1.Diurnal variation, moon phase and seasonal variation in onset of stroke

ObjectiveTo determine the diurnal variation,moon phase,and seasonal variation in the onset of stroke. MethodsThe time parameters,including the time,the moon phase and the season of the stroke occurrence for 905 in-patients from 1989-2001 were investigated.ResultsThere was a significant difference (D=0.155,P<0.01) for circadian rhythm in the onset of 321 cases with cerebral infarction,and the incidence frequency at dawn was much higher than that at other time,but the difference of the moon phase and seasonal rhythm in the onset of cerebral infarction were not significant(P>0.05).There were not diurnal variation,moon phase and seasonal rhythms in the onset for cerebral haemorrhage(P>0.05).ConclusionsThe cerebral infarction occur more frequently at dawn time.
2.Surgical techniques of autonomic nerve preservation in lymph nodes dissection for gastric cancer

Resection of the automonic nervous system in the process of lymph nodes dissection is a major factor leading to postoperative gastrointestinal dysfunction.The era of blind expansion of the wide excision has been questioned,and safety first,radical dissection of the metastatic lymph nodes,low invasive rate and organ function-saving surgery have been highly emphasized.Preserving the gastric autonomic nerve effectively reduces postoperative diarrhea,shortens the time for weight recovery,and prevents insulin secretion disorders and postoperative cholelithiasis.In the process of laparoscopic or open autonomic nerve preservation in lymph node dissection for gastric cancer,full understanding of the anatomy of the autonomic nervous system,recognizing the landmark of the nerves,proper designing the surgical space,dissecting according to the extracellular space in the membrane of the nerve fibers and nerve plexus are factors which ensures the quality and effects of the surgery.
3.Investigation and analysis of nursing undergraduate's graduation thesis writing

ObjectiveTo understand the nursing undergraduates' graduation thesis writing,to provide basis for the revised management plan and improve the overall quality of graduation thesis.Methods A self-designed questionnaire was given to 158 four-year program nursing graduates of Grade 2006 by group and the questionnaires were collected timely and analyzed with SPSS 17.0.Results63.19% of students thought it necessary to write thesis,86.71% thought internship beneficial for wrting and 64.6% hoped to finish the thesis writing before the practice because of great pressure.ConclusionStudents' recognition degree to graduation thesis directly affects the quality of thesis writing.A reasonable teaching plan should be made and school resources should be fully used to improve the quality of thesis writing.
4.Clinical Observations on Primary and Connecting Point Combination Acupuncture for the Treatment of Depression

ObjectiveTo investigate the clinical efficacy of primary and connecting point combination acupuncture in treating mild and moderate depression.MethodPatients meeting the inclusion criteria were randomly allocated to observation and control groups. Both groups of enrolled patients took fluoxetine hydrochloride (Prozac). The observation group received meridian-unblocking acupuncture at yuan-primary points and luo-connecting points and the control group, acupuncture at routinely selected points in the eleventh-five textbook. Treatment was given for four consecutive weeks. The Hamilton Depression Scale (HAMD) 24-item score was recorded in the patients before and after one month of treatment. The therapeutic effects were evaluated according to the score reduction rate in the two groups.ResultThere wasa statistically significant post-treatment difference in the HAMD 24-item score between the two groups (P<0.05). The total efficacy rate was83.3% in the treatment group and70.0% in the control group. There was a statistically significant post-treatment difference in therapeutic effect between the two groups (P<0.05). ConclusionMeridian-unblocking acupuncture at yuan-primary points and luo-connecting points is more effective than conventional acupuncture in proving the symptoms in patients with mild or moderate depression.
5.Surgical treatment of stage Ⅳ gastric cancer

Stage Ⅳ gastric cancer is defined as gastric cancer with non-curative factors,and the long-term survival rate of patients with stage Ⅳ gastric cancer is around 10%.Chemotherapy has been used for the main therapy of the patients with unresectable stage Ⅳ gastric cancer.Palliative resection of the primary tumor has been recommended only for presence of obstruction,bleeding and other surgical emergency.Debulking operation was irrelevant to the outcome that has been considered by research results.However,the patients with stage Ⅳ gastric cancer of only one non-curative factors (No.16a2,b1 lymph node enlargement;degree of peritoneal metastasis as P0 and positive peritoneal cytology;a small number of liver metastases)have good long-term survival when these patients have achieved R0 or R1 resection.In this regard,comprehensive treatment including surgical resection is the main treatment policy for the patients with stage Ⅳ gastric cancer.In recent years,some clinical studies on improvement of prognosis through surgical resection after conversion therapy have received extensive attention.
6.Analysis of 60 Gastrointestinal Tumor Cases Diagnosed by B Mode Ultrasonography

This paper reports on the real-tim ultrasonographic diagnosis of gastrointestinal tumors in 60 patients ; and the ultrasonograms of gastrointestinal tumors and their classification are analysed. In this report, the diagnostic value of ultrasound in gastrointestinal tumors has been deduced. We consider that pseudorenal signs are the ultrasonography of gastrointestinal system and most of them are gastrointestinal tumors . Our observations suggest that the sonography of pseudorenal sign may be used as an anatomic feature of abdominal masses.

