Journal of Zhejiang University. Science. B 2007;8(8):566-569
OBJECTIVETo identify the mutations of iduronate-2-sulfatase (IDS) gene, to reveal its mutation features, and to establish a basis for genetic counseling and prenatal gene diagnosis of Hunter syndrome.
METHODSUrine glycosaminoglycans (GAGs) assay, PCR and DNA sequencing were performed to detect mutation of IDS gene of the patient and his parents.
RESULTSThe result showed that the patient was: DS(++), HS(++), KS(-), CS(-), and that both of his parents were negative. A frame-shift deletion mutation (1062 del 16) was identified in exon 7 of the patient's IDS gene. His parents' genotypes were normal.
CONCLUSIONThe patient's mutation was not inherited by his parents but a novel one. The mutation probably altered the primary structure and tertiary structure of IDS enzyme protein remarkably and lowered the activity of IDS enzyme greatly. Therefore it is supposed to be the direct cause of the disorder.

OBJECTIVE:
To study the clinical efficacy and complications of Endobutton titanium plate and clavicle hook plate in the treatment of acromioclavicular dislocation.
METHODS:
Total 48 patients with Rockwood Ⅲ to Ⅴ acromioclavicular joint dislocation from March 2015 to May 2019 were retrospectively divided into two groups according to different surgical methods. Among the patients, 23 patients were treated with Endobutton loop titanium plate fixation (observation group), including 15 males and 8 females, ranging in age from 23 to 59 years old, with an average of(36.2±8.1) years old;Rockwood type Ⅲ in 6 cases, type Ⅳ in 11 cases and type Ⅴ in 6 cases. Twenty-five patients were treated with clavicular hook plate(control group), including 17 males and 8 females, ranging in age from 22 to 54 years old, with an average of (34.7±6.4) years old; Rockwood type Ⅲ in 6 cases, type Ⅳ in 14 cases and type Ⅴ in 5 cases. The operation time, intraoperative bleeding, hospitalization time, visual analogue scale (VAS) of pain, Constant-Murley score of shoulder function and postoperative complications were compared between the two groups.
RESULTS:
All the patients were followed up, and the duration ranged from 24 to 51 months, with a mean of (30.5±6.5) months. The amountof bleeding and hospitalization time in the observation group were (71.9±4.0) ml and(8.2±1.6) d respectively;and those in the control group were (97.6±13.4) ml and (12.8±1.2) d respectively. There was significant difference between the two groups (
CONCLUSION
Compared with clavicular hook plate internal fixation in the treatment of acromioclavicular joint dislocation, Endobutton loop titanium plate internal fixation technology has the advantages of less surgical bleeding, shorter hospitalization time, less postoperative pain, good recovery of shoulder joint function and less complications.

Chinese Journal of Cancer 2010;29(3):270-276
BACKGROUND AND OBJECTIVERadiotherapy (RT) is a major non-surgical modality in the comprehensive treatment for colorectal adenocarcinoma. The radioresistance of cancer stem cells (CSCs) is a key factor that influences therapeutic effectiveness. This study was to investigate the effects of specific chromosome structure and histone modification in CSCs in colorectal adenocarcinoma radioresistance.
METHODSSamples were collected from resected human colorectal adenocarcinomas. Subcutaneous colorectal cancer model was established in nude mice. Immunohistochemistry showed that xenografts generated from bulk colorectal cancer cells resembled the original tumor specimen. Flow cytometry was performed to sort CSCs (CD133+) and non-CSCs (CD133-) from both resected samples of colorectal adenocarcinoma and xenograft before and after high single-dose radiation. The markers labeling heterochromatin (H3K9me3, HP1-alpha and H3K4me1) and euchromatin (H3K4me3) in CD133+ and CD133- nucleus were detected by immunofluorescence.
RESULTSThere was distinct difference in chromatin structure between colorectal CSCs (CD133+) and non-CSCs (CD133-). The chromatin displayed compact patches in CD133+ nucleus, but loosely latticed structure in CD133- nucleus; immunofluorescence verified that the compact patches existing in CSCs was generated from heterochromatin construction. In addition, the vacuole-like defect in heterochromatin regions of CSCs was observed within 24 h after exposure to 10 gray (Gy) single-dose RT. Interestingly, this phenomenon was repaired from 96 h, and recovered to dense plaque structure in heterochromatin regions of CSCs after 144 h. However, no significant difference in non-CSCs was observed after RT exception for a loose chromatin structure.
CONCLUSIONSCSCs play a role in radiosensitivity in colorectal cancer. The mechanism may be related to heterochromatin formation and histone methylation.

OBJECTIVETo evaluate the feasibility and efficacy of laparoscopy-assisted radical gastrectomy with D2 lymph node (LN) dissection for elderly patients with gastric cancer.
METHODSFrom January 2007 to December 2009, 255 gastric cancer patients aged over 65 years underwent D2 LN dissection. Of these, 116 patients underwent laparoscopy-assisted radical gastrectomy (LAG group), while 139 patients underwent conventional open gastrectomy (OG group). The recovery and complication rate were compared between the two groups. Risk factors associated with postoperative complications were investigated by univariate and multivariate analyses.
RESULTSCompared to the OG group, the mean blood loss and number of patients who required blood transfusion were less, and the time to first flatus, the time to resume soft diet, and postoperative hospital stay were shorter in the LAG group (all P<0.01). However, the operation time and the mean number of LN retrieval were not significantly different between the two groups (both P>0.05). The postoperative morbidity in the LAG group was 15.5% (18/116), lower than that in OG group [28.1% (39/139), P<0.05]. Logistic regression analysis revealed that there were three significant factors associated with postoperative complications such as laparoscopy-assisted gastrectomy (P<0.05), operation time (P<0.01), and preoperative comorbidity (P<0.01). Operation time (P<0.05) and preoperative comorbidity (P<0.01) were independent risk factors for complication in laparoscopy-assisted radical gastrectomy of elderly patients. The mean survival time of LAG and OG were 23.0 months and 22.5 months, respectively, and the difference was not statistically significant (P>0.05).
CONCLUSIONSLaparoscopy-assisted radical gastrectomy with D2 LN dissection is oncologically compatible with open gastrectomy in elderly patients with gastric cancer less invasive. Operation time and preoperative comorbidity are independent risk factors for complication in laparoscopy-assisted radical gastrectomy of elderly patients.
