1.Application of transanus ilues tube in the case of left-semicolon cancer with intestinal obstruction

Objective To investigate the case of left-semicolon cancer with intestinal obstruction for the methods of one-stage resection and anastomosis.Methods The clinical data of ten patients with left-semicolon can- cer with intestinal obstruction treated by transanus ilues tube,were restrospectively analyzed from October 2004 to December 2006.Results No postoperative anastormotic leakage was found and the patients were clinically cured. Conclusion The technical problem that left-semicolon cancer with obstruction for one-stage resection and anastomo- sis was resolved by the application of transanus ilues tube.
2.The relationship between arsenic in drinking water and skin lesions in endemic arsenism area in Shanyin County of Shanxi Province

Objective To investigate the relationship between arsenic in drinking water and skin lesions in endemic arsenism area in Shanyin County of Shanxi Province,in order to provide epidemiologic data for further arsenism research.Methods One hundred and eighty-nine endemic arsenism patients and 59 controls were randomly selected in 17 endemic amenism countries in Shanyin County of Shanxi Province.The content of arsenic in drinking water which wa8 collected indoom was half-quantitatively screened by a kit made by Chinese Center for Disease Control and Prevention,then quantitatively determined by HPLC-ICP-MS.Patients of endemic arsenism were diagnosed by "The Standard of Diagnosis for Endemic Amenism"(WS/T 211-2001).Results There were 64.9% (87/134)samples above the arsenic level(50μg/L)of drinking water and the median value of arsenic in drinking water was 91.43 μg/L in 134 water samples.The OR(95%CI)value between arsenic in drinking water and hyperkeratosis,hyperpigmentation,depigmentation was 2.46(1.22-4.94),3.34(1.50~7.44)and 2.86(1.50-5.46),respectively.The prevalence of hyperkeratosis,hyperpigmentation and depigmentation increased,as the arsenic in drinking water increased(≤10,≤50,≤200,>200μg/L),especially in>200μg/L group(OR=6.15,13.96,11.41,P<0.05).The arsenic level in drinking water of Ⅲ degree of depigmentation patients(318.300μg/L)was higher(P<0.05)than that of 0,Ⅰ and Ⅱ degree groups(86.670,131.800,1 10.590μg/L,P<0.05).Conclusions Shanyin County is a medial arsenic pollution area. Arsenic in drinking water is considered as a risk factor of skin lesion. The degree of skin lesions increased,as the arsenic in drinking water increased.
3.Application of ring-like double tubes with negative pressure drainage after radical surgery of breast cancer

4.Antimicrobial resistanee and resistant genes of Pseudomonas aeruginosa

Objective To investigate the trend of antimicrobial resistance and the prevalence of resistant genes in Pseudomonas aeruginosa strains isolated from Hangzhou First People's Hospital.Methods Antimicrobial susceptibilities of 1489 Pseudomonas aeruginosa strains isolated from 2003 to 2007 were statistically analyzed using WHONET.MICs of 11 antimicrobisis to 36 multi-drug-resistant Pseudomonas aerugionosa strains were determined by agar dilution method.Genes of β-lactamases(BLA)and aminoglycoside-modifying enzymes(AMEs)were detected by PCR and the PCR products were sequenced.Results The resistant rates to aztreonam,imipenem,ceftazidime,cefepime,piperacillin,piperacillin/tazobactam.cefoperazone/sulbactam,ciprofloxacin,levofloxacin,gentamicin and amikacin were increased from 13.4%,10.6%,8.7%,7.9%,12.7%,12.7%,6.7%, 15.8%,20.5%,24.7% and 10.9%in 2003 to 35.3%,40.9%,18.4%, 32.4%,32.9%,32.0%,21.9%,37.8%,38.6%, 39.4% and 34.8% in 2007.respectively.Hish MICs of 11 antimicrobiMs for multi-drug resistant Pseudomonas aerouginosa were determined with MIC90≥128 μg/mL.In 36 multi-drug resistant Pseudomonas aeruginosa strains,21(58.3%)strains carried β-lactamase genes and 32 strains(88.9%)carried aminoglycosidemodifying enzyme genes,while the deletion rate of oprD2 was 80.6%(29/36).Conclusions The resistant rates to common antibiotics of Pseudomonas aeruginosa have increased.resulting in multi-drug resistance.Genes of β-lactamases and aminoglycoside-medifying enzymes are prevalent in multi-drug resistant Pseudomonas aeruginosa strains,with the common deletion of oprD2.
5.Long-term Prognostic Analysis of Re-operation in Patients With Functional Tricuspid Regurgitation After Left-sided Valve Replacement

