1.Effects of inhaled nitric oxide on hemodynamics and oxygenation in piglets with sepsis

Objective To explore the changes in cardiac index and oxygenation index in sepsis piglets after nitric oxide (NO) gas inhalation. Methods A piglet model of sepsis was induced by intravenous infusion of Gram-negative bacterial endotoxin (LPS), then the piglets were randomly divided into two groups. NO group (n=8) was administered with inhaled nitric oxide of 80ppm via volume control (VC) mechanical ventilation for one hour, while the control group (n = 4) received mechanical ventilation with VC and was observed for one hour to assess the stability of the model. The parameters of oxygenation and hemodynamics were measured by PICCO and arterial blood gas analysis every fifteen minutes for one hour. Results Injection of endotoxin induced a stable pig model of sepsis. PH, HCO3-, arterial oxygen pressure (PaO2), mean arterial pressure (MAP) and cardiac index in this model were significantly lower the baseline values (P < 0.01). Arterial oxygen pressure and cardiac index were significantly higher in N0 group than in the control group (P<0.01). Heart rate (HR), central venous pressure (CVP), global end-diastolic volume index (GEDI) and intrathoracic blood volume index (ITBI) did not significantly differ between NO group and the control group. Conclusions Inhalation of nitric oxide gas can significantly improve oxygenation and cardiac function in piglets with sepsis.
2.Genetic elements of drug-resistance in a strain of multidrug-resistant Klebsiella pneumoniae isolated from bronchoalveolar lavage fluid

Objective To analyze the resistance genes in a muhidrug resistant Klebsiella pneumoniae (MDRKP) strain.Methods A MDRKP strain was isolated from bronchoalveolar lavage fluid in Pediatric Intensive Care Unit of Children's Hospital Affiliated to Soochow University in February 2012.Acquired resistance genes to beta-lactams,aminoglycosides,quinolones,ompK35 and ompK36 gene for outer membrane porin protein,and carbapenems targeting PBP2 gene were analyzed by PCR and verified by DNA sequencing.Results Acquired resistance genes TEM-1,SHV-1 to beta-lactam antimicrobial agents and aac(6′)-I b to aminoglycoside antimicrobial agents were positive in the strain of MDRKP.While 16S rRNA methylase,ompK35 and ompK36 genes for outer membrane porin protein were negative.Compared with susceptible strains,there were 9 synonymous mutations in PBP2 gene sequence of this MDRKP strain,but the amino acid sequences were the same.No mutation in quinolone resistance determining region (QRDR) was observed.Conclusion The multidrug resistance of the isolated Klebsiella pneumoniae strain may be related to 2 kinds of beta-lactam acquired resistance genes,1 kind of aminoglycoside acquired resistance gene,ompK35 and ompK36 genes defects and synonymous mutation in PBP2 gene.
3.Acute adverse effects of radiotherapy on HIV-positive patients with malignancy

Objective To observe and evaluate acute toxicities in a series of human immunodeficiency virus (HIV)-positive cancer patients receiving radiation therapy.Methods The study retrospectively reviewed the acute radiation reaction of radiation therapy of 14 HIV seropositive patients diagnosed with carcinoma between Feb 2008 and Dec 2013 at the Yunnan Tumor Hospital during the radiotherapy period and 1 month following treatment.Acute adverse effects were classified according to the site of radiation therapy and analyzed using the Common Terminology Criteria for Adverse Events (CTCAE) version 3.0.Results Seven patients experienced interruptions or delays in treatment,and 2 stopped treatment entirely.The most common acute adverse effects were skin reactions and mucous membrane reactions,including dermatitis,stomatitis or diarrhea.Eight patients had grade 3 acute adverse effects,including 6 patients with grade 3 skin reactions and 2 patients with grade 3 mucosa reactions.Conclusions Radiotherapy is an effective treatment for HIV positive patients with tumors,however it frequently induced severe acute radiation responses.
4.The changes of VEGF level in patients with hepatocellular carcinoma after argon-helium cryoablation combined with transcatheter arterial chemoembolization treatment

