2.Clinicopathological analysis for postoperative recurrence in early gastric cancer patients

Objective To clarify the risk factors of postoperative recurrence of early gastric cancer (EGC)patients who underwent radical gastrectomy.Methods We investigated follow-up records of 336 EGC patients on whom curative operations were performed at the First Affiliated Hospital of Dalian Medical University between January 1994 and January 2008 with special reference to 12 cases of cancer recurrence.Results The 5-year survival rate of D1 was 89.0%,D1 + t was 91.4%,D1 + β was 91.8%,D2 was 92.5%,D3 was 90.0%(P =0.981).The recurrence rate was 3.6%(12/336).The recurrence time was 17-58 months,the survival time was 33-68 months.Six cases suffered from hematogenous recurrences,four cases from lymph node recurrences,two cases from remnant stomach recurrences.The recurrence rate of SM was higher than M(P =0.015),lymph node positive was high than negative(P =0.011),D1 was higher than D2(P =0.025).Conclusions The recurrence rate of EGC correlated with depth of invasion,lymph node metastasis and lymph node dissection.
3.Are cancer stem cells the sole source of tumor?

Tumors are believed to consist of a heterogeneous population of tumor cells originating from rare cancer stem cells (CSCs). However, emerging evidence suggests that tumor may also originate from non-CSCs. To support this viewpoint, we are here to present definitive evidence indicating that the number of tumorigenic tumor cells is greater than that of CSCs in tumor, and tumor can also derive from non-CSCs. To achieve this, an idealized mathematical model was employed in the present study and theoretical calculation revealed that non-CSCs could initiate the occurrence of tumor if their proliferation potential was adequate. Further, experimental studies demonstrated that 17.7%, 38.6% and 5.2% of tumor cells in murine B16 solid melanoma, H22 hepatoma and Lewis lung carcinoma, respectively, were potentially tumorigenic. Thus, based on the aforementioned findings, we propose that the scarce CSCs, if exist, are not the sole source of a tumor.
4.Expression of Hsp22 and VEGF after brain I/R injury in gerbils and its neuroprotective effects

Objective In this study we investigated the expression and protective mechanisms of Hsp22 and VEGF in brain tissue following cerebral ischemic reperfusion in gerbils and whether they are correlative. Method Forty five gerbils were randomly divided into there groups: Normal Group (n =5) ,Sham-operated Group and Ischemia-Reperfusion Group (I/R). Sham-operated Group and I/R Group were divided into four subgroups (each group has five animals) : 6 hours group, lday group, 3days group, 7days group. Histological analysis of tissue injury was carried out by staining with haematoxylin and eosin haematoxylin and eosin (HE). Result The tissues of normal group and sham-operated group was integrated, but that of I/R were changed with gliocyte hyperplasia and hypertrophia, interstitial edema, cellular edema, gliocyte and neuron edema , neuron necrosis. The data presented here provided a positive correlation between expression of Hsp22 and VEGF. Conelusions There was a positive correlation between expression of Hsp22 and VEGF after gerbils brain I/R injury.
5.Pylorus-preserving gastrectomy for early gastric cancer

Objective To discuss the effect of pylorus-preserving gastrectomy for early gastric cancer(EGC). Methods Between August 1995 and December 2005, 52 cases of EGC underwent pyloruspreserving gastrectomy(PPG) and 159 cases of EGC underwent distal gastrectomy(DG), Clinicopathlogic data and follow-up results of the two groups were analyzed retrospectively, and gastric emptying and gallbladder function of 15 cases PPG and 17 cases DG were compared at the same time.Results Compared with DG group, patients in PPG group maintain the body weight, gastric emptying and gallbladder function. There was no significant difference between PPG group (92. 3% ) and DG group (93.1% ) in overall 5-year survival rate ( P = 0. 881 ). The 5-year survival rate of the the PPG group with lymph node dissection was D1 100%, D1+α 92. 3%, D1+β 88.9%, D2 87. 5% respectively.Conclusions For early gastric cancer, the pylorus-preserving gastrectomy is effective for maintaining the postoperative function with similar long term survival as that of distal gastrectomy.
6.Quantong Recipe integrated with ureteroscopic laser lithotripsy for ureteral calculi.