Objective: To analyze the long-term prognosis of re-operation in patients with functional tricuspid regurgitation (FTR) after left sided valve replacement (LSVR) and hence evaluate the optimal timing of mentioned re-operation. Methods: A total of 59 FTR patients who had re-operation after their prior LSVR in our hospital from 1999-01 to 2013-01 were analyzed. The clinical information and post-operative follow-up results were recorded in all patients. Results: There were 5/59 (8.5%) patients died in peri-operative period and the overall post-operative mortality was 11.9% (7/59). The follow-up data of 54 survivors were available for the mean time of 51.1 (21-188) months. There were 19/54 (35.2%) patients suffered from MACE and 30 (55.6%) were beneifted by improved cardiac function. Uni-variable analysis indicated that pre-operative NYHA class IV (P=0.008), pre-operative right ventricular (RV) dysfunction (P=0.037), concomitant left-sided redo-operation (P=0.017) and TVR operation (P=0.002) were associated with all cause mortality of tricuspid re-operation. Multi-variable Cox regression analysis showed that pre-operative RV dysfunction was the only independent risk factor of long term MACE-free accumulating survival rate (HR=3.0, 95% CI 1.11-8.2,P=0.031); while TVR operation (HR=12.8, 95% CI 1.53-107.02,P=0.019) and pre-operative NYHA class IV (HR=5.3, 95% CI 1.20-24.51,P=0.032) were the independent risk factors for long-term mortality in patients after tricuspid re-operation. Conclusion: Patients with compensatory RV function showed better long term prognosis after secondary tricuspid operation. Aggressive re-operation before the occurrence of right ventricular dysfunction could be beneficial for relevant patients.
6.Micro-pore structure and mechanical properties of porous titanium scaffold using 3D print technology

Objective To study the micro-pore architecture and mechanical properties of porous titanium scaffolds with diamond molecule structure produced by 3D print technology, so as to guide the development of 3D-prinited porous titanium orthopedic implants. Methods Selective laser melting (SLM) and electron beam melting (EBM) were used to fabricate porous Ti6Al4V scaffolds with diamond molecule structure. The micro-pore architectures of those scaffolds were observed using optical microscope and scanning electron microscope (SEM), and universal material testing machine was used to conduct compressive test on the scaffolds. Results Both SLM and EBM techniques had machining error and half-melted metal particles were found on the strut surface. The relative error of strut size produced by SLM and EMB was 20.9%-35.8% and -9.1%-46.8%, respectively. The scaffold with strut width of 0.2 mm could not be produced by EBM. The compressive strength and elastic modulus of the scaffold fabricated by SLM was 99.7-192.6 MPa and 2.43-4.23 GPa, respectively. The compressive strength and elastic modulus of the scaffold fabricated by SLM was 39.5-96.9 MPa and 1.44-2.83 GPa, respectively. Conclusions The manufacturing precision of SLM is higher than that of EBM. Porosity is the main factor that affects the compressive strength and elastic modulus of the scaffolds. In the same process, with the increase of porosity, both the compressive strength and elastic modulus decrease. When the porosities are similar, the scaffolds fabricated by SLM possess higher compressive strength and elastic modulus than those by SLM.
7.Research on TALF expression in Escherichia coli.

The expression of cDNA encoding Tachyleus auti-lipoposaccharide (LPS) factor, which is of interest for use as a potential inhibitor of the common core subunit of Gram-negative bacterial endotoxin. First, the TALF gene was inserted into expression vectors pGEX-4T-2, pET22b and pET28a to construct recombinant expression plasmids. The recombinant plasmids were transformed to E. coli BL21 (DE3) and the expression of TALF was examined. Results show that TALF in pET22b and pET28a vectors can't be expressed. Only the fusion protein GST-TALF was expressed in E. coli BL21 existing as inclusion bodies. From 1 liter of culture, about 4mg of fusion protein GST-TALF with 91% purity was finally obtained. No apparent bactericidal activity and LPS neutralizing activity of the fusion protein GST-TALF were found. After digested with thrombin, the fusion protein GST-TALF exhibited strong bactericidal activity and LPS neutralizing activity.

