1.Effect of intravenous thrombolytic therapy with urokinase on neurological function and serum MMP-9 in patients with acute cerebral infarction

Objective To investigate the effect of intravenous thrombolytic therapy with urokinase on the neurological function and the concentration of serum matrix metalloproteinase 9 (MMP-9) in the patients with acute cerebral infarction.Methods The patients with acute cerebral infarction were divided into the experimental and control groups.The experimental group included 27 patients who were complied with thrombolytic criterion within 4.5 hours after stroke and were firstly treated by intravenous thrombolytic therapy with urokinase by 100 million units after 24 h and 300 mg aspirin by oral.The control group included 27 cases that were directly administrated by 300 mg aspirin 4.5 hours later after stroke.After 24 h,the two groups were administrated with other same conventional treatments such as neurotrophy,improvement of microcirculation,and control of blood-fat.The neurological function and dynamic concentration of serum MMP-9 were observed before treatment and after treatment.Results After treatment,the neurological deficit evaluation score in both groups was gradually reduced with the treatment time,and the neurological deficit evaluation score in the experimental group was significantly lower than that in the control group at the 1 st,3rd,and 14th day,respectively[(10.97 ± 1.53) Score vs (15.67 ±1.78)Score,t =8.35,P =0.03;(8.15 ± 1.40) Score vs(12.72 ± 3.31) Score,t =6.62,P =0.03; (5.87 ± 1.03) Score vs (11.92 ±2.05) Score,t =13.70,P =0.01].After treatment,the concentration of serum MMP-9 in both groups was reduced with the treatment time,and serum MMP-9 in the experimental group was significantly lower than that in the control group at the 1st,3rd,and 14th day,respectively[(282.84 ±37.51) ng/ml vs (316.90±36.75)ng/ml,t =3.37,P =0.00;(309.11±37.71)ng/mlvs (348.39 ±15.26) ng/ml,t =5.02,P=0.04;(264.68±31.91)ng/ml vs (302.81 ±36.30)ng/ml,t =4.10,P =0.03].Conclusions Intravenous thrombolytic therapy with urokinase can effectively reduce the neurological deficit and the produce of MMP-9 in patients with acute cerebral infarction.
2.Negative Modulation of NO for Diaphragmatic Contractile Reduction Induced by Sepsis and Restraint Position

In practice of forensic medicine, potential disease can be associated with fatal asphyxia in re-straint position. Research has demonstrated that nitric oxide (NO) and nitric oxide synthase (NOS) are plentifully distributed in skeletal muscle, contributing to the regulation of contractile and relaxation. In the current study, respiratory functions, indices of diaphragmatic biomechanical functions ex vivo, as well as NO levels in serum, the expressions of diaphragmatic inducible NOS (iNOS) mRNA, and the effects of L-NNA on contractility of the diaphragm were observed in sepsis induced by cecal ligation and punc-ture (CLP) under the condition of restraint position. The results showed that in the CLP12-18 h rats, respiratory dysfunctions; indices of diaphragmatic biomechanical functions (Pt, +dT/dtmax, -dT/dtmax, CT, Po, force over the full range of the force-frequency relationship and fatigue resistance ) declined progressive-ly; the NO level in serum, and iNOS mRNA expression in the diaphragm increased progressively; force increased significantly at all stimulation frequencies after L-NNA pre-incubation. Restraint position 1 h in CLP12 h rats resulted in severe respiratory dysfunctions after relative stable respiratory functions, almost all the indices of diaphragmatic biomechanical functions declined further, whereas little change took place in NO level in serum and diaphragmatic iNOS mRNA expression; and the effects of L-NNA were lack of statistical significance compared with those of CLP12 h, but differed from CLP18 h group. These results suggest that restraint position and sepsis act together in a synergistic manner to aggravate the great reduction of diaphragmatic contractility via, at least in part, the negative modulation of NO, which may contribute to the pathogenesis of positional asphyxia.
3.Experimental study of prevention effect of andrographolide against radiation exposure in rats