Background: Endoscopic surgery of Western medicine has developed rapidly in treating ureteral calculi while traditional Chinese medicine (TCM) therapy also has its advantage, thus the integrated traditional Chinese and Western medicine therapy may have an even better efficacy in dealing with this problem. Objective: To observe the clinical effects of sequential therapy of Quantong Recipe, a compound traditional Chinese herbal medicine, combined with ureteroscopic laser lithotripsy on ureteral calculi. Design, setting, participants and interventions: A total of 151 patients with ureteral calculi (stone diameter of 3 to 21 mm) from Department of Urology, Yueyang Hospital of Integrated Traditional Chinese and Western Medicine, Shanghai University of Traditional Chinese Medicine were included, of which 106 patients with stone diameter of 3 to 10 mm were treated with Quantong Recipe for the first two weeks. Twenty patients with unexpelled stones and 45 patients with stone diameter of more than 10 mm were treated with ureteroscopic laser lithotripsy. The 65 patients except one open operation were divided into regular treatment group (32 cases) and regular treatment combined with Quantong Recipe group (32 cases) after lithotripsy. The efficacy was assessed after one week. Main outcome measures: Cure rate in the patients with stone diameter of less than 10 mm after two-week Quantong Recipe treatment was calculated, and clearance rate in the patients with residual stone for ureteroscopic laser lithotripsy after one-week Quantong Recipe treatment was also observed. Results: After taking Quantong Recipe for two weeks, the stones in 86 patients were completely discharged, and the stones in 6 patients were not completely discharged, but the unexpelled stones had moved down more than one segment of ureteral stenosis; the total response rate was 86.79% (92/106). The 64 patients with stones unexpelled or with stone diameter of more than 10 mm were treated with ureteroscopic laser lithotripsy, and the clearance rate of residual stone was 93.8% in regular treatment combined with Quantong Recipe group, and was 71.9% in regular treatment group, and there was a significant difference between the two groups (P<0.05). Conclusion: Sequential therapy of Quantong Recipe integrated with ureteroscopic laser lithotripsy demonstrates a significant advantage of integrated traditional Chinese and Western medicine therapy in treating ureteral calculi.
8.Acute tibialis posterior tendon rupture: a literature review based on 24 cases

Objective To study the clinical features,treatment methods and clinical outcomes of acute tibialis posterior tendon rupture.Methods Using [(posterior tibial tendon) OR (tibialis posterior tendon)] AND (rupture OR injury OR disruption OR trauma OR tear) as the search words,we searched the PubMed database to retrieve the 19 articles involving 24 cases of acute tibialis posterior tendon rupture.The injury mechanisms,X-ray manifestations,operational findings,treatment methods and clinical outcomes were analyzed.Results The mechanisms of the fracture-related tibialis posterior tendon rupture included motor accident (50.0%,12/24),falling from a height (41.7%,10/24) and sports injury (8.3%,12/24).The tendon rupture was mostly complicated with malleolar fracture (91.7%,22/24),especially the pronation type (70.8%,17/24).Direct suture of the tendon was adopted in 91.7% of the cases (22/24).Favorable outcomes were achieved in 95.5% of the cases (21/24).Conclusions Acute tibialis posterior tendon rupture is mostly seen in high-energy medial malleolus fracture.The mechanism of the rupture might be tendon distraction or bone flake incision.Early operation can lead to favorable outcomes.
9.Treatment of Lithotripsy for Acute Renal Failure Resulting from Upper Urinary Calculi

Objective To study and evaluate the therapeutic safety and effect of frequency-doubled-double-pulse laser(FREDDY)lithotripsy for treating upper urinary calculi combined with acute renal failure.Methods The clinical data of 32 cases treated by frequency-doubled-double-pulse laser(FREDDY)lithotripsy were retrospectively analyzed.Results After the operation the serum BUN and Cr levels in the patients got close or returned to normal and the free rate of the stones was up to 90.6%(29/32).Conclusions The ureteroscopic frequency-doubled-double-pulse laser(FREDDY)lithotripsy has the advantage of safety,high efficiency and less trauma for treating the upper urinary tract obstruction combined with acute renal failure.It can also deal with the bilateral ureteral stones at one time.It can be the first choice when the condition is proper.
10.Clinical features and correlation between radiographic parameters and incidence of calcaneal spur

Objective To analyze the clinical features and the correlation between radiographic parameters and incidence of calcaneal spur in the patients from Nanfang Hospital,Southern Medical University,China.Methods Three experienced observers independently used the image acquisition and transmission system (PACS) to collect the data of lateral and axial X-ray images of calcaneus or ankle joint in neutral position from the patients with calcaneal spur and normal controls who had undergone radiological examination in Nanfang Hospital,Southern Medical University from July 2014 through December 2015.Ten radiological parameters of the foot (B(o)hler angle,Gissane angle,calcaneal inclination angle,talocalcaneal angle,talus horizontal angle,posterior facet inclination angle,calcaneal length,height of the posterior facet,absolute foot height,and calcaneal width) were measured in both the patients and normal controls.The location,morphology and length of calcaneal spurs were compared between genders,sides and age groups.Results A total of 216 parpatients were included in the study.Female patients were more than male ones,simple plantar spurs more than simple achilles tendon ones,type B spurs more than type A ones,the length of achilles tendon spurs larger than that of plantar ones,female plantar spurs more than males ones,and the length of right foot plantar spurs larger than that of left foot ones.All the differences above were statistically significant (P < 0.05).There were no significant differences in the location,morphology or length of calcaneal spurs between the age group of ≤ 60 years old and the age group of > 60 years old (P > 0.05).The incidence of calcaneal spur were significantly correlated to Gissane angle (P =0.000,OR =0.944,95% CI 0.917-0.973),posterior facet inclination angle (P=0.017,OR=0.957,95% CI 0.924-0.992) and height of the posterior facet (P =0.007,OR =0.933,95% CI O.886-0.981).Conclusions Calcaneal spur favored more females than males.Plantar spurs were more common than Achilles ones.Plantar spurs of Type B were more common than those of Type A.Achilles spurs were longer than plantar ones.More females suffered plantar spur than males.Right foot spurs were longer than left foot ones.Age had no significant influence on the spur characteristics.The incidence of spur might have been related to the Gissane angle,posterior facet inclination angle and height of the posterior facet of the foot.
