMG signals. Compared with convention- al wavelet thresholding methods, the adaptive approach can adjust thresholds based on different signal to noise ratios of the processed signal, thus effectively removing noise contamination and reducing distortion of the EMG signal. The advantage of the developed adaptive thresholding method was demonstrated using simulated and experimental sEMG recordings.

Objective To establish human choriocarcinoma JeG-3 cell line resistant to floxuridine (FUDR)and describe the characteristics of this FUDR-resistant subline. The thymidylate synthase (TS) expression level in FUDR-resistant subline was also discussed. Methods The FUDR-resistant sub-line JeG-3/FUDRA was established by intermitted exposure to grads increased FUDR.Reversed microscope was used to observe the morphological changes in FUDR-resistant sub-line.Population doubling time was calculated and compared based on the growth curve of these two cell lines, cell cycles and chromosomal ploidy were assayed with flow cytometry methods. The chemo-luminescence assay was used to detect the hormone secretion by two kinds of cell lines. The resistant index (RI) was measured by cell counting kit-8 (CCK-8)assay.Quantitative RT-PCR was used to detect the mRNA expression level of TS and we also detected the TS mRNA expression level in different doses exposed subline. Results The RI of JeG-3/FUDRA was 31.62.Compared with the JeG-3 cell, the FUDR-resistant cell line had gross changes in morphological, cell growth, cell cycles and chromosomal numbers. The ability of human chorionic gonadotrop(hCG) and progesterone secretion was lower in JeG-3/FUDRA subline. The trend of TS mRNA expression was:while exposed to low concentration of FUDR,the TS mRNA expression level was downregulated, then followed the increasing dose of the drug, the expression level of TS mRNA ascended gradually. When the terminal concentration was reached, the expression level of TS mRNA in JeG-3/FUDRA subline was higher than that of JeG-3 cell line (P<0.05).Conclusions We established the FUDR-resistant subline of JeG-3 successfully. The TS mRNA expression level is stage-related to the different concentration and different phase in FUDR exposure. Our data suggested that TS mRNA expression level may not be used as a biomarker to predict the chemosensitivity in FUDR-based chemotherapy.

This paper was aimed to analyze and study the process of Turmeric fine pulverizing; and the powder properties of Turmeric ultra-micro powder after the process. Based on d50, the powder properties of Turmeric ultra-micro powder were summarized by using orthogonal design to select the optimal Turmeric fine pulverizing. Compari-sons were made on powder properties, such as exterior characters, IR spectra, fluidity and hygroscopicity before and after fine pulverizing. The results showed that optimal fine pulverizing process was determined based on orthogonal design. The conditions were that the material was 1 200 g, with water of 5.5% and crushing for 40 min. Compari-son of powder properties of Turmeric powders before and after fine pulverizing showed that as the diameter of the particle decreased. Turmeric particle gradually showed signs of aggregation. At the same time, granular sensation disappeared;the color turned lighter;powder became finer;fluidity was reduced;balanced hygroscopic capacity el-evated. Although the chemical composition and molecular structure had not changed;initial velocity and capacity of hygroscopicity increased, acceleration declined. It was concluded that Turmeric fine pulverizing was a convenient, reliable and practical process, with small size of particle. It can be used for Turmeric fine pulverizing. The compre-hensive evaluation showed that ultrafine powder four as the optimum powder.

OBJECTIVETo investigate the clinical outcomes of cannulated screw combined with buttress plate in treating transarticular shear fractures of the distal humerus.
METHODSFrom July 2008 to December 2011, 17 patients with shear fractures of the distal humerus were treated with cannulated screw combined with buttress plate. Among them, included 10 males and 7 females aged from 20 to 59 years old (mean 36.5). All the fractures were classified into type I (8 cases), type II (1 case), type III (3 cases) and type IV (5 cases) according to Bryan-Morrey and McKee classification. According to AO/ASIF classification, 10 cases were type 13B3.1, 7 cases were type 13B3.3; according to Dubberley classification, 5 cases with type 1A, 2 cases with type 1B, 2 cases with type 2A, 2 cases with type 2B, 4 cases with type 3A, 2 cases with type 3B. There were 12 cases with fresh injuries, 4 cases with old injuries. All fractures were closed injuries. Complications and range of motion of elbow were observed, and clinical efficacy were evaluated by Broberg-Morrey standard.
RESULTSAll patients were followed up with a mean time of 21.3 months (ranged, 18 to 24). All fractures obtained bone healing from 10 to 15 weeks with a mean of 12.5 weeks. No malunion or delayed healing occurred. Two cases occurred ossification in lateral collateral ligament attachment. 2 cases occurred heterotopic ossification in the capitellum in front of capsule joint. The range of elbow extension was (16 +/- 7) degrees, flexion was (115 +/- 9) degrees, the average are of pronation and supination was (65 +/- 5) degrees and (60 +/- 5) degrees respectively. According to Broberg-Morrey standard, there were excellent in 7 cases, good in 8 cases, fair in 2 cases, and average score was 92.44 +/- 4.64.
CONCLUSIONFor transarticular shear fractures of the distal humerus, classification should be performed according to preoperative X-ray and CT, cannulated screw and buttress plate combined and early function rehabilitation could recover elbow function.
