1.Prognostic Value of 99mTC-MIBI SPECT Exercise Myocardial Imaging in Patients with Suspected or Diagnosed Coronary Artery Disease

ObjectiveTo examine the prognostic value of 99mTC-MIBI single-photon emission computed tomography (SPECT) in patients with suspected or diagnosed coronary artery disease (CAD).Methods116 patients with suspected or diagnosed CAD were examined with 99m TC-MIBI SPECT exercise and rest myocardial imagings, and followed by investigation of cardiac events.Results Follow-up information was complete in 106 patients. Over an average follow-up of 25±8 months, cardiac events occurred in 11 patients. Univariate analyses showed that age, old myocardial infarction, exercise peak heart rate, the change in heart rate from rest to exercise peak, exercise peak systolic blood pressure, the change in systolic blood pressure from rest to exercise peak, abnormal myocardial imaging, the number of myocardial perfusion defect and ischemic volume rate (IVR) were the effective predictors of subsequent cardiac events. Multivariate stepwise Logistic regression analyses revealed that the size of IVR was the only variable with independent predictive value for occurrence of subsequent cardiac events. IVR more than 25% was associated with a significantly increased frequency of subsequent cardiac events (relative risk ratio was 5.9, P<0.005). IVR less than 25% or even normal myocardial perfusion imaging was associated with a very low-risk of cardiac event and a very good prognosis.Conclusion 99mTC-MIBI SPECT myocardial imaging is the most effective noninvasive technique that predicted the occurrence of cardiac event in patients with suspected or diagnosed CAD. According to the size of IVR, high-risk and low-risk patients can be effectively identified.
2.Surgical treatment of Lisfranc fracture-dislocations by reconstruction of three-column with Kirschner wire fixation.

OBJECTIVETo explore the effect of reduction and fixation with multi-Kirschner wires for treatment of Lisfranc fracture-dislocations.
METHODSThere were 49 patients (37 male and 12 female aged from 20 to 28 years old) involved in the study. According to the Myserson damage typing, type A in 12 cases, B1 in 3, B2 in 28, C1 in 4 and C2 in 2. Kirschner wires were applied to fix and reconstruct the three-column in three directions according to the structural characteristic of midfoot. Reconstruction of three-column needed not only to reduce and to fix every single column, but also to establish union of the columns.
RESULTSAccording to the evaluation of AOFAS for midfoot, 14 cases were excellent (90 to100), 22 cases good (80 to 89), 8 cases fair (70 to 79) and 5 cases poor (60 to 69), with an average score of 84.200+/-9.663.
CONCLUSIONThe diagnosis and treatment of Lisfranc fracture-dislocations should comply with the theory of three-column reconstruction of foot arch, which can achieve the static balance of biomechanics and provide a stable environment for healing of fracture and soft tissue. Reduction and fixation with multi-Kirschner wires is an effective treatment method for Lisfranc fracture-dislocations.

3.Investigation of Evolution Rules of Phlegm and Blood Stasis Syndrome in Hyperlipidemia and Ath- erosclerosis by NMR-based Metabolic Profiling and Metabonomic Approaches.

OBJECTIVETo explore evolution rules of phlegm and blood stasis syndrome ( PBSS) in hyperlipidemia and atherosclerosis (AS) using NMR-based metabolic profiling and metabonomic approaches based on formulas corresponding to syndrome.
METHODSTotally 150 SD rats were divided into the normal group, the model group, the Erchen Decoction (ED) group, the Xuefu Zhuyu Decoction (XZD) group, the Lipitor group, 30 in each group. The hyperlipidemia and AS rat model was duplicated by suturing carotid artery, injecting vitamin D3, and feeding with high fat diet. ED and XZD were used as drug probes. Blood samples were withdrawn at week 2, 4, and 8 after modeling. Blood lipids, blood rheology, histopathology and metabolomics were detected and analyzed. Results Results of blood lipids and pathology showed hyperlipidemia and early AS rat models were successfully established. At week 2 after modeling, levels of total cholesterol (TC) and low-density lipoprotein cholesterol (LDL-C) significantly increased, which reached the peak at week 4 and maintained at higher levels at week 8. ED exerted obvious effect in improving TC and LDL-C levels of early models, while XZD could greatly improve levels of TC and LDL-C of late models. Rheological results showed at week 2, there was no significant difference in whole blood viscosity, plasma viscosity, or hematocrit between the model group and the normal group (P > 0.05). At week 4 partial hemorheological indicators (such as plasma viscosity) were abnormal. Till week 8 whole blood viscosity, plasma viscosity, and hematocrit were significantly abnormal (P <0. 05, P < 0.01). As time went by, whole blood viscosity, plasma viscosity, and hematocrit showed gradual increasing tendency in the ED group, while they showed gradual decreasing tendency in the XZD group. Results of metabonomics showed significant difference in spectra of metabolites between the normal group and the model group. As modeling time was prolonged, contents of acetyl glucoprotein and glucose in the model group increased in late stage, which was in. line with results of blood lipids and hemorheology. ED showed more obvious effect in early and mid-term modeling (at week 2 and 4), and increased contents of partial metabolites (such as choline, phosphatidyl choline, glycerophosphocholine), but these changes in the XZD group were consistent with those of the model group. In late modeling (at week 8) XZD showed more obvious effect in improving contents of lactic acid, acetyl glycoprotein, LDL, creatine, choline, and glucose.
CONCLUSIONSED and XZD not only showed regulatory effects on lipid disorders, but also could improve dysbolism of Chos. In formulas corresponding to syndrome, damp-phlegm was main pathogenesis of hyperlipidema and AS in early and mid stages. Blood stasis syndrome began to occur along with it progressed. Phlegm can result in blood stasis and intermingles with stasis. Phlegm turbidity runs through the whole process.

