1.CT scan findings of primary pulmonary non-Hodgkin's lymphoma and their relation to pathological features.

OBJECTIVETo investigate the CT scan findings of primary pulmonary non-Hodgkin's lymphoma (PPNHL) and their relation to pathological features.
METHODSThe CT scan findings of 17 PPNHL cases confirmed pathologically from August 1992 to June 2007 were analyzed in relation to their pathological foundation.
RESULTIn 17 cases the CT findings showed: 1) Five were single mass or nodule, including 3 with air-bronchograms, 1 with straddling-lobe distribution and 3 with slight enhancement; 2) One was multi-nodules with air-bronchograms; 3) Six were consolidation with air bronchogram,including 3 with straddling-lobe distribution and 3 with slight enhancement; 4) Five were combined changes, including 5 with air-bronchograms, 2 with straddling-lobe distribution and 4 with slight enhancement. The CT features of PPNHL corresponded to their pathological process and pathological features.
CONCLUSIONPrimary pulmonary non-Hodgkin's lymphoma should be considered when CT scan shows variety of images found in one patient, middle lobe of right lung involved, air-bronchograms and straddling-lobe distribution in consolidation with slight enhancement.

2.Cyclooxygenase-2 blockade inhibits accumulation and function of myeloid-derived suppressor cells and restores T cell response after traumatic stress.

Myeloid-derived suppressor cells (MDSCs) play a crucial role in T cell dysfunction, which is related to poor outcome in patients with severe trauma. Cyclooxygenase-2 (Cox-2) contributes to immune disorder in trauma and infection via production of prostaglandin E2. However, the role of Cox-2 in the accumulation and function of MDSCs after traumatic stress has not been fully elucidated. In the present study, we treated murine trauma model with NS398, a selective Cox-2 inhibitor. Then the percentages of CD11b+/Gr-1+ cells, proliferation and apoptosis of CD4+ T cells were determined. Arginase activity and arginase-1 (Arg-1) protein expression of splenic CD11b+/Gr-1+ cells, and delayed-type hypersensitivity (DTH) response were analyzed. The results showed that Cox-2 blockade significantly decreased the percentages of CD11b+/Gr-1+ cells in the spleen and bone marrow 48 and 72 h after traumatic stress. NS398 inhibited arginase activity and down-regulated the Arg-1 expression of splenic CD11b+/Gr-1+ cells. Moreover, NS398 could promote proliferation and inhibit apoptosis of CD4+ T cells. It also restored DTH response of traumatic mice. Taken together, our data revealed that Cox-2 might play a pivotal role in the accumulation and function of MDSC after traumatic stress.
3.Locating the displacement of the steel wire implantation with the stereotactic mammography

Objective To analyze the manifestation,reason,the processing method of the steel wire implantation with the sereotactic mammography to improve the accuracy of the preoperative positioning.Methods Seventy-nine cases which got the stereotactic steel wire implantation.In 96 lesions, 13 had steel wire displacement.Among them,5 cases got steel wire displacement during the sereotactic process,5 cases got steel wire displacement after the stereotactic process,2 cases got steel wire displacement during the operation,one case did not show the calcification on the postoperative radiography.Results The steel wire displacement occurred in 5 cases during the stereotactic process came from the patients and doctors respectively and the repositioning was needed.The steel wire displacement after the stereoscopic positioning was attributed to the overdose injection of local anesthesia,which led to the mismatch between the depth of Z axis of the mammary gland and the actual depth the computer given,the incorrect method for needle placement,and,neglecting whether the steel wire have got the lesion anchored when pulling out the needle set of steel wire hood,besides,these three kinds of instances above were all exaggerated by the accordion effect.For the displacement within 2 cm,the lesion can be excised toward the pathological change direction according to the position that steel wire prompted and re-place the second steel wire,putting the J-shaped steel wire into the needle hood and taking it out of the body.After repositioning,2 cases had the steel wire prolapse during operation,which resulted from the over-lifting of the steel wire.After placing the steel wire, the radiologist should give an accurate description on the depth and direction to the surgeon and the notch should be taken for incision from the steel wire head end which is proximate to skin.The postoperative specimen from one case had no calcification,which might be related to the condition that the calcification was located in the gland body,which got destruction from the surgical electrical electrotome.The excisionscope should be extended and the short term reexamination is recommended to make sure the complete excision of the calcification.Conclusion It is the gold standard method that implanting the steel wire with the stereotactic mammography to guide the surgical dissecting technique to diagnose non-palpable breast lesion(NPBL).Thorough understanding of the displacement manifestation of implanting steel wire with stereotactic technique and the treatment methods will be helpful in the surgical dissecting guidance.
4. Intestinal flora changes in a mouse model of transverse aortic constriction

