1.CT scan findings of primary pulmonary non-Hodgkin's lymphoma and their relation to pathological features.

OBJECTIVETo investigate the CT scan findings of primary pulmonary non-Hodgkin's lymphoma (PPNHL) and their relation to pathological features.
METHODSThe CT scan findings of 17 PPNHL cases confirmed pathologically from August 1992 to June 2007 were analyzed in relation to their pathological foundation.
RESULTIn 17 cases the CT findings showed: 1) Five were single mass or nodule, including 3 with air-bronchograms, 1 with straddling-lobe distribution and 3 with slight enhancement; 2) One was multi-nodules with air-bronchograms; 3) Six were consolidation with air bronchogram,including 3 with straddling-lobe distribution and 3 with slight enhancement; 4) Five were combined changes, including 5 with air-bronchograms, 2 with straddling-lobe distribution and 4 with slight enhancement. The CT features of PPNHL corresponded to their pathological process and pathological features.
CONCLUSIONPrimary pulmonary non-Hodgkin's lymphoma should be considered when CT scan shows variety of images found in one patient, middle lobe of right lung involved, air-bronchograms and straddling-lobe distribution in consolidation with slight enhancement.

2.Surgical treatment of Lisfranc fracture-dislocations by reconstruction of three-column with Kirschner wire fixation.

OBJECTIVETo explore the effect of reduction and fixation with multi-Kirschner wires for treatment of Lisfranc fracture-dislocations.
METHODSThere were 49 patients (37 male and 12 female aged from 20 to 28 years old) involved in the study. According to the Myserson damage typing, type A in 12 cases, B1 in 3, B2 in 28, C1 in 4 and C2 in 2. Kirschner wires were applied to fix and reconstruct the three-column in three directions according to the structural characteristic of midfoot. Reconstruction of three-column needed not only to reduce and to fix every single column, but also to establish union of the columns.
RESULTSAccording to the evaluation of AOFAS for midfoot, 14 cases were excellent (90 to100), 22 cases good (80 to 89), 8 cases fair (70 to 79) and 5 cases poor (60 to 69), with an average score of 84.200+/-9.663.
CONCLUSIONThe diagnosis and treatment of Lisfranc fracture-dislocations should comply with the theory of three-column reconstruction of foot arch, which can achieve the static balance of biomechanics and provide a stable environment for healing of fracture and soft tissue. Reduction and fixation with multi-Kirschner wires is an effective treatment method for Lisfranc fracture-dislocations.

3.Constructing Support Vector Machine Ensembles for Cancer Classification Based on Proteomic Profiling

In this study, we present a constructive algorithm for training cooperative support vector machine ensembles (CSVMEs). CSVME combines ensemble architecture design with cooperative training for individual SVMs in ensembles. Unlike most previous studies on training ensembles, CSVME puts emphasis on both accuracy and collaboration among individual SVMs in an ensemble. A group of SVMs selected on the basis of recursive classifier elimination is used in CSVME, and the number of the individual SVMs selected to construct CSVME is determined by 10-fold cross-validation. This kind of SVME has been tested on two ovarian cancer datasets previously obtained by proteomic mass spectrometry. By combining several individual SVMs, the proposed method achieves better performance than the SVME of all base SVMs.

6.Expression of tissue factor in rabbit pulmonary artery in an acute pulmonary embolism model

