1.Genetic subtypes and drug resistance characteristics of newly reported HIV-1 cases in Tongzhou District, Beijing, 2021-2022

Objective To investigate the distribution characteristics of genetic subtypes among newly reported human immunodeficiency virus type 1 (HIV-1) cases in Tongzhou District, Beijing, from 2021 to 2022, and to analyze the molecular epidemiological characteristics and drug resistance patterns of HIV-1 in the area. Methods Newly reported HIV-1 cases in Tongzhou District, Beijing, from 2021 to 2022, were taken as the study population. Nested PCR was used to amplify the HIV-1 pol region gene, and the gene sequences were obtained through first-generation sequencing. A phylogenetic tree was constructed to analyze the HIV-1 genetic subtypes and drug resistance. Results There were 308 new HIV-1 cases reported in Tongzhou District, Beijing from 2021 to 2022. A total of 230 HIV-1 pol region gene sequences were obtained, with a case coverage rate of 74.68%. Seven HIV-1 genotypes were identified, with CRF07_BC (42.17%) and CRF01_AE (36.09%) being the main prevalent strains. Furthermore, new epidemic types such as CRF55_01B, CRF01_AE, and BC recombinants were detected. There were statistically significant differences (χ2=17.845, P<0.05; χ2=7.731, P<0.05) in the genotype composition ratio among newly reported HIV-1 cases across different age groups and detection routes, while no significant differences (P>0.05) were observed concerning gender, transmission route, marital status, and others. A total of 64 cases (27.83%) showed drug-resistant mutations, with 27 mutation sites identified in total. There were significant differences (χ2=53.674, P<0.05) in the mutation rates among different HIV-1 genotypes, with the highest mutation rate (100.0%) observed in CRF55_01B. Drug resistance to PIs, NRTIs, and NNRTIs was observed in 18 cases (7.83%), with resistance rates of 3.04%, 0.87%, and 4.35%, respectively. High-level drug resistance was observed in eight cases, including one case of NRTI resistance (mutation site M184V) and the remainder exhibiting NNRTI resistance, involving sites such as M184V, K103N, Y188L, and Y188C. Conclusions The genotype of HIV-1 in Tongzhou District is diverse, with CRF07_BC and CRF01_AE as the predominant strains. The total drug resistance rate reached a moderate level. We should strengthen the analysis of HIV-1 genotypes and drug resistance monitoring, focusing unique recombinants and resistance-related gene mutations.
2.Analysis of Hormone Levels in Patients with Hematological Diseases Before and After Hematopoietic Stem Cell Tansplantation.

OBJECTIVE:
By analyzing the hormone secretion of the adenohypophysis, thyroid glands, gonads, and adrenal cortex in patients with hematological diseases before and after hematopoietic stem cell transplantation (HSCT), this study aims to preliminarily explore the effect of HSCT on patients' hormone secretion and glandular damage.
METHODS:
The baseline data of 209 hematological disease patients who underwent HSCT in our hospital from January 2019 to December 2023, as well as the data on the levels of hormones secreted by the adenohypophysis, thyroid glands, gonads and adrenal cortex before and after HSCT were collected, and the changes in hormone levels before and after transplantation were analyzed.
RESULTS:
After allogeneic HSCT, the levels of thyroid-stimulating hormone (TSH), triiodothyronine (T3), free triiodothyronine (FT3) and estradiol (E2) decreased, while the levels of luteinizing hormone (LH) and follicle- stimulating hormone (FSH) increased. The T3 level of patients with decreased TSH after transplantation was lower than that of those with increased TSH after transplantation. In female patients, the levels of prolactin (PRL), progesterone (Prog), and testosterone (Testo) decreased after HSCT. Testo and PRL decreased when there was a donor-recipient sex mismatch, and the levels of adrenocorticotropic hormone (ACTH) and cortisol (COR) decreased when the HLA matching was haploidentical. The levels of T3, FT3, and PRL decreased after autologous HSCT. In allogeneic HSCT patients, the levels of TSH, T4, T3, FT3, and ACTH in the group with graft-versus-host disease (GVHD) were significantly lower than those in the group without GVHD. Logistic regression analysis showed the changes in hormone levels after transplantation were not correlated with factors such as the patient's sex, age, or whether the blood types of the donor and the recipient are the same.
CONCLUSION
HSCT can affect the endocrine function of patients with hematological diseases, mainly affecting target glandular organs such as the thyroid, gonads, and adrenal glands, while the secretory function of the adenohypophysis is less affected.

