1.The impact of HBeAg positivity/negativity and HBV DNA loads on the prognosis of chronic severe hepatitis B.

OBJECTIVETo explore the impact of HBeAg positivity/negativity and HBV DNA loads on the prognosis of chronic severe hepatitis B.
METHODS206 patients with chronic severe hepatitis B hospitalized in Beijing Ditan Hospital from July 2002 to Dec. 2004 were analyzed. HBeAg positivity/negativity, HBV DNA loads and other factors relating to the prognosis of the patients were studied with univariate and multivariate analyses.
RESULTSChi2 univariate analysis showed that there was no significant difference in the prognosis between different HBeAg groups (chi2 = 0.440, OR = 0.777, 95% CI 0.424-1.425, P = 0.50). But there was a significant difference in the prognosis between different HBV DNA load groups: the prognosis of patients with lower HBV DNA loads was better than those with higher loads (chi2 = 9.806, OR = 3.055, 95% CI 1.554-6.007, P = 0.002), and the improving rates of the two groups were 53.1% and 27.0% respectively. Using multivariate logistic regression analysis, 9 screened factors showed great impact on the prognosis of chronic severe hepatitis B. Cirrhosis, hepatorenal syndrome, hepatic encephalopathy, PTA < 20%, TBil > 513 mmol/L, Alb < 30 g/L, CHO < 1.6 mmol/L, PLT < 5 x 10(9)/L, and higher HBV DNA loads (HBV DNA > 3 x 10(4) copies/ml in HBeAg negative patients and > 1 x 10(5) copies/ml in HBeAg positive patients) were shown to be associated with a poor prognosis. Coefficients of regression of the above factors were 1.539, 21.356, 1.398, 1.650, 2.440, 2.266, 1.738, 2.631 and 2.656 respectively. The coefficients of regression of HBV DNA loads were: B = 2.656, Wald = 7.768, P = 0.005, EXP(B) = 14.235, and 95.0% CI for EXP(B) = 2.199-92.133.
CONCLUSIONSOur results indicate that the HBV DNA loads were one of the most important factors influencing the prognosis of the chronic severe hepatitis B patients, the importance is only next to hepatorenal syndrome and over grade II hepatic encephalopathy. HBeAg positivity/negativity has no influence on the prognosis, but HBV DNA loads are important; the lower the viral loads, the better the prognosis.

OBJECTIVECytokine mediated cell immunity is the main mode of anti-tumor immunity in organism, and the disequilibrium of cytokine network is the main cause of tumor cells escaping immunologic surveillance. High mobility group box 1 (HMGB1), a nuclear protein, has recently been identified as an important mediator of local and systemic inflammatory diseases when released into the extracellular milieu. In the present study, the investigators explored the clinical significance of alteration in the serum levels of HMGB1 in childhood acute lymphocytic leukemia (ALL) and the mechanism of HMGB1-induced tumor necrosis factor (TNF)-alpha secretion in leukemic cells.
METHODSThe serum levels of HMGB1 in healthy children and childhood ALL were assayed by Western blotting. K562 leukemic cells were stimulated with recombinant HMGB1 protein in vitro, and the secretion of TNF-alpha was determined by using ELISA. The effects of HMGB1 on activation of p38, c-Jun amino-terminal kinase (JNK), and extracellular-signal regulated protein kinase (ERK) and mitogen-activated protein kinase (MAPK) in K562 cells were assayed by using Western blotting. The effects of inhibitors specific for the MAPK on HMGB1-induced TNF-alpha secretion were assayed by using ELISA.
RESULTSThe serum levels of HMGB1 were significantly higher in ALL initial treatment group (n = 15, 43.78 +/- 4.62 microg/ml) than those in healthy control group (n = 15, 0.60 +/- 0.48 microg/ml, P < 0.01) and ALL complete remission group (n = 15, 0.89 +/- 0.62 microg/ml, P < 0.01). No significant difference was found between the healthy control group and ALL complete remission group in HMGB1 levels (P > 0.05). TNF-alpha started to become detectable at 2 h and was still increasing at 16 h after HMGB1 (1 microg/ml) treatment in K562 cell culture. TNF-alpha was also secreted from K562 cells in a dose-dependent manner after HMGB1 (1 ng/ml-1 microg/ml) exposure. HMGB1 induced the phosphorylation of p38, JNK and ERK in k562 cells. Inhibitors specific for the JNK (SP600125), MEK (PD98059), and p38 MAPK (SB203580), abrogated HMGB1-induced TNF-alpha secretion.
CONCLUSIONSThe measurement of serum HMGB1 is helpful to evaluate the prognosis of the childhood ALL. HMGB1 stimulates leukemic cells to secrete TNF-alpha through a MAPK-dependent mechanism.

