1.Blood vessels and nerves surrounding the seminal vesicles: A clinical anatomic study.

OBJECTIVETo investigate the precise locations of the blood vessels and nerves surrounding the seminal vesicles (SV) in men and provide some anatomical evidence for SV-related minimally invasive surgery.
METHODSWe observed the courses and distribution of the blood vessels and nerves surrounding SVs and obtained the data for positioning the SV neuroplexes in 20 male pelvises.
RESULTSOne branch of the neuroplexes was distributed to the SVs bilaterally with the neurovascular bundles, (2.85 ± 0.18) cm from the median sulcus of the prostate (MSP), while another branch ran through the Denonvillier fascia behind the SV, (0.81 ± 0.06) cm from the MSP. The arterial SVs (ASV) originated from the inferior vesical artery and fell into 4 types, 55% going directly to the SVs as one branch, 15% running between the SV and the ampulla of the deferent duct as another branch, 25% downward as 2 branches to the SV and between the SV and the ampulla of the deferent duct respectively, and 5% as the other ASVs. The shortest distance from the ASV through the prostatic neuroplexus to the posterior SV was (1.08 ± 0.09) cm.
CONCLUSIONIn SV resection, neuroplexus injury can be reduced with a bilateral distance of < 2.85 cm and a posterior distance of < 0.81 cm from the MSP, and so can bleeding by vascular ligation between the SV and the ampulla of the deferent duct.

2.Comparison of holmium laser enucleation and plasmakinetic enucleation of the prostate for benign prostatic hyperplasia

Objective To evaluate and compare the efficacy of holmium laser enucleation of the prostate(HoLEP) and plasmakineticenucleation of the prostate(PKEP) for treating benign prostatic hyperplasia (BPH).Methods A total of 86 cases of BPH were selected from the Shandong Energy Zibo Mining Group Co.Ltd Central Hospital from January 2013 to June 2014.The patients received either HoLEP (40 cases) or PKEP(46 cases) treatment.Clinical data including postvoid residual volume (PVR),international prostate symptom score(IPSS),quality-of-life score(QOL score),maximum urinary flow rate (MFR) were evaluated before and 3 months after operation.Blood loss in operation,operation time,weight of resected prostate tissue,bladder irrigation time,catheterization time and length of hospital stay were also collected from the perioperative period.Results Three months after surgery,PVR,IPSS,QOL and MFR were all significantly improved in both HoLEP and PKEP groups compared with the data before operation (P =0.000),while there was no significant difference between the two groups(P>0.05).Patients from the HoLEP group showed markedly lower values of bone loss in operation than that of PKEP group((69.5±23.6) ml vs.(87.5±38.0) ml,P=0.011).There were no significant differences in terms of other indexes (P> 0.05).Conclusion HoLEP and PKEP have similar efficiency in treating BPH,while HoLEP is suggested superior in regard of bone loss.

4.Preliminary Study on Solid-state Fermentation of Streptomyces Strain Men-myco-93-63

The culture medium and cultural conditions of solid-state fermentation of Streptomyces Menmyco-93-63 were tested in this study. The suitable medium which contains rice, sorghum, millet bran, and rice hull with the proportion of 2:2:3:3 was developed for the spore production of Streptomyces Men-myco-93-63 using single substrate screening, mixture substrate screening and orthogonal experiments, and the sporulation was up to 2.52?109 CFU/g. And then, initial charge, initial ratio of water to solid, inoculating quantity, and culture temperature impact to sporulation of Streptomyces Men-myco-93-63 were tested. The favorite cultural conditions are developed as the following: the initial charge is 15 g in 500 mL Erlenmeyer flask; initial ratio of water to solid is 1.7:1.0 (V/W, rice hull excluding), inoculating quantity is 7 mL, culture temperature is 28℃.
5.Comparative study of absorbable screw and metallic internal fixation in treatment of tibial plateau fractures

Objective To assess the effectiveness of absorbable screws and metallic devices for treating tibial plateau fractures.Methods Fifty-seven cases of tibial plateau fractures were treated with internal fixation using absorbable screws(Group A),cannulated screws(Group B)and condylar buttress plates(Group C)in our hospital.According to Schatzker classification,there were 15 cases with typeⅠfractures,21 with typeⅡ,nine with typeⅢand 12 with typeⅣ,who were followed up for 6-51 months. Statistical analysis was performed to compare bone union and functional restoration of the knee between three groups.Results Bone union was achieved in all cases.Based on Merchant scale for knee func- tion,excellence rate was 84%(16/19)for Group A,92%(11/12)for Group B and 85%(22/26) for Group C,with no significant statistical difference.Conclusion Absorbable screws is as effective as metallic devices for treating tibial plateau fractures and has advantages of minimal invasion,few complica- tions and avoiding secondary operation.
6.Integrin-ligands binding reaction upregulates the antioxidant activity of rabbit bronchial epithelial cells

