1.Progress of Chinese joint surgery

Great and extensive progress has been made in joint surgery in the last decade. With the evolution of surgical concepts and the development of operational technique and equipment, indications, selective range for surgica l operation and management in the period of perioperation have been surveyed in new perspectives and with deeper insights. This article introduces the situation of joint surgery of China in these years, briefly reviews the latest progresses in the fields of the arthroscopy, the arthroplasty and the joint cartilage repa ration, and thus suggests developmental strategies for Chinese joint surgery in the future.
2.Morphological Alteration and the Protein Expression of Caspase -3 in Denervated Posterior Cricoarytenoid Muscles

Objective To investigate the morphological alteration and the protein expression of caspase -3 in long -term denervated posterior cricoarytenoid muscles (PCAMs)in order to find the appropriate time point of rein‐neration in long -term denervated PCAMs .Methods A total of 45 patients with vocal paralysis were recruited and devided into 3 groups ,the 3~6 months denervation group ,the 6~12 months denervation group ,the 1~2 year denervation group .12 adults served as control group .The morphological alteration was evaluated using HE staining and the change in expression of caspase -3 ,an apoptosis related factor ,were observed using immunehistochemistry stain and western blot .Results With elongation of denervation time ,there were increased denaturation in the mus‐cle fibers .The nucleus moved inside and some of them concentrated .Caspase-3 showed weak staining in innerva‐ted ,however ,by 3-6 months of muscle denervation there was a significant accumulation of caspase -3 protein in myofibers ,6~12 months and 1~2 years of denervation ,expression of caspase -3 protein in myofibers was de‐creased significantly .In western blot ,the change in protein expression of caspase -3 was observed an 21-fold (P< 0 .01) increase from 3~6 months denervated muscles to innervated muscles ,11-fold (P< 0 .01) increase from 6~12 months denervated muscles to innervated muscles ,3~fold (P< 0 .01) increase from 1~2 year denervated muscles to innervated muscles .Conclusion The morphological alterations and changes in expression of caspase -3 indicated there was a high amplitude of apoptosis in denervated posterior cricoarytenoid muscles within 1 year .
3.The application of Midwife Core Competency Scale in Beijing midwives

Objective To evaluate the reliability and validity of Midwife Core Competency Scale.Methods A total of 369 midwives from 32 hospitals in Beijing were investigated by using the Midwife Core Competency Scale,and the reliability and validity were measured.Results The scale had good internal consistency (Cronbach's α =0.950) and stability (the test-retest reliability was 0.832).The distribution of 54 items in 6 public factors (the cumulative variance=68.405%) extracted from them was consistent with the original scale,yielding an acceptable construct validity.The scale also had good validity to discriminate different categories.Conclusions The Midwife Core Competency Scale had good reliability and validity,which provides an useful tool to evaluate midwifes' core competency.Larger sample from different areas need to be considered for scale improvement and application.
4.Value of Hydrotherapy in Rehabilitation of Heart Failure (review)

With water immersion, the water exerts a pressure on the body surface. Thus there is a blood volume shift from the periphery to the central circulation, resulting in marked volume loading of the thorax and heart. This article illustrated hemodynamic and hurohumoral responses of hydrotherapy, and dicussed whether patients with left ventricular dysfunction (LVD) or chronic heart failure (CHF) can immerse, take balneotherapy, exercise in the water and swimming. A positive effect of therapeutic warm-water tub bathing has been observed in patients with LVD and CHF, which was assumed to be from afterload reduction due to peripheral vasodilatation caused by the warm water. Optimal swimming as used for cardiovascular training programs may be allowed exclusively for NYHA class Ⅱ patients with myocardial infarction older than 6 weeks, myocarditis suffered more than 6 months ago, and cardiomyopathy. Patients with previous severe myocardial infarctions and/or CHF may bathe in a half sitting position, but immersed no deeper than the xiphoid process, because immersion up to the neck could produce abnormal increase in mean pulmonary pressure and pulmonary capillary wedge pressure temporarily. In coronary artery disease (CAD) patients with reduced LV function the heart is working more efficiently with mild cycling in water than cycling outside of water. Decompensated heart failure is the absolute contraindication to hydrotherapy.
5.Application of high resolution melting analysis in detecting IDH1 mutation in glioma patients

Objective To evaluate the feasibility of detecting the mutation of IDH1 in glioma patients by high resolution melting(HRM) curve analysis.Methods The gene mutation of IDH1 was detected by HRM method in 9 surgical resection paraffin specimens of glioma,and the result was verified by gene sequencing.Results 6 cases of R132H(CGT > CAT) mutation and 1 case of R132C(CGT > TGT) mutation were found by HRM method.The resluts of direct gene sequencing were consistent with HRM method.Conclusion The HRM method in detecting IDH1 mutation is more efficient and convenient than direct sequencing.Moreover,it has advantages of low-cost and suitable for clinical test.
6.Research and Application of Analytical Technique onδ18Op of Inorganic Phosphate in Soil

