1.Clinical analysis of retroperitoneal laparoscopic renal adipose capsule outer pathway in the treatment of chyluria

Objective To evaluate the surgical techniques and clinical efficacy of retroperitoneoscopic ligation of renal lymphatic vessels outside adipose capsule and inside adipose capsule for comparison in the treatment of chyluria.Methods Retrospective anal-ysis of the retroperitoneal laparoscopic adipose capsule of kidney pedicle lymphatic duct ligation(group Ⅰ)and adipose capsule pathways in the kidney totally or subtotally free renal pedicle lymphatic ligation(group Ⅱ)and 1 1 1 patients with chyluria clinical data,compared two groups of operation time,bleeding volume,the rate of analgesia,postoperative gastrointestinal function recovery time,drainage time,postoperative recovery time,postoperative hospital stay and postoperative complications and other index differ-ence.Results Retroperitoneal laparoscopic operation group of adipose capsule in operation time,bleeding volume,postoperative an-algesia,postoperative gastrointestinal function recovery time,drainage time,postoperative recovery time,postoperative hospital stay and postoperative complications were better than the adipose capsule of kidney totally or subtotally free operation group,the differ-ence was statistically significant(P<0.05).Conclusion Retroperitoneal laparoscopic renal adipose capsule of kidney pedicle lym-phatic disconnection for the treatment of chyluria effect,and adipose capsule operation ways,the method less trauma,quicker recov-ery,can completely replace the renal adipose capsule total or Sub-total of free kidney pedicle lymphatic disconnection operation,wor-thy of clinical application.
2.Air pollution network attention and its impact factors on basis of Baidu search indexes

Objective To investigate public network attention to air pollution and their desire to prevent haze. Methods Daily Air Quality Index(AQI) and PM2.5 concentration data and daily Baidu Index data were obtained in 31 provinces of China from January 1st, 2015 to December 31st, 2017.The Baidu indexes of air pollution and its protection were used to compose public′s network attention index for analysis on its spatial distribution in the 31 provinces and for linear regression research on air pollution, its protection and its network attention impact factors. Results With Baidu search index, it was noted that the network attention to air pollution was significantly higher than to air pollution protection.And a significant difference was found in the public network attention to air pollution between different provinces.It was found that education and consumption level of residents significantly affected public network attention to air pollution, while AQI and consumption level of residents significantly affected public network attention to air pollution hazards protection behaviors. Conclusions Even though the public has already paid high attention to air pollution, the awareness of air pollution hazards protection behaviors is still weak.Well-designed education campaigns are needed to enhance public awareness of air pollution and motivate the public to take actions to reduce pollution air exposure, especially in those provinces with a high AQI and PM2.5concentration.
3.Primary development of visual uroflow scale.

OBJECTIVE:
To develop the visual uroflow scale (VUS), analyze the relationship of VUS score and index of free uroflowmetry, assess urination function preliminarily and improve the work efficiency in the clinic.
METHODS:
Male lower urinary tract symptoms (LUTS) patients, who attended the Department of Urology in Peking University People's Hospital from March 2016 to March 2017, were assessed for their urination function according to the Visual Uroflow Scale without help from clinicians before undertaking a free uroflowmetry test. And afterwards, a free uroflowmetry was undertaken, and variables including maximal flow rate (Qmax), the average flow rate (Qave) and voiding volume (VV) was obtained. During the study, 124 cases were collected and 53 cases met the inclusion and exclusion criteria and were included in the study cohort. The Spearman correlation analysis was used for analyzing the correlation of VUS scores with free uroflowmetry variables and age. The validity of VUS was evaluated.
RESULTS:
Most of the patients could choose the very figure matched with self-condition by first instinct without any help from the clinician. The data were analyzed by Spearman correlation analysis. In the present study, voiding time was positively correlated with the VUS score (correlation coefficient, 0.62, P < 0.05). In the present cohort, the patients chose the third and fourth figures to take longer time to urinate, implying worse LUTS situation. Flow time and VUS scores were positively correlated (correlation coefficient, 0.61, P < 0.05). The patients with higher VUS scores would spend more time on urinate, no matter how long urinary hesitation was. Both Qmax and Qave were negatively correlated with the VUS score (correlation coefficient -0.54, -0.62, P < 0.05). The study illustrated that the VUS score suggested that the Qmax basically and further reflected the urination function. And its relationship to age revealed the decreased urination function of aging male, which had reached a consensus.
CONCLUSION
Development of VUS has helped the clinician assess the urination function preliminarily at the first time. Patients are assessed for a VUS score before getting surgery or receiving the drug for treatment, and can be re-assessed after. The VUS score can provide an objective quantitative basis to evaluate the treatment efficacy. In addition, considering that it is convenient, timesaving and easy to understand, the VUS is available for follow-up.

4.Determination of propofol in human serum by improved reversed phase high-performance liquid chromatography with fluorescence detection.