3.Synthesis and antitumor activity of 20-O-linked camptothecin ester derivatives.

AIMTo improve the profile of 20 (S)-camptothecin, a series of 20-O-linked camptothecin phenoxyacetic acid ester derivatives have been designed.
METHODSThese derivatives were synthesized by the method of acylation. Their chemical structures were confirmed with 1HNMR, IR, MS, and HRMS. The cytotoxicities of the compounds were tested by MTT assay. The in vivo antitumor activities of these esters were evaluated against mouse liver tumor H22 in mice.
RESULTSTwelve derivatives of camptothecin ester are new compounds.
CONCLUSIONIn vitro and in vivo antitumor activity has indicated that some derivatives appeared significantly more effective than topotecan in the H22 mouse liver tumoral model.

4.Quality of life survey on patients with chronic rhinosinusitis by using Chinese version of the 22-item sinonasal outcome test(SNOT-22)

Objective To explore the health-related quality of life(QOL)status of patients with chronic rhinosinusitis. Methods Sixty four patients with chronic rhinosinusitis and twenty healthy volunteers were enrolled, and their QOL scores were assessed by using SF-36 questionnaire(Chinese version)and SNOT-22 questionnaire translated into Chinese. Results The feasibility, reliability,validity,and responsibility of Chinese version of SNOT-22 questionnaire all passed the test. It showed that by Chinese version of SNOT-22 questionnaire the most five important items affecting health status were nasal obstruction,runny nose,loss of smell or taste,dizziness and post-nasal discharge respectively. The coefficient of correlation was 0.233 between the SNOT-22 questionnaire and the Lund-MacKay CT score of Patients with chronic rhinosinusitis. Conclusions Chinese version of SNOT-22 questionnaire can effectively assess the QOL status of patients with chronic rhinosinusitis. It showed less correlation between the SNOT-22 questionnaire and the Lund-MacKay CT score of patients with chronic rhinosinusitis.
5.Quantative analysis of eupatilin and jaceosidin in folium of Artemisia argyi from different areas in China by RP-HPLC based on ancient medicine books.

In order to evaluate the quality of Artemisia argyi from Qichun, Ningbo, Anguo and Nanyang, the contents of eupatilin and jaceosidin were determined by RP-HPLC. The determination was performed on Agilent Eclipse XDB-C₁₈ (4.6 mm×250 mm, 5 μm) with mobile phase consisted of acetonitrile-0.2% phosphoric acid(35∶65) at the flow rate 1.0 mL•min ⁻¹. The detection wavelength was 350 nm and the column temperature was 25 ℃. The results showed that the amount of eupatilin and jaceosidin had a clear linear relationship in the range of 0.003-0.126 g•L ⁻¹ (r=0.999 9) and 0.005-0.200 g•L ⁻¹ (r=0.999 9), and the average recovery rates for them were 99.14% (n=6, RSD 1.2%) and 99.40% (n=6, RSD=0.73%), respectively. The results showed that RP-HPLC can be used for the quantification of eupatilin and jaceosidin in the folium of A. argyi. With this method, we found there was no significant difference of jaceosidin content within all the samples collected, but the content of eupatilin was significantly higher in samples from Qichun, Ningbo, Xiangyang and Nanyang, located in the south of Huaihe River compared with these from other areas.
6.Clinical and molecular biologic characteristics of 36 cases of leukemia with 11q23/mll.

This study was aimed to analyze the clinical and cytogenetic characteristics of acute leukemia with 11q23/mll rearrangement and explore the reasonable therapeutic principles. Characteristics in general situation, morphology, immunology, molecular biology, cytogenetics, treatment and overall survival of 36 cases of acute leukemias with mll gene rearrangement were studied and analyzed. The results showed that 36 cases with mll gene rearrangement were found positive (7.2%) in 494 patients with acute leukemia. Among the 36 cases of mll rearrangement positive, 32 cases were diagnosed as acute myeloid leukemia (AML) with myeloid antigen expression, of which 5 cases expressed lymphoblastic differentiation antigen; 4 cases were classified as B-lineage acute lymphoblastic leukemia (ALL), of which non-lineage myeloid expression pattern were found in 3 cases. In 29 out of 36 cases (80%) the clonal chromosomal aberration were detected, of which chromosome 11 aberration were observed in 22 cases. All patients received chemotherapy with a total response rate of 47.2%. Of the responded patients, 10 cases relapsed within 6 months, with a recurrence rate of 40%; 9 cases received hematopoietic stem cell transplantation (HSCT), 7 cases of which survived after transplantation. The median survival time of 36 cases was 16 months (range 2 - 46) and their 2-year overall survival rate was 41.4%. The 2-year overall survival rate of 9 patients who received HSCT was 87.5%. It is concluded that acute leukemia patients with mll gene rearrangement show poor response to chemotherapy, high recurrence rate and poor prognosis. Hematopoietic stem cell transplantation may be a reasonable treatment principle to improve these patients' survival situation.

