AIM: To investigate the clinical efficacy and safety of intravitreal injection of triamcinolone combined macular grid photocoagulation treatment for macular edema.
METHODS: Totally 150 cases (150 eyes) with macular edema in our hospital from July 2009 to November 2013 were selected, which were randomly divided into study group (75 cases, 75 eyes) and control group (75 cases, 75 eyes) . The cases in control group were treated with macular grid photocoagulation treatment, those in the study group used triamcinolone acetonide combined macular grid photocoagulation treatment. Best corrected visual acuity ( BCVA ) , parallel optical coherence tomography ( OCT) and fundus fluorescein angiography (FFA) were detected before treatment, after treatment 7d, 1, 3, and 9mo.
RESULTS:After the treatment, patients' vision were significantly improved in two groups (P<0. 05). In the study group 7d, 1, 3, and 9mo after operation, the visual acuity was better than the control group and preoperative (P<0. 05); fovea macular neurosensory layer thickness decreased significantly (P<0. 05). In the control group, the point omentum macular neurosensory retinal thickness was not statistically significant at 7d, 1, 3, and 9mo after operation compared with before treatment (P>0. 05). Fovea macular neurosensory retinal thickness in the study group was significantly lower than that in control group (P<0. 05). Intraocular pressure of 7 cases in the study group increased slightly, and were normal after treatment.
CONCLUSION: Triamcinolone acetonide combined macular grid photocoagulation treatment is accurate, can effectively improve the visual acuity, reduce macular edema, it is safe and reliable, and suitable for clinical application.

To explore the diagnosis and treatment of emphysematous pyelonephritis so as to improve its diagnosis and reduce its mortality. The clinical data were retrospectively analyzed for 2 diabetic patients with emphysematous pyelonephritis.Their symptoms included high fever, chills and lumbago.Computed tomography (CT) scans were performed to reach a definite diagnosis of emphysematous pyelonephritis with gas within renal parenchyma and/or perirenal tissue. Both received intravenous antibiotics. One patient underwent open abscess drainage while another percutaneous puncture under ultrasonic guidance.Their conditions improved and healed.CT scan is an efficient imaging modality for early diagnosis. The combination of antibiotics and early drainage may offer a better outcome.

Objective To observe the preventive effect of mannatide on infection and relapse of idiopathic thrombocytopenic purura(ITP).Methods One hundred and twenty children with ITP were randomly divided into mannatide treatment group and prednisone control group.Control group venous dexamethasone of 3 d;then treated by prednisone.Treatment group added mannatide tablets for 1 month. The rates of remission clinical blood, platelet, control time, complicated infection and relapse rates were observed. The levels of plasma immunoglobulin(Ig)G,IgA,IgM were determined before and after mannatide treatment. Results The rate of clinical blood, platelet, control time, infection time was not different in 2 groups. The rates of infection complicated and relapse were all significant lower than that in control group. The plasma IgG,IgA significantly increased than that in control group. The plasma IgM had no significant difference. Conclusion Vaccine therapy can be helpful in protecting and decreasing infection, diminishing relapse of children with ITP,and improve the level of IgG,IgA,and thus improve their immune function.

Objective To approach rehabilitation nursing measures in using the allogeneic ten-don to re-establish the anterior cruciate ligament of knee through the tibia twin tunnel under arthroscopy. Methods Mental nursing and direct functional exercises was given to 30 patients who received this kind of operation from November,2006 to July,2007,and functional exercises of flection and extention of knee joint by steps, functional exercises of muscle force of quadficeps muscle and back muscle group of legs and the correct using of artificial brace of knee joint. Results Range of joint motion could reach 95°after 2 weeks and 125°after 4 weeks postoperation.Score of joint function(Lysholm)could reach(74.8±5.7)after 6 weeks,(80.0±2.3)after 9 weeks,(91.8±3.4)after 12 weeks, and(94.5±2.2)after 1 year. Conclusion Selective perioperative rehabilitation nursing instruction is the important guarantee of function recovery.

OBJECTIVETo study the protective effect of Ligustrazine Injection (LI) against cisplatin-induced ototoxicity and to explore its mechanism.
METHODSThirty healthy adult guinea pigs were randomly divided into three groups, 10 in each group, i.e., the normal control group, the cisplatin group, and the LI group. Guinea pigs in the normal control group were intraperitoneally injected with normal saline at 3 mL/kg for 7 consecutive days. Those in the cisplatin group were intraperitoneally injected with cisplatin at 3 mg/kg for 7 consecutive days. Those in the LI group were intraperitoneally injected with LI at 140 mg/kg for 7 days, but cisplatin (3 mg/kg) was intraperitoneally injected from the opposite side starting from the 4th day. Brainstem auditory evoked potential (BAEP) was performed in all animals before and after injection. All animals were sacrificed after the final testing under anesthesia and their cochleas collected. Half the cochleas of each group were collected for silver nitrate staining of cochlear basilar membrane stretched. The other half the cochleas of each group made into paraffin sections to observe the apoptosis of cochlea cells by TUNEL method and the expression levels of c-Jun detected by immunohistochemistry.
RESULTSThere was no statistical difference in the difference of BAEP threshold among the 3 groups before injection (P > 0.05), but the BAEP threshold increased in the cisplatin group and the LI group (P < 0.05). Besides, it was higher in the cisplatin group (P < 0.05). In the cisplatin group, most hair cells were missing, spiral ganglion cells obviously decreased, more TUNEL positive cells occurred, and the expression of c-Jun was stronger. But the aforesaid impairment in the LI group was obviously lessened (P < 0.05).
CONCLUSIONSLI showed certain antagonist effect on cisplatin-induced ototoxicity. Its mechanism might be associated with scavenging oxygen radicals of the cochlea tissue, improving the microcirculation, and fighting against apoptosis.

Microbiology 1992;0(03):-
The sfa1 gene encoded a bifunctional enzyme with the activities of both alcohol dehydrogenase and glutathione-dependent formaldehyde dehydrogenase in Saccharomyces cerevisiae. The gene disruption cassette produced by PCR using the same long oligonucleotides which comprise 19 or 22 nucleotides complementary to sequences in the templates(pUG6 and pUG66 marker plasmid)at 3' end and 45 nucleotides at 5' end that annealed to sites upstream or downstream of the genomic target sequence to be deleted. After two linear disruption cassettes with a Cre/loxP mediated marker were transformed into the cells of Saccharomyces cerevisiae YS-1,the positive transformants were checked by PCR to correct the integration of the cassette and concurrent deletion of the chromosomal target sequence.Once correctly integrated into the genome, the select marker can be efficiently rescued by transformating the plasmid pSH47 into YS-1 and inducing the Cre expression with a Cre/loxP-mediated marker removal procedure. The expression of the Cre recombinase finally resulted in the removal of the marker gene, leaving behind a single loxP site at the chromosomal locus. The diploid mutant YS-1-sfa1 was generated, which could enhance the output of ethanol with 8.0% by shaking culture in flask compared with the original strain YS-1.
