3.The New Idea about Early Intervention for Type 2 Diabetes Based on Gan Disease Transferring to Pi in Metabolic Diseases.

Excess energy has become a main reason for increasingly serious human health hazards. Excess energy, mainly ectopically deposits in the liver, pancreas and other organs in the form of triglycerides, and produces chronic oxidative, nitrosative stress (ONS) , and fat toxicity, resulting in insulin resistance and impaired insulin secretion, and further impaired glucose regulation (Pidan). By combining Chinese medical pathogeneses and symptoms analyses, authors found this process has features of Gan disease transferring to Pi. Based on a number of related guidelines and clinical practice, we demonstrated treating sputum and stasis by the same method was one treatment method for intervening liver disease transferring to spleen in metabolic diseases. This idea helps to organic integrating prevention and treatment of major metabolic diseases including non-alcoholic fatty liver disease (NAFLD) and type 2 diabetes mellitus, which can improve clinical effectiveness and efficiency of Chinese medicine.

6.Magnetic resonance imaging changes and analysis of related factors after treatment in childhood hepatolenticular degeneration

Objectives To observe the changes of magnetic resonance imaging(MRI) during the course of treatment in hepatolen-ticular degeneration (HLD) and to evaluate its related factors.Methods Sixty - three patients with HLD received routine SE sequence MRI of brain and liver.Twenty - two patients with abnormal MRI in 42 patients and each of them had 2-4 times of serial MRI ex-aminations at interval of 1.0- 1.5 year.Results Ten patients had lesion in both brain and liver in 22 patients. Nine patients only in brain, 3 patients in liver.The major lesions disappear or decrease after rational treatments. There were only slight or no changes of MRI abnormalities in patients receiving treatments or age of patients was over 15 year old. Abnormal MRI findings shown again after stopped treatments over 2 months. In such cases, the clinical and MRI improvements were slow.The rate of clinical and MRI improvement in patients with adequate treatment were markedly higher than patients with inadequate treatment.Conclusions Liver and brain lesion will show in MRI disappear after adequate treatment;There are only slight or no change or showing again in MRI abnormalities in pa-tients with inadequate treatments compared with poor clinical improvement. MRI is beneficial to understanding therapeutic effect and prognosis of HLD.
7.MRI appearance of malignant melanoma in the sinonasal tract and nasopharynx

ObjectiveTo investigate the MRI appearance of malignant melanoma in the sinonasal tract and nasopharynx.MethodsMRI Findings and clinical data from 8 patients with biopsy or operation proved malignant melanoma were retrospectively reviewed.Gadolinium-enhanced imaging and dynamic contrast enhancement scanning were performed in all cases.ResultsThe majority of lesions were located in the nasopharynx (n =2),middle turbinate (n =1 ),or a combination of the nasal cavity,ethmoidal sinus,and maxillary sinus ( n =5 ).The aggressive nature of the tumors was demonstrated by bone destruction ( n =5 ) and invasion of adjacent structures,involving anterior fossa (n =2 ),orbits (n =4 ),infratemporal fossa ( n =2 ),pterygopalatine fossa ( n =3 ),and parapharyngeal space ( n =1 ).The MRI appearance included two patterns.In the first pattern,the tumors were round and small (smaller than 2 cm in maximum dimension in three patients).MR signal intensity of the lesions was hyperintense to gray matter on T1 WI and hypointense on T2.In the second pattern,the lesions were irregular and large ( larger than 3 cm in maximum dimension in five patients).They showed heterogeneous low-signal on T1 WI.On T2WI,one lesion showed isointensity or little hypo-intensity,and four lesions showed hyperintense.They demonstrated mild heterogeneous enhancement on gadolinium-enhanced images.Four patients demonstrated as plateau type time-signal intensity curve (TIC) pattern,and another four patients demonstrated as wash-out type TIC pattern.ConclusionsThe MR signal characteristic of the malignant melanoma in the sinonasal tract and nasopharynx were related with the tumor size.When the lesion was small,the MR signal intensity was more typical.When the lesion was large,they had mixed signal intensity as other malignant tumors,and specific mild heterogeneous enhancement.

9.Clinical application of the penetrating needling technique from Yemen (TE 2) to Zhongzhu (TE 3).

The penetrating technique of acupuncture from Yemen (TE 2) to Zhongzhu (TE 3) acts on the extraordinary effect of promoting Shaoyang meridian qi. The technique is specially good for heat pattern/syndrome of the triple energizer and it is commonly used in the treatment of common cold, high fever, anhidrosis in heat pattern/syndrome, migraine, redness, swelling and pain of the eye, tinnitus, ear pain, swelling and pain of the teeth, ulcers in the mouth and tongue, sore throat, neck rigidity, etc. In the manipulation, the needle is inserted from Yemen (TE 2), going obliquely upward along the metacarpal interspace to Zhongzhu (TE 3). With this technique, the Ying-spring and Shu-stream acupoints can be stimulated simultaneously with one needle. This method is characterized as less acupoints, exquisite needling, strong sensation, easy and safe manipulation, remarkable immediate effect and high curative rate of single treatment. Hence, it is worthy of application in clinical practice.

10.Intralesional curettage and wide excision for treatment of giant cell tumors (GCTs) of the distal radius: A Meta-analysis.

OBJECTIVETo search all studies that had been published in the world with regarding to the effectiveness of the extent of intralesional curettage and wide excision for recurrence rate and complications and comparative functional outcomes in patients with giant cell tumours (GCT) of the distal radius and analyze them which were in high quality by means of Meta analysis, in order to give some evidences for the choice of method dealing with giant cell tumors GCT in surgery.
METHODSCochrane central register of controlled trials(Issue 8 2014), PubMed(1970-01-01/2013-01-01), Ovid (1970-01-01/2013- 01-01), Elsevier (1970-01-01/2013-01-01), CNKI (1970-01-01/2013-01-01) were searched. Including intralesional curettage and wide excision were performed to treat giant cell tumors (GCTs) of the distal radius in the literatures, selecting on meet eligibility in the standard literatures underwent strict quality assessment. The Meta-analysis was performed with software RevMan5.0 from the Cochrane collaboration. Additionally, the analysis checked the heterogeneity of data. The effectiveness of the extent of intralesional curettage and wide excision for recurrence rate and complication in patients with giant cell tumours of the distal radius were evaluated and Odds Ratio was calculated.
RESULTSSeven relevant articles were identified involving total 163 cases. Among them, 92 cases were intralesional curettage (PMMA, n = 54; bone graft, n = 33; no PMMA or bone grafts, n = 5) and 71 cases were wide excision. The patients in the intralesional curettage group had a higher recurrence rate [OR = 3.87, 95% CI (1.42, 10.53)],especially for Campanacci grade 3 GCTs [OR = 10.12, 95% CI (1.57, 65.27)], yet fewer major complications [OR = 0.13, 95% CI (0.04, 0.40)] than the wide excision group. The use of PMMA versus bone graft did not affect the recur- rence rate [OR = 0.96, 95% CI (0.26, 3.56)]. By selecting the system evaluation of MSTS, the VAS and dynamometer, the result showed that the intralesional curettage group was equivalent or preferable to wide excision in terms of function rehabilitation.
CONCLUSIONBased on data obtained from the limited number of studies available, intralesional curettage appears to be moreappropriate for the treatment of local lesions (Grade 1 and 2) than Grade 3 GCTs of the distal radius. Moreover, PMMA was not additionally effective as an adjuvant, the intralesional curettage group was found to be equivalent or preferable to wide excision in terms of function rehabilitation.