BACKGROUND:Tissue factor (TF) is the initiation factor of the extrinsic coagulation pathway, and plays a critical role in the process of thrombosis. This study aimed to investigate the expression of TF and to explore their clinical effect on the pulmonary artery after acute pulmonary thromboembolism. METHODS:Thirty-four Japanese white rabbits (Level II animals) supplied by Tianjin Medical University were randomly assigned into:group A, specimens of the pulmonary artery taken 3 hours after pulmonary embolism (n=8); group B, specimens of the pulmonary artery taken 8 hours after pulmonary embolism (n=8); group C, specimens of the pulmonary artery taken 24 hours after pulmonary embolism (n=8); and control group, pseudo-operations performed without injection of autologous blood clots (n=10). The animal model of pulmonary thrombo-embolism was established by injection of autologous blood clots into the jugular vein through a 5F catheter, and was confirmed by digital subtraction angiography. The mRNA expression of TF in different parts of the pulmonary artery was accessed by RT-PCR. Theq test was used if there was a significant difference in a given continuous variable among the three groups assessed by ANOVA. The experiment equipment was supplied by the State Key Laboratory of Experimental Hematology, Institute of Hematology and Blood Diseases Hospital, the Chinese Academy of Medical Sciences and Peking Union Medical College. RESULTS:The TF expression in the specimen adjacent to emboli was stable at 3, 8 or 24 hours after embolism. The mRNA expression of TF at 3 and 8 hours after embolism was lower in the specimens taken from the distal end of the morbid pulmonary artery than those adjacent to emboli. While at 24 hours after embolism, there were similar mRNA levels in specimens either adjacent or distal to emboli. CONCLUSION:The high level of TF expression in pulmonary artery tissue adjacent to emboli could lead to locally increased coagulation activity, indicating the necessity of initiating anti-coagulation treatment as soon as possible after acute pulmonary embolism.
7.Conversion from bladder to ileal drainage for the treatment of metabolic acidosis following simultaneous pancreas and kidney transplantation (a case report)

Objective To investigate the therapeutic method of metabolic acidosis in long-term sur- vival patients undergoing simultaneous pancreas and kidney transplantation.Methods A 45-year-old fe- male patient,who had undergone simultaneous pancreas and kidney transplantation(due to diabetic ne- phropathy and uremia)with bladder drainage 2 years before,developed severe metabolic acidosis,and thus underwent surgical conversion from bladder to ileal drainage.The procedure was as follows.The stoma of duo- denocystostomy was isolated and resected.The site of cystostomy was closed in two layers.The graft duode- num was then anastomosed to a loop of the recipient's ileum,which was proximal 40 cm from the ileocecum in a side-to-side manner.Results The metabolic acidosis resolved postoperatively.The patient received conventional immunosuppressants.The hospital stay was 30d.Follow-up of 4 years showed normal pancreas and kidney functions.Conclusions Conversion from bladder to ileal drainage is safe and effective for metabolic acidosis related to the exocrine secretions of bladder drained pancreas graft in simultaneous pancre- as and kidney transplant recipients.

Background Diabetic nephropathy (DN) is the leading cause of end-stage renal disease.Various treatment regimens and combinations of therapies provide only partial renoprotection.Therefore new approaches are needed to retard the progression of DN.The aim of the present study was to evaluate the role of a novel spiroalkaloid from Acorus tatarinowii named acortatarin A (AcorA) in inhibiting high glucose-induced extracellular matrix accumulation in mesangial cells (MCs).Methods The cytotoxity of AcorA on MCs was examined by 3-(4,5-dimethylthiazol-2-yl)-2.5-diphenyltetrazolium bromide (MTT) assay.The expression of fibronectin and collagen Ⅳ was examined by real time PCR and western blotting.The expression of p22phox and p47phox was detected by western blot.The interaction between p22phox and p47phox was examined by co-immunoprecipitation.The phosphorylation of p47phox was examined by immunoprecipitation.The phosphorylation of protein kinase C (PKC) α,PKCβ,phospholiase C gamma (PLCγ1),and the p85 subunit of PI3K was determined by Western blotting.Results AcorA significantly inhibited high glucose-induced activation of NADPH oxidase,a ROS-generating enzyme,by increasing phosphorylation of p47phox and enhancing interaction between p22phox and p47phox.Preincubation of AcorA with MCs inhibited high glucose-induced collagen Ⅳ and fibronectin production in a dose-dependent manner.Moreover,AcorA attenuated high glucose enhanced phosphorylation of PKCα,PKCβ,PLCγ1,and the p85 subunit of PI3K.Conclusion AcorA inhibits high glucose-induced extracellular matrix production via blocking NADPH oxidase activation.
