1.A prospective study on cigarette smoking and liver cancer risk: a Meta-analysis

Objective:To explore the relationship between tobacco smoking and liver cancer risk.Methods:Cohort studies of cigarette smoking and risk of liver cancer were identified by searching MEDLINE and CBM databases from January 1989 to September 2009. Results from individual studies were combined synthetically by Meta analysis using R software and Meta package. Results:A total of 19 articles were included in our analysis. The cohort totally included 3 023 558 followed-up persons and 9 525 cases of liver cancer patients. The pooled relative risk (RR) of liver cancer was 1.49 (95%CI: 1.17-1.92) for smokers compared with non-smokers. The results showed a statistically moderate association between tobacco smoking and the risk of liver cancer. Subgroup analysis indicated that the pooled RRs for smoking were 1.53(95%CI : 1.29-1.83)for male smokers and 1.70(95%CI :1.17-2.48)for female smokers, respectively.Conclusion:Tobacco smoking is a risk factor for liver cancer in both males and females.

The establishment of quality evaluation of traditional Chinese medicine system that not only accords with Chinese medicine function characteristics but also is recognized as international medical circles, is an arduous task in urgent need of solving the current modernization of traditional Chinese medicine in the process of internationalization. It is difficult to evaluate atraditional Chinese medicine by detection of single active components in traditional Chinesemedicinewiththe western medicine quality controlmethod due to the overall effects of traditional Chinese drugs, the components of the overall diversity, targets, and the complexity of the interaction between components of unpredictable make the Long-term since, domestic and foreign scholars continue to explore and put forward a series of quality evaluation of traditional Chinese medicine to promote the development of traditional Chinese medicine. This article summarized the related academic ideas and developments to, providea new thought and perspective for the quality control of traditional Chinese medicine.

3.Influence of Akt inhibitor MK2206 in proliferation and apoptosis of tongue squamous cell carcinoma TCA-8113 cells and its mechanism

Objective To explore the influence of Akt inhibitor MK2206 in the proliferation and apoptosis of tongue squamous cell carcinoma TCA-8113 cells,and to clarify the possible mechanism.Methods The tongue squamous carcinoma TCA-8113 cells at the logarithmic phase were randomly divided into control group and 1,5,25,125, 250 nmol·L-1 MK2206 groups.The inhibitory rate of proliferation of TCA-8113 cells was detected with MTT method,and the apoptotic rate of TCA-8113 cells was determined with flow cytometry(FCM),and the expressions of caspase-9,Bad,GSK-3β,p-Akt and T-Akt proteins in the TCA-8113 cells were detected with Western blotting method.Results The IC50 of tongue squamous cell carcinoma TCA-8113 cells after treated with MK2206 for 12, 24,and 36 h were (112.54±1.67),(79.67±2.01),and (33.33±1.98)nmol·L-1 .The FCM results showed that the apoptotic rates of TCA-8113 cells after treated with 1,5,25,125,and 250 nmol·L-1 MK2206 for 12 h were (14.2±0.74)%,(19.3±0.45)%,(35.1±0.45)%,(39.6±0.48)% and (52.1±0.19)%;there were significant differences compared with control group(P<0.01).The Western blotting method results showed that the expressions of p-Akt, Bad and GSK-3βwere decreased with the increasing of dose and time of MK2206;compared with theβ-actin in control group,the bands got darken;the expression level of caspase-9 was increased, compared with theβ-actin in control group, the bands got darken;the T-Akt protein expression did not change significantly;compared with the β-actin in control group, the color of bands had no significant difference.Conclusion Akt inhibitor MK2206 can inhibit the proliferation of tongue squamous cell carcinoma TCA-8113 cells and induce apoptosis.
4.Biological characteristics of bacteriophage D218 of multiple drug resistance Acinetobacter baumanii

Objective To screen the bacteriophage for splitting multiple drug resistance Acinetobacter baumanii to provide a new scheme for the phage therapy of multiple drug resistance Acinetobacter baumanii infection.Methods Multiple drug resistance Acinetobacter baumanii isolated from hospitals in Shanghai served as the host bacteria.The bacteriophages were isolated from different environments.The morphology and size of bacteriophages were observed by electron microscope with negative staining.The influence of pH and temperature on its stability was analyzed.The phage DNA was extracted and sequenced.The related gene function was analyzedResults A virulent phage named D218 of drug-resistance Acinetobacter baumanii was isolated from the effluent sewerage of hospital.The electron microscopy observation showed that it is a long-tail phage of polyhedron symmetric structure,with a diameter of about 65nm,the tail length of about 100nm and width of about 15 nm.The phage kept high activity at pH 3-11 and temperature 4-50 ℃.The genome size is 102449bp with GC content of 37.2%.It encodes 167 ORFs.Conclusion A new virulent phage D218 of drug-resistance Acinetobacter baumanii is isolated,which is stable to pH and temperature,which increases the new data for its application in hospital infection prevention and control,and the phage library setup.
5.Tumor vascular normalization improves cancer treatment efifcacy

Therapeutic strategies targeting tumor angiogenesis have been approved for cancer therapy. Vasculature normalization induced by anti-angiogenic drugs can restore abnormal tumor vessels, and improve the tumor microenvironment characterized by hypoxia, extracellular acidosis, and high interstitial lfuid pressure, improve the cancer treatment results by chemoradiotherapy and immunotherapy.

