2.The efficacy of early intratympanic steroid of high concentration combined with systemic steroid on idiopathic sudden sensorineural hearing loss

Objective To evaluate the therapeutic effects of early intratympanic steroid of different concentrations and types combined with systemic steroid on severe idiopathic sudden sensorineurl hearing loss(ISSNHL).Methods We reviewed 95 patients with unilateral ISSNHL from January 2012 to June 2016 in the Department of Otolaryngology of the People's Hospital of Changshou and according to the steroid treatment methods,Patients were divided into systemic steroid therapy alone group(group A,30 cases),early intratympanic steroid of low concentration combined with systemic steroid group (group B,32 cases) and early intratympanic steroid of high concentration combined with systemic steroid group (group C,33 cases).Results Total effective rates of the group A,group B and group C were 46.67 %,56.25 %,78.79%.Group C was significantly higher than that of group A and group B(P<0.05);the average pure tone threshold improvement was (46.67±9.32) in group C which was better than group A (20.53±8.83) and group B(25.19± 7.46),the differences were stastically significant (P<0.01).Conclusion Severe ISSNHL should be treated by intratympanic steroid of high concentration combined with systemic steroid as early as possible in order to rescue the hearing,the efficacy of which is much better than other steroid treatments so far.
4.Investigation on Impacts of Industrial Fluoride-induced Pollution on Environment and Residents' Health

ve To understand the impacts of industrial fluoride-induced pollution on environment and human health. Methods The contents of fluorides in environmental media, such as air, soil, vegetables, weeds, branches and leaves in area around a certain chemical factory (polluted area) were monitored. The prevalance of dental fluorosis, the contents of fluorides in hair and nail were investigated among 87 individuals without history of exposure to industrial fluorides, living in polluted area for more than 5 years, and 132 individuals in control area. Results Higher fluoride levels in environmental medias, higher prevalance rates of dental fluorosis, high contents of fluorides in hair and nails of population were found in polluted area compared with those in control area. Conclusion The in-dustrial fluoride-induced pollution from this chemical factory had resulted in the hazardous impacts on environmental quality and human health.
5.Application of intraaortic balloon counterpulsation during primary percutaneous coronary intervention in patients with cardiogenic shock complicating with acute myocardial infarction

Objective To investigate the safety,efficacy and effects of emergent percutaneous coronary intervention (PCI) in patients from Second Affiliated Hospital,Medical College,Zhejiang University with cardiogenic shock (CS) complicating acute myocardial infarction (AMI).Method Twenty-seven patients with CS complicating with AMI were treated by PCI with intraaortic balloon counterpulsation (IABP) support.The change of hemodynamics before and after IABP and PCI,the characteristics of PCI,the mortality during hospitalization, the major adverse cardiac events (MACE) and left ventricular ejection fraction at 30-day follow-up were observed.Results The hemodynamics were significantly improved after IABP.No patients died during PCI.Two patients died after PCI and the total mortality was 7.4% in hospital.During the period of 30-day follow-up, one patient died of heart failure.The left ventricular ejection fraction greatly improved at 30 days after PCI. Conclusions The data suggested that the use of IABP during PCI in patients with CS complicating AMI was safe, decreased mortality and improved prognosis.

9. Lentiviral vector-mediated RNA interference of HBs inhibits replication of HBV

Objective: To construct a lentiviral vector (LV) of RNA interference (RNAi) targeting HBs gene and to observe its effect on the replication of HBV and expression of antigens. Methods: The effective sequence of siRNA targeting HBs gene was confirmed in our previous study. The complementary DNA containing both sense and antisense Oligo DNA of the targeting sequence was designed, synthesized and cloned into the lentivirus vector (pGCLM-GFP). The resulting lentivirus vector containing HBs shRNA was named LVshHBs, and it was confirmed by PCR and sequencing. 293T cells were cotransfected with lentivirus vector pGCLM-GFP, pHelper 1.0 and pHelper 2.0. All virus stocks were produced by calcium phosphate-mediated transfection. The titer of virus was tested according to the expression level of GFP. HepG2. 2.15 cells were infected with LVshHBs and the supernatant of the cells was subjected to ELISA, Western blotting analysis and HBV DNA quantitative analysis. Results: PCR and DNA sequencing demonstrated that the LVshHBs-producing HBs shRNA was successfully constructed. The titer of concentrated virus was 5 × 108-2 × 109 TU/ml. The inhibitory effect was efficient and the corresponding viral transcript and expression of antigens were decreased after infection. The inhibitory effect was observed 4 days after infection and peaked 9 days after the initial treatment with RNAi. Secreted HBsAg was reduced by > 70% in cell culture compared with the negative control, which is also confirmed by Western blotting and real time PCR. After quantification of HBV DNA, the level of DNA relative to the controls was also significantly reduced after RNAi treatment(P<0.05). Conclusion: The lentivirus RNAi vector of HBs has been successfully constructed. The lentiviral microRNA-based RNAi targeting HBs can specifically mediate the down regulation of HBs expression, inhibiting HBV replication and antigen expression.
10.Hemodynamic effects of open-loop and closed-loop target controlled infusion of propofol and fentanyl for general anesthesia induction

Objective To observe the hemodynamic effects of open-loop target controlled infusion (OLTCI) and closed-loop target controlled infusion (CLTCI) of propofol and fentanyl for general anesthesia induction. Methods Twenty-four female patients with ASA grade I-II who were performed thyroidectomy were randomly allocated into two groups: OLTCI group and CLTCI group(n=12). In OLTCI group, anesthesia induction and maintenance were performed with propofol and fentanyl at the target concentrations of 3 ?g/mL and 3 ng/mL, respectively. In CLTCI group, double CLTCI were performed. Titrations of propofol and fentanyl were guided with bispectral index (BIS) and product of systolic pressure and heart rate (HR). Initiative concentrations of this closed-loop system were 3 ?g/mL and 3 ng/mL, step-up or step-down concentrations were 0.5 ?g/mL and 0.5 ng/mL, and the highest concentrations were 6 ?g/mL and 5 ng/mL, respectively. HR, mean arterial pressure (MAP), HR variability, BIS value and the dosages of propofol and fentanyl in various time of the two groups were recorded. Results One min after intubation and simulative incision stimulation, BIS value of both groups were increased, but the BIS value in CLTCI group was less increased than OLTCI group(P
