1.Effect of short pulse gastric electrical stimulation on the c-fos expression in nucleus tractus solitarius and hypothalamus and its possible mechanism

Objective To investigate whether enteric nervous system(ENS)involve in mediating the effect of short pulse gastric electrical stimulation on c-fos expression in nucleus tractus solitarius (NTS)and hypothalamus.Methods Fifteen Wistar rats were divided into control group,gastric electrical stimulation(GES)group and ENS free group with 5 rats each.A pair of electrodes was implanted on the gastric serosa.In ENS free group,0.5%benzalkonii chloridum was applied on the serosa during the surgery.A set of short pulse GES with frequency of 20 cpm,pulse width of 300 μs and amplitude of 2 mA was applied via the electrodes for 30 rain in the stimulation group and ENS free group.The c-fos expression in NTS/hypothalamus was detected 1 h after the stimulation.Results The number of neuron with c-fos expression was(71.6±7.4)/HP in NTS group and(63.4±10.8)/HP in ENS free group,both were significantly higher than those in control group(36.4±8.6/HP,P<0.05).The number of neuron with c-fos expression in hypothalamus was higher in GES[(224.2±47.3)/HP]and ENS free groups[(249.1±44.0)/HP]compared with control group[(90.2±47.3)/HP,P<0.05].There was no significant difference in c-fos expression in NTS/hypothalamus between GES group and ENS free group(P>0.05).Conclusions Short pulse gastric electrical stimulation significantly increases c-fos expression in NTS/hypothalamus,which suggests that NTS/hypothalamus may participate in mediating the effect of GES on gastric function.This effect may be not mediated by ENS.
2.Analeptic effect of Shenfu injectio in patients emerging from general anesthesia

Objective To determine if Shenfu injectio(SFI)has any analeptic action in patients emerging from general anesthesia.Methods Eighty-six ASAⅠorⅡadult patients undergoing elective abdominal surgery under general anesthesia were randomly divided into 2 groups(n=43 each):SFI group and control group.The patients were premedicated with intramuscular phenobarbital 0.1g and atropine 0.5mg.Anesthesia was induced with propofol 2 mg?kg~(-1),fentanyl 4-5?g?kg~(-1) and vecuronium 0.1 mg?kg~(-1) and maintained with propofol infusion(2-4 mg?kg~(-1)?h~(-1)),0.5%-1.0% isoflurane inhalation and intermittent i.v.boluses of fentanyl and vecuronium.The patients were intubated and mechanically ventilated.The propofol infusion and isoflurane inhalation were stopped during skin closure.The patients were still unconscious and on mechanical ventilation at the end of surgery and transferred to PACU with a tube in trachea.As soon as the patients reached the PACU,SFI 1 ml?kg~(-1) in Ringer's solution 100 ml was infused over 10 min.In control group the patients received Ringer's solution 100 ml without SFI.The following times were recorded:(1)the time when the patients opened their eyes on command;(2)the time when mechanical ventilation was stopped;(3)the time when oxygen inhalation was stopped;(4)the extubation time;(5)the time of staying in PACU.Venous blood samples were taken before(T_0) and 5,15 and 45 min(T_(1,2,3))after SFI infusion for determination of plasma?-endorphin concentration.Results The awakening time,the mechanical ventilation time,oxygen inhalation time,extubation time and duration of PACU stay were significantly shorter in SFI group than in control group.There were no significant differences in MAP and HR after SFI between the two groups.The plasma?-endorphin concentration was significantly higher in group SFI than in control group.Conclusion Shenfu injectio can make patients emerging from general anesthesia faster.

4.Measurement of left ventricular torsion by 2-dimensional speckle-tracking imaging in patients with chronic cor pulmonale

Objective To evaluate the sensitive indicator of left ventricular rotation/torsion assessed by two-dimensional speckle tracking imaging (STI) and Logistic regression analysis,and to investigate the clinical value of the sensitive indicator for assessment of left ventricular dysfunction in patients with chronic cor pulmonale (CCP).Methods 36 patients with CCP (CCP group) and 38 healthy controls (control group) were included in this study.Imaging in parasternal short-axis view (in basal and apical level) were selected.Parasternal short-axis views at mitral valve and apical levels were collected.Basal peak rotation,apical peak rotation,peak torsion,basal end-systolic rotation,apical end-systolic rotation and end systolic torsion were measured with Echo PAC software.Relevant indicators of left ventricular rotation/torsion were selected by using logistic regression analysis and the regression equation was established.Optimal values of specific parameters (Peak torsion and end-systolic torsion) were calculated with receiver-operating characteristic (ROC) curve.Results Specific parameters of rotation/torsion were significantly reduced in patients with CCP as compared with controls (all P<0.01).Logistic regression analysis showed that end-systolic torsion and peak torsion were correlated with CCP (OR=0.473 and 0.706,P=0.007 and 0.011).Cut-off value of peak torsion for predicting left ventricular dysfunction was 12.070°,the area under the ROC curve (AUC) was 0.819 (95% CI:0.683-0.956),the sensitivity was 84.6%,and the specificity was 73.9 %.Cut-off value of end-systolic torsion for predicting left ventricular dysfunction was 10.680°,AUC was 0.875(95%CI:0.744 1.000),the sensitivity was 84.6%,and the specificity was 91.3%.Conclusions Two-dimensional speckle tracking imaging can sensitively assess left ventricular torsion and evaluate the left ventricular dysfunction in patients with CCP.