Objective To explore the effects of andrographolide(AP),extracted from the traditional Chinese herb Andrographlis paniculata(AP),on injury induced by radiation exposure.Methods Sixty male rats were randomly divided into 4 equal groups and irradiated with 60Co γ-rays at the doses of 1,2,and 4 Gy,respectively:low dose AP group(intragnstrically administered with AP at the dose of 100 ms/kg daily for 10 d before irradiation),and high dose AP group(intragastrically administered with AP at the dose of 200 ms/kg daily for 10 d before irradiation),model group(administered with the same volume of normal saline instead of AP for 10 d before irradiation),and control group(irradiated only at 3 different doses).One day after irradiation all rats were killed with their livers being fixed to make paraffin section.The morphological feature was observed under light microscope after HE staining,and the cell apoptosis was detected by TUNEL technology.Results Compared to the control and model groups,the pathological changes of liver were significantly gentler in the AP treatment groups.The apoptosis rates of the liver cells of all the AP sub-groups were significantly lower than those of the control and model subgroup(t=2.19-4.80.P<0.05).Conclusions AP might have prevention effect against radiation exposure.
4.Current Status and Time Trends of Cancer Incidence and Mortality Worldwide

As the second most common cause of death in the world, cancer has become a persistent public health challenge. The incidence and mortality in different countries and regions or of multiple cancer types are significantly different, which is closely related to economic development level, lifestyle and environmental factors. A large number of epidemiological studies have focused on cancer burden, epidemic pattern, etiology and prevention, which is very important for the government to formulate cancer prevention policies based on medical evidence and protect population health. Therefore, based on the latest epidemiological publications, this paper reviews the incidence and mortality of cancer as well as the time trend in major countries and areas.
5.A case-control study on the risk factors of work-related acute pesticide poisoning among farmersfrom Jiangsu province

Objective To explore the risk factors on cases regarding work-related acute pesticide poisoning among farmers of Jiangsu province.Methods A population-based,1 ∶ 2 matched case-control study was carried out,with 121 patients as case-group paired by 242 persons with same gender,district and age less then difference of 3 years,as controls.Cases were the ones who had suffered from work-related acute pesticide poisoning.A unified questionnaire was used.Data base was established by EpiData 3.1,and SPSS 16.0 was used for both data single factor and multi-conditional logistics regression analysis.Results Results from the single factor logistic regression analysis showed that the related risk factors were:lack of safety guidance,lack of readable labels befores praying pesticides,no regression during application,using hand to wipe sweat,using leaking knapsack,body contaminated during application and continuing to work when feeling ill after the contact of pesticides.Results from multi-conditional logistic regression analysis indicated that the lack of safety guidance (OR=2.25,95% CI:1.35-3.74),no readable labels befores praying pesticides (OR=1.95,95% CI:1.19-3.18),wiping the sweat by hand during application (OR=1.97,95%CI:1.20-3.24)and using leaking knapsack during application (OR=1.82,95% CI:1.10-3.01) were risk factors for the occurrence of work-related acute pesticide poisoning.Conclusion The lack of safety guidance,no readable labels befores praying pesticides,wiping the sweat by hand or using leaking knapsack during application were correlated to the occurrence of work-related acute pesticide poisoning.
6.Epidemiological study on alcohol consumption and agricultural profession-related injuries among minority residents from the northern parts of China.

OBJECTIVETo understand the incidence rates of agricultural profession-related injuries and the relationship to alcohol consumption.
METHODSA multistage sample of 2366 villagers was selected from Minority Nationality Villages, Heilongjiang province. Face-to-face interviews were conducted by trained interviewers during May 2008. 2198 (92.9%) of the eligible questionnaires were available.
RESULTSIncidence rate of agricultural profession-related injury rate was 11.8% (260/2198) from May 2007 to April 2008. Higher proportions of injuries were seen for males, 30 - 49 year olds, farmers and among those people working on farms for 10 years or more, smoking during farm work, with sleeping disorder, using motor vehicles, and those using agricultural machinery. Rate of alcohol drinking was 29.1% (640/2198) in the month prior to the investigation. Results from logistic regression models showed that alcohol consumption patterns and other alcohol-related behavior were examined in separate logistic models because of collinearity by controlling the variables that were associated with agricultural profession-related injury including sex, age, years of farm work, driving a motor vehicle, and agricultural machinery use etc. In each model, the reference group was those villagers who did not drink in the past month. The odds of injury among villagers with history of past month drinking, who drank distilled spirits, and alcohol drinking at breakfast and lunch were 1.80 (95%CI: 1.24 - 2.62), 2.09 (95%CI: 1.38 - 3.15), 2.15(95%CI: 1.43 - 3.22) respectively. The odds of agricultural injury also significantly increased with greater average amounts of pure alcohol per day, with increased frequency of drinking per week, and reported years of drinking.
CONCLUSIONIntervention efforts should include increasing awareness about alcohol drinking as a major risk factor causing agricultural injuries as well as controlling alcohol drinking, improving knowledge about driving motor vehicle and using agricultural machinery.

