3.Clinical observation on controlling antihypertension with the general anesthesia of TEAS and anesthetics in endoscopic endonasal surgery.

OBJECTIVETo study whether the dose of controlling antihypertensive drug is reduced by transcutaneous electrical acupoint stimulation (TEAS) and the anesthetics, as well as the control of blood pressure (BP) and heart rate (HR) in endoscopic endonasal surgery with general anesthesia.
METHODSSixty patients for selective endoscopic endonasal surgery with general anesthetics and controlling antihypertension involved were selected and randomized into a TEAS group, a sham-TEAS group, 30 cases in each one. The electric pads were attached to bilateral Hegu (LI 4), Zusanli (ST 36), Sanyinjiao (SP 6) and Quchi (LI 11), stimulated with Hans-200 apparatus, 3 to 5 mA, 2 Hz/100 Hz in the TEAS group based on the patients' response to comfort. No electric stimulation was applied to the sham-TEAS group. The general anesthesia started after 30 min intervention and lasted till the end of surgery. The BP and HR were observed and recorded at the end of monitoring in operation room, 10 min after tranquilization (T0), 30 min after intervention (Tj, after induction~of general anestiesa (T2), 30 min after surgery start (T3), 60 min after surgery start (T4) and 30 min after extubation (T5). The doses of vecuronium bromide, propofol and nitroglycerin were recorded statistically in surgery, as well as the operative bleeding volume, the operative time, the resuscitation time and the visual analogue scale (VAS) score after resuscitation.
RESULTSCompared with that at T0, the mean arterial pressure (MAP) at T2, T3, T4 and T5 in the TEAS group and at T3 and T4 in the sham-TEAS group was all reduced, indicating the significant difference (all P < 0.01). MAP at T2 and T5 in the TEAS group was lower than that in the sham-TEAS group (both P < 0.01). Compared, with that at T5, except at T2 in the TEAS group (P<0. 05), HR was not different significantly at the rest time points (all P > 0.05). HR was different at T2 to Ts in the sham-TEAS group statistically (all P < 0.01). The doses of vecuronium bromide, propofol and nitroglycerin, the operative bleeding volume, the operative time, the resuscitation time and VAS after resuscitation were not different significantly between the two groups (all P > 0.05).
CONCLUSIONThe general anesthesia with TEAS and anesthetics involved for controlling antihypertension contributes to the control of BP and HR in the patients in endoscopic endonasal surgery. The impacts are not obvious on the doses of antihypertensive drug, the general anesthetics, the operative bleeding volume, the time of resuscitation and the postoperative analgesia.

4.Bibiometric analysis on papers produced by Peking university hospital

Objective To evaluate the academic status of the researchers in pediatric department of Peking university first hospital, we conducted bibliometric analysis of research on the scientific publications. Methods The total number, first authors, and the citing and cited conditions of these papers published from 1999 to 2000 were analyzed by bibliometric methods. Results There were total 513 papers including 275 original articles published in 59 journals from 1999 to 2003.There were 238 papers written in Chinese, of which 188 papers (79 %) were from the core journals. In the 37 original articles written in English, 31 papers(83.8 %) were select-ed by science citation index(SCI).The average of impact factors (IF) of sourcing journals was 1.447(0.107 -7.717). The averages of IF per year ranged from 0. 768 to 2.206.The original articles selected by SCI were totally cited 38 times. Every article was cited 1.41 times in average.Of all 275 papers,each had an average of 9.8 citing reference, increasing from 8.1 in 1999 to 12.0 in 2003.About 80.7 % of those references were written in foreign languages.The Price's index of those papers was 43.2%.Conclusions The number of papers kept improving in the past 5 years, and had a significant improvement in 2003.The authors showes great talents to use English articles as their main information sources. But the utilization of the last published papers in this subject should be im-proved.
5.Proteomic Analysis of Hypertrophied Left Ventricle in Spontaneously Hypertensive Rats

