1.Effect of Joint Mobilization Therapy on Osteoarthritis of Knee Joint

Objective To observe the efficacy and safety of joint mobilization therapy for osteoarthritis of knee joint.Methods 120 patients with osteoarthritis of knee joint were randomly divided into the treatment group and control group with 60 cases in each group. The treatment group were treated with joint mobilization and synthesis rehabilitation therapy, and control group only with synthesis rehabilitation therapy.Results The total effective rate was 90.00% in the treatment group and 76.67% in the control group according to analysis of intention-to-treat, and 92.73% in the treatment group and 80.77% in the control group based on analysis of per-protocol population. There was a significant difference between the two groups ( P<0.05). No adverse effect was found in the treatment group.Conclusion The joint mobilization therapy is efficacy and safety for osteoarthritis of knee joint.
2.Detection of mutations of exons 5 and exons 8 of PTEN gene in oral squamous cell carcinoma

Objective:To detect mutations of exons 5 and exons 8 of PTEN gene in oral squamous cell carcinoma(OSCC),and to explore the relationship between gene mutations and development of OSCC.Methods: Mutations of exons 5 and exons 8 of PTEN gene were detected by polymerase chain reaction-single strand conformation polymorphism(PCR-SSCP).Results: The whole sequences of exons 5 and exons 8 of PTEN gene of all cases of OSCC were expanded.There was no mutation in exons 5 and exons 8 of PTEN gene of all cases of OSCC.Conclusion: There is no mutation in exon 5 and exon 8 of PTEN gene in OSCC.This might show that there is no correlation between mutations in exon 5 and exon 8 of PTEN gene and the development of OSCC.

5.Meta-analysis on acupoint sticking therapy for facial paralysis.

OBJECTIVETo evaluate the therapeutic effect and safety of acupoint sticking therapy for facial paralysis.
METHODSSearch the literatures in the whole paper databank of China figure medical library (Jan. 1994-Dec. 2004) and China Biomedical Literature Disk Databank (Jan. 1995-Dec. 2004). Meta-analysis was conducted with RevMan 4. 2. 5 software.
RESULTSSixteen controlled trials involving 2 157 patients were included. Meta-analysis indicated that there was high statistically difference between the acupoint sticking therapy and simple acupuncture therapy or Western medicine therapy.
CONCLUSIONThe acupoint sticking therapy is effective and safe for facial paralysis.

6.A meta-analysis of treatment of infantile diarrhea with bifid triple viable bacterial tablet.

OBJECTIVETo assess the efficacy and safety of bifid triple viable bacterial tablet in treatment of infantile diarrhea.
METHODSAccording to the requirements of Cochrane systematic review, a thorough literature search was performed among Chinese Digital Hospital Library (www.chkd.cnki.net) and Chinese Biomedical Literature Disk Database (CBMdisk). A meta analysis was performed on a total of 1326 patients involved in 12 papers which met the inclusion criteria.
RESULTSPublication bias analysis showed that the funnel plot was symmetrical. Test for heterogeneity showed that the groups treated with bifid triple viable bacterial tablet and antibiotics or anti-viral agents or placebo control had clinical homogeneity and statistical homogeneity (P = 0.66, 0.67, 0.85, respectively, I(2) = 0%), which allowed to use fixed effect model analysis. The bifid triple viable bacterial tablet and Smecta did not have statistical homogeneity (P = 0.02, I(2) = 70.9%), therefore random effect model analysis was applied. Incorporation analysis showed that in comparison of the bifid triple viable bacterial tablet versus antibiotics or anti-viral agents or placebo control, the odds ratios were 5.34, 4.74 and 6.43, respectively, and 95% CIs were [2.81, 10.16], [2.47, 9.09], and [2.61, 15.83], on test for overall effect, Z = 5.11, 4.67 and 4.04, P < 0.00001. In the forest plot the 95% CI horizontal line of incorporation odds ratio droped to the right side of the vertical line indicating the border of effectiveness. However, no statistically significant difference was found between bifid triple viable bacterial tablet and Smecta.
CONCLUSIONThe clinical evidences available so far indicated that treatment of infantile diarrhea with bifid triple viable bacterial tablet is safe and effective although rigorously designed large sample size randomized double blind clinical trials are required to further demonstrate and support the conclusion.