3.Independent and Interactive Effects of Air Pollutants, Meteorological Factors, and Green Space on Tuberculosis Incidence in Shanghai.

OBJECTIVE:
To assess the independent and combined effects of air pollutants, meteorological factors, and greenspace exposure on new tuberculosis (TB) cases.
METHODS:
TB case data from Shanghai (2013-2018) were obtained from the Shanghai Center for Disease Control and Prevention. Environmental data on air pollutants, meteorological variables, and greenspace exposure were obtained from the National Tibetan Plateau Data Center. We employed a distributed-lag nonlinear model to assess the effects of these environmental factors on TB cases.
RESULTS:
Increased TB risk was linked to PM 2.5, PM 10, and rainfall, whereas NO 2, SO 2, and air pressure were associated with a reduced risk. Specifically, the strongest cumulative effects occurred at various lags: PM 2.5 ( RR = 1.166, 95% CI: 1.026-1.325) at 0-19 weeks; PM 10 ( RR = 1.167, 95% CI: 1.028-1.324) at 0-18 weeks; NO 2 ( RR = 0.968, 95% CI: 0.938-0.999) at 0-1 weeks; SO 2 ( RR = 0.945, 95% CI: 0.894-0.999) at 0-2 weeks; air pressure ( RR = 0.604, 95% CI: 0.447-0.816) at 0-8 weeks; and rainfall ( RR = 1.404, 95% CI: 1.076-1.833) at 0-22 weeks. Green space exposure did not significantly impact TB cases. Additionally, low temperatures amplified the effect of PM 2.5 on TB.
CONCLUSION
Exposure to PM 2.5, PM 10, and rainfall increased the risk of TB, highlighting the need to address air pollutants for the prevention of TB in Shanghai.

4.Characteristics and pathophysiology of simple liver cysts in Beijing

Objective:To explore the onset characteristics and pathophysiological changes of simple liver cyst in Beijing.Methods:A retrospective cohort study was used. The physical examination data of Department of Health Management of Xuanwu Hospital of Capital Medical University for 10 years from January 1, 2012 to December 31, 2021 were analyzed. Selected clinical data of 37 389 subjects with 2 or more repeated ultrasound examinations, including 17 759 males, 19 630 females, aged (44.4±16.2) years, ranged from 22-103 years. 3 431 cases hepatic cyst were confirmed by repeated ultrasound examination, the data of the liver cyst formation after the same physical examination were the study group ( n=3 431), and the data before cyst formation were the control group ( n=3 431). The observation indicators included: (1) the epidemiological characteristics of liver cysts; (2) the age distribution of the incidence of liver cysts; (3) the gender distribution of the incidence of liver cysts; (4) the pathophysiological changes of liver cysts.Measurement data of normal distribution were expressed as mean±standard deviation ( ± s). The measurement data of skewed distribution were expressed as M ( Q1, Q3), using Wilcoxon signed rank sum test for comparing groups and chi-square test for comparing count data. The factors associated with hepatic cyst pathogenesis were summarized by multivariate Logistic regression. Results:The overall incidence of hepatic cysts was 9.18%, 9.78% in males, 8.63% in females, and the incidence of males was greater than that of females. The incidence of males over 70 to 79 years old decreased slightly, and the incidence in males and females in the other age groups increased with age.Results of multivariate Logistic analysis showed age ( OR=1.01, 95% CI: 1.01-1.02, P<0.01), waist circumference( OR=1.02, 95% CI: 1.01-1.02, P<0.01), systolic blood pressure ( OR=1.00, 95% CI: 0.99-1.00, P=0.013), ALT ( OR=0.99, 95% CI: 0.98-1.00, P<0.01), AST ( OR=1.03, 95% CI: 1.02-1.04, P<0.01), triglyceride lipids ( OR=0.90, 95% CI: 0.86-0.95, P<0.01), HDL( OR=0.71, 95% CI: 0.60-0.83, P<0.01), uric acid ( OR=1.00, 95% CI: 1.00-1.00, P<0.01), creatinine ( OR=0.99, 95% CI: 0.99-0.99, P<0.01) were the factors influencing the occurrence of liver cysts. Conclusions:The incidence of liver cysts increased linearly with age, and the incidence of males was greater than that of females. Age, waist circumference, systolic blood pressure, ALT, AST, triglycerides, HDL, uric acid, and creatinine may interact with the occurrence and development of liver cysts.
5.Epidemiological investigation of iron deficiency among preschool children in 10 provinces, autonomous regions, or municipalities in China