Objective To investigate the molecular mechanisms for the development of cardiac hypertrophy in hypertension,the present study provided the differential protein expression analysis of hypertrophied heart at differ- ent stages in spontaneously hypertensive rats (SHR).Methods The profiles of protein expression of left ventricu lar myocardium in SHR and its normotensive control Wistar-Kyoto (WKY) rats at the age of 4 and 20 weeks were analyzed with two-dimensional gel electrophoresis (2-DE) in combination with matrix assisted laser desorption ioniza- tion-time of flight (MALDI-TOF-TOF) mass spectrometry.Results Although the blood pressure of SHR was normal at 4 weeks age,hypertrophy of the left ventricle had already developed.The expression pattern in the hy- pertrophic myocardium was found 27 modulated proteins,20 of which were identified.These proteins are involved in reactions of energy metabolism,mitoehondrial oxidative phosphorylation and oxidative stress,etc.The expres- sion of 13 proteins was significantly changed in SHR rats at early stage prior to the development of sustained hyper- tension,while the expression changes of other 7 proteins occurred only at late stage in SHR rats when the blood pressure was significantly elevated.Conclusions lncrease in glycolysis and decrease in oxidation of fatty acid and glucose was shown in the hypertrophied myocardium from early stage in SHR prior to the development of hyperten sion.The significant changes in protein expression of the mitochondrial electron transport chain and antioxidative molecules support the hypothesis that oxidative stress promotes and accelerates the development of hypertensive car- diac hypertrophy.
6.The effect of palliative resection on incurable colorectal cancer survival

Objective To quantify the benefit of primary Pdmor removal in patients with differently presenting incurable coloreetal cancer,while no other therapy combined.Methods One hundred and forty-three consecutive patients were operated for incurable colorectal cancer(91 undergoing resective and 52 non-resective procedures),with the purpose of comparing homogenous populations and of identifying whether the patients got benefit from primary tumor resection.Results In patients with resectable primary tumors,resective procedures were associated with longer median survival than non-resective procedures(10 months vs 3 months),patients with distant spread without neoplastic ascites/implanting metastasis got benefit from primary tumor removal(P<0.01).The complication of resective procedures was not significantly differ-ent from that of non-resective procedares(P>0.05).Conclusion Palliative resection of primary colorectal cancer should be pursued in patients with unresectable distant metastasis whenever the primary tumor is technically resectable.
7.Pharmacokinetic effect of combined administration on spinosin and ferulic acid in monarch drug Ziziphi Spinosae Semen kernel.

To study the pharmacokinetic effect of different combined administration with monarch drug Ziziphi Spinosae Semen on its main components in rats. Sprague-Dawley (SD) rats were randomly divided into Ziziphi Spinosae Semen group, Ziziphi Spinosae Semen-Fructus Schisandrae Chinensis group, Ziziphi spinosae Semen-Salviae Miltiorrhize Radix et Rhizoma group and Zaoren Ansheng prescription group. After oral administration, HPLC was eluted with the mobile phase of acetonitrle-0.03% phosphate acid water in a gradient mode. The detection wavelength was 280 nm. The pharmacokinetic parameters of spinosin and ferulic acid were calculated by DAS 2. 0 software. Compared with Ziziphi Spinosae Semen group, Ziziphi Spinosae Semen-Fructus Schisandrae Chinensis group, Ziziphi Spinosae Semen-Salviae miltiorrhizae Radix et Rhizoma group showed a lower maximum plasma concentration (C(max)) and area under curve (AUC(0-t)) for spinosin and ferulic acid but higher clearance speed (CL/F); whereas the Zaoren Ansheng prescription group showed higher maximum plasma concentration (C(max)) and area under curve (AUC(0-t)) for spinosin and ferulic acid but lower clearance speed (CL/F). 8.Investigations on allergens of allergic diseases in children from rural Shanghai