6.Construction and Characterization of a ppbR gene Mutant of Pseudomonas sp. M18

Pseudomonas sp. M18, one of plant-growth-promoting rhizobacteria, can produce secondary metabolites including phenazine-1-carboxylic acid (PCA) and pyoluteorin (Plt). PA2572 gene coding protein is a probable two-component response regulator in Pseudomonas according to homologous speculations. In order to investigate its genetic function, PA2572 homologous gene, ppbR, was amplified from M18 genome, inactivated by inserting a Gm cassette. The resulting reconstruct was introduced into the M18 genome using homologous recombination technique, so as to obtain the null mutant M18P. The results showed that the M18P has less flagellar swimming and swarming motility, and yielded fewer PCA. The production of PCA was only 50% of the wild type. However, there was no remarkable difference between mutant and wild type in producing pyoluteorin in KMB medium.

Neuritin is a new neurotrophic factor found recently. In order to identify the function of Neuritin clearly, the coding sequence of human neuritin was amplified by PCR from neuritin cDNA , this fragment digested by NocI and NotI was inserted into pET32a by T4 ligase and transformed into E. coli BL21 then the recombinant plasmid named pET32a-neuritin was constructed successfully . Neuritin was expressed distinctly after inducing by EPTG. The product was identified as neuritin by SDS-PAGE and Western blot analysis . The expression production was purified on Ni2+-NTA column.
9.Normal sperm morphology and the outcomes of routine in vitro fertilization.

OBJECTIVETo explore the application value of morphology assessment of sperm from fresh semen in routine in vitro fertilization (IVF).
METHODSWe analyzed the morphology of the sperm from fresh or optimized semen samples and, based on the sperm morphology of the raw semen, allocated 908 IVF cycles due to the pure tubal factor to different groups: morphologically normal sperm (MNS) ≤ 4%, > 4% - ≤ 15%, and > 15% in Trial 1 and MNS ≤ 1%, > 1% - ≤ 2%, > 2% - ≤ 3%, and > 3%-- ≤ 4% in Trial 2. We compared the rates of fertilization, cleavage, high-quality embryo, -blastocyst formation, and pregnancy among different groups.
RESULTSThe total fertilization rate was significantly lower in the MNS ≤ 4% than in the MNS > 4% - ≤ 15% and >15% groups (74.40% vs 78.61% and 80.03%, P < 0.01). Compared with the MNS ≤ 1%, > 1% - ≤ 2%, and > 2% - ≤ 3% groups, the MNS > 3% - ≤ 4% group showed remarkably increased rates of 2PN normal fertilization (77.23%, 78.97% and 78.99% vs 85.47%, P < 0.01), cleavage (95.71%, 96.01% and 97.27% vs 98.73%, P < 0.05), and blastocyst formation (53.85%, 49.01% and 49.55% vs 63.41%, P < 0.01). No statistically significant differences were observed in the rates of clinical pregnancy, implantation, early abortion, live birth, or malformation at birth among different groups (P > 0.05).
CONCLUSIONMNS ≤ 4% affected the total rate of fertilization while MNS ≤ 3% reduced the rate of normal fertilization in IVF. However, even MNS ≤ 1% did not result in fertilization disorder or failure. Therefore, teratozoospermia alone was not an indicator of ICSI and sperm mor- phology assessment had no obvious value for predicting the rates of embryo quality, clinical pregnancy, and live birth in IVF.

10.Security evaluation of bupivacaine, ropivacaine combined with fentanyl in postoperative continuous epidural analgesia.

OBJECTIVETo investigate the effects, side-effects and security of bupivacaine, ropivacaine combined with fentanyl in postoperative continuous epidural analgesia.
METHODSA total of 1 600 postoperative continuous epidural analgesia patients receiving different agents in SICU were divided into two groups: 0.1% bupivacaine +5 microg/ml fentanyl group (group B, n = 920) and 0.2% ropivacaine +2 microg/ml fentanyl group (group R, n = 680). The effects (visual analog-scale score and content to analgesia), side effects were analyzed retrospectively in the two groups.
RESULTSCompared with group B, patients in group R had higher analgesia contentment (P < 0.05), but no difference in visual analog-scale score was found in the two groups. The incidences of urinary retention, nausea and vomiting, skin itching in group B were significantly higher than those in group R (P < 0.05). In each group, patients over sixty had higher ratio of hypotension than those under sixty (P < 0.05); The female patients had a higher incidence of nausea and vomiting than male patients (P < 0.05); The incidence of debility and numbness of lower limbs in patients with lumbar segments epidural analgesia was higher than those with thoracic analgesia (P < 0.05).
CONCLUSIONS0.1% bupivacaine +5 microg/ml fentanyl and 0.2% ropivacaine +2 microg/ml fentanyl can provide adequate pain relief in postoperative continuous epidural analgesia, and 0.2% ropivacaine +2 microg/ml fentanyl comes with less side effects. The incidence of complication is related with analgesics, age, gender and the position of epidural puncture.