Objective:To understand the current status of anemia, iron deficiency, and iron-deficiency anemia among preschool children in China.Methods:A cross-sectional study was conducted with a multi-stage stratified sampling method to select 150 streets or townships from 10 Chinese provinces, autonomous regions, or municipalities (East: Jiangsu, Zhejiang, Shandong, and Hainan; Central: Henan; West: Chongqing, Shaanxi, Guizhou, and Xinjiang; Northeast: Liaoning). From May 2022 to April 2023, a total of 21 470 children, including community-based children aged 0.5 to<3.0 years receiving child health care and kindergarten-based children aged 3.0 to<7.0 years, were surveyed. They were divided into 3 age groups: infants (0.5 to<1.0 year), toddlers (1.0 to<3.0 years), and preschoolers (3.0 to<7.0 years). Basic information such as sex and date of birth of the children was collected, and peripheral blood samples were obtained for routine blood tests and serum ferritin measurement. The prevalence rates of anemia, iron deficiency, and iron-deficiency anemia were analyzed, and the prevalence rate differences were compared among different ages, sex, urban and rural areas, and regions using the chi-square test.Results:A total of 21 460 valid responses were collected, including 10 780 boys (50.2%). The number of infants, toddlers, and preschoolers were 2 645 (12.3%), 6 244 (29.1%), and 12 571 (58.6%), respectively. The hemoglobin level was (126.7±14.8) g/L, and the serum ferritin level was 32.3 (18.5, 50.1) μg/L. The overall rates of anemia, iron deficiency, and iron-deficiency anemia were 10.4% (2 230/21 460), 28.3% (6 070/21 460), and 3.9% (845/21 460), respectively. The prevalence rate of anemia was higher for boys than for girls (10.9% (1 173/10 780) vs. 9.9% (1 057/10 680), χ2=5.58, P=0.018), with statistically significant differences in the rates for infants, toddlers and preschoolers (18.0% (475/2 645), 10.6% (662/6 244), and 8.7% (1 093/12 571), respectively, χ2=201.81, P<0.01), and the rate was significantly higher for children in rural than that in urban area (11.8% (1 516/12 883) vs. 8.3% (714/8 577), χ2=65.54, P<0.01), with statistically significant differences in the rates by region ( χ2=126.60, P<0.01), with the highest rate of 15.8% (343/2 173) for children in Central region, and the lowest rate of 5.3% (108/2 053) in Northeastern region. The prevalence rates of iron deficiency were 33.8% (895/2 645), 32.2% (2 011/6 244), and 25.2% (3 164/12 571) in infants, toddlers, and preschoolers, respectively, and 30.0% (3 229/10 780) in boys vs. 26.6% (2 841/10 680) in girls, 21.7% (1 913/8 821), 40.0% (870/2 173), 27.1% (2 283/8 413), 48.9% (1 004/2 053) in Eastern, Central, Western, and Northeastern regions, respectively, and each between-group showed a significant statistical difference ( χ2=147.71, 29.73, 773.02, all P<0.01). The prevalence rate of iron-deficiency anemia showed a significant statistical difference between urban and rural areas, 2.9% (251/8 577) vs. 4.6% (594/12 883) ( χ2=38.62, P<0.01), while the difference in iron deficiency prevalence was not significant ( χ2=0.51, P=0.476). Conclusions:There has been a notable improvement in iron deficiency and iron-deficiency anemia among preschool children in China, but the situation remains concerning. Particular attention should be paid to the prevention and control of iron deficiency and iron-deficiency anemia, especially among infants and children in the Central, Western, and Northeastern regions of China.
6.Application of adipose-derived stem cells in skin graft and skin flap transplantation

Skin graft and skin flap transplantation are the most widely used reparative and reconstructive method in plastic surgery. How to avoid the necrosis of skin graft and skin flap together with the improvement of survival quality of skin graft and skin flap have always been one of the basic problem in plastic surgery basic and clinical research. The emergence of adipose-derived stem cells has brought a new idea to solve this problem. In this paper, the isolation, identification and biological characteristics of adipose-derived stem cells, application of adipose-derived stem cells in skin graft and skin flap transplantation were reviewed. At the same time, the problems of adipose-derived stem cells and their future prospects were discussed.

