Objective To evaluate the surgical technique and clinical application of retroperitoneal laparoscopic pyelolithotomy and ureterolithotomy with no suture of the incision. Methods Twenty-five patients who had upper urinary tract calculi with the course from 10 months to 6 years were enrolled. The size of the calculi was 1.2 to 3.1 cm in diameter.ESWL was ineffecative in 14 of the patients and ureteroscopic lithotomy failed in 2 of them. Then the 25 patients underwent retroperitoneal laparoscopic pyelolithotomy and ureterolithotomy.Details of the procedure were presented. Results All the operations were successful in the 25 patients. The operations took 60 to 190 min with a mean time of 105 min.All the patients fully recovered with no complication observed during the 9~12 months' follow-up. Conclusions Retroperitoneal laparoscopy surgery with no suture of the incision has advantages of minimal invasive, safe, effective and rapid postoperative rehabilitation. The surgical procedure has a good prospect in the urologic clinical practice.

Chinese Journal of Contemporary Pediatrics 2015;17(7):726-730
OBJECTIVETo investigate the impacts of biological factors (age and sex) and family factors (socioeconomic status and parenting style) on the early lexical and intellectual development of children in a longitudinal tracking study.
METHODSA total of 38 Mandarin-speaking children aged from 18 to 24 months were surveyed using the Putonghua Chinese Communicative Development Inventory (PCDI), the Ages and Stages Questionnaire (ASQ), and a self-designed Questionnaire for Parents. All of the subjects were retested using PCDI and ASQ after 6 months.
RESULTSBiological factors accounted for 65% of the variance in lexical development, 10% of which was attributed to gender, in the first survey. After six months, the contribution of age decreased to 26% and gender had no significant impact. Lexical development could positively predict the intellectual development of children. When age and gender were controlled, it accounted for 22% of the variance in intellectual development. Family socioeconomic factors had no significant impacts on lexical and intellectual development. Children's recognition of people and objects around them with guidance of parents in parenting styles could positively predict the intellectual development of children six months later, which accounted for 10% of the variance.
CONCLUSIONSBiological factors play an important role in the early lexical development of children. However, the influence decreases with the increase of age (months). Biological factors, lexical development, and parenting style have a combined influence on children's intellectual development.

Objective To investigate the effects of dynamic standing bed training on the functional recovery of hemiplegic patients. Methods Sixty hemiplegic patients with Barthel index score of 40 were randomized into a standing rehabilitation group and a control group. The patients in standing rehabilitation group were asked to take part in dynamic standing bed training in addition to drug treatment. Those in the control group were treated with medication only. After 1 to 2 weeks of treatment, the weight-bearing ability of the lower limbs, balance, muscle tone, trunk control and the cardiovascular response were evaluated and compared with those before the treatment. Results The scores of weight-bearing, balance, muscle tone and trunk control of the patients in the standing rehabilitation group were better improved than those in the control group. The changes of blood pressure and pulse of patients were reduced and within the range of safety after 2 weeks of training. Conclusion Dynamic standing bed training is beneficial for the functional recovery of hemiplegic patients. It is safe for the patients in terms of cardiovascular response.

AIM:To establish a rat hyperlipidemia model for studying the aortic expression of heat shock protein 22 (HSP22), tumor necrosis factor alpha (TNF-α) and endothelial nitric oxide synthase (eNOS) and the effect of atorvasta-tin intervention.METHODS:Hyperlipidemia model was established in SD rats.Afterwards, the rats were divided into nor-mal control group, high fat group and high fat+atorvastatin intervention group. The expression of HSP22 and TNF-αin the rat aortas was detected by immunohistochemical assay and the expression of eNOS was assessed by Western blotting.RE-SULTS:No detectable expression of HSP22 and TNF-αin the normal control group was observed. However, the expression of HSP22 and TNF-αwas positive in the high fat group and the atorvastatin intervention group. The mean densities of HSP22 and TNF-αpositive particles were significant lower in the atorvastatin intervention group as compared with high fat group ( both P<0.05) .The expression of eNOS protein in the high fat group and atorvastatin intervention group was significantly lower than that in normal control group (P<0.01).However, no marked difference of eNOS protein expression between high fat group and atorvastatins intervention group was observed.CONCLUSION:The expression of HSP22 and TNF-αin the rat aortas is increased in the hyperlipidemia rat model. This effect can be restored by atorvastatin treatment. The expression of eNOS in the rat aortas is decreased in the hyperlipidemia rat model, but this tendency could not be attenuated by atorvastatin.

Objective To evaluate the results of prosthesis augmentation, relieve suffering patient experience, improve shape and touch of breast. Methods According to design,in dual-plane mammaplasty transaxillary approach with endoscopic assistance,we used soft-tissue expansion before prosthesis, saline volume in expander was the same as prosthesis size at beginning; adjusting expander volume and position, then determined prosthesis size according to expander volume lastly.after intraoperative sustained expansion for 30 to 60 minutes twice prosthes volume, replace expander with prosthesis. Results This method was applied in 120 patients, showing that pain significantly reduced after expansive without use of analgesia pump; complications included that 3 patients appeared seroma and absorbed soon; 2 patients had poor prosthese location, which was adjusted in reoperation; one capsular contracture was repaired later. After 6-months to 2-years follow-up,the results showed that breast was nature and dynamic; satisfactory rate reached to 95 %.Conclusions In augmentation mammaplastic operation,pre-adoption of an expander can make it easy to adjust the location of prosthesis,to avoid being injured and to determine prosthesis volume by expander injection of saline volume, especially for patient with different size of both breasts.

Objective To explore the clinical features and treatment of children with acute severe viral myocarditis. Methods The clinical data of presentation, diagnosis, therapy and prognosis of children who were admitted in our hospital from Jan 2005 to Jan 2012 with acute severe viral myocarditis(severe myocarditis group) were analyzed retrospectively.Twenty-three cases of normal healthy children in the same period were selected as control group. The levels of serum cardiac troponin(CTn)-Ⅰ and N-terminal pro-brain natriuretic peptide(NT-proBNP) were detected by ELISA method, the changes of left ventricular ejection fraction and left ventricular fraction shortening were understood by color doppler echocardiography. Results The level of CTn-Ⅰin severe myocarditis group was significantly higher than that of control group, the difference was statistically significant [(18.67 ± 12.31) ng/ml vs (0.02 ±0.01) ng/ml,P <0.05].Compared with the acute phase, the level of CTn-Ⅰshowed a trend of gradual decline in 7 d [(0.55 ±0.24) ng/ml],basic close to normal in 14 d [(0.06 ±0.03) ng/ml] (P <0.05).The level of NT-proBNP increased significantly in severe myocarditis group compared with control group [(3 067.26 ± 902.79) pg/ml vs (80.04 ± 17.79) pg/ml,P <0.05].Compared with acute phase, the levels of NT-proBNP were closed to normal in 7 d [(648.63 ±342.37) pg/ml] and 14 d [(213.58 ± 129.51) pg/ml] (P < 0.05).The left ventricular ejection fraction [(52.63 ± 6.98) % vs (71.39 ± 2.41) %] and left ventricular fraction shortening [(32.1 ± 2.97) % vs (40.04 ± 2.31) %] in severe myocarditis group were significantly lower than those in control group (P < 0.05).Conclusion Acute severe viral myocarditis of children was characterized by rapid onset, severe illness and high mortality.Early use of adrenal cortical hormone and gamma globulin under the comprehensive treatment and application temporary pacemaker can help patients to recover from the disease.