7.A primary study of arytenoid within-shift combining with recurrent laryngeal nerve transposition

OBJECTIVE To study the therapeutic effect of arytenoid within-shift combining with recurrentlaryngeal nerve (RLN) anastomosis to ansa cervicalis for patients with long-term unilateral vocal cord paralysis (UVCP) . METHODS Twelve patients with traumatic RLNs paralysis were studied. The courses of injury varied from 3 to 22 years. RLN was anastomosed to ansa cervicalis and arytenoid was within-shifted on ipsilateral side in all patients. Therapeutic efficacy was evaluated by laryngoscope, strobe-laryngoscope, voice evaluation, voice acoustics parameter analysis, and laryngeal electromyography (LEMG) . RESULTS The voice was improved in all cases after arytenoid within-shift immediately. The mean values of voice acoustics parameters (jitter, shimmer, NNE) were significantly smaller and maximum phonation time (MPT) were significantly longer after operation compared with those of before treatment. Laryngoscope showed that arytenoid was significantly within-shifted. The glottic posterior chink vanished in 9 cases and reduced in 3 cases. The voice resumed normal, obviously improved, improved and no improved were found in 9、 3、0、0 cases respectively 12 months after operation. The mass and tension of all treated vocal cords may become the same as the contralateral normal vocal cords, thus resuming symmetric vibration of the vocal cords. The reinnervation of the RLNs were approved by LEMG in these patients whose RLNs were restored. CONCLUSION Arytenoid within-shift combining with RLN anastomosis to ansa cervical can restored normal voice in patients with long-term UVCP.
8.Cost-utility analysis of comprehensive intervention on epilepsy

Objective To discuss the cost-utility regarding the effect of comprehensive intervention program on epilepsy.Methods 874 cases with epilepsy were investigated in 9 pilot sites in Hcnan province.Cost of epilepsy comprehensive intervention was calculated based on the research projects of epilepsy prevention and management of rural areas.Disability-adjusted life year (DALY)and quality adjusted life year (QALY) were used as utility index for cost-utility analysis.Results Of the 874 cases,8.58 DALY was lost and 27.95 QALY was obtained per capita in the intervention group (422 cases).The costs per QALY and DALY obtained were 2454.10 and 7995.40 Yuan,respectively.Relative to the non-intervention control group (452 cases),the costs for QALY obtained by the intervention group had reduced.The investment would reduce 21 735.10 Yuan when each additional DALY was saved.Conclusion Epilepsy caused serious economic burden to the society,families and the patients themselves.Comprehensive intervention program could reduce the economic burden of the disease thus yielding high cost-benefit.This program was deserved to be promoted.
9.Clinical observation and mechanism study on acupuncture combined with massage for treatment of vertebrobasilar insufficiency.

OBJECTIVETo search for the best program increasing clinical therapeutic effect on vertebrobasilar insufficiency (VBI) and to study the mechanism.
METHODSThe patients were randomly divided into an acupuncture plus massage group, an acupuncture group and a massage group. Their therapeutic effects were observed and compared. And transcranial Doppler (TCD) was used for detection of systolic flow velocity (Vs), enddiastolic peak flow velocity (Vd), mean peak flow (Vm) and pulsatility index (PI) before and after treatment.
RESULTSThe cured rate and the total effective rate in the acupuncture plus massage group were significantly higher than the acupuncture group and the massage group (P < 0.05). And there were significant differences in Vm, PI and DI in the left vertebral artery (LVA), the right vertebral artery (RVA) and the basilar artery (BA) before and after treatment (P < 0.01).
CONCLUSIONAcupuncture combined with massage can improve Vm, PI, DI of VA-BA and the clinical therapeutic effect on VBI as well. It is superior to the simple acupuncture therapy and simple massage therapy. At present, it is one of the best therapy for VBI.

10.High expression and identification of DNA mismatch repair gene mutS in Escherichia coli.

DNA mismatch repair gene mutS (2.56 kb) was PCR modified and cloned into a secretive prokaryotic expression vector pET32a (+) which carries a N-terminal His.tag + and thioredoxin sequence. MutS protein was expressed with high level after IPTG induction using the strain E. coli AD494(DE3). SDS-PAGE revealed that the expected protein with a molecular weight of 108 kD which is about 35% of the total bacterial proteins is almost soluble. The expected protein was purified directly by immobilized metal (Ni2+) chelation affinity chromatography and the purity is over 90%. MutS protein activity verified using mismatch DNA showed that the expression product can recognize and bind to base-pair mismatch specifically.
