1.Treatment of fresh subtrochanteric fracture combined with old femoral neck fracture with hemiarthoplasty through anterolateral approach.

OBJECTIVETo explore the clinical effects of one-stage hemiarthroplasty in treating fresh subtrochanteric fracture combined with old femoral neck fracture.
METHODSFrom October 2003 to October 2013, 7 patients with fresh subtrochanteric fracture and old femoral neck fracture were treated with hemiarthroplasty in one stage. There were 5 males and 2 females, aged from 69 to 80 years old with an average of 74.5 years. According to the Garden classification, 2 cases were type IV and 5 cases were type V. According to the A0 classification of subtrochanteric fracture, all cases were type A1 (long oblique femoral subtrochanteric fractures). Biological coating long handle femoral prosthesis was used in the patients. The hip joint functions were assessed according to Charnley scoring system after all hemiarthroplasty.
RESULTSSeven patients were followed up from 12 to 24 months with an average of 18 months. There were 1 case with urinary system infection and 1 case with deep venous thrombosis. Of all, 5 cases recovered the walk ability at 6 months after operation and 2 cases could walk with the help of the crutch at 1 year after operation. According to Charnley scoring system to assess the hip joint function, the total Charnley scores reached 15.0±2.5, and 4 cases got an excellent results, 2 good, 1 fair.
CONCLUSIONArthroplasty through anterolateral approach can obtain satisfactory clinical effects in treating fresh subtrochanteric fracture combined with old femoral neck fracture, it has advantages of good joint stability, little pain, less complication, quickly recover, early activities on fields.

2.Advances in expansion property of stent in vessels with different shapes

The implantation of an intravascular stent has become a widely used minimally invasive treatment for coronary heart diseases due to its minimal invasiveness and high efficiency.But the long-term outcomes are often compromised by in-stent restenosis (ISR),which severely restricts its practical effectiveness.In this paper,the formation mechanism of ISR was discussed,and the effects of mechanical environment on ISR were analyzed from the view of solid mechanics.The research progress of stent expansion in vessels with 4 different shapes,including straight vessel,tapered vessel,bifurcation vessel and curved vessel was reviewed.Moreover,the correlations between vessel stress and vessel shape change caused by stent expansion and ISR were reviewed.Finally,based on the influence of different vessel shapes on stent expansion,a multi-objective optimization design method was proposed to improve the stent expansion performance and reduce the occurrence of ISR.

4.The immunostimulatory study on peripheral blood mononuclearcell and CD4+ T Lymphocyte of HBV infections that were activated by CpG oligonucleotide

Objective To study the effects of CpG oligodeoxyribonuclentide (ODN)as adjuvant on the immune responses in PBMC and CD4+ T cell with chronic hepatitis B virus. Methods The selected 20 infections were averagely divided two groups. The frequency of IFN-γ secreting PBMC and CD4+ T cell in immune tolerant phase and in the immune clearance phase that had stimulated by CpG ODN, HBsAg and Mixture[CpG ODN+HBsAg] were analyzed by enzyme linked immune spot(ELISOT). Results The PBMC and CD4+ T cell were differently incubated by CpG ODN+ HBsAg and M[CpG ODN + HBsAg]. The number of IFN-γ spot differently are 3±8, 339±429, 375±496, 1±4, 5±16 and 5±12;the results of immume tolerance are 3±8,361±153, 375±276, 0±2, 2±2 and 4±4; but the results of immune clearance are 3±21, 289±345, 405±656, 2±14, 8±40 and 7±30. The IFN-γ spots statistical analysis of PBMC were differently incubated by HBsAg and M,the total is P = 0.720, The IFN-γ spots statistical analysis of CD4+ T cell were differently incubated by HBsAg and M, the total is P = 0. 890, The IFN-γ spots statistical analysis of PBMC and CD4+ T cell were differently incubated by M, the total is P=0.000. Conclusions The ability that CpG ODN can not significantly increase the IFN-γsecreting of PBMC and CD4+ T cell that were incubated by HBsAg to the infection in immune tolerant phase and in the immune clearance phase, but the PBMC outweighed The CD4+ T cell.
5.Management of medical equipment maintenance

Objective To explore the management methods of medical equipment maintenance. Methods Based on the staffing of hospital engineering technician, the difference between levels of hospitals in medical equipment maintenance management was explored from the aspects of maintenance, preventive maintenance, use training of medical equipment, medical equipment maintenance management system and etc. Results The management methods of medical equipment maintenance were different corresponding to the configurations of the departments of clinical engineering. Conclusion The maintenance management method corresponding to the staffing of hospital engineering technician as well as the correct use of modern medical equipment contribute to insuring the safety and reliability and enhancing the availability and utilization rate.