4.Application of minimally invasive, decompression bone graft implantation combined with metal trabecular bone reconstruction system for early stage osteonecrosis of femoral head.

OBJECTIVETo observe the application effect of minimally invasive decompression, bone graft implantation and metal trabecular bone reconstruction system for early stage osteonecrosis of femoral head and discuss the treatment of hip-salvage operation in early stage osteonecrosis of femoral head;
METHODSFrom January 2010 to June 2011, 50 patients (62 hips) Which were osteonecrosis of femoral head of early stake,were treated with minimally invasive decompression, bone graft implantation and metal trabecular bone reconstruction system, including 31 males (40 hips), 19 females (22 hip) with an average age of 36.2 years old ranging from 22 to 54 years old. The course of disease was from 6 to 15 months (averaged 10.5 months). Among them, 19 cases (23 hips) were steroid-induced, 25 cases (33 hips) were alcohol-induced, 6 cases (6 hips) were idiopathic; According to ARCO stage, 28 hips were at stage I, 34 hips were at stage II. All of them were diagnosed as femoral head necrosis by imaging examination before operation. Then each patient was followed to assess by Harris hip score, curative effect, and conduct the femoral head survival analysis during the postoperation.
RESULTSAll patients had finished operation, the operation time was between 30 and 85 min, intraoperative blood loss was 50 to 220 ml, and 47 cases (58 hips) were follow-up from 24 to 46 months with an average of 34.05 months. As compared with preoperative, the Harris hip score at the last follow-up was improved, the difference was statistically significant (P<0.01). The Harris hip score, curative effect and survival time of femoral head in ARCO stage I was superior to these in ARCO Stage II, the difference was statistically significant (P< 0.05).
CONCLUSIONEffect of minimally invasive decompression,bone graft implantation combine with the metal trabecular bone reconstruction system for early stage osteonecrosis of femoral head was good,it could significantly improve the Harris hip score, increase the femoral head survival time, delay the hip replacement, and performance better in ARCO stage I.

5.Successful pregnancy following laser-assisted selection of viable but immotile spermatozoa for intracytoplasmic sperm injection: A report of 2 cases.

OBJECTIVETo investigate the feasibility and clinical application value of selecting viable spermatozoa by noncontact diode laser.
METHODSWe obtained immotile spermatozoa from 2 infertile men with obstructive azoospermia or severe asthenospermia and selected viable spermatozoa using a single laser shot at the sperm tail. Those that responded to the laser shot by a curling reaction of the tail were regarded as presumably viable and used for intracytoplasmic sperm injection (ICSI).
RESULTSThe mean fertilization rate was 88.89% after ICSI with the laser-selected viable spermatozoa. Both of the embryo transfers resulted in a single pregnancy.
CONCLUSIONNoncontact diode laser is a useful alternative for the assessment of sperm viability, which may help to achieve successful pregnancy.

7.Neonatal fluoxetine exposure induced depression-like behaviors in adult Kunming mice and the antidepressant-like effect of agmatine.

