1.Effect of lacrimal duct laser with lacrimal drainage tubes and stents for complexity dacryagogatresia

AIM: To explore the effect of lacrimal duct laser with lacrimal drainage tubes and stents implantation for complexity dacryagogatresia.
METHODS: There were 65 patients ( 82 eyes ) with compound tears nasolacrimal duct obstruction who received lacrimal drainage tubes and stents implantation after laser. The lacrimal duct stents were removed through nasal cavity after 1mo. Lacrimal drainage tubes were removed after 3-6mo. Follow-up periods were 6mo to 1a.
RESULTS: In the 65 patients (82 eyes), 71 eyes were cured, 5 eyes improved, the efficient rate was 93%; there were 6 eyes (7%) with epiphora.
CONCLUSION: Lacrimal duct laser with lacrimal drainage tubes and stents implantation was efficient for complexity dacryagogatresia.
2.Effect of mild hypothermia on the brain edema of severe head injury

Objective To investigate the effect of mild hypothermia on brain edema after severe head injury (SHI) and its clinical significance. Methods Thirty-six patients with SHI were randomly divided into conventional therapy group (control group) and mild hypothermia therapeutic group (therapeutic group), with 18 patients in each group. The patients in the therapeutic group were treated with ice-cap, ice-blanket and ice-bag and lytic cocktail Ⅰas adjuvants, and a rectal temperature of 33 ℃ was maintained for 3~5 d. Intracranial pressure (ICP) of all the patients after operation were monitored postoperatively. The size of brain edema was measured with CT on day 0, 1, 3, 7, 14, 21 respectively after SHI. The size of brain edema at the trauma cite and ICP as well as the clinical outcome were compared and analysed. Results ICP of the control and therapeutic groups were(2.87±0.26)kPa and (1.67±0.23) kPa respectively on day 3 (P<0.01), and were (3.15±0.24)kPa and (1.78±0.24) kPa respectively on day 7 after brain injury (P<0.01). The area of brain edema was the largest on day 14 in the control group (140.90±22.95 cm3)and on day 3 in the therapeutic group (95.83±14.97 cm3). At day 14 posttrauma, the edema area in therapeutic group was 42% smaller than that in the control group (P<0.05). The rates of consciousness recovery within a week were 22.2% (4/18) in the control group and 55.6% (10/18) in the therapeutic group (P<0.05). The rates of unfavorable prognosis was 61.1% (11/18) in the control group and 33.3% (6/18) in the therapeutic group. Conclusion Mild hypothermia treatment can improve the prognosis through alleviating brain edema and stabilizing ICP after SHI.

5.Ultrastructure change of conjunctiva and fascia tissue of conjunctivochalasis

Background Bulbar conjunctiva tissue appears to be thinning,elasticity declined,tension reduced and fascia atrophied in conjunctivochalasis.Histopathological examination of conjunctivochalasis shows decrease of elastic fibers and melt of collagen fibers.But there are fewer studies on the ultrastructure of conjunctiva of conjunctivochalasis up to now.Objective This study was to investigate the ultrastructure change of conjunctiva tissue in conjunctivochalasis.Methods Five loose conjunctiva samples of conjunctivochalasis and 5 normal conjunctival tissue samples were collected and ultra-microstructure changes of these samples were observed under the transmission electron microscope.Results The number of fibroblasts in conjunctivochalasis lamina was progressively decreased.The shape of fibroblasts was long and fusiform.Somatic synapse was slim.Nucleus-cytoplasm ratio was increased.Disorder,scattered and broken of the collagen fibril were seen,and some areas were dissolved or lacunae.Normal conjunctival fibroblasts were oval or polygonal,with wieners and long somatic synapse,and intercellular matrix was full of collagen fibril and dense arranged fibers.Fibroblasts in fascia of eonjunctivochalasis were linear,and collagen fibril was seriously defected.Fascia fibroblasts of normal bulbar conjunctiva were spindleshaped and bigger than conjunctivochalasis fibroblasts.There were full of collagen fibrils in intercellular matrix.Conclusions The collagen fibril is decreased and fibroblast cells are degenerated in lamina and fascia of conjunctivochalasis.
7.Effect of excessive iodine intake on sodium-iodide symporter mRNA and protein expression of breast in lactating rats

