Objective To investigate the clinical importance and the relationship between the levels of plasma total homocysteine (Hcy), serum folic acid and vitamin B 12 , and the occurrence and development of ischemic leukoaraeosis (LA). Methods Ischemic LA group and normal control group with 100 cases for each were studied. The 100 cases in ischemic LA group were firmly diagnosed by CT or / and MRI. The levels of plasma total Hcy, serum folic acid and vitamin B 12 of the patients in ischemic LA group were measured, and compared with that in control group. Results The level of plasma Hcy in ischemic LA group (19.79?5.25?mol/L) was significantly higher than that in control group (12.02?2.11?mol/L), while the levels of serum folic acid and vitamin B 12 in ischemic LA group (2.08?0.84ng/ml and 140.13?45.49pg/ml) were lower than that in control group (4.39?1.30ng/ml, 297.45?120.23pg/ml). 3.Efficacy of Rehabilitation on 216 Patients with Nonspecific Low Back Pain

ObjectiveTo assess different treatments on nonspecific low back pain.Methods246 subjects were assigned to 3 groups, a control group(n=82), a home exercise group(n=82) or a strengthening exercise group(n=82). Follow-up data were obtained at 1 months, 3 months and 6 months after the treatment. Primary outcome measures were modified Oswestry Low Back Pain Disability Questionnaire for disability and Numeric Pain Rating Scale for pain.ResultsThe home exercise and strengthening exercise groups experienced greater improvements in disability than the control group 6 months after the treatment. The strengthening exercise group demonstrated greater change than home exercise group.ConclusionActive rehabilitation therapy may benefit nonspecific low back pain patients.
4.Surveillance of common diseases among primary and middle school students in Xinjiang Production and Construction Corps

Objective:
To analyze the surveillance results of common diseases among primary and middle school students in Xinjiang Production and Construction Corps in 2020, so as to provide the evidence for formulating the control measures for common diseases among primary and middle school students.
Methods:
The comprehensive health surveillance data of 31 467 primary and middle school students were collected from 132 schools in 14 divisions of Xinjiang Production and Construction Corps in 2020, and the prevalence of poor vision, overweight and obesity, dental caries, malnutrition and elevated blood pressure were descriptively analyzed.
Results:
The 31 467 students included 15 580 boys ( 49.51% ) and 15 887 girls ( 50.49% ). There were 19 632 students ( 62.39% ) from urban areas and 11 835 ( 37.61% ) from suburban areas, and there were 11 371 primary students ( 36.14% ), 11 169 junior high school students ( 35.49% ) and 8 927 high school students ( 28.37% ). The detection rates of poor vision, overweight and obesity, dental caries, elevated blood pressure, and malnutrition were 57.22%, 29.69%, 27.57%, 15.03%, and 4.81%, respectively. The detection rates of overweight and obesity ( 36.07% vs. 23.45%, P<0.05 ) and malnutrition ( 5.71% vs. 3.93%, P<0.05 ) were significantly higher, while the detection rates of poor vision ( 51.61% vs. 62.72%, P<0.05 ), dental caries ( 24.46% vs. 30.63%, P<0.05 ) and elevated blood pressure ( 14.61% vs. 15.43%, P<0.05 ) were significantly lower in boys than in girls. The detection rates of overweight and obesity ( 30.17% vs. 28.91%, P<0.05 ) and poor vision ( 61.55% vs. 50.04%, P<0.05 ) were significantly higher, while the prevalence rates of malnutrition ( 4.55% vs. 5.24%, P<0.05 ), dental caries ( 24.47% vs. 32.73%, P<0.05 ) and elevated blood pressure (14.66% vs. 15.63%, P<0.05) were significantly lower in students from urban areas than from suburban areas. In addition, the prevalence of malnutrition and poor vision appeared a tendency towards a rise, while the prevalence of overweight and obesity, dental caries and elevated blood pressure appeared a tendency towards a decline among primary and middle school students with the study period (P<0.05 ).
Conclusions
The prevalence of poor vision, overweight and obesity, and dental caries is relatively high among primary and middle school students in Xinjiang Production and Construction Corps, and comorbid overweight and obesity and malnutrition is found.
5.Application value of different minimally invasive approach in treatment of patients with intra-articular calcaneal fractures