Objective To investigate the therapeutic effect of argon-helium cryoablation combined with transcatheter arterial chemoembolization (TACE) for primary hepatocellular carcinoma (HCC) and its influence on vascular endothelial growth factor (VEGF) level, and to compare it with simple TACE treatment. Methods During the period from Aug. 2013 to Aug. 2014 a total of 50 patients with primary HCC were admitted to Hunan Provincial People’s Hospital. The patients were randomly and equally divided into group A (TACE group, n=25) and group B (argon-helium cryoablation+TACE group, n=25). VEGF levels in the peripheral blood were determined before and after the treatment, and the results were statistically compared between the two groups. Results After the treatment the VEGF level in group A was significantly increased when compared with preoperative VEGF level (P<0.05). In group B, the VEGF level was decreased after the treatment, which was statistically significant lower than the preoperative VEGF level (P<0.05). The differences in VEGF level between the two groups were statistically significant (P<0.05). Conclusion The statistic analysis of VEGF levels indicates that argon-helium cryoablation combined with TACE is probably superior to simple TACE in the inhibition of tumor angiogenesis.
5.The properties of different orthodontic rectangular arch wires in torsion.

OBJECTIVETo study the properties of orthodontic rectangular arch wires in torsion.
METHODSBy using the torsion measurement apparatus, torsion moments and angles of four kinds of orthodontic wires (0.46 mm x 0.63 mm and 0.48 mm x 0.63 mm stainless steel wires, and the same size nickel-titanium wires) were tested respectively in a 0.56 mm-slot first maxillary incisor bracket. According to the torsion moments and angles, scatter graphs were drawn to fit the load-deflection curves of four kinds of wires and their linear equations were calculated. According to the equations, the torsion stiffness and the play angles of the wires at 20 N mm correspondingly were calculated.
RESULTSThe torsion stiffness of four kinds of wires were 4.909, 6.417, 1.325 and 1.363 N mn/degrees, the torsion angles of different wires were 19.14 degrees, 15.33 degrees, 36.83 degrees and 35.07 degrees at 20 N mm respectively.
CONCLUSIONThe torsion stiffness of stainless steel wires was bigger than that of the same cross-section dimensional nickel-titanium wires. The difference of torsion stiffness between two dimensional nickel-titanium wires is smaller, while the difference of torsion stiffness between two stainless steel wires is larger.

6.Experimetal study of different processed products of Shudihuang on bored of growing stomach in rats