8.Persistence of the Effects of Nucleoside(Acid)Analogues in the Treatment of Chronic Hepatitis B after Drug Withdrawal

OBJECTIVE:To study the persistence of the effects of nucleoside (acid) analogues in the treatment of chronic hepatitis B (CHB) after drug withdrawal. METHODS:150 cases of CHB were selected and received nucleoside (acid) ana-logues for one year at least. They were divided into the standard group(n=56)and non standard group(n=87)according to the drug withdrawal standard stated in Guidelines for Prevention and Treatment of CHB,and followed up for one year except 7 cas-es of drug withdrawal who were followed up less than one year. HBV serum markers HBVM and HBV-DNA were detected quan-titatively in 2 groups. CHB recurrence,HBsAg disappearance and the change of HBV in serum were observed in 2 groups after drug withdrawal. RESULTS:The recurrence rate of CHB patients with HBeAg (+) in standard group and non standard group were lower than those with HBeAg(-),with statistical significance(P<0.01 or P<0.05);that of CHB patients with HBeAg (+) in standard group was lowe than those with HBeAg (+) in non standard group,with statistical significance (P<0.05);there was no statistical significance in recurrence rate of CHB patients with HBeAg(-)between standard group and non stan-dard group(P>0.05);the total recurrence rate of standard group was lower than that of non standard group,with statistical sig-nificance(P<0.05). There was no statistical significance in recurrence rate between 2 groups after drug withdrawal following 4 kinds of nucleoside (acid) analogue monotherapy (P>0.05). Among sustained responders,HBsAg of 3 patients disappeared in standard group,including 2 cases of HBsAg seroconversion,1 case of no HBsAg seroconversion. Among patients with recurrent CHB,2 patients suffered from HBeAg(+)again in standard group,and 1 patient with HBeAg(-)developed liver cancer;5 patients suffered from HBeAg(+)again in non standard group,including 1 patient died and 1 patient with HBeAg(-)devel-oped liver cancer. CONCLUSIONS:Taking Guidelines for Prevention and Treatment of CHB as drug withdrawal standard,there still is a high recurrence rate after stopping nucleoside(acid)analogues;the recurrence rate of patients with HBeAg(-)is high-er than those with HBeAg(+),and that of patients in non standard group is higher than in standard group. The types of nucleo-side(acid)analogues do not influence recurrence rate after drug withdrawal.
9.Efficacy Observation of Cinepazide Maleate Combined with Oxiracetam in the Treatment of Vascular Dementia

OBJECTIVE:To observe the clinical efficacy and safety of cinepazide maleate combined with oxiracetam in the treat-ment of vascular dementia. METHODS:62 patients with vascular dementia were randomly divided into treatment group and control group,with 31 patients in each group. Patients in control group were treated with Oxiracetam injection 4g/d for 3weeks. Patients in treat-ment group were treated with cinepazide maleate 160 mg/d for 3 weeks on the basis of control group. The patients of 2 groups were exam-ined with Mini-mental State Examination (MMSE)and Activity of Daily Living Scale (ADL)before and after treatment. RESULTS:There was statistically significant difference in MMSE and ADL scores of 2 groups before and after treatment(P<0.05);MMSE score and ADL score of treatment group was better than that of control group,with statistical significance(P<0.05);Obvious ADR was not found in both groups. CONCLUSIONS:Cinepazide maleate combined with oxiracetam can significantly improve ADL and MMSE score in patients with vascular dementia,improve the quality of life,showing significant and good clinical efficacy.
10.Rapid Distinguishment of Drug Consumption Patterns by the Seasonal Decomposition Method

OBJECTIVE:To establish a simple and feasible method for rapid analysis of drug consumption patterns,and to im-prove the level of hospital pharmacy management. METHODS:Taking Motilium tablet as an example,the actual order amount da-ta of 24 months were analyzed by time series seasonal decomposition method;several key components of change trend were extract-ed by SPSS software,i.g. smoothed trend-cycle(STC),seasonal factors(SAF)and seasonally adjusted series(SAS). The varia-tion of components data series was observed to obtain the conclusion,and then compared with direct-vision method. RESULTS:The seasonal decomposition method had high prediction accuracy,the mean absolute percent error (MARE)(0.17) was less than that of error value by direct-vision method (0.24) significantly. CONCLUSIONS:This method can found drug consumption pat-terns quickly and accurately with the help of software. Based on this aspect,stock adjustment strategy can combine daily manage-ment and long-term dynamic management conveniently so as to improve the level of drug management.