Analytical technique on oxygen isotope composition of phosphate in soil has important applications in biogeochemical research. As the composition of the soil is complex with multiple oxygen sources, a liable phosphate enrichment and purification technology is important for phosphate oxygen isotope research. Here we presented a protocol on the analysis of the δ18 Op of inorganic phosphate in soil, and phosphate was purified with multiple mineral precipitations and finally precipitated as silver phosphate. The efficiency of NH4 OH purification method was examined and the purity of the product was verified through XRD and element analysis. The result showed that treatment of the Ag3 PO4 sample by recrystallizing with ammonia could remove the impurity efficiently without significantly modifying oxygen isotope compositions. The validity of sample processing and reliability of isotope analyses were confirmed by processing a KH2 PO4 standard in parallel with soil P sample. The analytical result of three soil types indicated that this method provided good precision of 0. 2‰. The differences of δ18 Op between three soils groups are significant ( From 15. 2‰ to 19. 5‰) and can be used to tracing P sources.
7.Comparative Study of Complete and Incomplete Cervical Spinal Cord Injuries Complicated with Venous Thromboembolism

Objective To study the incidence of venous thromboembolism (VTE) between complete and incomplete spinal cord injured patients. Methods 177 cases of acute cervical spinal cord injury were observed from April 2007 to July 2010. According to ASIA classification, 52 cases of grade A involved in observation group, while 125 cases of grades B~D involved in control group. Intervention included low molecular weight heparin and rehabilitation training. Incidence of VTE in two groups was compared. Results There was 11 cases complicated with VTE in the observation group (21.2%) while 6 cases in the control group (4.8%). There was a difference in incidence of VTE between two groups (P<0.05). Conclusion There is a higher incidence of VTE in complete than incomplete cervical spinal cord injury
8.Evaluation of Regional Left Ventricular Cardiocyte Viability and Diastolic Function by Strain Curves in Patients with Myocardial Infarction

Objective To detect the strain features of regional wall abnormalities in patients with coronary artery disease during each diastolic period, and its possibility to evaluate regional left ventricular cardiocyte viability and diastolic function.Methods 54 patients with anterior myocardial infarction (MI group) and 78 normal subjects (NOR group) underwent Doppler tissue imaging, which were performed in 2-chamber-view by strain curves synchronously.Results In the NOR group, strain value of 66 cases (84.62%) showed an gradually increasing negative value from the apex to base to middle of left ventricle, while in the MI group, there were 9 cases (16.67%) with such a trend. IR phase: in the NOR group, 564 segments (90.38%) were upward wave bands, but in the MI group, there were 123 segments (28.47%) having such waves ( P<0.05). RF phase: in the NOR group, 576 segments (92.31%) were upward and steep wave bands, but in the MI group, the number of upward waves were obviously less (102 segments,23.61%) ( P<0.01). SF period: compared with the NOR group, which was horizontal, the MI group had upward wave bands ( P<0.05).Conclusion Regional myocardial ischemia and infarction can cause significant regional diastolic wall abnormalities of strain value in active diastolic phase. Regional diastolic wall motion abnormalities can be evaluated quantitatively and synchronously with high sensitivity by strain curve which has the potential value in cardiocyte viability and diastolic function.
9.Disrupting sfa1 Gene to Enhance Biosynthesis of Ethanol in Saccharomyces cerevisiae

The sfa1 gene encoded a bifunctional enzyme with the activities of both alcohol dehydrogenase and glutathione-dependent formaldehyde dehydrogenase in Saccharomyces cerevisiae.The gene disruption cassette produced by PCR using the same long oligonucleotides which comprise 19 or 22 nucleotides complementary to sequences in the templates(pUG6 and pUG66 marker plasmid)at 3' end and 45 nucleotides at 5' end that annealed to sites upstream or downstream of the genomic target sequence to be deleted.After two linear disruption cassettes with a Cre/loxP mediated marker were transformed into the cells of Saccharomyces cerevisiae YS-1,the positive transformants were checked by PCR to correct the integration of the cassette and concurrent deletion of the chromosomal target sequence.Once correctly integrated into the genome,the select marker can be efficiently rescued by transformating the plasmid pSH47 into YS-1 and inducing the Cre expression with a Cre/loxP-mediated marker removal procedure.The expression of the Cre recombinase finally resulted in the removal of the marker gene,leaving behind a single loxP site at the chromosomal locus.The diploid mutant YS-1-sfa1 was generated,which could enhance the output of ethanol with 8.0% by shaking culture in flask compared with the original strain YS-1.
10.Recombinant human interleukin-6 inhibits mRNA expressions of adiponectin,its receptor 1 and adiponectin secretion in SW872 adipocytes

SW872 preadipocytes were cultured and induced to differentiate by oleic acid in vitro.The levels of adiponectin and its receptors (AdipoR1 and AdipoR2) mRNA were measured by semiquantitative RT- PCR.The concentration of adiponectin in the culture medium was assayed by ELISA.The results showed that rhIL- 6 could inhibit adiponectin,AdipoR1 mRNA expressions and adiponectin secretion in SW872 adipocytes in a time- and dose-dependent manner,but did not influence adipoR2 mRNA expression.