Objective To compare the application value and the clinical curative effect of longitudinal incision and tarsal sinus minimal-ly invasive approach in treatment of patients with intra-articular calcaneal fractures. Methods A retrospective statistical analysis was made by collecting and comparing the clinical data of 67 patients with intra-articular calcaneal fractures from March 2008 to March 2012,and they were divided into the longitudinal incision minimally invasive approach group ( ZW group,36 patients,37 feet) and the tarsal sinus minimally invasive approach group ( FW group,31 patients,33 feet) . The Bohler angle, Gissane angle before and after operation,complications healing time,and AOFAS scores were compared. Results The healing time,Bohler angle,Gissane angle of ZW group and other indicators were better than those of the FW group,but there was no significant difference between the two groups (P>0. 05). The infection of incision and compli-cations in ZW group was obviously less than those of the FW group (P<0. 05). According to the AOFAS scoring system,ZW group scored 82. 49 points while FW group scored 86. 53 points,which indicated no statistical significance (P>0. 05). Conclusion The two kinds of treatments are of no obvious difference,but the longitudinal small incision minimally invasive approach could receive better effect in terms of soft tissue damage,healing time,and postoperative complications.
6.Preparation and primary application of 14-3-3 specific polyclonal antibody

Objective Preparation of specific antibody of neuron protein 14 3 3 and application for the diagnosis of prion diseases. Methods The rabbits were immunized with prokaryotic expressed GST 14 3 3 fusion protein and the collected antiserum was evaluated by ELISA and Western blot assays. Results ELISA revealed that the reactive titer of the prepared antibody against purified 14 3 3 protein was up to 1:4 096 000; Western blot assays confirmed that both prokaryotic expressed,and the natively presenting 14 3 3 proteins in the brain extracts were recognized by the prepared antibody. Two cases of clinically diagnosed Creutzfeldt Jakob disease (CJD)with typical EEG changes showed the positive 14 3 3 protein in cerebral spinal fluid (CSF). Four patients with dementia and narcoma but without typical EEG change were 14 3 3 negative in CSF,three of them being even further tested by brain biopsies without typical histological change of CJD and presentation of protease resistant PrP Sc protein. 35 samples CFS collected from patients without dementia were 14 3 3 negative in CSF. All the results were the same as that of standard 14 3 3 polyclonal antibody.Conclusion Using purified prokaryotic expressed GST 14 3 3 fusion protein as antigen,the specific antibody was elicited in the immunized animals. The prepared antibody can be used in identification of 14 3 3 protein in CSF for the diagnosis of CJD.
7.Circulating exosomal inflammation-related protein S100A8 as a potential biomarker for the severity of diabetic retinopathy

Objective:To observe the expression of S100A8 in plasma exosomes, microvesicles (MV), plasma and vitreous in patients with diabetic retinopathy (DR), and verify it in a diabetic rat model, and to preliminarily explore its role in the occurrence and development of DR.Methods:A case-control study. From September 2018 to December 2019, a total of 73 patients with type 2 diabetes, hospitalized patients undergoing vitrectomy, and healthy physical examinations in the Tianjin Medical University Eye Hospital were included in the study. Among them, plasma were collected from 32 patients and vitreous fluid were collected from 41 patients, which were divided into plasma sample research cohort and vitreous sample research cohort. The subjects were divided into simple diabetes group (DM group), non-proliferative DR group (NPDR group) and proliferative DR group (PDR group) without fundus changes; healthy subjects were regarded as normal control group (NC group). In the study cohort of vitreous samples, the control group was the vitreous humor of patients with epimacular membrane or macular hole. Plasma exosomes and microvesicles (MVs) were separated using ultracentrifugation. Transmission electron microscopy, nanometer particle size analyzer and Western blot (WB) were used to characterize exosomes and MVs. The mass concentration of S100A8 was determined by enzymelinked immunosorbent assay. Eighteen healthy male Brown Norway rats were divided into normal control group and diabetic group with 9 rats in each group by random number table method. The rats of diabetes group were induced by streptozotocin to establish diabetic model. Five months after modeling, immunohistochemical staining and WB were used to detect the expression of S100A8 in the retina of rats in the normal control group and the diabetes group. t test was used for the comparison of measurement data between the two groups. Results:Exosomes and MVs with their own characteristics were successfully separated from plasma. The concentrations of plasma exosomes and vitreous S100A8 in the PDR group were higher than those in the NPDR group, DM group, NC group, and the difference was statistically significant ( P=0.039, 0.020, 0.002, 0.002, P<0.000,<0.000). In the plasma sample cohort study, It was not statistically significant that the overall comparison of the S100A8 mass concentrations of plasma and plasma MV in the four groups of subjects ( F=0.283, 0.015; P=0.836, 0.996). Immunohistochemical staining showed that retinal ganglion cells, bipolar cells, cone rod cells and vascular endothelial cells in the diabetic group all expressed S100A8 protein. Compared with the normal control group, the expression level of S100A8 in the retina of the diabetic group increased, and the difference was statistically significant ( t=8.028, P=0.001). Conclusions:The level of S100A8 protein in circulating exosomes increases significantly with the severity of DR in patients with type 2 diabetes. S100A8 may be an influential factor in the inflammatory environment of DR and a potential anti-inflammatory therapeutic target.
8.Preliminary study on positioning of Damon Q self-ligating brackets in a digital integration model.

