2.Anterolateral minimally invasive plate osteosynthesis technique for distal humeral shaft fracture

Objective: To evaluate the clinical effcacy of the modiifed anterolateral minimally invasive plate osteosynthesis technique for distal humeral shaft fracture, and to explore its feasibility, security, advantage and disadvantage. Methods: hTe clinical effcacy of 17 patients with distal humeral shatf fracture, who were treated with the anterolateral humerus minimally invasive plate osteosynthesis during 2009 to 2012, were retrospectively analyzed. hTe operative time, bleeding volume, complications, esseous union time and range of motion (ROM) of elbow were recorded, and the functional outcome of elbow joint was evaluated by Mayo elbow performance score (MEPs). The varus angle was measured in the malunion patients atfer the distal humeral shatf fracture healed. Results: All of the 17 patients obtained bony union at an average of 19.2 weeks postoperatively, an average of 4.5 screwes were inserted in distal humerus. hTe mean ROM of elbows was 133° and the MEPS were 98.2. Seven patients suffered humeral malalignment and the mean varus degrees were 8.3°. Conclusion: hTe technique of anterolateral humerus minimally invasive plate osteosynthesis is safe and feasible for distal humeral shatf, and the satisfactory clinical outcomes can be obtained by this modiifed technique. However, some of the patients may appear malunion with varus angulation of humerus.
3.Ophthalmic manifestations of tuberous sclerosis: CT and MRI findings

Objective To describe CT and MRI imaging findings of retinal hamartomas with tuberous sclerosis and evaluate their clinical value. Design Retrospective case series. Participants 7 patients of tuberous sclerosis complex (TSC) who have ocular lesions found with CT and MRI examination. Methods 7 patients with tuberous sclerosis were diagnosed by clinical data and head CT. The size, morphology, density and enhancing situation of the retinal lesions were observed with CT and MR.I, as while as with direct or indi-rect fundoscopy. Main Outcome Measures CT and MRI features of ocular lesions. Results 7 cases displayed intraeranial calcification and/or non-calcified tubercles, situating in subendyrna (7 cases, accounting for 100%) by head CT. Orbital CT and MRI showed 4 cases (57.1%) of flat retinal hamartomas 3 cases (42.9%) 9f retinal lobular and nodular hamartomas, 2 cases(28.6%) scattered calcification patches in hamartoma, and 2 cases (28.6%) of nanophthalmos. The lesions displayed unenhancement on contrast-enhanced MR images in 7 cases(100%) . There was 1 case (14.3%) of progressive retinal astrocytic hamartoma. Conclusion The examination of CT and MRI may find the bigger retinal hamartomas, which can play a very important role at the diagnosis with the help of whole brain examination. For progressive retinal nstrocytic hamartoma,systemic evaluation with CT and MRI can provide evidence for choosing appropriate thera-pies. (Ophthalmol CHN, 2009, 18: 257-260)

5.Analysis of the failed internal fixations for thoracolumbar fractures

Objective To discuss the common causes for postoperative complications after treatment of spinal fractures and their preventive measures.Methods To analyze the 32 failed eases who received internal fixation for their thoracolumbar fractures between July 1998 and April 2005 in our department.Their preoperative and postoperative results of X-ray,CT and MIR examinations were reviewed to find out the causes for failure.Three of them were treated through anterior approach and 29 through posterior approach.Eighteen screws broke in eight eases, the rods got loosened in eight cases and broken in four cases,15 pedicte screws were mis-located in eight cases and the implants got loosened in three cases.Results Thirty patients had to get a reoperation.Two were cured through non-surgical treatment.The causes for failure were found to be as follows:1,incorrect operation approach;2,in- stability of the front-middle colunm;3,unskillful operation;4,ignorance or the lesion to the adjacent vertebral discs; 5,flaws in pedicle screw design.Conclusion To prevent postoperative complications after internal fixation for spinal fractures,surgeons should strictly stick to the indications before operation,choose a proper approach and method for internal fixation,try to stabilize the front-middle column by the first intention,and have a good command of anatomy and operation skills.
6.Analyze Antibacterial Resistance Character of Stenotrophomonas maltophilia in ICU

Stenotrophomonas maltophilia is among eight strain of broad resistance to antibacterial. It has been pay attention to global students, and is very difficulty in clinic treatment. Infection of Stenotrophomonas maltophilia is more familiar in ICU. Its test rate is 13.45%(62/461) in the study .The resistance character: IPM﹑CZO﹑CRO﹑AMP and MNO is highest(resistive rate 100%). ATM、GEN 、 KAN 、MEM 、CTX and PIP is much high(≧86.5%), LVX﹑GAT and FEP is high (=50%).Conclusion: Stenotrophomonas maltophilia should be monitored in the hospital. Used of antibacterial must be rational. Severe disinfection and isolation should been put in practice to the infected patients. CSL is very active antibacterial (active rate 90.6%) against Stenotrophomonas maltophilia in the study, and ought be selected headmost in treated its infection.
7.Effect of Sinusitis Mixture on Mucosa Cells after Functional Endoscopic Sinus Surgery.

