6.Changes of Plasma Brain Natriuretic Peptide and N-Terminal Pro-B-Type Natriuretic Peptide of Sepsis Combined with Myocardial Injury in Newborns

Objective To explore the changes of plasma brain natriuretic peptide(BNP) and N-terminal pro-B-type natriuretic peptide (NT-proBNP)of sepsis combined with myocardial injury in newborns.Methods According to neonatal sepsis treatment program,45 cases of sepsis newborns in NICU of the Second Hospital in Lanzhou University from Jul.2007 to Jun.2008 were collected.According to the myocardial injury diagnostic criteria,45 cases neonatal sepsis were divided into myocardial injury group(n=22) and non-myocardial injury group(n=23).Myocardial injury group was also divided into congenital heart disease group and non-congenital heart disease group accor-ding to echocardiography.At the same time,30 healthy newborns were collected as healthy control group.Every newborns were tested the level of plasma BNP,NT-proBNP,creatine kinase-MB(CK-MB) and cardiac-troponin I (cTnI).Results There were significant difference between myocardial injury group,non-myocardial injury group and healthy control group in the levels of plasma BNP,NT-proBNP,CK-MB and cTnI,those in congenital heart disease group were higher than those in non-myocardial injury group and the healthy control group(Pa0.05).Conclusions BNP and NT-proBNP can be early used to diagnose myocardial injury and heart failure of neonatal sepsis associated with CK-MB and cTnI.In NICU,infants with sepsis should normally test BNP and NT-proBNP in order to early diagnose myocardial injury of neonatal sepsis.
7.Alteration of bone matrix components in degenerative vertebra metabolism of cervical spondylosis

Objective:To investigate the biological mechanism of the degeneration of cervical spine in cervical spondylosis(CS) by analyzing the alteration of bone matrix components.Methods:Twenty five degenerative cervical vertebra and 8 blood samples from CS cases were collected.The contents of hyaluronic acid(HA),laminin(LN),pro collagen Ⅲ,collagen Ⅳ were detected by radioimmunoassay.Calcium,phosphate and total protein levels were detected by automatic biochemical analyzer.Results:Ruling out the inference of blood, the contents of HA, LN, pro collagen Ⅲ and collagen Ⅳ were significantly lower in degenerated CS vertebra than in the control, so as the levels of calcium and phosphate. Conclusion:Bone matrix components are obviously decreased during the degeneration of cervical vertebra, resulting in the alleviation of bone tenacity and hardness. This may be one of the biological mechanisms of cervical vertebra degeneration and deterioration of cervical spine biomechanics.
9.Effect of crocetin on cardiac hypertrophy induced by overloading pressure in rats.

AIMTo study the influence of crocetin on cardiac hypertrophy induced by overloading pressure in rats.
METHODSThe model of cardiac hypertrophy was produced by overloading pressure in rats. The animals were divided into five groups: sham-operation group (0.5% CMC-Na, ig), model group (operation + 0.5% CMC-Na, ig), captopril group (operation + 50 mg x kg(-1), ig), crocetin I (100 mg x kg(-1), ig) and crocetin II (50 mg x kg(-1), ig). All animals were treated for 30 d by ig. Then, cardiac indexes were examined. The activity of ATPase and the hydroxyproline content in heart were assayed by colorimetric analysis. Matrix metalloproteinases (MMPs) activity was assayed by SDS-PAGE zymography.
RESULTSCompared with the model group, crocetin was found to significantly reduce the cardiac indexes and the content of hydroxyproline in heart, increase the activity of Na+ , K+ -ATPase, Ca2+, Mg2+ -ATPase and inhibit MMPs activity.
CONCLUSIONThe activity of MMPs may play a key role in the cardiac hypertrophy induced by overloading pressure, and proprably as a result of decreasing the activity of MMPs. Crocetin was shown to prevent remodeling of cardiac hypertrophy induced by overloading pressure.

10.Clinical Features and Antibiotic Sensitivity of Gram-Negative Diabetic Foot Osteomyelitis

Objective To investigate the clinical features and antibiotic susceptibility of osteomyelitis infected by Gram-negative bacteria (G-) in patients suffered from diabetic foot ulcers (DFU). Methods The clinical data of 91 DFU pa-tients accompanied with osteomyelitis (DFO) were retrospective studied. These patients hospitalized in the Tianjin Metabolic Diseases Hospital were divided into two groups, Gram-negative bacteria (G-) group (n=44) and Gram-positive bacteria (G+) group (n=42), respectively. The clinical features were compared between two groups. Logistic regression analysis was used to determine the risk factors for Gram-negative bactreial infection. The Gram-negative antibiogram was summarized. Results A total of 112 pathogens were isolated from 91 patients. G-bacteria were the most frequent pathogens (48.2%), following by G+ bacteria (47.3%) and fungi (4.5%). Pseudomonas aeruginosa was the majority of the G-bacteria. Comparing the two groups, the rate of antibiotic use within the previous 6 months was significantly higher in G-group (75.0%) than that of G+group (52.4%, P<0.05). There were no significant differences in the other indicators between two groups. The Logistic re-gression analysis revealed that the history of antibiotic use was the independent risk factor of G-bacterial infections in DFO patients. Antibiotics susceptibilities reflected G- bacteria were more prevalent to resist to cephalosporins and quinolonem, but sensitive to imipenem, ceftazidine and cefperazone-sulbactam. Conclusion Gram negative bacteria were not only the main pathogens isolated from DFO patients, but also frequently resistant to several popular antibiotics in China. The proper bacteria culture and antibiotic sensitivity test are especially emphasized to patients with DFU.
