1. Expression of S100A4 protein in hepatocellular carcinoma and its clinical implications

Objective: To explore the expressions of S100A4 protein in hepatocellular carcinoma (HCC), cirrhotic liver, adjacent non-cancerous liver and normal liver tissues and their clinical implications. Methods: Tissue microarray combined with immunohistochemistry (IHC) was used to evaluate the differential expressions of S100A4 protein in HCC-related tissues, and their clinical implications were statistically analyzed. Results: Immunohistochemical staining showed that the positive staining of S100A4 protein was higher in the specimens of primary HCC (70.2%), liver cirrhosis (54.4%), and adjacent non-cancerous liver tissues (41.7%) compared with that in normal liver tissues (0.0%). Biological statistic analysis revealed that S100A4 protein expression influenced the survival time of HCC patients. The patients with weak expression of S100A4 had longer survival time than those with over-expression of S100A4 protein (P = 0.040). Conclusion: The order of the expression levels of S100A4 protein in HCC-related tissues is: HCC>adjacent non-cancerous liver > cirrhotic liver > normal liver. In HCC patients without hepatic microvascular invasion, positive expression of S100A4 protein significantly decreases their postoperative survival rate.
2. Expression, subcellular localization and clinical implications of TC 21/R-Ras 2 in hepatocellular carcinoma

0bjective: To explore the expression, subcellular localization and clinical implications of TC 21/R-Ras 2 gene in hepatocellular carcinoma. Methods: TC21 mRNA transcription was detected by semi-quantitative Reverse Transcription Polymerase Chain Reaction (RT-PCR) in HCC and adjacent liver tissues. Immunohistochemistry (lHC) and Western blot were used to measure the differential expression of TC21 protein in HCC related tissues, and subcellular localization of TC21 was determined with tissue microarray combined with IHC staining. Results: RT-PCR showed that higher TC21 mRNA expression was occurred in 7 HCC cell lines such as SMMC-7721, BEL-7402, Huh-7, Hep3 B, HepG2. MHCC-97L, MHCC-LM3, and immortal human liver cell lines L-02 and Chang's liver, also 7 HCC tissues and matched noncancerous livers. Western blot indicated an accordant trend of TC21 mRNA and protein expression in 7 HCC cell lines above. TC21 overexpression was detected in 4 HCC and their adjacent liver tissues by Western blot. Furthermore, immunohistochemical staining showed that TC21 positive staining trends: primary HCC > matched noncancerous liver > cirrhotic liver > normal liver, in which the positive staining rates were 84. 2%, 77. 2%, 41. 7% 0% , respectively. Compared to cirrhotic liver and normal liver tissues, TC21 was significantly over-expressed (P < 0.05, P < 0.01). TC21 expression in HCC tissues was appeared higher than that in noncancerous livers, but no significance was found (P > 0.05). Biological statistic analysis revealed that TC21 over-expression in HCC positively correlated to the tumor sizes (P < 0.05) but negatively correlated to cirrhosis or not (P < 0.05). Beside above-mentioned findings, it was found that TC21 protein mainly localized to the nuclei of HCC cells, parts to cytoplasms, but not to plasma membranes. Nuclear staining of TC21 was higher in HCC tissues than that in their adjacent livers (P < 0.001). Conclusion: Present study firstly shows that up-regulation and nuclear localization of TC21 protein may play important roles in hepatocellular malignant transformation and hepatocarcinogenesis, although its mechanism is to be elucidated.

The culture medium and cultural conditions of solid-state fermentation of Streptomyces Menmyco-93-63 were tested in this study. The suitable medium which contains rice, sorghum, millet bran, and rice hull with the proportion of 2:2:3:3 was developed for the spore production of Streptomyces Men-myco-93-63 using single substrate screening, mixture substrate screening and orthogonal experiments, and the sporulation was up to 2.52?109 CFU/g. And then, initial charge, initial ratio of water to solid, inoculating quantity, and culture temperature impact to sporulation of Streptomyces Men-myco-93-63 were tested. The favorite cultural conditions are developed as the following: the initial charge is 15 g in 500 mL Erlenmeyer flask; initial ratio of water to solid is 1.7:1.0 (V/W, rice hull excluding), inoculating quantity is 7 mL, culture temperature is 28℃.

OBJECTIVETo study clinical effects of unilateral pedicle screw-rod system fixation combined with the single the cage of interbody fusion in the treatment of lumbar degenerative instability via paraspinal intermuscular approach under MAST Quadrant retractor.
METHODSFrom February 2010 to December 2011, 39 patients with lumbar degenerative instability after invalid conservative treatment were treated with unilateral fixation combined with interbody fusion via the paraspinal intermuscular approach under MAST Quadrant retractor. The indexes such as the operative time, blood loss, complications, VAS pain scores, JOA (subjective symptoms of low back pain,lower limb pain and numbness, sensory disturbance, movement disorders) scores, modified Macnab criteria for curative effect evaluation were observed before treatment, at the 1st month after treatment and the latest follow-up. The intervertebral fusion was studied on the X-ray at the latest follow-up.
RESULTSAll the patients were followed up, and the mean during was (22.3 +/- 8.6) months. The operation time was (138 +/- 46) min, and the amount of bleeding was (335 +/- 152) ml. There were no complications such as cerebrospinal fluid leakage and spinal nerve injury during operation, and no incision infection after operation. The VAS pain score was reduced from preoperative 7.93 +/- 1.27 to 2.05 +/- 1.18 on the 1st month after operation and 1.89 +/- 0.42 at the latest follow-up. The JOA total score was improved from preoperative 1.59 +/- 0.42 to 8.86 +/- 0.37 on the 1st month and 9.02 +/- 0.29 at the latest follow-up. According to modified Macnab criteria, there were 17 cases got an excellent result, 19 good, 3 case fair. Thirty-five patients got intervertebral bony fusion at the latest follow-up.
CONCLUSIONUnilateral pedicle screw-rod system fixation combined with single cage interbody fusion in the treatment of lumbar degenerative instability via paraspinal intermuscular approach under MAST Quadrant retractor is a safe, minimally invasive, satisfactorily effective methods to treat lumbar degenerative instability.