Objective To study effect of excessive iodine intake on sodium-iodide symporter(NIS)mRNA and protein expression of breast in lactating rats.Methods60 Wistar rats,having been weaned for one month,were randomly divided into three groups according to their body weights,I.e,①normal iodine(NI,30 rats);②ten fold high iodine(10 HI,15 rats);③one hundred fold high iodine(100 HI,15 rats).Eating food containing iodine of 300μg/L and drinking water of iodine at 5,1845,20 295μg/L,respectively.After fed for 3 months,the rats mated and had offspring,and urine and milk iodine of lactating rats were determined by As-Ce-catalytic spectrophotometric method.Their marmnary glands were sampled at lactation day 10.Then NIS mRNA expression by RT-PCR was determined and NIS protein by immunohistochemistry(SABC)was observed.Results The urine iodine of 10 HI group(3597.5μg/L)and 100HI group(25 404.3μg/L)increased obviously compared with that of NI group(344.7μg/L).The milk iodine of 10HI group(27.1×103μg/L)and 100HI group(191.0×1μg/L)was higher than that of NI group(6.0×103μg/L),but the increased fold of milk iodine was not paralleled with that of urine iodine.Difference of NIS mRNA expression was significant(F=24.19,P<0.01)among the groups,and the NIS mRNA expression in 10HI(1.250±0.034)and 100HI(1.272±0.039)group were less than that in NI (1.532±0.044)group(P<0.01).The breast NIS mRNA expression in lactating rats(1.532±0.044)was significantly higher than that in unlactating rats(0.879±0.018,P<0.01).With the increasing iodine uptake,NIS protein expression decreased.Conclusions The NIS mRNA and protein in rat breasts is down-regulated by excessive iodine intake.So increasing extent of milk iodine concentration is inhibited,which is important to prevent off-spring from getting excessive iodine intake from parental generation.
8.Diffusion tensor imaging in evaluating the injury of corticospinal tract in patients with hypertensive basal ganglia hemorrhage

Objective To investigate the value of diffusion tensor imaging (DTI) in evaluating the injury of corticospinal tract (CST), including the motor dysfunction and the integrity of CST, in patients with hypertensive intracerebral hemorrhage (HICH), and explore the relation between DTI results and the recovery of myodynamia. Methods Clinical data of 10 healthy volunteers and 35 patients with HICH, admitted to our hospital and performed evacuation of hematoma from November 2006 to May 2009, were retrospectively analyzed; DTI by using a 3.0T MR canner were chosen on them and the imaging was analyzed by Functool software. The changes of directionality and integrity of SCT were analyzed on fractional anisorropic (FA) map, color-coded directional map, and three-dimensional white matter tractography. Myodynamic examination at Brunnstrom standard 2 months after rehabilitation nursing was performed in these patients. The correlation between the injured degree of CST and the recovery of myodynamia was analyzed. Results Fiber tractography derived from DTI was consistent with known white matter fiber anatomy in 10 healthy volunteers. Three patterns of corticospinal tract involvement were identified in 35 patients: the intact one in 11 patients (the ipsilateral corticospinal tract was almost the same as contralateral corticospinal tract), the partially damaged one in 18 patients (the ipsilateral corticospinal tract was less than 2/3 but larger than 1/3 contralateral corticospinal tract) and the almost completely damaged one in 6 patients (the ipsilateral corticospinal tract was less than 1/3 contralateral corticospinal tract). Different pattern had totally different clinical outcome and the intact one had a full recovery. The ipsilateral corticospinal tract showed significantly lower Brunnstrom scores of the upper extremities than the ipsilateral one (P<0.05); the Brunnstorm scores and the value of myodynamia among the 3 patterns were statistically different (P<0.05). The recovery of motor function was negatively correlated to the injured degree of CST (r=-0.931, P=0.000). Conclusion The DTI, offering the optimal visualization of white matter tracts can detect the injured degree of the corticospinal tracts and predicate the prognosis to some extent, thus, early evaluation of corticospinal tract injury based on DTI can provide predictive value for motor functional outcome in patients with HICH.

Methods Recombinant human aFGF (10mg/ml) was continuously injected into the intraventricle for 12 h before brain traumatic injury in rats, and the brain water content was determined and the histological and ultrastructural changes in the brain tissue observed. Results Brain edema and neuronal damage were reduced remarkably by the administration of aFGF. Conclusion aFGF can obviously alleviate brain edema and neuronal damage.