7.In vitro study of directional inducible differentiation of inner ear hair cells from human adipose-derived mesenchymal stem cells

Objective To explore the feasibility of directionally inducing human adipose-derived mesenchymal stem cells (hAD-MSCs) towards inner ear hair cells. Methods Mesenchymal stem cells from human adipose tissue were isolated, purified and cultured in vitro, hAD-MSCs were induced to neural stem/progenitor-like cell, and then co-cultured with embryonic chick otic vesicle cells. Processed hAD-MSCs were tested by immunostaining to ascertain whether they expressed characteristic hair cell markers. Results Morphologically, hAD-MSCs were induced to differentiate into neural stem/progenitor cells and expressed specific neural markers. After being co-cultured with embryonic chick otic vesicle cells, hAD-MSCs expressed specific surface markers of inner ear hair cells. Conclusions hAD-MSCs can be directionally induced to differentiate towards hair cell-like cells in vitro.
8.Design and implementation of data checking system for Chinese materia medica resources survey.

The Chinese material medica resources (CMMR) national survey information management system has collected a large amount of data. To help dealing with data recheck, reduce the work of inside, improve the recheck of survey data from provincial and county level, National Resource Center for Chinese Materia Medical has designed a data checking system for Chinese material medica resources survey based on J2EE technology, Java language, Oracle data base in accordance with the SOA framework. It includes single data check, check score, content manage, check the survey data census data with manual checking and automatic checking about census implementation plan, key research information, general survey information, cultivation of medicinal materials information, germplasm resources information the medicine information, market research information, traditional knowledge information, specimen information of this 9 aspects 20 class 175 indicators in two aspects of the quantity and quality. The established system assists in the completion of the data consistency and accuracy, pushes the county survey team timely to complete the data entry arrangement work, so as to improve the integrity, consistency and accuracy of the survey data, and ensure effective and available data, which lay a foundation for providing accurate data support for national survey of the Chinese material medica resources (CMMR) results summary, and displaying results and sharing.

In this paper, a data acquisition system based on mobile terminal combining GPS, offset correction, automatic speech recognition and database networking technology was designed implemented with the function of locating the latitude and elevation information fast, taking conveniently various types of Chinese herbal plant photos, photos, samples habitat photos and so on. The mobile system realizes automatic association with Chinese medicine source information, through the voice recognition function it records the information of plant characteristics and environmental characteristics, and record relevant plant specimen information. The data processing platform based on Chinese medicine resources survey data reporting client can effectively assists in indoor data processing, derives the mobile terminal data to computer terminal. The established data acquisition system provides strong technical support for the fourth national survey of the Chinese materia medica resources (CMMR).
10.Production suitability regionalization study of Pinus massoniana.

The distribution, yield and sample information data of Pinus massoniana was obtained by document literature and sample investigation. Based on sample data from 12 provinces including 414 sample plots and environment factors in China,the distribution regionalization of P. massoniana was predicted by using Maxent and spatial analysis function of ArcGIS. The results showed that the northernmost distribution of P. massoniana was 33.5 degrees north latitude, and it mainly distributed in the southeast in China. Based on plant age, plant height, yield per plant and other growth index from 414 sample plots, combined vegetation form and other data, the growth regionalization of P. massoniana was carried out by using SPSS and related functions of ArcGIS. The results showed that Fujian, Guizhou and Guangxi had a lager distribution area of P. massoniana, meanwhile, it had a relatively higher yield of fresh pine needles. The relational model between environmental factors and shikimic acid,and procyanidin, and the total lignans was constructed by using SPSS regression analysis method. Then the spatial calculation function of ArcGIS was used tocarry out the quality regionalization of P. massoniana based on the relational model. The results showed that east of Sichuan, Guizhou, Chongqing had a good pine needles quality. Based on the distribution, growth and quality regionalization, the production suitability regionalization of P. massoniana was carried out. The results showed that the optimal planting base region mainly distributed in east of Sichuan, middle and east of Guizhou, and east of Guangxi.