6.Systematic evaluation on direct economic burden of healthcare-associated infection due to multidrug-resistant organisms

Objective To understand the direct economic burden of healthcare-associated infection(HAI)due to multidrug-resistant organisms(MDROs).Methods Computer retrieval of CNKI,Wanfang,VIP,PubMed,Sci-enceDirect,and Cochrane databases on literatures about economic burden of MDRO HAI at home and abroad were performed,the retrieval time was from database establishment to December 2015,systematic evaluation of the liter-atures was obtained.Results According to the inclusion and exclusion criteria,as well as through Newcastle-Otta-wa Scale (NOS)for evaluating the literatures,19 literatures were included.In 12 studies about methicillin-resistant Staphylococcusaureus infection,the direct economic cost varied from $916.61 to $62908.00;in 4 studies about MDRO Acinetobacterbaumannii infection,the direct economic cost varied from$4644.00 to $98575.00.Direct economic cost due to extended-spectrumβ-lactamases-producing Enterobacteriaceae was $2824.14-$30093.00. Conclusion MDRO HAI will increase economic cost of both hospitals and patients,prevention and control measures should be taken .

AIM:To investigate the expression of transmembrane protein 16A(TMEM16A) in Fischer rat thy-roid follicular epithelial ( FRT) cells and its electrophysiologic properties .METHODS: The eukaryotic expression vector of pUB6/V5-TMEM16A was constructed and transfected into FRT cells by liposome-mediated transfection .In order to ob-tain the high efficiency of gene transfection and expression , the quantity and ratio of lipid/DNA complexes were optimized . The FRT cells stably expressing TMEM16A were gained by the selection with blasticidin and confirmed by the techniques of RT-PCR and immunofluorescence .The expression and location of TMEM 16A in the FRT cells were observed under an in-verted fluorescence microscope .TMEM16A protein was associated with calcium-dependent chloride current , as measured with halide-sensitive fluorescent protein and patch-clamp technique .RESULTS: The results of double digestion and se-quencing indicated that TMEM16A was cloned into pUB6/V5.The results of RT-PCR and immunofluorescence confirmed that TMEM16A was expressed in the FRT cells after transfection with TMEM16A.The classical calcium-activated chloride channel currents were recorded in the FRT cells stably expressing TMEM 16A by the technique of patch-clamp and halide-sensitive fluorescent protein YFP-H148Q/I152L.CONCLUSION:The protein expression of TMEM16A in the FRT cells was observed.TMEM16A is the molecular identity of calcium-activated chloride channels .

Objective To investigate the daily total fluoride intake in relating to the prevalence of skeletal fluorosis in two villages in Jiangsu Province,in order to provide the scientific evidences for the control and prevention of endemic fluorosis.Methods Adults sampled from a high-fluoride Village,Wamiao,and a low-fluoride Village,Xinhuai,were surveyed in this study according to the fluoride concentration in their household shallow well.The average daily total fluoride intake from difierent sources and the skeletal fluorosis were investigated in each subject.Then the subjects from two villages were allocated into five subgroups(<2.00,2.00~,3.00~,4.00~,≥5.00 mg/d),the relation fluoride intake and prevalence of osteofluorosis was analyzed.Results The prevalence of skeletal fluorosis in Wamiao Village was 31.06%(41/132),but no skeletal fluorosis case(0/35)was found in Xinhuai Village.According to the daily total fluoride intake,subjects with higher daily total fluoride intake tended to associated with a higher prevalence of skeletal fluorosis in a significant dose-response relationship(regression equation:y=2.624-6.855x+3.424x2:r=0.997).The benchmark dose lower limitation of daily total fluoride intake with 95% confidencewas 2.50 mg/d calculated according to this dose-response relationship,the reference dose(RfD)was 2.50 mg/d.In Wamiao Village a significant difference was also found between daily total fluoride intake in 41 subjects[(5.09±1.20)mg/d]with X-ray detectable skeletal fluorosis and in 91 subjects[(3.08±1.12)mg/d]without X-ray detectable skeletal fluorosis(t=-9.32,P<0.01).Conclusions These findings indicate that the daily total fluoride intake has a significant dose-response relationship with the prevalence of skeletal fluorosis in an endemic fluorosis area associated with high-fluoride drinking water;and the RfD in this study was lower than that in the national standard of"Chinese hygienic standard for daily total fluoride intake(WS/T 87-1996)"(3.50 mg/d).