This study is to explore a behavioral and pathological model for depression in mice, and evaluate the anti-depressant-like effect of agmatine. Neonatal Kunming mice were treated with fluoxetine (10 mg x kg(-1), ip, qd) for 17 d (between day 4 and 21 after birth), and then the mice were normally housed till being adult (about 10 weeks after birth). The behaviors of the mice were measured by using open-field test, novelty suppressed feeding test and tail-suspension test. Hippocampal adenylate cyclase (AC) activity was measured by radioimmunoassay. Neonatal exposure to fluoxetine induced a "depression-like" behaviors in the adult mice, shown as the decreased locomotor activity, increased feeding latency and immobility time in the open-field test, novelty suppressed feeding test, and tail-suspension test, respectively. Chronic agmatine treatment (10 mg x kg(-1), ig, bid) for 3 weeks significantly increased the locomotor activity, and decreased the feeding latency in the neonatal fluoxetine exposed mice. Furthermore, single treatment with agmatine (40 mg x kg(-1), ig) also decreased the immobility time in the tail-suspension test, and increased the hippocampal AC activity in the mice. These results indicate that neonatal exposure to fluoxetine induces depressive-like behaviors in the adult mice. Agmatine reverses these behaviors, which may be closely related to the enhancement of the hippocampal AC activity.

8.An effectiveness evaluation of health education on diabetes patients

Objective To evaluate the effectiveness of health education approach on the awareness of the diabetes knowledge among diabetic patients and to explore its influencing factors. Methods A total of 130 patients with diabetes mellitus who were hospitalized from 2015 to 2016 were enrolled in this study. Using self-control method, we compared the awareness rates of the diabetes knowledge at pre -hospital admission with pre-discharge from the hospital after the health education, and a logistic model was used to analyze related factors which might influence the awareness of the diabetes knowledge. Results Through health education, awareness rate of overall diabetes knowledge among patients increased from 18.90% to 75.59%. All the awareness rates of diabetes knowledge points increased significantly (all P<0.001) . After health education, the awareness rates of the knowledge on diabetes diagnosis basis and harmfulness of diabetes were still at the lowest level, accounting for 58.27% and 61.42% respectively, while the awareness rates of other diabetic knowledge were higher than 70%. Multivariable logistic regression analysis indicated that the OR value of improvement of overall awareness rate among diabetes patients younger than 50 years old was 0.040, compared to patients older than 50 years. The OR values among diabetes patients with less than 10 years, 10-19 years disease course were only 0.003 and 0.012, compared with patients with more than 20 years disease course. Conclusion Health education can effectively improve the awareness rates of diabetes knowledge in general. However, the intervention effectiveness was unsatisfactory among elder people or patients with short disease course.
9.The effectiveness of 3D printed titanium alloy trabecular reconstruction rod for the treatment of early osteonecrosis of femoral head

Objective To evaluate the safety of 3D printed titanium alloy trabecular reconstruction rod, bone grafts and its clinical efficacy. Methods Sixty patients with osteonecrosis of femoral head (ONFH) stageⅡ(ACROⅡ) were randomly divided into experimental group (titanium rod group) and control group (tantalum rod group). Patients were followed up at 6, 12 and 24 months after the operation. Harris score and visual analogue scale (VAS) were used to evaluate the postoperative function. Changes of X-ray were observed before and after operation. The imaging evaluation was analyzed after 24 months. Results The Harris score was decreased in titanium rod group compared with that of tantalum rod group at six-month. There were no significant differences in Harris scores for other time points between two groups. Repeated measures analysis of variance showed that there was a significant difference in time effect. The X-ray staging results changed gradually before and after operation in two groups, stage Ⅲand stage Ⅳgradually increased with time. There was progress in ONFH after operation in two groups. No infection or rejection was found in the two groups. There were higher improvement rates of stageⅡA and stageⅡB. There were no significant differences in improvement rate and hip preservation rate between two groups. The transparent lines disappeared in the positive and lateral X-ray films 6 months after the operation. No new transparent lines were found. The bone enhancement was not found in two groups of patients during follow-up period. No stress occlusion and bone trabecular bone enhancement were observed in two groups. Conclusion Titanium rods are satisfactory for the treatment of femoral head necrosis in the early stage of ACRO Ⅱ, which is not recommended for the treatment of relatively large ACROⅡC stage of necrotic area.
10.New method of multi-components quantitation by one marker new method for quality evaluation of Guanmaikang capsula.

OBJECTIVETo establish a new quality evaluation method for traditional Chinese medicine preparations, using one chemical reference substance to calcutate multi-components simultaneously.
METHODEmployed puerarin as the maker component, puerarin relative correction factors (RCF) of salvianolic acid B to puerarin and paidzein to puerarin were calcatated in the chromatographic conditions for determination of the three components in Guanmaikang capsules. The contents of Puerarin were determined by external standard method, and those of salvianolic acid B and paidzein were calculated by puerarin and their RCF. The accuracy of the new method was evaluated by comparing the calculated contents with the determined.
RESULTThe analysis methods were established, and it has been no significant difference between the calculated contents and determined contents.
CONCLUSIONSThe method can control the components without providing salvianolic acid B and paidzein reference. It is to be a suitable quality evaluation pattern for TCM Preparation.