8.Effect of Pulmonary Rehabilitation on Pulmonary Function and Activities of Daily Living in Patients with Chronic Schizophrenia Complicated with Pulmonary Infection at Stable Stage

Objective To explore the effect of pulmonary rehabilitation on activities of daily living(ADL)and pulmonary function in patients with chronic schizophrenia complicated with pulmonary infection at stable stage. Methods From January,2014 to July,2017,62 patients with chronic schizophrenia complicated with pulmonary infec-tion at stable stage were randomly divided into control group(n=31)and observation group(n=31).Both groups accepted routine medication,while the observation group received pulmonary rehabilitation for two months in ad-dition.The distance of 6-Minute Walk Test(6MWT),the modified Barthel index(MBI),modified British Medi-cal Research Council Dyspnea Scale (mMRC), C-reactive protein (CRP), Zung Anxiety Self-Rating Scale and Zung Depression Self-Rating Scale were compared before and after training. Results The total effective rate was higher in the observation group than in the control group(χ2=5.415,P=0.020).Com-pared with the control group,the distance of 6MWT increased(t=5.463,P=0.008),the score of MBI increased(t=2.142, P=0.028), the score of mMRC decreased (t=3.689, P=0.013), and the level of CRP decreased (t=-2.179,P=0.031)in the observation group.The scores of Zung Anxiety Self-Rating Scale(t=5.048,P=0.009) and Zung Depression Self-Rating Scale(t=-1.388,P=0.045)both decreased. Conclusion An early implementation of pulmonary rehabilitation could improve ADL and the exercise endurance,and reduce the incidence of infection in patients with chronic schizophrenia complicated with pulmonary infection at stable stage.
9.Effects of purinergic analogues on spontaneous contraction and electrical activities of gastric antral circular muscle in guinea-pig.

In order to investigate the role of non-adrenergic non-cholinergic nerves in regulating mechanical and electrical activity of gastric circular smooth muscle, the effects of ATP and its analogues on gastric motility and electrical activities were observed in guinea-pig. In organ bath system, isometric force of the circular smooth muscle of guinea-pig gastric antrum was measured. Electrical activity of the muscle was recorded by using intracellular microelectrode. Electrical and mechanical activities were recorded by chart recorder. ATP and 2-MeSATP potentiated the mechanical activity but did not affect electrical activity in gastric circular smooth muscle. ATP and 2-MeSATP-induced contraction was effectively blocked by nonselective P2y-purinoceptor antagonist, reactive-blue-2 and suramin, but ATP-induced contraction was not blocked by alpha,beta-MeATP-induced desensitization of P2x-purinoceptors. However, alpha,beta-MeATP, P2x-purinoceptor agonist, attenuated slow waves with membrane hyperpolarization and inhibited contraction. The relaxation by beta,gamma-MeATP was blocked by alpha,beta-MeATP-induced desensitization of P2x-purinoceptors. ATP-induced contraction was blocked by external calcium-free, but not by nicardipine, a L-type calcium channel blocker. Indomethacin, a nonselective cyclooxygenase inhibitor, did not block ATP-induced contraction. The results suggest that: (1) ATP- and analogues-induced contraction is mediated by P2y-purinoceptor, whereas alpha,beta-MeATP-induced relaxation by P2x-purinoceptor in guinea-pig gastric antral circular smooth muscle. (2) ATP-induced contraction is dependent on extracellular calcium, but Ca2+ entry is not mediated by L-type calcium channel. (3) Prostaglandins are not involved in ATP- and analogue-induced contraction of gastric circular smooth muscle in guinea-pigs, and alpha,beta-MeATP-induced relaxation is related to membrane hyperpolarization.

10.First 30 endobronchial ultrasound-guided transbronchial needle aspirations: a single institution's early experience.

BACKGROUNDA new technique developed in 2002, real time endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA), has been one of the most important tools in lymph nodes (LNs) staging before lung cancer surgery. EBUS-TBNA was introduced into China in 2008.
METHODSBetween June 2009 and October 2009, 30 patients with mediastinal/hilar lymphadenopathy and thoracic masses previously detected with CT scan underwent EBUS-TBNA without rapid onsite cytological examination.
RESULTSFrom 30 patients, 33 samples were obtained from LNs and seven samples from intrapulmonary lesions. Twenty out of the 23 lung cancer diagnoses were clarified through the procedure, with sensitivity, specificity, positive predictive value, negative predictive value and accuracy being 87%, 100%, 100%, 70% and 90%, respectively. All three false negative cases were found in the first five procedures. Additionally, among the 33 LNs examined, three specimens that had no lymphocytes were also found within the first five procedures. There were no major complications, and the procedures were uneventful.
CONCLUSIONSEBUS-TBNA seems a safe and effective technique in making diagnosis for mediastinal/hilar LNs and intrapulmonary masses. For pulmonologists experienced in bronchoscopy, the sensitivity of the procedure for diagnosing lung cancer should be no less than 90% after the initial five procedures.