7.Diagnosis of small focal nodular lesions in patients with liver cirrhosis: comparison between contrast-enhanced ultrasound and contrast-enhanced helical CT

Objective To compare the diagnostic value of contrast-enhanced ultrasound (CEUS) and contrast-enhanced helical CT (CECT) for various small focal nodular lesions (≤2 cm) in patients with liver cirrhosis. Methods Eighty-one small hepatic space-occupying lesions in 72 patients with liver cirrhosis were detected with CEUS and CECT, respectively. The diagnostic performance was calculated by histological results obtained from biopsy or surgery, which was considered as the gold standard, Results Fifty-three of the 81 small nodules were hepatocellular carcinoma, 26 were regenerative nodules and 2 were hemangioma. On CEUS, 51 (96.2%,51/53) HCC were hypervascular during arterial phase. On CECT, 41 (77.4%, 41/53) HCC were hypervascular (P < 0.01).Nodules which appeared by contrast enhancement during the arterial phase and contrast wash-out during the portal/late phase on CEUS or CECT were considered as HCC. The sensitivity, specificity and accuracy were 86.8% (46/53) ,82.1% (23/28) ,and 85.2 % (69/81) in CEUS, and 73.6% (39/53), 92.9 % (26/28), and 80.2 % (65/81) in CECT, respectively. Overall, there was no significant difference between CEUS and CECT in the diagnostic confidence for small hepatic nodules (P >0.05).Conclusions CEUS is superior to CECT in the detection of arterial vascularization for small hepatocellular carcinoma with a diameter ≤2 cm. The ability of CEUS in the characterization of focal nodular lesions in cirrhotic livers is similar to that of CECT.

As a depot drug delivery system, injectable polylactide-polyglycolide (PLGA) sustained-release microspheres have been successfully used to treat many diseases since the first microsphere product Lupron depot was approved for marketing in the United States in 1989. It has the ability of long-term release in the body for several days to several months, which can not only reduce the times of administration, but also reduce the drug blood concentration fluctuations, significantly improve the safety and patient compliance.
9.Practice and application of problem-based learning in evidence - based medicine teaching

AIM: To investigate the effect of problem - based learning ( PBL) used in the teaching of medical students'evidence-based medicine ( EBM) .
METHODS: Five classes ( total 147 students ) were randomly selected as experimental ( PBL ) group, at the same time, another 5 classes ( total 149 students ) were also randomly selected as control group, using traditional teaching method ( lecture-based learning, LBL ) in 2010 grade. The final examination scores of the experimental group were compared with control at the end of term. In addition, all students were interviewed using self -administered questionnaire to obtain their evaluation for PBL practice. SPSS13. 0 software was used for statistical analysis.
RESULTS: The homogeneity test in baseline survey showed that the basic characteristics between the two groups of students were no significant differences, and were comparable (P>0. 05). Final exam results showed that in addition to the scores of the EBM basic knowledge indicated no significant difference between two groups of students (P>0. 05), for the 5 steps of EBM procedure, namely, asking questions, finding the best evidence, evaluating the evidence, using and practicing the evidence, re - evaluating the evidence, and the total scores between the two groups, there were significant statistically differences (P<0. 05). The results to student learning evaluation showed that there was no significant difference between the two groups (P>0. 05) in aspects of better understanding classroom knowledge, improving language expression ability, and writing skill exercises. And other residual items had a significant difference ( P<0. 05), especially in aspects of improving enthusiasm for learning, self - study ability, improving learning efficiency, information analysis and utilization ability, team collaboration, and communication between teachers and students, however, there was a very significant difference (P<0. 001) between the two groups.
CONCLUSION: PBL teaching mode can effectively improve teaching effectiveness and the quality of EBM teaching, so the this teaching mode is worth further popularizing.
10.Effect of Huanglian Jiedu decoction in combination with fluconazole on ergosterol of fluconazole-resistant Candida albicans.

OBJECTIVETo investigate the effects of ethyl acetate extract of Huanglian Jiedu decoction (EAHD) , alone and in combination with fluconazole (FLZ) on FLZ-resistant Candida albicans.
METHODThe minimum inhibitory concentrations (MIC) and sessile MIC80 (SMIC80) of EAHD and FLZ to FLZ-resistant C. albicans were determined by CLSI M27-A3 microdilution method, and the synergy of EAHD combined with FLZ were examined by the checkerboard microdilution assay. Agar plate-method was adopted to observe the rate of antifungal activity according to time-kill curve. HPLC and qRT-PCR were utilized to evaluate the changes of ergosterol content and expressions of related genes, respectively.
RESULTMICs of EAHD ranged from 156 to 1,250 mg · L(-1), those of FLZ from 256 to above 2,048 mg · L(-1) with FICI approximate 0.066 in combination; SMIC80 of EAHD were higher than 1,250 mg · L(-1), SMIC80 of FLZ were higher than 512 mg · L(-1) and up to above 2,048 mg · L(-1). Combination group also showed synergy effect except one group showing addition effect. The results of T-K experiment also confirmed obviously fungicidal effect when treated for 12 h. When compared with control groups, the ergosterol was reduced 85% and 50% in the treatments of combination and EAHD alone by HPLC, respective- ly. The expressions of ERG1, ERG2, ERG6, ERG7 and ERG11 were upregulated, and ACS1, ACS2, MET6 were downregulated when exposed to FLZ. The expressions of the above genes were downregulated by treatment of EAHD. The expressions of ERG2, ERG6, ERG11 were upregulated, while ERG1, ERG7, ACS1, ACS2, MET6 were downregulated in combination group.
CONCLUSIONThe combination of EAHD and FLZ exhibited synergy against FLZ-resistant C. albicans through decreasing the synthesis of ergosterol, and resulting in the breakage of cell membrane.
