3.Development and Practice of Wartime Military Medical Service Information System in Field Medical Clinic

Objective To provide information support for military medical service drilling with actual arms in field medical clinic. Methods The local netwok was built based on Intranet mode for military medical service. A distributed wartime military medical service information system was developed. The long-range consultation was carried through a consultation vehicle. The field medical clinic information system was integrated with No.1 Military Medical Project. Results The system has been used in drilling with actual arms of field medical clinic for three years. Conclusion The result shows that the system can satisfy the need of information management of field medical clinic and enhanced the efficiency of management greatly.
4.Association of single nucleotide polymorphism of transcription factor 7-like 2 gene with gestational diabetes mellitus

Objective To investigate the relationship between gene polymorphism of transcripion factor 7-like 2 (TCF7L2) at positions rs290487, rs11196205, rs11196218 and gestational diabetes mellitus (GDM) in Chinese women.Methods In 1140 unrelated pregnant Northern Chinese women (335 women with GDM, 158 gestational cases with impaired glucose tolerance and 647 pregnant non-diabetic controls) ,three single nucleotide polymorphisms (rs290487, rs11196205, and rs11196218) in the TCF7L2 gene were genotyped using ligase detection reaction (LDR).In the present study, cases with GDM and impaired glucose tolerance (IGT) were indistinguishable clinically and biochemically, and were combined into case group.Results The frequency of C allele of rs290487 was 41.6% in case group, being significantly higher than that in control group (36.3%, P=0.012).There was significant difference in the frequency of CC genotype between case group and control group (18.7% vs 14.0%, P=0.033).Compared with T allele carriers, CC genotype carriers had a 1.418-fold increased risk of GDM (95% CI 1.028-1.955).After adjusting for age, body mass index, family history of diabetes,systolic blood pressure,and diastolic blood pressure, pregnant women with CC genotype carriers of rs290487 were more prone to hyperglycemia compared with the T allele carriers (OR 1.518, 95% CI 1.064-2.166).Conclusions The TCF7L2 rs290487 variant may contribute to the genetic predisposition to GDM.CC genotype is likely to be associated with an increased risk of GDM in the pregnant Chinese women.
5.Identification of phenylpropanoids in ciwujia injection by HPLC-MS.

There are reports about the chemical compounds of Ciwujia herbs, but with no study report about the chemical material basis of Ciwujia injection (CWJI). In this study, LC-MS(n) and LC-Q-TOF-MS techniques were adopted for a qualitative analysis on phenylpropanoids in CWJI. The Ultmate XB-C18 column (4.6 mm x 250 mm, 5 microm) was adopted and eluted with the mobile phase of 0.5% formic acid-water and acetonitrile, with the flow rate at 0.8 mL x min(-1) and the column temperature at 20 degrees C. Based on the data of high-resolution and multi-stage MS, control products and literatures, altogether 54 phenylpropanoids were identified in Ciwujia Injection, including 34 phenylpropanoids, 16 ligans and 4 coumarins. Among them, 28 were reported for the first time in Ciwujia, and 14 compound structures were identified in comparison with the control products. The method established in this study could be used to simply and rapidly identify phenylpropanoids in CWJI. The findings provide scientific data for defining the chemical material basis of CWJI.

6.Research progress on preparation technology of nanocrystal drugs

Nanocrystal drugs have many advantages, such as no carrier materials, easy industrialization, diversified dosage forms, and can significantly improve the solubility and bioavailability of insoluble drugs, so many drugs have been on the market. The traditional nanocrystal preparation technology has the problems of low preparation efficiency and process limitation of the smallest achievable particle size. With the progress of pharmaceutical preparation technology, the preparation technology of nanocrystal drugs is constantly improving, and new preparation technologies are constantly emerging. The emergence of new technologies has greatly shortened the process time and makes it possible to prepare nanocrystal drugs with smaller particle diameters. In this paper, the preparation technologies of nanocrystal drugs, especially the new preparation technologies such as high gravity controlled precipitation, microfluidic reaction technology and various combination technologies, are reviewed from three aspects: "Top-down" technology, "Bottom-up" technology and combination technology. This article also prospects the development of new preparation technologies, hoping to provide reference for the related research of nano-preparations.
7.Mechanical properties of thermodynamic nickel-titanium archwires in a simulated oral environment in vitro.