OBJECTIVETo develop a new high-performance liquid chromatographic (HLPC) method for determination of propofol in human serum.
METHODSHuman serum samples were precipitated with 20% perchloric acid and centrifuged to obtain 50 microl of the supernatant for analysis by HPLC coupled with fluorescence detection. The analysis was performed with a C(18) reversed-phase column using a acetonitrile-water (90:10) phase delivered at 1.0 ml/min, with the excitation wavelength of 276 nm and emission wavelength of 310 nm.
RESULTSThe calibration curves were linear (r=0.997 5) within the concentration range of 0.05-10 microg/ml, the limit of propofol quantification was 50 ng/ml and the intra- and inter-day precisions were between -/+15%.
CONCLUSIONSThe method is accurate, sensitive and simple for propofol determination in clinical anesthesia.

5.Impact of previous abdominal operations on the outcome of laparoscopy-assisted radical total gastrectomy.

OBJECTIVETo explore the impact of previous abdominal operations on the outcome of laparoscopy-assisted radical total gastrectomy (LATG).
METHODSClinical data of 328 patients with gastric cancer who underwent LATG from January 2008 to December 2010 were analyzed retrospectively. Among the patients, 57 cases had previous abdominal surgeries (PAS group) and 271 cases had no previous abdominal surgeries (NPAS group). The intraoperative and postoperative parameters were compared, and risk factors associated with postoperative complications were analyzed using univariate and multivariate analysis.
RESULTSThere was no significant difference between the two groups in the mean number of lymph nodes harvested (30.2±10.5 vs. 31.1±9.4, P>0.05). The operative time [(247.0±60.5) min vs. (214.7±57.0) min, P<0.01] and the postoperative complication rate [21.1%(12/57) vs. 11.1%(30/271), P<0.05] were significantly different between the two groups. However, there were no differences in intraoperative blood loss, transfused patients, conversion, postoperative ambulation time, postoperative flatus passage time, resumption of liquid diet, removal of gastric tube, and postoperative hospital stay (all P>0.05). Multivariate logistic regression analysis showed that previous abdominal surgeries was not an independent risk factor associated with postoperative complication morbidity (P>0.05).
CONCLUSIONSLATG is feasible for gastric cancer patients who has previous abdominal surgeries. Previous abdominal surgery may prolong operative time but is not an independent risk factor associated with postoperative complication.

6.MRSA colonization characteristics and nursing of patients in RICU

Objective To investigate the characteristics of MRSA colonization and their influence to the prognosis of patients in respiratory intensive care unit (RICU),so as to provide the basis for the caring of the RICU patients.Methods A prospective study included 246 cases of patients admitted to RICU in our hospital during Jan 2010 to Neb 2011.Swab specimens from nares,pharynx,axillae,groin,chest and hands of the patients were collected and cultured at the first day in RICU,and then repeated every 3 days.The patients were divided into MRSA colonization group (n =30) and MRSA non-colonization group (n =216) according to MRSA culture results.Results The MRSA colonization rate was 12.19% in RICU patients,including 16 cases of hospital-acquired colonization.No significant difference was found in the gender,the APACHE Ⅱ score and APACHE Ⅲ between MRSA colonization group and the MRSA non-colonization group (P > 0.05).The mean age in MRSA colonization group was significantly greater than that of the MRSA non-colonization group [(81.13 ± 6.86) vs (74.78 ± 11.89) ; t =2.955,P =0.003)] ; The number of hospitalization and the mortality in one year of the MRSA colonization group was significantly more than those of MRSA non-colonization group [(1.83 ±1.216) vs (1.06 ±0.257),40.0% vs 16.7% ;t =2.998,x2 =9.132;P<0.01)].MRSA was mainly colonized in nares.Conclusions MRSA colonization has an important impact on the prognosis of RICU patients.More than 50% of MRSA colonization cases are hospital-associated,suggesting that we should strengthen the bedside care,prevention of cross infection.
7.Value of multiparameter flow cytometry in the diagnosis and prognostic evaluation of childhood myelodysplastic syndrome.

OBJECTIVETo investigate the value of multiparameter flow cytometry (MFC) and flow cytometric scoring system (FCSS) in the diagnosis and prognostic evaluation of childhood myelodysplastic syndrome (MDS).
METHODSA retrospective analysis was performed for the clinical data of 42 children who were diagnosed with MDS. MFC was performed to investigate the phenotype and proportion of each lineage of bone marrow cells. The correlations of FCSS score with MDS type, International Prognostic Scoring System (IPSS) score, and revised IPSS (IPSS-R) score were analyzed.
RESULTSOf all the 42 children, 20 (48%) had an increase in abnormal marrow blasts, 19 (45%) had a lymphoid/myeloid ratio of >1, 14 (33%) had abnormal cross-lineage expression of lymphoid antigens in myeloid cells, 8 (19%) had abnormal CD13/CD16 differentiation antigens, 5 (12%) had abnormal expression of CD56, 3 (7%) had reduced or increased side scatter of granulocytes, 3 (7%) had reduced expression of CD36 in nucleated red blood cells, 2 (5%) had reduced expression of CD71 in nucleated red blood cells, 1 (2%) had absent expression of CD33 in myeloid cells, 1 (2%) had reduced or absent expression of CD11b in granulocytes, and 1 (2%) had absent expression of CD56 and CD14 in monocytes. There were significant differences in the median overall survival time and event-free survival time among the low-, medium-, and high-risk FCSS groups (P<0.05). Among the low-, medium-, and high-risk FCSS groups, the low-risk FCSS group had the highest 2-year overall survival rate, while there was no significant difference between the medium- and high-risk FCSS groups (P>0.05). The three groups had a 2-year event-free survival rate of 95%, 60%, and 46% respectively (P<0.05). FCSS score was positively correlated with MDS type, IPSS score, and IPSS-R score (P<0.05).
CONCLUSIONSMFC and FCSS help with the diagnosis and prognostic evaluation of childhood MDS.
8.Comparison of sensitivity of audiological tests to identify otitis media with effusion in newborn infants