OBJECTIVEThis study was performed to determine the feasibility of digital virtual positioning of braces in clinical applications.
METHODSWe determined the positions of brackets in 28 teeth of 15 cases according to positioning methods that use bracket height or root information. Final status was generated by OrthoRx software. Treatment results were assessed with American board of orthodontics objective grading system (ABO-OGS).
RESULTSThe deduction points in the marginal ridge height were significantly decreased after treatment using the two methods (P<0.05). However, no statistically significant difference was found in the deduction points in the marginal ridge height between the two methods (P>0.05). No improvement was found in the deduction points in root parallelism with both methods. No statistical significance was found in the deduction points in root parallelism before and after treatments using both methods. The deduction points in alignment and the three scoring components were significantly decreased after treatment using the two positioning methods. In addition, statistically significant differences were found in the deduction points in alignment between two methods (P<0.05).
CONCLUSIONBy using OrthoRx software, we achieved effective therapeutic treatment in reconstructing three-dimensional digital modes using two different bracket-positioning methods. The positioning method that used root information is more accurate compared with the bracket height positioning method. This study provides an experimental basis for bracket accuracy in the mouth.

9.Relationship between quantitative detection of Tiam1 mRNA and invasion, metastasis and prognosis of patients with colorectal cancer

Objective To investigate the relationship between quantitative detection of T lymphoma invasion and metastasis inducing factor (Tiaml) mRNA and invasion,metastasis and prognosis of patients with colorectal cancer.Methods The quantitative expression of Tiam1 mRNA in cancer tissues in colorectal cancers (n =97),in tissues of colorectal adenomatous polyposis (n =49) and normal control (n =30) were detected with real-time fluorescence quantitative-polymerase chain reaction (PCR).The relationship between the quantitative expression of Tiam1 mRNA in colorectal cancer tissues and the invasion,metastasis and the prognosis were analyzed.Results The levels of Tiam1 mRNA expression in the colorectal cancer group,were significantly higher than those in colorectal adenomatous polyposis group and normal controlgroup [(14.45±4.87)2-△△△Ct vs (3.77 ±3.15)2-△△Ct,(1.06 ±0.65)2-△△Ct,P <0.01].The levels of Tiam1 mRNA expression in the adenomatous polyposis with high grade intraepithelial neoplasia were signifiCantly higher than those in the adenomatous polyposis with low grade intraepithelial neoplasia [(5.26 ±2.45)2-△△Ct vs (3.78 ±2.19)2-△△Ct,P <0.01],which were both significantly higher than those in the adenomatous polyposis with no intraepithelial neoplasia [(2.13 ± 1.64)2-△△Ct,P <0.05 or P < 0.01].The levels of Tiam1 mRNA expression in colorectal cancer group were positively related to the differentiated degree of tumor,depth of invasion,lymph node metastasis,tumor node metastasis (TNM) stages and the postoperative recurrence (P < 0.01),and were negatively related to the postoperative survival time (P <0.01).Multivariate Cox regression analysis revealed that levels of Tiam1 mRNA expression in cancer tissues were independent prognostic factor for colorectal cancer (Exp =4.043).Conclusions The high Tiam1 mRNA expression levels in cancer tissues are closely related to invasion,metastasis and poor prognosis of colorectal cancer,and may be a prognostic indicator of colorectal cancer.The increase expression of Tiam1 mRNA in colorectal adenomatous polyposis may be related to colorectal tumorigenesis.
10.Hashimoto's encephalopathy with disturbance of consciousness and dementia as initial symptoms: report of 2 cases and literature review

Two eases of Hashimoto's encephalopathy (HE) are reported and relative literature is reviewed in this article.Both cases were female,with disturbance of consciousness and dementia as initial symptoms.Marked increase of anti-thyroid antibodies was found in serum of the patients.The symptoms disappeared after steroids and maintenance therapies.A total of 126 cases of HE in Chinese literature were retrieved from 2006 to 2015 through the databases of China National Knowledge Infrastructure and Wanfang Med Online.The male/female radio was 1 ∶ 6 with an average age of 51.3 years.The clinical manifestations included cognitive impairment (44.5%),epilepsy (38.3%),psychiatric disorders (27.3%),stroke-like symptoms (25.8%),disturbance of consciousness (24.2%),and extrapyramidal symptoms (20.3%).Good response to steroids was observed in 77.2% HE patients.Analysis indicates that HE is an autoimmune disease with the presence of neurological and psychiatric manifestations,elevated titers of anti-thyroid antibodies in serum and well response to steroids,with a good prognosis.