Objective To analyse the distribution characteristics of major allergens initiating allergic diseases in children from rural Shanghai. Methods Eight hundred children with allergic diseases from rural Shanghai (rural ease group), 450 children with allergic diseases from urban Shanghai (urban ease group) and 100 healthy children from rural Shanghai (rural normal control group) underwent skin prick tests (SPT), and children of rural case group were subdivided into infant group, preschool age group and school age group according to age. The positive rates of allergens and SPT were compared among groups. Results The positive rate of SPT of rural case group was significantly higher than that of rural normal control group (73.38% vs 26.00%, P<0.05), and was significantly lower than that of urban ease group (73.38% vs 80.22%, P<0.05). Dermatophagoidesfarinae and Dermatophagoides pteronyssinus were the major allergens in rural ease group, with the positive rates of 57.88% and 59.13%, respectively. Except weed and rubber, there were significant differences in positive rates of the other allergens between rural ease group and the other two groups(P<0.05). There were significant differences in positive rates of SPT among different age groups of rural children with allergic diseases (P<0.05). Conclusion Dermatophagoides farinae and Dermatophagoides pteronyssinus are the major allergens in children with allergic diseases from rural Shanghai, whose positive rates of SPT are lower than those of children with allergic diseases from urban Shanghai. The positive rate of SPT is related to age to some extent.
9.Correlation between invasion and metastasis of gastric cancer and serum angiopoietin-2

Objective To study the clinical significance of the serum angiopoietin-2(Ang-2) in the diagnosis,recurrence,invasion and metastasis of gastric cancer.Methods The serum Ang-2 and carcino-embryonic antigen (CEA) levels in 158 patients with gastric cancer (gastric cancer group) and 30 normal controls(control group) were measured by enzyme linked immunosorbent assay(ELISA) technique respectively.The serum Ang-2 and CEA levels were also measured 2 weeks after operation in gastric cancer group and reexamined in the recurred gastric cancer patients in 2 years after operation (recurred and metastasis group).The correlation between the serum Ang-2 level and pathologic c haracterization of gastric cancer was evaluated.Results The serum Ang-2 and CEA levels in gastric cancer group [ (331.8 ±64.3),(42.6 ±37.3)μg/L] and recurred and metastasis group [(318.7 ±72.9),(40.5 ±36.7)μg/L] were significantly higher than those in control group [ (187.4 ± 32.7),(4.2 ± 3.1 )μ g/L] (P < 0.01 ),and the serum Ang-2 level 2 weeks after operation [ (211.6 ± 75.1 ) μ g/L ] was significantly decreased to the control group (P > 0.05 ),while the serum CEA level [ (33.4 ± 30.6) μ g/L ] was still significantly higher than the control group (P < 0.01 ).The sensitivity of the serum Ang-2 for diagnosis of gastric cancer was markedly higher than that of the serum CEA (P < 0.01 ).There was correlation between serum Ang-2 and degree of tumor differentiation,TNM pathological staging,lymphatic metastasis,invasion depth and tumor size (p <0.01 ),but there was no correlation between serum Ang-2 and tissue classification and location of gastric cancer (P> 0.05).Conclusion The serum Ang-2 level is suggested to be a valuable gastric cancer marker and conduce to the diagnosis of gastric cancer,the monitoring of recurrence after operation and evaluation of prognosis.
10.Selection of De-emulsification Strains and Optimization of Fermentation Conditions

Petroleum-field emulsions,which are produced in the process of exploration,production and re-covery,represent a major environmental and processing problem for the petroleum industry.Biological de-mulsifier as a biological agent,will become a novel method of petroleum-field emulsions demulsifier instead of chemical demulsifier for its high efficiency,low cost,high adoptability,environment-friendly,repeated use.A set of high efficiency and convenient method was established to isolate de-emulsification strain and studied on the fermentation conditions of the de-emulsification strains.A de-emulsification strains,isolated from a petroleum-contaminated site,was evaluated for its de-emulsification capabilities using a kero-sene/water model emulsion system keeping stable for more than 200 hours which was mixed by stirring Tween 60-water(0.072%,V/V)and Span 60-oil(0.028%,V/V)at the ratio(oil:water)of 6.5:3.5.We have isolated two strains with high and steady de-emulsification activity by separation and purification,blood-plate test,excluding-oil test and de-emulsifying test.The average de-emulsification rates were more than 80.0% at 24 h.The two strains are both belonging to bacillus by traditional and molecular genetic iden-tification.By optimizing the demulsification bacteria fermentation conditions,the fermentation conditions,which is the most suitable for accumulating the fermentation products,were determined as follows:tem-perature 25?C,stirring speed160 r/min,pH 9,inoculated quantities 20%.When the fermentation conditions were temperature 35?C,stirring speed120 r/min,pH 9,inoculated quantities 2%,the fermentation products appeared the highest activities of demulsification.