Objective To observe and compare the effects of 4 kinds of different processed products of Radix Rehmanniae (RR) Preparata on the phenomenon of bored of growing stomach in rats.Methods Four kinds of different processed products of prepared RR were selected and named as following:Shudihuang-A (steamed RR);Shudihuang-B (stewed with wine RR Preparata and RR);Shudihuang-C (nine steamed nine sun Shudihuang RR Preparata);Shudihuang-D[RR nine steamed nine sun (Que Sharen)].The Shudihuang decoction in A,B,C,D groups were preparation.SD rats were intragastrically administrated for 14 d after fasting 24 h with carbon semisolid paste each 0.5 g by gavage.After 30 min,the gastrin in serum,plasma motilin and content of substance P were determined by ELISA.After blood collection,the abdominal cavity was exposed,and the gastric net weight was calculated.Results The residual rate of gastric contents in rats of blank group was (3.18 ±0.22)%,the residual rate of gastric contents in rats of Shudihuang-A,-B,-C,-D groups were (3.78 ±0.27)%,(1.90 ±0.16)%,(2.40 ± 0.78) %,(2.65 ± 0.67) % respectively with significance (P < 0.05 in A-group,P < 0.01 in B,-C,-D groups).Compared with A-group,the difference in B-group with significance(P <0.01).The content of gastrin in blank group was (146.19 ± 123.51) pg · mL-1,the content of gastrin in rats of Shudihuang-A,-B,-C,-D groups were (131.50 ±59.94),(151.97 ± 120.49),(233.46 ± 15.99),(179.18 ±36.02) pg · mL-1 respectively with significance(P < 0.05 in Shudihuang-A group,P < 0.01 in Shudihuang-C,-D groups).Compared with C-group,the difference in D-,A-,B-groups with significance(all P < 0.01).Compared with A-group,the difference in B-group with significance (P < 0.01).The plasma motilin content in rats of blank group was (84.14 ± 125.93)pg · mL-1,the plasma motilin content in rats of Shudihuang-A,-B,-C,-D groups were (81.91 ± 313.77),(87.30 ± 67.10),(188.68 ± 32.59),(170.08 ± 83.08) pg · mL-1 respectively with significance(all P > 0.05 Shudihuang-A,-B groups;all P < 0.01 in Shudihuang-C,-D groups).Compared with C-group,the difference in D-,A-,B-groups with significance(all P <0.01).The substance P content ofin rats of blank group was (19.82 ± 13.87) pg · mL-1,the substance P content of in rats of Shudihuang-A,-B,-C,-D groups were (13.38 ±6.47),(23.01 ±10.41),(37.46±0.44),(29.93±7.77) pg· mL-1 with significance(P < 0.05 in Shudihuang-A group with decreased while Shudihuang-B,-D groups with increased;P < 0.01 in Shudihuang-C group with increased).Compared with C-group,the difference in D-,A-,B-groups with significance (all P < 0.01).Conclusion The bored of growing stomach phenomenon caused by steamed Shudihuang is most serious in rats.The nine steamed nine sun Shudihuang can effectively eliminate greasy effect in rats by increasing the contents of gastrin,motilin,substance P and reducing the residual of gastric contents in rate.
7.Oridonin inhibits proliferation of Jurkat cells via the down-regulation of Brg1.

OBJECTIVETo investigate the effect of oridonin on the human acute lymphocytic leukemia cell line Jurkat and its mechanism.
METHODSJurkat cells were cultured in vitro and treated with various concentrations (0, 1.25, 2.5, 5, and 10 μmol/L) of oridonin for different lengths of time (24, 48, and 72 hours). The proliferation of Jurkat cells was analyzed by MTT assay. The changes in nuclear morphology were evaluated by fluorescence microscopy at 12 hours after treatment with various concentrations of oridonin. The expression levels of Brg1, P53, and C-myc were determined by semi-quantitative Western blot in Jurkat cells treated with various concentrations of oridonin for 24 hours or 5 μmol/L oridonin for various lengths of time (0, 2, 6, 12, and 24 hours). The expression levels of P53 and C-myc and proliferation of Jurkat cells were evaluated after Brg1 expression was knocked down by Brg1-specific siRNA.
RESULTSCompared with the control group, the proliferation of oridonin-treated Jurkat cells was significantly inhibited in a concentration- and time-dependent manner (P<0.05). According to the florescence microscopic analysis, oridonin treatment led to nuclear pyknosis in Jurkat cells. Compared with the control group, Jurkat cells treated with 5 μmol/L oridonin had reduced expression of Brg1 and C-myc but elevated expression of P53. Brg1 knock-down led to a significant reduction in proliferation of Jurkat cells (P<0.05), up-regulated expression of P53, and down-regulated expression of C-myc.
CONCLUSIONSOridonin can inhibit the proliferation of Jurkat cells, probably via the Brg1 signaling pathway.

8.8.5/11.5F transurethral seminal vesiculoscopy in the diagnosis and treatment of refractory hematospermia.

