Objective To explore the feasibility and cosmetic results of video-assisted subcutaneous mastectomy and immediate reconstruction. Methods Two patients who suffered from breast diseases and need to excise the breasts received a video-assisted total subcutaneous mastectomy and immediate reconstruction in our center. Results Both patients felt satisfied on the cosmetic result. According to the operative experience of these two patents combined with review, the key point of the operation was summarized as follows. The incisions on the anterior axillary line were small and concealing. External retraction method was used to establish the working space. Video-assisted operation was combined with euthyphoria. Blunt dissection was combined with electrical surgery unit and ultrasound knife to isolate the breast skin flap. After breast glandectomy was completed, silica gel prosthesis was put into the interspace behind ectopectoralis. Pressure dressing for 3 weeks was done to prevent the prosthesis upward displacement. Conclusion The video-assisted subcutaneous mastectomy and immediate reconstruction is a safe and feasible operation method with the better cosmetic result.
3.Effects of Jianpi Xiaopi extract on gastric secretion of spleen deficiency rats

Objective To observe the effects of Jianpi Xiaopi extract on gastric secretion of experimental SD rats with spleen deficiency. Methods Animal model of spleen deficiency was established by using Rhubarb decoction. The gastric secretion was determined by titration method, and the gastrin in plasma was measured by radioimmunoassay. 4.Coating for dexamethasone-eluting intravascular stent and its drug sustained release in vitro

Objective To prepare the coating of dexamethasone-eluting intravascular stent and observe the features of its delayed release in vitro.Methods Spray painting and dipping coating were used respectively to prepare stent coating,and then high performance liquid chromatography(HPLC)was used to detect the drug loading.The stents prepared by dipping coating were set in a flow of 50 ml PBS buffer at a velocity of 20 ml/min,and 0.5 ml buffer was collected at 6 h and 1,2,4,6,8,10,12,15 d respectively with another 0.5 ml fresh PBS being added immediately.The drug content of the 0.5 ml buffer was detected by HPLC for drug sustained release in vitro.Results The drug loading of stents prepared by spray painting was(24.26?5.23)?g,while that stents prepared by dipping coating for 4 d was(93.15?7.83)?g.Drugs were released slowly and the release rate reached 84% at 15 d.Conclusion The dipping coating-prepared stents have evident effect of sustained release.
5.Study of placebo used in the Chinese medicinal clinical trail

Objective To study the influencing factors of the colorants used in the placebo of herbal medicine.Methods The effect of pH value,temperature,and illumination and sterilization method on the coloring of the pigments like carmine,sunset yellow 60,brilliant blue 60 and caramel were investigated by determining the Lab value with the instrument.Results PH value of solution had effects on the coloring of carmine and sunset yellow 60,but no effects on the brilliant blue 60 and caramel.When pH values of the solutions were adjusted to 10,chromatic aberration values of carmine and sunset yellow 60 were 17.29 and 27.29,respectively; In the conditions of acidification,the color of carmine and sunset yellow 60 solutions were not changed much; short heating and illumination had no effects on all the four pigments,and all the chromatic aberration values were less than 1.5; 60Co-ray irradiation had great effects on the coloring,and the chromatic aberration values of carmine,sunset yellow 60,brilliant blue 60 and caramel solutions after 60Co-ray irradiation were 12.43,22.45,37.53 and 19.22,respectively; While autoclaving (121 ℃,20 min) would not change the color of solution,and all those chromatic aberration values were less than 1.5.Conclusion These colorants are suitable for the placebo preparation of Chinese medicine decoction,which is recommended to be added in the preparation of the final link.60Co-ray irradiation sterilization is forbidden.
6.A study on expression of basic fibroblast growth factors in periodontal tissue following orthodontic tooth movement associated with low power laser irradiation.

OBJECTIVEThe purpose of this study was to investigate the effects of low power laser on basic fibroblast growth factors (bFGF) expression in periodontal tissue during tooth movement.
METHODS18 white rabbits were randomly divided into 6 groups with 3 rabbits in each group, including groups of 1, 3, 5, 7, 14 and 21 days. Under an anesthesia condition by 2% pentobarbital sodium, the stainless coil springs were fixed between the first maxillary molar and the incisor producing the force of 80 g. The right side of maxilla was considered as the experimental group under the irradiation of low power laser with the left side as the control groups. The expression of bFGF was investigated half-quantitatively through immunohistochemical analysis.
RESULTSThe expression of bFGF in periodontal tissue with irradiation of low power laser was higher than the control side. There were significant differences among the 5, 7, and 14 day groups. In the tension area of the experimental side, the expression of bFGF in the osteoblastic surface of alveolar bone was characteristically greater than that of the control side.
CONCLUSIONThe laser of low power promotes the expression of bFGF in the periodontal tissue and alveolar bone remodeling.

