Respiratory diseases are common in the elderly and often the main causes of death among this population. In addition, it is expected that chronic obstructive pulmonary disease, lower respiratory tract infections, lung cancer, and pulmonary tuberculosis will be listed in the top ten diseases in 2020. Therefore, screening, diagnosis and management of respiratory diseases should be strengthened among the elderly; meanwhile, studies on geriatric respiratory medicine should be further enhanced.

2.The immunity effects of quorum sensing in the pulmonary infection in rats due to pseudomonas aeruginosa

Objective To investigate the immunity effects of quorum sensing in the pulmonary infection in rats due to Pseudomonas aeruginosa.Method(1)300 healthy,clean Sprague-Dawley rats were divided into 3 groups randomly:two different Pseudomonas aeruginosa(the wide-type Pseudomonas aeruginosa PAO1 and its double mutant PAO-JP2,in which the quorum sensing systems were defective,embedded in minute seaweed algiante beads which was made by an ejection set with an acuminate hole were inoculated to Sprague-Dawley rats to establish animal model of chronic pulmonary infection.The control group were dealed with the same methods using the sterile brine instead.(2)The levels of antibody IgG,IgM,IgG1,IgG2a to Pseudomonas aeruginosa in sera and cytokines including interferon-?(IFN-?),Interleukin-4(IL-4)in lung homogenate was measured at 3,7,14,28 days after infection.Results(1)The mortality in the PAO1-JP2-infected group(11.0%)was significantly lower than that of the PAO1-infected group(29.7%)and the control group(4.21%).(2) Compared with the PAO-JP2 group,during the early stages of infection(3 days after infection),the IFN-?level in lung homogenates was significantly higher;during the middle stages of infection(7 days after infection),the levels of IFN-?and IL-4 of PAO1-infected rats were significantly higher;In the later stages of infection(14 to 28 days after infection),levles of IFN-?,IgG2a were lower,while levels of IL-4,IgG,IgG1 were higher persistantly in PAO1-infected rats.Conclusions Quorum sensing system play an important role in pathogenesis and immunity effects of pseudomonas aeruginosa infection via modulating the expression of virulence factors and interfering with the immune system of hosts.
3.Study and Application of Bacterial Mercury-resistance Mechanism and Evolution

There exist a number of mercury-resistant bacterial in environment, Mer operon is involved in the resistant mechanism, MerRTPA of Mer operon encodes the proteins related to the regulation, transport and reduction of mercury ion, respectively. The toxic mercury ion is transported by MerTP from medium to cytoplasmic mercuric reductase, MerA, and deoxidized to non-toxic and volatile element mercury, Hg(0). Bacterial mercury-resistant system originated from ancient times, and evolved into the Mer operon with diversity by gene integration and insertion. Mercury-resistant bacteria highly specifically absorb mercury ion, and can be used in recovering the mercury-polluted environment as well as the genetic selective marker.
4.Preliminary Study on Solid-state Fermentation of Streptomyces Strain Men-myco-93-63

The culture medium and cultural conditions of solid-state fermentation of Streptomyces Menmyco-93-63 were tested in this study. The suitable medium which contains rice, sorghum, millet bran, and rice hull with the proportion of 2:2:3:3 was developed for the spore production of Streptomyces Men-myco-93-63 using single substrate screening, mixture substrate screening and orthogonal experiments, and the sporulation was up to 2.52?109 CFU/g. And then, initial charge, initial ratio of water to solid, inoculating quantity, and culture temperature impact to sporulation of Streptomyces Men-myco-93-63 were tested. The favorite cultural conditions are developed as the following: the initial charge is 15 g in 500 mL Erlenmeyer flask; initial ratio of water to solid is 1.7:1.0 (V/W, rice hull excluding), inoculating quantity is 7 mL, culture temperature is 28℃.
5.Influence of anti-depression therapy on cardiac function and short-term prognosis in elderly patients with congestive heart failure

Objective To investigate the influence of anti-depression therapy on cardiac function and short-term prognosis in elderly patients with congestive heart failure (CHF) and the safety and effectiveness of fluoxetine in these patients.Methods Ninety six elderly hospitalized patients with CHF accompanied with depressive disorder were randomly divided into two groups:fluoxetine (20mg once daily)and placebo treated groups based on the routine cardiac drug therapy for 3 months. Results The prevalence rate of depressive disorder in elderly patients with CHF was 31.1%.The length of stay of fluoxetine group was shorter than that of placebo group.The post-treatment depressive exponents of self-rating depression scale(SDS)in fluoxetine group (51.39%?8.63)was lower than those in pretreatment and in placebo group.The improvement of cardiac function in fluoxetine group(53.4%?4.5%) was much better than that of placebo group (P
6.Genetic susceptibility in children with incomplete Kawasaki disease.

