1.Analysis of epicardial adipose tissue volume associated with the blank period after radiofrequency ablation of atrial fibrillation recurrence

Objective To explore the correlation of epicardial adipose tissue (EAT) volume with atrial fibrillation (AF) and its recurrence after radiofrequency ablation (RA).Methods Eighty-five AF patients (AF group) and 90 non-AF patients (control group) were chosen between January 2014 and May 2016.Their EAT volumes were measured by CT scanning.Patients in AF group after RA were followed up 6-18 months,and the recurrence of AF was recorded.The recurrence of AF within 3 months of RA was defined as blanking recurrence group (n =27) and non-blanking recurrence group (n =58),and that after 3 months of RA was defined as the 1 ong-term recurrence.Results The total EAT volume and left atrial EAT volume were significantly larger in blanking recurrence group than in non-blanking recurrence group [(118.71 ±28.94) cn3 vs (97.73 ±24.86)cm3,(29.98 ±8.09)cm3 vs (23.11 ±8.30)cm3,t =6.219,4.451,P < 0.01].Multivariate logistic resgression analysis showed that total EAT volume and left atrial EAT volume were the independent risk factors for AF.All showed that total EAT volume and left atrial EAT volume were the independent risk factors for blanking recurrence.The incidence of long-term recurrence was significantly higher in blanking recurrence group [40.7% (11/27)] than in non-blanking recurrence group [15.5 % (9/58)] (x2 =7.142,P < 0.05).Conclusions The incidence of AF is higher in patients with a Iarger total EAT volume and a larger left atrial EAT volume and AF is easier to recur even though after RA.
2.Relation between epicardial adipose tissue thickness and left ventricularremodeling in dilated cardiomyopathy patients

Objective To study the relation of epicardial adipose tissue (EAT) thickness with 1eftventricular remodeling and dysfunction in dilated cardiomyopathy (DCM) patients.Methods One hundred and twenty DCM patients who received treatment from Febuary 2012 to February 2016 were served as DCM group,and 76 healthy subjects undergoing physical examination were served as control group.Their left ventricular end-diastolic diameter (LVEDD),left ventricular end systolic diameter (LVESD),left ventricular end diastolic volume (LVESV),left ventricular end systolic volume (LVEDV),left ventricular end diastolic volume index (LVEDVI),left ventricular end systolic volume index (LVESVI),sphericity index (SI),leftventricular ejection fraction (LVEF),and EAT thickness were measured by routine cardiac ultrasonography and compared between two groups.Results The levels of LVESD,LVEDD,LVESV,LVEDV,LVESVI,LVEDVI and EAT thickness in DCM group were significantly higher,and the levels of LVEF,SIS and SID in DCM group were significantly lower (P<0.05).The EAT thickness in DCM group with NYHA class Ⅱ,Ⅲ,Ⅳ was (8.1 ± 1.8),(7.8 ± 2.0),(7.9 ± 1.7) mm,and there was significant difference (F=1.973,P> 0.05) Linear correlation analysis showed that the EAT thickness was positively related with the LVESD,LVEDD,LVESV,LVEDV,LVEDVI,LVESVI,SISand SID (r =0.247,0.231,0.256,0.267,0.293,0.281,0.261,0.237,P<0.05).There was no relationship between EAT thickness and LVEF (r =0.132,P> 0.05).Logistic multifactor regression analysis showed that EAT thickness was an independent risk factor for left ventricular remodeling in DCM patients (OR =0.793,95%CI:0.431-1.734,P =0.039).Conclusions The EAT thickness is significantly related with the left ventricular remodeling and can be used as an independent risk factor for predicting left ventricular remodeling in DCM patients.
3.Expressions and clinical significances of cyclooxygenase-2,p53 in osteosarcoma

[Objective]To observe the expressional variability and correlation of cox-2 and p53 protein between osteosarcoma and osteochondroma and explore their clinical significance.[Method]Sixty Specimens divided into 2 groups were taken from the department of pathology,Third Hospital of Hebei Medical University during 1998~2004.Among them,40(24 male and 16 female)aged 8~38(18.68?6.47)at time of therapy were from cases of osteosarcoma excision;20(13 male and 7 female)aged 9~27(17.95?4.77)from benign lesion of bone(osteochondroma)operation.Immunohistochemistry staining(Streptavidin-Peroxidasc,S-P method)was performed to detect the expressions of cox-2 protein and p53 protein in 40 cases of osteosarcoma.The criteria of positive expression was that brown granule was found in cytoplasm(cox-2)or in nucleus(p53).The percentage of cox-2 or p53 positive expression in five high power fields was cunted randomly in each case.Positive(-):positive oncocytes account 50%.Software SAS v6.12 was used in data analysis.The difference of cox-2 and p53 expressions between osteosarcoma and osteochondroma was analyzed with Chi-square Test.Spearman method was used,with the standard ?=0.05,to analyze the relationship between cox-2 and p53 expression among osteosarcoma.[Result]The positive rate of cox-2 and p53 expressions were 67.5% and 55.0% in osteosarcoma,there was noticeable distinction between osteosarcoma and osteochondroma,most of them was high.There was a positive correlation between them,rs=0.58,P
4.Distribution and antimicrobial resistance of pathogens causing healthcare-associated infection in a hospital in 2011-2015