Objective To investigate intestinal microbiota changes of model mice with transverse aortic constriction (TAC) to find a new target point for preventing and treating cardiovascular disease. Methods Twelve C57BL/6 mice were randomly divided into TAC model group (group TN) and control group (group N) of 6 mice each. The group TN received minimally invasive surgery for ligating the aorta to make its constriction to the appropriate degree, while the group N received same operation but no constriction. Faecal samples were collected 22 days after the treatment. Intestinal flora were determined by 16S rDNA pyrosequencing and bioinformatics clustering analysis were performed with software of Quantitative Insights into Microbial Ecology. Results A higher abundance of Parabacteroides and a lower abundances of Lactobacillaceae, Lactobacillus and Cocleatum were present in TAC mice compared with the controls. Conclusion Intestinal flora changes would take place in the TAC mice. Intestinal flora may be a potential target for prevention and treatment of cardiovascular diseases, but further validation should be performed to verify the relationship between those intestinal flora changes and disease progression in animal models.
5.Inhibitory effect of dutasteride on the expressions of epididymal Claudin1 and β-catenin in male rats.

OBJECTIVETo explore the molecular mechanism of dutasteride inhibiting fertility by studying its effects on the expressions of the epididymal epithelial junction proteins Claudin1 and β-catenin in rats.
METHODSSixteen 3-month-old SD male rats were equally divided into an experimental and a negative control group to be treated intragastrically with dutasteride at 40 mg/kg per day and the same dose of solvent, respectively, for 14 consecutive days. Then, the sperm motility and morphology of the rats were detected by computer-assisted sperm analysis, the serum levels of testosterone (T) and dihydrotestosterone (DHT) measured by ELISA, changes in the tight junction of epididymal cells observed under the transmission electron microscope, the protein and gene expressions of Claudin1 and β-catenin determined by RT-PCR and immunohistochemistry, and the conception rate of the mated female rats calculated.
RESULTSDutasteride significantly suppressed the serum DHT level, sperm motility, and fertility of the rats (P <0.05). Interspaces between epididymal epithelial cell tight junctions were observed, the volume of epididymal fluid obviously increased, and the expressions of Claudin1 and β-catenin gene and protein remarkably downregulated in the experimental rats (P <0.05).
CONCLUSIONDutasteride can significantly inhibit the fertility of male rats by reducing the serum DHT level, suppressing Claudin1 and β-catenin expressions, and damaging epididymal epithelial cell junctions.

6.Study on law using Chinese drug of famous old docter of traditional Chinese medicine to coronary heart disease based on association rules.

OBJECTIVETo analyze the law that famous old doctor of traditional Chinese medicine (TCM) treat angina pectoris using Chinese drugs, explore the method that well-known doctor's experience was arranged.
METHODTypical medicine case 115 of angina pectoris were collected and well-known doctor coronary heart disease database making a diagnosis was set up, then used association rules to analyze well-known doctor's prescription compatibility law curing coronary heart disease.
RESULTIn 175 kinds of traditional Chinese medicinal used, the compatibility among blood-activating drug, phlegm-resolving drug and tonic was the most frequently used medicinal groups, Gualou Xiebai decoction, Huoxue decoction and Shengmai powder were the basic formula that well-known doctor treat coronary heart disease, matching and employing between the three form the universal law that the well-known doctor used medicine.
CONCLUSIONDrug groups drew with association rules that well-known doctor treat coronary heart disease reacted well-known doctor's therapeutic thought of curing, would provide the method reference to research in the hereafter.

