[ ABSTRACT] AIM:To study the effects of nuclear factor kappa B ( NF-κB) on human hepcidin expression in fer-ric ammonium citrate ( FAC)-induced HH4 hepatocytes.METHODS:Non-transformed HH4 cells were exposed to FAC at concentrations of 0.1, 1, 5 and 10 mmol/L for 48 h.The expression of iron regulatory gene hepcidin was determined by semi-quantitative RT-PCR.The effects of NF-κB on hepcidin transcriptional activity were detected using chromatin immuno-precipitation (ChIP), electrophoretic mobility shift assay (EMSA) and dual-luciferase reporter assay system, combined with the inhibition experiments of intracellular NF-κB activity.RESULTS: FAC at concentrations of 5 mmol/L and 10 mmol/L significantly enhanced the expression of hepcidin. The results of ChIP and EMSA showed the binding of NF-κB to the upstream of hepcidin promoter.Treatment with NF-κB inhibitor BAY 11-7082 attenuated hepcidin expression. The lucif-erase activity in the cells transfected with recombinant luciferase reporter plasmid was obviously higher than that in control group.CONCLUSION:NF-κB is the transcription factor that contributes to hepcidin expression in iron overload-induced HH4 cells.

OBJECTIVETo explore therapeutic effects of minor external fixator for open comminuted phalangeal fractures with non-transarticular.
METHODSFrom January 2013 to December 2014,13 patients with open comminuted phalangeal fractures were treated by minor external fixator with non-transarticular, including 9 males and 4 females aged from 18 to 56 years old with an average of 35 years old. According to Gustilo classification, 8 cases were type II, and 5 cases were type III. The time from injury to operation ranged from 2 to 7 h averaged 5 h. Seven patients were diagnosed as phalangeal fracture, 6 patients were involved with joint surface of phalangeal, which 1 case combined with inherent nerve damage and 1 case combined with soft tissue defect repaired at stage II. The patients were fixed by plaster for 2 weeks and removal external fixtor from 6 to 8 weeks after operation and received functional exercise.
RESULTSAll patients were followed up from 3 to 12 months with an average of 7 months. One case occurred metacarpophalangeal joints stiffness. No screw breakage, inflammation occurred. Skin soft tissue recovered well and obtained bone healing, the average time of bone healing was (4.6 ± 1.0) weeks. According to TAM scoring system used by Hand Surery Association of Chinese Medical Association, 7 cases got excellent results, 4 good, 1 moderate and 1 poor.
CONCLUSIONMinor external fixator for open comminuted phalangeal fractures with non-transarticular has advantages of simple operation, good stability, and could adjust at the later stage, less damage for soft tissue periosteum, low inflammatory rate, earlier functional exercise. It is worth to be popularized and applied.

4.Government compensation for Australian public hospitals
Chinese Journal of Hospital Administration 2012;28(2):153-157
As introduced in the paper,Australian government subsidize the public hospitals with all their budget needs as compensation, with the two levels of governments constituting the largest buyer of health products and services in the country. In addition, the government supports the hospitals with their facilities.A rational government investment mechanism is proposed with reference to such a model for the ongoing reform of public hospitals in China.
5.Detection and comparison of plasma calprotectin in different stages of diabetic retinopathy in patients with type 2 diabetes mellitus
Chinese Journal of Experimental Ophthalmology 2012;30(4):367-370
BackgroundThe neutrophils infiltration and vascular endothelium damage are found in the patients with diabetic retinopathy (DR).Calprotectin existes in the cytosol outside lysoome. It is thought to be a marker of inflammation. The effect of calprotectin in the development of DR is still in the study. Objective This study was to investigate the contents of plasma calprotectin in different stages of DR in patients with type 2 diabetes mellitus. Methods This was a case-control study.Sixty consecutive patients with type 2 diabetes mellitus were enrolled in this study. The patients were assigned to non-DR (NDR) group,non-proliferative DR (NPDR) group and proliferative DR (PDR)group according to fundus appearance and fundus fluorescein angiography(FFA) manifestation and 20 patients for each group.Twenty healthy subjects matched in gender, age and blood biochemical indicators were collected as the normal control group. The periphery blood samples were collected from the subjects for the detection of plasma calprotectin by ELISA.The plasma calprotectin levels were compared among different stages of DR and normal subjects. All subjects had signed informed consents. Results The contents of plasma calprotectin were (57.70±12.29 ),( 72.07± 10.14 ),( 87.70 ± 10.37 ),( 94.36 ± 9.40 ) ng/L in the normal control group,NDR group,NPDR group,PDR group respectively, with a statistically significant difference among 4 groups (F =73.09,P<0.001 ).The content of calprotectin in PDR group showed a highest value in comparison with normal control group,NDR group and PDR group(q =20.157,10.648,4.497,P<0.01 ).The content of calprotectin in NPDR group was significantly higher than that in NDR group( q=6.216,P<0.01 ). ConclusionsPlasma calprotectin may play a role during the development of DR in type 2 diabetes mellitus patient.

Objective To discuss the efficacy of the modified subinguinal approach for the treatment of acetabular anterior column and wall fractures. Methods A clinical retrospective study was performed of the 5 patients who had been treated at our department from January 2011 to April 2015 for acetabular anterior column and wall fractures and conformed to follow-ups.They were 3 males and 2 females, aged from 19 to 45 years (mean,35.6 years). The duration from injury to surgery averaged 7.6 days (range, from 4 to 10 days). The subinguinal ligament approach was adopted in the 5 cases, through which the inguinal ligament was preserved intact after sharp dissection of its insertion at the anterior superior iliac spine and medial-upper advancement, the advantage of expanded exposure of the first window by the Farid sub-ilioinguinal approach was retained, and additional lesions due to iliac osteotomy were avoided. The operation time, intraoperative bleeding, postoperative reduction and time for fracture union were documented.Merle d' Aubigne and Postel scoring system was used to assess the hip joint function of the affected limb at the final follow-ups.Results The operation time ranged from 110 to 150 minutes (mean,125 minutes);the intraoperative bleeding ranged from 415 to 550 mL (mean,450.2 mL).By the Matta's criteria, the postoperative reduction was rated as excellent in 4 cases and good in one.The 5 patients were followed up for an average of 32.4 months (range, from 6 to 43 months).Their fractures united clinically after an average of 4.6 months (range, from 3 to 6 months). The hip joint function was rated as excellent in 2 and good in 3 by the Merle d' Aubigne and Postel scoring system at the final follow-ups.Follow-ups observed no serious complications like inguinal hernia, internal fixation failure, myositis ossificans,or avascular necrosis of the femoral head. Conclusions As the modified subinguinal approach can provide a broad surgical exposure, lead to minimal injury to the inguinal ligament, avoid lesions caused by iliac osteotomy, and decrease operation time and bleeding,it may be a better approach for acetabular anterior column and wall fractures.