Antioxidant activity of bronchial epithelial cells (BECs) plays an essential role in preventing the airway epithelium integrity from damage in structure and function. Integrin expressed by BECs is the receptor of extracellular matrix such as fibronectin (Fn), and it is involved in modulation of proliferation, differentiation and metabolism of the cells. In order to test the hypothesis that integrin-ligand binding reaction supports the ability of cells to withstand oxidant attack, the present study evaluated the antioxidant activity of primary cultured rabbit BECs treated with fibronectin or its sequence Arg-Gly-Asp (RGD peptide), by determining changes in the activity of glutathione peroxidase (GSH-Px), superoxide dismutase (SOD) and catalase (CAT) and in the level of glutathione (GSH). The results are as follows: (1) Fn (10 μg/ml) increased significantly the activity unit of GSH-Px (P<0.05, n=5), which was inhibited by calmodulin-inhibitor W7 (10-5 mol/L) (P<0.05). Both Fn (5~20 μg/ml) and RGD (15~60 μg/ml) showed a dose-dependent upregulatory effect (respectively r=0.93 and r=0.73). (2) Treatment with Fn increased SOD activity (P<0.01, n=7), which was abolished by W7 (P<0.01). (3) Catalase activity was also stimulated by Fn (P<0.05, n=6) and reversed by W7 (P<0.01). (4) A dose-dependent increase of GSH level was observed in both Fn (r=0.82) and RGD treatment (r=0.84). The data suggest that the binding of integrin with extracellular matrix can upregulate activity of antioxidant enzymes, and increase the content of GSH and improve the ability of BECs to resist oxidant injury.

9.Diagnosis and treatment and prevention of iatrogenic functional aphonia.

OBJECTIVETo discuss the diagnosis, treatment and prevention of iatrogenic functional aphonia.
METHODSTwenty three patients who either lost their voice or only could whisper after surgery in other hospitals were included in this study as the first group, history was well collected and laryngostroboscopy performed. All cases were confirmed as iatrogenic functional aphonia patients and received phonation therapy. In another group of patients who received vocal cord surgery in our hospital from 2003 to 2005, speaking was restricted while not prohibited after surgery, voice quality was closely observed, and 1028 cases were included.
RESULTSAll 23 cases of functional aphonia were cured with phonation therapy. No iatrogenic functional aphonia occurred in the second group of patients.
CONCLUSIONSThe iatrogenic functional aphonia can be caused by post operative mistreatment and could be cured with phonation therapy, and it is preventable if speaking is not strictly prohibited after surgery.

10.Effect of human urinary kallidinogenase on spatial learning and memory functions and nestin expression in peri-infarction cortex of rats after focal cerebral ischemia-reperfusion

Objective To investigate the effects of human urinary kallidinogenase (HUK) on the abilities of spatial learning and memory and the expression of nestin in the peri-infarction cortex of rats after focal cerebral ischemia-reperfusion. Methods Sixty rats were equally randomized into 5 groups:sham-operated group, model group, low dose HUK treatment group (3.5 ×10-3 PNAU/kg), median dose HUK treatment group (8.75×10-3 PNAU/kg) and high dose HUK treatment group (17.5×10-3 PNAU/kg).The focal cerebral ischemia-reperfusion models in the model group and HUK treatment groups were established by introducing an intraluminal filament into the right middle cerebral artery of the rats. HUK was administered intraperitoneally right after the operation and afterward once daily for 2 weeks. The spatial learning and memory functions were studied by Morris water maze test, and the nestin expression in the peri-infarction cortex was measured by immunohistochemistry on the 15th d. Results The model group exhibited seriously spatial learning and memory deficits in both place navigation trail and spatial probe trial. In the place navigation trial, the mean values of escape latency in the median dose and high dose HUK treatment groups were shorter than those in the model group (P<0.05). In the spatial probe trial, significant differences in the percentages of time spending in the former platform quadrant and frequency of crossing the former platform site were noted between the model group and both median dose and high dose HUK treatment groups (P<0.05). Immunohistochemical analysis showed that the nestin expression in the peri-infarction cortex of median dose and high dose HUK treatment groups increased significantly as compared with that in the model group (P<0.05). Conclusion Treatment with HUK improves the spatial learning and memory abilities in rats after focal cerebral ischemia-reperfusion, which may result from the increasing expression ofnestin and the proliferation of neural stem cells.