Purpose/Significance To analyze the influencing factors of user satisfaction on mobile medical platform,and to put for-ward suggestions to improve the quality of medical service and user satisfaction.Method/Process Taking the evaluation texts of users from eight tertiary hospitals in Guangxi in the online mobile medical service platform of haodf.com as the research object,the paper uses the software of ROST content mining system to filter meaningless words,carries out emotional analysis,word frequency statistical analysis and co-occurrence matrix semantic network analysis.Result/Conclusion The degree of satisfaction and recognition of users to the mo-bile medical service platform is high.From the perspective of influencing factors and departments,it puts forward some suggestions on how to improve the satisfaction of mobile medical service users.
8.In vitro expression and functional analyses of the mutants p.R243Q,p.R241C and p.Y356X of the human phenylalanine hydroxylase

Objective To study the in vitro expression of three phenylalanine hydroxylase(PAH)mutants(p.R243Q,p.R241C,and p.Y356X)and determine their pathogenicity.Methods Bioinformatics techniques were used to predict the impact of PAH mutants on the structure and function of PAH protein.Corresponding mutant plasmids of PAH were constructed and expressed in HEK293T cells.Quantitative reverse transcription polymerase chain reaction was used to measure the mRNA expression levels of the three PAH mutants,and their protein levels were assessed using Western blot and enzyme-linked immunosorbent assay.Results Bioinformatics analysis predicted that all three mutants were pathogenic.The mRNA expression levels of the p.R243Q and p.R241C mutants in HEK293T cells were similar to the mRNA expression level of the wild-type control(P>0.05),while the mRNA expression level of the p.Y356X mutant significantly decreased(P<0.05).The PAH protein expression levels of all three mutants were significantly reduced compared to the wild-type control(P<0.05).The extracellular concentration of PAH protein was reduced in the p.R241C and p.Y356X mutants compared to the wild-type control(P<0.05),while there was no significant difference between the p.R243Q mutant and the wild type control(P>0.05).Conclusions p.R243Q,p.R241C and p.Y356X mutants lead to reduced expression levels of PAH protein in eukaryotic cells,with p.R241C and p.Y356X mutants also affecting the function of PAH protein.These three PAH mutants are to be pathogenic.[Chinese Journal of Contemporary Pediatrics,2024,26(2):188-193]
9.Influencing factors of bone nonunion after intramedullary needle operation for tibial fracture

Intramedullary needle(IMN)has the advantages of high healing rate and low incidence of complications in treatment of tibial fracture,and has become one of the most commonly used fixation methods for the treatment of tibial fracture.However,due to the patient's own factors,fracture location and fracture type,infection and surgical treatment,bone nonunion after IMN still occurs in clinic.Bone nonunion leads to the increase of medical cost and prolonged the hospitalization time of patients,which causes great pain to patients,and also brings great challenges to the treatment of orthopedic surgeons.Therefore,this paper reviews the influencing factors of bone nonunion after IMN for tibial fracture,in order to provide reference for clinical treatment.
10.Construction of a nursing discipline construction indicator system for ClassⅢ Grade A general hospitals

Objective:To construct an indicator system for the development of nursing disciplines in ClassⅢ Grade A general hospitals.Methods:An initial draft of the indicator system was established through literature research and expert meetings. The Delphi method and purposive sampling were used from January to April 2023 to conduct two rounds of expert consultations with 17 nursing experts from four medical units nationwide and three nursing colleges. The indicators were revised and refined, resulting in the establishment of a nursing discipline construction indicator system for ClassⅢ Grade A general hospitals. The level of expert engagement was represented by the questionnaire recovery rate, expert authority by the expert authority coefficient ( Cr), and the degree of expert opinion coordination by Kendall 's harmony coefficient (Kendall 's W) . Results:The recovery rates for both rounds of expert consultation questionnaires were 100.00% (17/17), with expert Cr of 0.87. The CV values ranged from 0.049 to 0.260 and 0 to 0.140, respectively, while Kendall 's W were 0.221 and 0.257 ( P< 0.01). The final indicator system for the construction of nursing disciplines in ClassⅢ Grade A general hospitals included six primary indicators, 18 secondary indicators, and 63 tertiary indicators. Conclusions:The nursing discipline construction indicator system developed for ClassⅢ Grade A general hospitals in this study is scientific, reliable, and comprehensive. It can provide a reference for the construction of nursing disciplines in Class Ⅲ Grade A general hospitals.