7.Preliminary exploration on interactive follow-up mode for rehabilitation ofpostoperative breast cancer patietns in new medical environment

Objective To evaluate the effectiveness and prospect of innovative interactive follow-up mode for rehabilitation ofpostopera-tive breast cancer patietns in new medical environment. Methods The novel patient-centered medical care was promoted,and Pink Ribbon Clubs was established in different regions. Meanwhile,the patients’ self-support groups were formed, and a specialist for out-patients follow-up of outpatients by telephone or on the internet was arranged to provide psychological assessment,with timely and effective response. All data concerning to the follow-up were recorded and the patients were required to complete the relevant questionnaires. Results Compared with the same period in 2013,the follow-up rate was significantly increased to 80. 25%,and the patients showed higher satisfaction. The incidence of lymphedema in the patients was significantly reduced from 13% in 2013 to 7%. The quality of life was significantly improved and self-rat-ing score on depression was decreased from (51. 90 ± 8. 55) to (35. 81 ± 6. 21). Conclusion The kind of“Trinity” interactive mode is an effective follow-up mode with the clinical benefit in patients’ rehabilitation of physical and mental health,which is worthy of further applica-tion.
8.Feasibility of ultra-large pitch Turbo Flash scan mode in chest scan in preschool children without sedatives

Objective To investigate the feasibility of chest examination using ultra-large pitch Turbo Flash in Force CT in preschool children without sedative.Methods A total of 90 preschool children evaluated as nervous or frightened were equally divided into 3 groups according to the scanning time.For group A,the routine scan with the mode of CARE Dose 4D/CARE kV was performed,and the sedative was used.For group B,the scan with ultra-large pitch Turbo Flash model and pitch as 3.0 was performed,and the sedative was used.For group C,the same scan mode as group B was performed but without sedative.The scanning time,radiation dose,image quality and the diagnostic efficiency were compared among 3 groups.Results There were no significant differences of the CT values of the aorta root,spinal posterior muscles and subcutaneous fat,the noise of the aorta root,SNR and CNR among 3 groups (all P>0.05).Moreover,no significant differences of the subjective score of image quality in pulmonary window and mediastinum window images were found among 3 groups (both P>0.05).The scanning time and radiation dose indexes (CT dose index volume [CTDIvo1],doselength product [DLP],effective dose [ED]) of group A were significantly higher than those of group B and group C (all P <0.05),while there were no significant differences between group B and group C (all P>0.05).There were no significant differences between the CT diagnosis and clinical diagnosis results in all 3 groups (all P>0.05).Conclusion It can shorten the scanning time and decrease the radiation dose by using the mode of ultra-large pitch Turbo Flash.And the satisfactory image quality can be obtained without sedative for nervous or frightened preschool children.
9.MICROSURGICAL ANATOMY OF THE SKIN FLAP OF LATERAL BRACHIUM

The arterial source of the flap,the anastomoses of cutaneous arteries in theskin and subcutaneous tissue and nervous distribution in the flap were observed andsurveyed in 42 upper limbs of adult cadavers.1.The arterial source of the flap comes mainly from the cutaneous branches ofprofund brachial artery,radial collateral artery,lateral humeral cutaneous artery andcutaneous branches of the posterior circumflex humeral artery.In most cases theprofund brachial artery and radial collateral artery may be served as the vascularpedicle of the flap of lateral brachium in transplantation.2.The cutaneous arteries in this flap anastomose each other to from a networkin the skin and subcutaneous tissue.Cutaneous arteries arising from the medial brac-hial region and the upper part of forearm also participate in the formation of thisvascular network.3.The veins of the flap contain both superficial and deep groups:The superfi-cial group is the cephalic vein of brachium which goes upward along the lateralsulcus of m.biceps brachii and its outer caliber is somewhat wider;the deep groupfollows the profund brachial artery or radial collateral artery as their venae comit-antes.Both groups may be sutured together or separately with veins of the recipientin skin grafting.4.The lateral brachial cutaneous nerve and posterior antebrachial cutaneousnerve pierce through the lateral intermuscular septum at various levels,and innervateover the skin in lateral brachial and posterior forearm regions.Since the posteriorbrachial cutaneous nerve is accompanied closely by the radial collateral artery,muchattention should be paid to it in cutting skin flap.5.The extent of cutting a skin flap in lateral brachial region can be enlargeddue to free anastomoses with arteries of adjacent regions which was demonstrated byperfusing red ink into profund brachial artery.The flap of lateral brachium maybe subdivided into following three parts:the upper,middle and lower,the vascularpedicle of which are the cutaneous branch of posterior circumflex humeral artery,lateral humeral cutaneous artery and profund brachial artery(or radial collateralartery)respectively.
10.Cerebral infarction complicated with multiple arterial thrombosis caused by cystathionine beta-synthase gene mutation in youth: a case report

Hyperhomocysteinemia (HHcy) is one of the independent risk factors for youth cerebral infarction. Gene mutation of key enzymes in homocysteine metabolism is the main cause of HHcy. Few cases of cystathionine beta-synthase (CBS) compound heterozygous mutation complicated with pulmonary embolism and lower extremity artery embolism have been reported. This article reported a young cerebral infarction patient complicated with pulmonary embolism and lower extremity artery embolism, who was subsequently detected with significantly elevated blood Hcy, and finally etiologically diagnosed with CBS 833 T>C/1082C>T compound heterozygous mutation. With the treatment of folic acid, methyl cobalt amine, vitamin B 6 and anticoagulant, the blood Hcy has been gradually declined, and no new thrombotic events occurred during the follow-up period of a year.