Objective To explore the distribution and antimicrobial resistance of pathogens causing healthcare-associated infection(HAI) in a hospital,and provide theoretical basis for rational use of antimicrobial agents and control of HAL Methods HAI occurred in patients in this hospital between 2011 and 2015 were chosen,distribution of major pathogens causing HAI and antimicrobial resistance were analyzed.Results A total of 852 pathogenic strains were isolated,including 458 (53.75 %) strains of gram-negative bacteria,259 (30.40 %) fungi,and 135 (15.85 %) gram-positive bacteria,the top 4 pathogens were Klebsiella pneumoniae (n =159,18.66 %),Candida albicans (n =119,13.97%),Acinetobacter baumannii (n =81,9.51%),and Escherichia coli (n =74,8.69 %).Isolation rate of gram-negative bacteria increased from 47.41 % in 2012 to 61.57% in 2015,fungi increased from 24.78% in 2011 to 34.45% in 2014 (2015 was 27.95%),gram-positive bacteria decreased from 26.55% in 2011 to 10.48% in 2015.Resistance rates of Escherichia coli and Klebsiella pneumoniae to imipenem and meropenem were 8.11%-12.58%;Acinetobacter baumannii had the lowest resistance rate to cefoperazone/sulbactam(23.46%),resistance rate of Candida albicans to fluconazole was up to 59.66%,vancomycin-resistant gram-positive bacteria was not found.Conclusion The major pathogens causing HAI are opportunistic pathogens,antimicrobial resistance is serious,the species and distribution varies with different years,it is necessary to enhance specimen detection rate,strengthen the monitoring on pathogen resistance,and use antimicrobial agents rationally.
5.Application of enteral nutrients in bowel preparation of diabetes patients before electronic colonoscopy

Objective To explore the effect of enteral nutrition applied to bowel preparation of diabetes patients before electronic colonoscopy. Methods A total of 172 diabetes patients were randomly allocated to the experimental group and the control group with 86 patients in each group. The experimental groups executed no dietary restriction within 3 days before the examination, and received Ensure 1h before colonoscopy. However,the control group was given low residue diet within 3 days before the examination, and received no Ensure before colonoscopy. The enteral cleanness and incidence of hypoglycemic reaction, abdominal pain and horror was compared between the two groups. Results 72 cases(83.72%) of the experimental group and 70 cases (81.40%) of the control group had better enteral cleanness, and there was no statistical difference in intestinal cleanness between the two groups. In the experimental group the incidence of hypoglycemic reaction, abdominal pain, horror was significantly lower than those in the control group. Conclusions Bowel preparation with unrestricted diet and ingested ensure before colonoscopy can not only ensure intestinal cleanness but also enhance endurance of colonoscopy.
6.Study on performance evaluation indicators of central venous pressure measuring system for critical patients in clinical trials

Objective We aimed to study the performance evaluation indicators of central venous pressure (CVP) measuring system for ICU patients in clinical trials and discuss its impact factors.Methods Ac-cording to the performance evaluation indicators of CVP measuring system based on the Delphi method,a form was created to evaluate the continuous CVP monitoring of 197 ICU patients and then the data were collected.A comparative analysis was performed to verify the factors affecting the performance of CVP measuring system.Results The performance of CVP measuring system was affected by the number of threeway switches,vasoactive drugs and positive end-expiratory pressure,the statistical value was 8.577,-6.773and 3.244.There were no statistically significant differences with respect to performance evaluation indicators of CVP measuring system among the patients with different body positions.Conclusions The performance evaluation indicators of CVP measuring sys-tem proves to be feasible and practical by clinical empirical research.Their influencing factors were dis-cussed and they offer a valuable reference for the determination of the performance of CVP measuring sys-tem in clinical practice.
7.The significance of X-ray imaging in rehabilitation treatment of cervical spondylosis of vertebral artery type

Objective To characterize the x-ray image of vertebral artery type of cervical spondylosis and explore its significance in rehabilitation treatment. Methods The X-ray films of 42 cases of vertebral artery type of cervical spondylosis were measured and analyzed. The treatment approaches including restriction of cervical movement,traction,epidural block and surgery were planned and implemented accordingly. Results The X-ray image revealed the following changes in the patients: the straightened physiologic curvature of the cervical spine,cervical vertebral unsteadiness,zygapophyzeal joint hyperplasia. Various treatment approaches are effective to patients with various manifestations. Conclusion Employing treatment approaches on the basis of X-ray images is of great signi-ficance for the effective treatment of patients with cervical spondylosis of vertebral artery type.

Aim To investigate the protective effect of cinnamic aldehyde ( CA ) on hormone-induced osteo-clasts proliferation and bone resorption in vitro and its molecular mechanisms. Methods RAW264. 7 cells induced into osteoclast were treated with RANKL and M-CSF and then were divided into control group, dexa-methasone ( DEX ) group and different doses of CA (11. 6, 23. 2, 46. 4 μg·L-1 ) groups. OCs were ob-served after tartrate resistant acid phosphatase( TRAP) staining. The cell proliferation was determined by MTT assay at different time points. The expression levels of TRACP5 b in cell cultured supernatants were measured by ELISA. RT-PCR technique was applied to examine the transcriptional levels of RANK and NFATc1 . Re-sults In MTT assay, the proliferation of osteoclasts stimulated by dexamethasone was promoted seriously compared with negative control group ( P < 0. 05 ) . Meanwhile, DEX could strengthen the content of TRACP5 b and up-regulate the expressions of RANK and NFATc1 mRNA. After administration of CA, the proliferation was inhibited, while the enhanced expres-sion of TRAP5b was reversed,and the over-expressions of RANK and NFATc1 mRNA were obviously down-regulated in a time-and-dose-dependent manner ( P <0. 05 ) . Conclusion The results suggest that CA in-hibits proliferation and bone resorption of osteoclast in-duced by DEX, which may be mediated by down-regu-lation of RANK and NFATc1 mRNA.