OBJECTIVETo explore the application of sinusitis mixture (SM) in endoscopic sinussurgery, thereby improving clinical curative rate of chronic sinusitis and nasal polyps.
METHODSA totalof 50 chronic sinusitis patients were equally assigned to the experimental group (nasal douching by SM)and the control group (nasal douching by Compound Sodium Chloride Injection). Mucosa tissue 0.1 cmbefore natural opening was collected before surgery, at week 4, 12, and 24 after surgery. Changes ofmucosa cilia cells, goblet cells, stroma of mucosal membrane, inflammatory cells, and mucous glandwere observed. The numbers of goblet cells in the upper epithelia and ciliated cells, as well as their ratioswere calculated.
RESULTSThere was statistical difference in cavity cleaning time, cavity mucosal epithelization time, numbers of goblet cells in the upper epithelia and ciliated cells, as well as their ratio between the two groups (t = -2.342, -2.015, -2.145, respectively; P < 0.05).
CONCLUSIONSM could effectively promote and accelerate cleaning and mucosal epithelization of functional endoscopic sinus surgery, and significantly promote mucosal ciliary structure and function recovery of ostium-meatus nasicomplex.

8.Feasibility Study of Test Method for Histamine-like Substances in Aclarubicin Hydrochloride

Objective:To establish a test method for histamine-like substances in aclarubicin hydrochloride. Methods:According to the method in the appendix of Chinese Pharmacopoeia ( edition 2010 ) , the test of histamine-like substances was carried out. Re-sults:The test of histamine-like substances was not interfered by aclarubicin hydrochloride with the concentration below 0. 01 mg· ml-1 . Histamine-like substances in 4 batches of samples were determined by the established method, and the results were consistent with those of the test method for depressor substances. Conclusion: The test method for histamine-like substances is feasible with the limit of 1 μg·mg-1 .
9.Differential expression and influence of caoguo zhimu decoction on kindling epilepsy related genes.

OBJECTIVETo screen the relative genes associated with the genesis of kindling epilepsy and the effect of caoguo zhimu decoction (CZD) on these genes.
METHODSmRNA differential display reverse transcription PCR was used to screen differential gene bind, and Northern blotting hybridization was applied to exclude the false positive reaction.
RESULTSThere were as more as 110 differential expression bindings in the normal group, model group and the treated group, among them 11 binds with differential expression only appeared in the model group but not in the normal group and disappeared in the treated group. It indicated that the 11 genes are correlated with the genesis and treatment of kindling epilepsy. By searching in the Gene-bank of NCBI, 7 bindings of the 11 were homologous genes and 4 gene fragments are novel genes with unknown function, which have been registered at the Gene-bank, with the registered numbers of CK325391, CK325392, CK325393, CK325394, respectively, and false positive possibility of 3 novel genes was excluded.
CONCLUSIONCZD has effect in treating kindling epilepsy may be through influencing the expression of partial gene fragments in the hippocampus.

10.Discussion of Chinese syndrome typing in acute hepatic failure model.

OBJECTIVETo study Chinese syndrome typing of acute hepatic failure (AHF) mice model by screening effective formulae.
METHODSLipoplysaccharides (LPS)/D-galactosamine (D-GaIN) was intraperitoneally injected to mice to establish the AHF mice model. Yinchenhao Decoction, Huanglian Jiedu Decoction, Buzhong Yiqi Decoction, and Xijiao Dihuang Decoction were administered to model mice respectively by gastrogavage. The behavior and the survival rate were monitored. The liver function and pathological changes of liver tissues were detected.
RESULTSIn all the tested classic recipes, the survival rate was elevated from 10% to 60% by administration of Xijiao Dihuang Decoction. Five h after modeling, the serum alanine aminotransferase (ALT) level was (183.95 +/- 52.00) U/L, and aspartate aminotransferase (AST) (235.70 +/- 34.03) U/L in Xijiao Di-huang Decoction Group, lower than those of the model control group, but with insignificant difference (ALT: 213.32 +/- 71.93 U/L; AST: 299.48 +/- 70.56 U/L, both P > 0.05). Xijiao Dihuang Decoction could obviously alleviate the liver injury.
CONCLUSIONSXijiao Dihuang Decoction was an effective formula for LPS/D-GaIN induced AHF model. According to syndrome typing through formula effect, heat toxin and blood stasis syndrome dominated in the LPS/D-GalN induced AHF mice model.