BACKGROUNDGlucolipotoxicity might play an important role in the β cell decompensation stage during the development of obesity-associated type 2 diabetes. Tissue inhibitor of metalloproteinase-1 (TIMP-1) inhibits matrix metalloproteinase (MMP) activity and regulates proliferation and apoptosis of a variety of cell types, including pancreatic β-cells. In the present study, we investigated whether TIMP-1 counteracts glucolipotoxicity in the pancreatic β-cell line INS-1.
METHODSINS-1 cells were incubated in normal or high glucose, with or without palmitate (0.4 mmol/L), in the presence of TIMP-1 or MMP inhibitor GM60001. In some experiments, cells were pretreated with phosphatidylinositol-3 (PI-3) kinase inhibitor, LY294002 or wortmannin. The amount of dead INS-1 cells was determined by HO342 and propidium iodide staining. Akt phosphorylation was evaluated by Western blotting analysis to investigate a possible mechanism of TIMP-1's action.
RESULTSTIMP-1 protected INS-1 cells from glucolipotoxicity independent of MMP inhibition. TIMP-1 stimulated Akt phosphorylation. Inhibition of the PI-3 kinase pathway abolished the survival effect of TIMP-1.
CONCLUSIONTIMP-1 may counteract glucolipotoxicity induced β-cell death via a PI-3 kinase pathway.

7.Effective inhibition of hTERT expression by RNA interference on the radiosensitivity of human laryngeal cancer Hep-2 cell line.

OBJECTIVETo construct an eukaryotic expression vector of human telomerase reverse transcriptase (hTERT) gene specific shRNA, and investigate the effect of pshRNA-hTERT combined with gamma-irradiation on cell survival and telomerase activity.
METHODSAccording to the coding sequence of hTERT mRNA, the target of RNAi was designed, and recombinant expression plasmid pshRNA-hTERT was constructed. The vector was transfected into Hep-2 cells. The radiosensitivity of Hep-2 cells was determined by clonogenic assay. Telomeric repeat amplification protocol (TRAP-PCR-ELISA) was used to observe the telomerase activity in each group. Results Recombinant expression vector pshRNA-hTERT was successfully transfected into Hep-2 cells. The hTERT expression inhibition rate reached 60. 8%. pshRNA-hTERT not only inhibited telomerase activity of Hep-2, but also inhibited the raise of telomerase activity induced by gamma-irradiation. Exposure of Hep-2 cells to pshRNA-hTERT for 24 hrs before irradiation resulted in a decrease in mean surviving fraction of Hep-2 cells compared with cells of group with irradiation alone (67. 7% vs 85. 7%, P <0. 05) .
CONCLUSIONRNAi showed a significant inhibitory effect to the expression of hTERT. The results indicate that pshRNA-hTERT can effectively inhibit telomerase activity of Hep-2 cells treated or untreated with 2 Gy gamma-irradiation and significantly enhance the radiosensitivity of Hep-2 cells in vitro. The role of radiosensitization of pshRNA-hTERT may be related with the inhibition of telomerase activity.

OBJECTIVES: In Fujian Province, China, gastric cancer is one of the leading causes of mortality among all malignant tumors. Nanjing county and Minqing county are located in inland Fujian and have similar general demographics. However, the adjusted mortality rate of gastric cancer in Minqing was found to be much higher than that in Nanjing. We sought to explore factors associated with this increased risk of gastric cancer between the two counties. METHODS: We recruited 231 and 224 residents from Nanjing and Minqing, respectively, and analyzed differences between their dietary habits, Helicobacter pylori infection rates, and concentrations of serum pepsinogen I, pepsinogen II, gastrin-17, and ratio of pepsinogen I:II. RESULTS: Subjects in Minqing had more first-degree relatives who had been diagnosed with upper gastrointestinal tumor, more unhealthy dietary habits, a higher Helicobacter pylori positive rate, and greater proportion of abnormal serum gastrin-17 than those in Nanjing did. CONCLUSIONS: The factors that differed between these two counties might indicate that residents in Minqing have a higher risk for developing gastric cancer than those in Nanjing do.

10.Role of microRNA-181a in the apoptosis of tubular epithelial cell induced by cisplatin.

BACKGROUNDCisplatin (DDP) is one of most effective and most commonly used therapeutic agent in treating tumors, it can accumulate in the kidney and lead to acute renal failure. MicroRNA-181a can induce cell apoptosis by suppressing the expression of Bcl-2 family. In the present study, we investigated the role of microRNA-181a in the apoptosis of tubular epithelial cell induced by DDP.
METHODSHK-2 cells were cultured, transfected with microRNA-181a inhibitor for 48 hours, and stimulated with 50 µmol/L cisplatin for 24 hours. MicroRNA-181a expression was analyzed by real time PCR, and cell apoptosis was detected by flow cytometry. Moreover, Bcl-2 and Bcl-2-associated X protein (Bax) expression were measured by Western blotting.
RESULTSMicroRNA-181a expression significantly down-regulated in cells transfected with microRNA-181a inhibitor, compared with that in untransfectd cells (21.19 ± 2.01 vs. 38.87 ± 1.97, P < 0.05). Cell apoptosis induced by DDP significantly decreased in cells transfected with MicroRNA-181a inhibitor. Compared with DDP treated cells alone, Bcl-2 expression strikingly was up-regulated and Bax expression was down-regulated in cells transfected with microRNA-181a inhibitor.
CONCLUSIONOne pathway of DDP induces apoptosis of tubular epithelial cell by suppressing Bcl-2 expression is achieved by regulating the target gene of MicroRNA-181a.