7.Levels and variation of radon equilibrium factor in an office of Nanning city

Objective:To study the levels and variation of equilibrium factor in indoor environment.Methods:A one-year continuous measurement of radon concentration and equilibrium equivalent radon concentration was carried out in an indoor office building of Nanning city. The effective data acquisition rates of radon gas and radon progeny were 99.9% and 86.7%, respectively.Results:The annual average activity concentration and equilibrium equivalent radon concentration in indoor environment were (50.9±20.7)and (15.5±10.1)Bq/m 3, both of which had the same diurnal and seasonal variation. The average annual value of equilibrium factor was 0.30±0.12, showing no obvious diurnal variation. The distribution of monthly mean value of equilibrium factor showed a similar trend to that of radon and radon progeny. The highest and the lowest value appeared in November and June, respectively, with 0.47±0.24 and 0.19±0.06. Conclusions:Due to the large variation range of monthly mean value of equilibrium factor in indoor environment, when annual effective dose of radon exposure was estimated based on radon gas concentration, attention should be paid to choose the quantity value of equilibrium factor and the uncertainty caused by the change of equilibrium factor should be considered.
8.Research and suggestion on the plan of environmental radioactivity level investigation

The investigation of environmental radioactivity level is an important basic work of ecological environment protection. It can provide basic data and scientific basis for the evaluation of radioactive environment quality and the formulation of radiation safety regulations and standards. Based on many years of practical experience of environmental radioactivity level investigation, combined with relevant regulations and standards, this paper summarizes the common investigation methods in the investigation and research of environmental radioactivity level at home and abroad, summarizes a set of environmental radioactivity level investigation scheme with strong applicability, and gives detailed suggestions on the ideas, methods, technical routes and other key points of the investigation scheme. The research results of this paper can provide a reference for the preparation of environmental radioactive level investigation scheme.
9.Investigation and evaluation of radioactivity level in Weizhou Island of Beihai

Objective The radioactivity level survey was carried out in Weizhou Island area of Beihai, to comprehensively master the radiation environment status in the area, understand the natural radioactivity level and its distribution law, and provide basic data for scientific evaluation of radiation environment quality. Methods According to the relevant standards and technical specifications, the air absorbed dose rate of gamma radiation in Weizhou Island area was monitored from April 2020 to March 2021, the concentrations of radionuclides in water, soil and other environmental samples were sampled and analyzed, and the monitoring results were analyzed and discussed in combination with the regional characteristics. Results The results show that the air absorbed dose rate of gamma radiation in Weizhou Island area ranges from 0.2~122 nGy/h; the activity concentrations of 238U, 226Ra, 232Th, 40K and 137Cs in solid samples range 17.0~37.3 Bq/kg, < 0.501~29.5 Bq/kg, < 0.766~54.3 Bq/kg, 18.7~369 Bq/kg and < 0.212~1.48 Bq/kg, respectively, the concentrations of U and Th in well water and reservoir water range 0.065~0.25 μg/L, 0.046~0.079 μg/L, the activities of 226Ra, 40K, total α and total β range 1.42~3.08 mBq/L, 0.069~0.231 Bq/L, 0.025~0.163 Bq/L, 0.082~0.572 Bq/L, respectively, the concentrations of U and Th in seawater samples range 1.81~2.25 μg/L, 0.634~0.648 μg/L, and the activities of 226Ra, 40K, 90Sr and 137Cs range 9.38~19.7 mBq/L, 11.3~11.8 Bq/L、0.193~0.866 mBq/L、1.13~1.42 mBq/L. Conclusion The environmental ionizing radiation level in Weizhou island of Beihai is in the range of background fluctuation and at a relatively low level, indicating that the radiation environmental quality of Weizhou Island and its surrounding areas is good.