7.Mental disorder and suicide among youths in rural China: a case control study based on consecutive samples from Hunan, Liaoning and Shandong provinces

Objective To study the prevalence of mental disorders among the Chinese youths aged 15-34 years,in rural areas and to identify risk factors related to suicide.Methods A consecutive sampling strategy was used for suicidal cases in 16 randomly selected counties in Hunan,Liaoning,and Shandong provinces.Between 2005 and 2008,a total of 392 suicide cases were recruited with 416 community controls at the same age range,selected from the same areas one family member together with one close friend of each suicidal case were interviewed,using the psychological autopsy (PA) method.The same method with structured instruments was performed on the two informants for each control in the same community.SCID was used for the diagnosis of mental disease.Results 48.0% of the suicides were diagnosed as having at least one mental disorder episode,in comparison with only 3.8% among the controls.It was found that mental disorder was the most important risk factor for the Chinese young suicide cases in the rural areas.Conclusion As seen in the Western countries,mental disorder had also been the number one correlate on suicidal cases in China,with the difference as other social and psychological factors might have played relatively more important roles in China.

9.Clinical observation and mechanism study on acupuncture combined with massage for treatment of vertebrobasilar insufficiency.

OBJECTIVETo search for the best program increasing clinical therapeutic effect on vertebrobasilar insufficiency (VBI) and to study the mechanism.
METHODSThe patients were randomly divided into an acupuncture plus massage group, an acupuncture group and a massage group. Their therapeutic effects were observed and compared. And transcranial Doppler (TCD) was used for detection of systolic flow velocity (Vs), enddiastolic peak flow velocity (Vd), mean peak flow (Vm) and pulsatility index (PI) before and after treatment.
RESULTSThe cured rate and the total effective rate in the acupuncture plus massage group were significantly higher than the acupuncture group and the massage group (P < 0.05). And there were significant differences in Vm, PI and DI in the left vertebral artery (LVA), the right vertebral artery (RVA) and the basilar artery (BA) before and after treatment (P < 0.01).
CONCLUSIONAcupuncture combined with massage can improve Vm, PI, DI of VA-BA and the clinical therapeutic effect on VBI as well. It is superior to the simple acupuncture therapy and simple massage therapy. At present, it is one of the best therapy for VBI.

10.Development of laparoscopic technology in biliary surgery in the past 23 years: a single-center experience.

OBJECTIVETo summarize the 23-year experience of laparoscopic biliary surgery in General Hospital of PLA and evaluate the application of laparoscopic surgery in the treatment of biliary diseases.
METHODSWe retrospectively analyzed the clinical data of 11 419 consecutive patients with biliary diseases undergoing laparoscopic surgery from April, 1992 and December, 2014. The disease spectrum was compared between patients treated before December 31, 2003 and those treated after the time point.
RESULTSThe 11419 patients receiving laparoscopic surgery accounted for 56.3% of the total patients undergoing biliary surgeries during the 23 years, including 4701 male and 6718 female patients with a mean age of 50.9∓13.2 years (6-93 years). Most (80.83%) of the patients received laparoscopic surgery for gallbladder stones, and 12.53% patients had the operation for gallbladder polyps. The laparoscopic operation rate was 84.81% in patients with gallbladder stones and 34.91% in patients with extrahepatic bile duct stones, but remained low in patients with biliary carcinoma. In laparoscopic operations, laparoscopic cholecystectomy was the most frequent (96.18%) followed by operations for extrahepatic bile duct stones, in which primary suture accounted for 1.38%, traditional T tube drainage for 0.90% and laparoscopic transcystic duct exploration for 0.72%. For malignant tumors, laparoscopic technique was used mainly for the purpose of exploration (0.34%). The application of laparoscopic technique in biliary surgery tended to increase after the year 2004, especially for benign gallbladder diseases and extrahepatic bile duct stones (P<0.05).
CONCLUSIONLaparoscopic technique in biliary surgery is gradually replacing the traditional open operation and becomes the gold standard for the treatment of benign biliary diseases.