OBJECTIVETo investigate the clinical application value of 8.5/11.5 F transurethral seminal vesiculoscopy in the diagnosis and treatment of refractory hematospermia.
METHODSWe retrospectively analyzed 78 cases of refractory hematospermia diagnosed and treated by 8.5/11.5 F transurethral seminal vesiculoscopy from June 2012 to June 2014. The patients underwent serum PSA examination, transrectal ultrasonography, seminal vesicle ultrasonography, and pelvis CT or MRI before surgery, and all received transurethral seminal vesiculoscopy under the 8.5/11.5 F rigid ureteroscope.
RESULTSOperations were all successfully accomplished, which revealed abnormal opening of the ejaculatory duct in 5 cases, mucosal inflammatory hyperemia in the prostatic utricle and seminal vesicle in 78, dark red mucilage substance in the seminal vesicle in 34, seminal vesicle stones in 19, small polyp in the seminal vesicle in 2, and ejaculatory duct or seminal vesicle cyst in 4. All the patients received symptomatic treatment during the surgery. After surgery, hematouria was found in 13 cases, which disappeared within 2 weeks, pelvic hematoma in 1 case, which was cured by conservative treatment within 3 months, and epididymitis in 2 cases, which was controlled by anti-infection treatment. Hematospermia recurred in 3 cases during the 1-year postoperative follow-up.
CONCLUSION8.5/11.5 F transurethral seminal vesiculoscopy, with its advantages of easy operation, wide field of vision, large channel for operation, and few complications, deserves general clinical application in the diagnosis and treatment of refractory hematospermia.

9.Effect of loop configuration on appliance stiffness of stainless steel round wire.

OBJECTIVETo investigate the effect of loop configuration on appliance stiffness of stainless steel round wire.
METHODSSome stainless steel round wires with various cross-sectional dimension (0.36, 0.41, 0.46 mm) were selected. Box loops, double horizontal loops, and double vertical loops were manufactured according to established parameters, and loading experiments were carried out with a micro-orthodontic archwire testing apparatus.
RESULTSWhen the cross-sectional dimension and the span were identical, the appliance stiffness of the box loops, double horizontal loops, and double vertical loops increased successively.
CONCLUSIONConfiguration of loops exert a great influence on appliance stiffness, and the mechanical properties of box loop are optimal. The force generated by box loop was lighter, slowly decreasing, and easier to be controlled. The activation range of box loop (0.41 mm) was appropriate when the produced optimal moment was between 0.01-0.02 N.m. Box loop (0.41 mm) is preferred to rectify the individual tooth with mild or moderate mesiodistal displacement.

10.In vitro effects of anti-CD44 monoclonal antibody on the adhesion and migration of chronic myeloid leukemia stem cells.

OBJECTIVETo explore the effects of anti-CD44 monoclonal antibody-IM7 on the in vitro adhesion and migration of chronic myeloid leukemia stem cell (CML-LSC) and its mechanism.
METHODSCD34(+)CD38(-)CD123(+) leukemic stem cells (LSC) from 20 newly-diagnosed chronic myeloid leukemia (CML) patients BM cells and CD34(+)CD38(-) hematopoietic stem cells (HSC) from 20 full-term newborn cord blood cells were isolated with EasySep(TM) magnet beads. The CD44 expression of the LSC and HSC was detected by flow cytometry (FCM), and the adhesion and migration ability of the LSC and HSC pre- and post-incubated with IM7 in vitro by MTT assay and transendothelial migration assay, respectively.
RESULTS(1) After incubated with IM7, the LSC and HSC CD44 expression rates were (86.60 ± 2.10)% vs. (25.40 ± 1.70)% (P < 0.05), respectively. (2) The adhesive ability of the LSC to endothelial cells was decreased markedly after incubated with IM7, the OD value (A(570)) changing from pre-incubation of (0.62 ± 0.11) to post-incubation of (0.34 ± 0.07), while there was little change of A(570) in the HSC group. (3) The migration ability of the LSC group was inhibited evidently after incubated with IM7, the inhibition rate being 46% ∼ 63%, while little change of that in HSC group was detected. (4) The adhesive ability of the LSC group to marrow stromal cells was decreased markedly after incubated with IM7, while little change was found in that of HSC group.
CONCLUSIONThe anti-CD44 monoclonal antibody-IM7 can effectively inhibit the adhesion and migration abilities of the LSC in vitro, which might provide a theoretical evidence for targeting therapy.