Objective To investigate the sensitive factors which Were used in routine audiological tests to find out otitis media with effusion(OME)in newborn infants.Methods Subiects of this study were 48 infants.including 31 males and 17 females.who failed in the universal newborn heating screening.The age ranged from 1.5 to 12 months with the average age of 4.3 months.All subjects accepted temporal bone CT and routine audiological assessments.including air.conduction and bone.conduction auditory brainstem response(ABR),40 Hz-auditory event related potential(40 Hz-AERP),distortion.product otoacoustic emission(DPOAE),acoustic reflex,tympanometries using 226 Hz and 1000 Hz probe tone.Nine factors were statistically analyzed using Kappa test,Univariate X2 test and multivariate condition Logistic stepwise regression analysis.which included the results of acoustic immitance.the air-conduction and boneconduction ABR thresholds,the difierence between air-conduction and bone-conduction ABR thresholds.the latency of ABR wave Ⅰ,duration between ABR wave Ⅰ and Ⅴ,40 Hz-AERP thresholds,amplitudes and thresholds of DPOAE,and acoustic reflex thresholds(ART).Results Seventy-seven ears were diagnosedwith OME.and 19 ears were normal.CT scan of temporal bone was set as a comparative standard.Kappa test indicated that the results of tympanometry with 1000 Hz probe tone(Kappa:0.745,P<0.001),the air-conduction ABR threshold(Kappa=0.453,P<0.001),the latency of ABR wave Ⅰ(Kappa=0.430,P<0.001),the threshold of 40 Hz-AERP(Kappa=0.582,P<0.001),and DPOAE(Kappa=0.495,P<0.001)had agreement with the results of temporal bone CT on evaluating the function of middle ear.Univariate analysis indicated that sensitive factors of middle ear function in newborn infants were tympanometry with 1000 Hz probe tone(P<0.001),ART(P<0.001),the air-conduction ABR threshold (P<0.001),the difierence between air-conduction and bone-conduction ABR thresholds(P<0.001).the latency of ABR wave I(P<0.001),the threshold of40 Hz-AERP(P<0.001)and DPOAE(P<0.001).And multivariate conditional Logistie stepwise regression model showed that tympanometry with 1000 Hz probe tone(P<0.001)and 40 Hz-AERP threshold(P=0.004)can be substituted into Logistic stepwise regression equation.Conclusions Tympanometry with 1000 Hz probe tone and are sensitive factors to find out OME in newborn infants.The air conduction ABR threshold.ABR wave Ⅰ latency,40 Hz-AERP threshold and DPOAE could reflect the middle ear function of newborn infants effectively.

10.Multi-central clinical evaluation of ginger-partitioned moxibustion for treatment of leukopenia induced by chemotherapy.

OBJECTIVETo prove the therapeutic effect of ginger-partitioned moxibustion on leukopenia induced by chemotherapy and effect on life quality of the patient with tumor after chemotherapy.
METHODSRandomized, controlled, multi-central cooperative method was used and the patients confirmed to the enrolled criteria were divided in-to two groups. The test group were treated with ginger-partitioned moxibustion at Dazhui (GV 14), Geshu (BL 17), Pishu (BL 20), etc.; and the control group with oral administration of Chinese patent medicine.
RESULTSOut of the 221 cases confirmed to program analysis, 113 cases were in the test group and 108 cases in the control group. After 10 days, the cured rate and the effective rate were 84.1% and 66.4% in the test group and 35.2% and 33.3% in the control group, respectively, with very significant differences between the two groups (both P < 0.01); fifteen days later, the therapeutic effects in the two groups were maintained. The two methods could improve clinical symptoms, with the test group being better than the control group. Any adverse response was not found in the two groups, and the injuries of functions of the heart, lung and kidney induced by chemotherapy had some improvement.
CONCLUSIONThe therapeutic effect of ginger-partitioned moxibustion on luekopenia induced by chemotherapy is reliable and is better than oral administration of Chinese patent medicine, with a better duplication.