OBJECTIVEThis study was to investigate the mechanical properties of thermodynamic nickel-titanium (NiTi) archwires of different brands under controlled temperature and in artificial saliva to provide some reference for its clinical application.
METHODSUsing a modified partial dental arch bending system, four types of thermodynamic NiTi archwires were tested under controlled temperature (25, 33, 37, 60 degrees C) and in artificial saliva. An Instron Universal Testing Machine was used to apply 3 mm deflections of archwires in the lateral incisor area. The load-deflection curves were determined from the passive position to deflections of 3 mm. The following variables from unloading curves were calculated: Stiffness between 2.5 mm UDP (unloading deflection point) to 0.5 mm UDP after unloading. Any statistically significant differences in these variables for the different brands, temperature and moments levels were analyzed using one-way analysis of variance.
RESULTSUnloading values of four types thermodynamic nickel-titanium archwires increased with controlled temperature rising. The unloading stiffness of the same brand archwires were approximate at 25, 33, 37 degrees C, while their unloading stiffness increased obviously at 60 degrees C.
CONCLUSIONDifferent brands thermodynamic NiTi archwires demonstrated different mechanical behaviours, but all of them exhibited super elasticity and practical shape memory effect. The four thermodynamic NiTi archwires displayed temperature sensitivity at the four test temperatures.

8.Clinical significance of diagnosis of varicocele with infrared ray imaging.

OBJECTIVETo investigate the temperature change of scrotum caused by varicocele(VC) and its significance.
METHODSOne hundred and sixty-six VC patients were examined by infrared ray imaging and colored ultrasonic scanning, and the results were analyzed. Comparisons of the infrared ray images and the routine semen analysis of 106 of the patients were made between before and after the operation.
RESULTSThe difference in the infrared ray images between before and after the operation on the VC patients was significant (P < 0.01), and so was the difference in the results of semen analysis(P < 0.01; some of the items P < 0.05).
CONCLUSIONSThe infrared ray imaging, together with the semen analysis and colored ultrasonic scanning, had a directive value for the diagnosis and treatment of VC patients.

9.N-terminal of ?-Synuclein Involved in Regulation of Mitochondrial Function

Objective: To identify the functional domain of ?-Synuclein in affecting mitochondrial function and how the function to be impaired,especially,the mitochondrial membrane potential and the release of Cytochrome c.Methods: Harvest of ?-Syn-N and ?-Syn-△N by PCR,then subcloned into the pCMV-Myc mammalian expression vector.The recombinant plasmids were transfected into HEK293T cells by Lipofectamine 2000.After detecting the protein expression by Western blot,the functional domain was detected by co-immunoprecipitation.The mitochondrial membrane potential through flow cytometry and immunofluorescence,at the same time,the release of Cytochrome c through flow cytometry to detect.Results: The recombinant plasmids were constructed successfully.CO-IP has proved that N-terminal may be the functional domain of ?-Synuclein in affecting mitochondria.Over-expression of N-terminal could depolarize the mitochondrial membrane potential and induce the Cytochrome c releasing in MN9D cells.Conclusion: N-terminal may be the functional domain of ?-synuclein and over-expression of N-terminal could decrease mitochondrial activity.
10.Post-disaster medical rescue strategy in tropical regions

BACKGROUND: Earthquakes, floods, droughts, storms, mudslides, landslides, and forest wild fires are serious threats to human lives and properties. The present study aimed to study the environmental characteristics and pathogenic traits, recapitulate experiences, and augment applications of medical reliefs in tropical regions. METHODS: Analysis was made on work and projects of emergency medical rescue, based on information and data collected from 3 emergency medical rescue missions of China International Search and Rescue Team to overseas earthquakes and tsunamis aftermaths in tropical disaster regions Indonesia-Aceh, Indonesia-Yogyakarta, and Haiti-Port au Prince. RESULTS: Shock, infection and heat stroke were frequently encountered in addition to outbreaks of infectious diseases, skin diseases, and diarrhea during post-disaster emergency medical rescue in tropical regions. CONCLUSIONS: High temperature, high humidity, and proliferation of microorganisms and parasites are the characteristics of tropical climate that impose strict requirements on the preparation of rescue work including selective team members suitable for a particular rescue mission and the provisioning of medical equipment and life support materials. The overseas rescue mission itself needs a scientific, efficient, simple workflow for providing efficient emergency medical assistance. Since shock and infection are major tasks in post-disaster treatment of severely injured victims in tropical regions, the prevention and diagnosis of hyperthermia, insect-borne infectious diseases, tropic skin diseases, infectious diarrhea, and pest harms of disaster victims and rescue team staff should be emphasized during the rescue operations.