OBJECTIVETo study the frequency distribution of single nucleotide polymorphisms (SNPs) in two genes associated with incomplete Kawasaki disease (KD) (rs1569723 in CD40 gene and rs2736340 in BLK gene), and to investigate its association with the genetic susceptibility and clinical phenotypes of incomplete KD.
METHODSA total of 184 children with incomplete KD and 203 normal children were recruited to carry out a case-control study. The genotypes of SNPs in CD40 gene and BLK gene were determined using polymerase chain reaction-restriction fragment length polymorphism. The frequency distribution of genotypes was compared between the KD and control groups. The association between gene polymorphisms and clinical features of incomplete KD was analyzed.
RESULTSThere were no significant differences in genotype (AA, AC, CC) and allele frequencies in CD40 SNP rs1569723 between the KD and control groups. There were significant differences in the frequency distribution of three genotypes (TT, CT, CC) in BLK SNP rs2736340 between the KD and control groups (P=0.031), and the KD group had a significantly higher frequency of T allele than the control group (P=0.007). There were significant differences in the incidence of conjunctival hyperaemia among the patients with different genotypes (rs1569723 in CD40 gene) (P=0.036). The SNP rs2736340 in BLK gene was associated with the extremity changes in KD patients (P=0.017).
CONCLUSIONSThe SNP rs2736340 in BLK gene is associated with the susceptibility to incomplete KD, and the SNP rs1569723 in CD40 gene and SNP rs2736340 in BLK gene are associated with some of clinical phenotypes of incomplete KD.

7.Survey of lower urinary tract symptoms in community-dwelling elderly females

Objective To investigate the incidence of lower urinary tract symptoms(LUTS) and bladder function in the community-dwelling elderly females. Methods The questionnaires were administered,including complaints of LUTS,International Prostate Symptoms Score(IPSS) and quality of life assessment.Meanwhile,uroflowmetry was performed and volume of residual urine was measured. Results The total number of women surveyed was 359.According to IPSS,the prevalence of moderate to severe LUTS(IPSS≥8) was 39.0%.The prevalence in those with age of 50-59,60-69 and ≥70 years were 35.1%,46.2% and 54.8%,respectively(P=0.034).Of all the women surveyed,73.8% had predominantly irritative symptoms.The maximum flow rates(Qmax)were(24.5?11.5) mL/s,(22.7?11.0) mL/s and(14.5?8.2) mL/s,respectively for these age groups(P
8.Diagnosis and treatment and prevention of iatrogenic functional aphonia.

OBJECTIVETo discuss the diagnosis, treatment and prevention of iatrogenic functional aphonia.
METHODSTwenty three patients who either lost their voice or only could whisper after surgery in other hospitals were included in this study as the first group, history was well collected and laryngostroboscopy performed. All cases were confirmed as iatrogenic functional aphonia patients and received phonation therapy. In another group of patients who received vocal cord surgery in our hospital from 2003 to 2005, speaking was restricted while not prohibited after surgery, voice quality was closely observed, and 1028 cases were included.
RESULTSAll 23 cases of functional aphonia were cured with phonation therapy. No iatrogenic functional aphonia occurred in the second group of patients.
CONCLUSIONSThe iatrogenic functional aphonia can be caused by post operative mistreatment and could be cured with phonation therapy, and it is preventable if speaking is not strictly prohibited after surgery.

9.Comparison of lung function measurement values with two different apparatus.

This paper introduces a random measurement analysis of, lung function measurement values with two different apparatus. in 41 patients. It shows that the differences are not statistically significant (P>0.05) between two apparatus measurement values except DLCO, FEF25, FEF75 in the group of normal ventilation, FVC in the group of abnormal ventilation. The two groups are both correlated closely (r> 0.9) except MMF(r=0.7725, RV r=0.808) in the normal group of ventilation, and FEF75 (r=0.58) in the abnormal group of ventilation (p<0.001). The two apparatus with different measuring theories have a good correlation.
