Background Vitretomy and lenstomy with silicone oil tamponade is an effective method for complicated vitreous retinopathy. The severe anisometropia after silicone retention is usually treated by two-point transscleral suture fixation for posterior chamber intraocular lens(IOL)implantation. In order to reduce the number and difficulty and complication of the operation, the surgical method should be improved. Objective The goal of this study was to observe the resuh of silicone oil removal combined with four-point trans-scleral suture fixation of posterior chamber IOL after vitrectomy. Methods A retrospective case-observational study design was adopted.Twenty eyes with silicone oil tamponade from 20 patients without lens and posterior capsule after vitrectomy were included in this study.Silicone oil removal with four-point trans-scleral suture fixation of posterior chamber IOL was performed. The anterior ocular inflammatory response, intraocular pressure, uncorrected and corrected acuities before and after operation, corneal endothelial cell counting and postoperative complications were observed and analyzed.Written informed consent was obtained from all patients prior to the operation. Results All of the operative eyes in this study showed improvement of visual acuity after operation.Of the 20 eyes,a visual acuity of ≥0.8 was seen in 2 eyes,0.6-0.7 in 6 eyes,0.3-0.5 in 8 eyes and 0.05-0.2 in 4 eyes 3 months after the removal of silicone oil.The uncorrected acuity postoperation was significantly improved in comparison with preoperation(H=10.147,P<0.01),but no significant difference was found in the corrected acuity between preoperation and postoperation(X =2.089,P< 0.01).The number of the corneal endothelial cells was(2064±329)cells/mm2 before operation, and that after operation was(1987±269)cells/mm2,showing an insignificant change between them(t =1.660,P > 0.05).No abnormality of IOL position was found in all 20 operated eyes. There was not serious postoperative complication in all 20 patients. Conclusions The combination of silicon oil extraction with four-point transscleral suture fixation IOL is effective in eyes without posterior capsule or lens after vitrectomy. It can reduce the operation time and improve the postoperative acuity and the quality of life of patients.

To investigate the relationship between Snail and renal tubular epithelial-mesenchymal transition (EMT) or tubulointerstitial fibrosis (TIF) at tissue and cellular levels and to observe the changes of miRNA profile after transfecting Snail gene into human renal tubular epithelial cells (HK-2),to further elucidate the importance of miRNA in the pathogenesis of renal fibrosis. Methods The expression of Snail and EMT-related proteins vimentin,SMA,E-cadherin was detected by immunohistochemistry in renal tissues of 40 patients with IgA nephropathy. The expression of Snail,E-cadherin and SMA in normal control group, empty transfection group and Snail gene transfection group was detected by Western blot and RT-PCR.Furthermore,differentially expressed miRNAs were screened by gene chip. Results By immunohistochemistry,Snail expression was positively correlated with vimentin and SMA,negatively correlated with E-cadherin in IgA nephropathy. The higher degree of the TIF,the stronger the expression of Snail.Compared with the control group, the expression of Snail, vimentin and SMA in the snail transfected group increased.However,E-cadherin decreased at gene and protein level by the RT-PCR and Western blot (P < 0.05).The difference was statistically significant.Five distinctly different miRNAs were screened by gene chip after Snail gene was transfected into HK-2 cells, and then 5 026 potential target genes were predicted. Conclusion Snail expression is closely related with renal tubular epithelial mesenchymal transition and tubulointerstitial fibrosis, and it may be used as a new target in EMT prevention.Differentially expressed miRNAs may be involved in the development of EMT and TIF.

Objective: To investigate the effects of electro-acupuncture on obestatin in rat with simple obesity. Methods: Obesity was induced in male SD rats with high-fat and -glucose diet for 10 weeks. Twenty-four rats with obesity were randomly divided into untreated group and electro-acupuncture group, with 12 rats in each group. Another 12 normal SD rats serving as controls were fed standard rat chow. Rats in the electro-acupuncture group were treated with electro-acupuncture on bilateral Zusanli (ST36), Tianshu (ST25) and Pishu (BL20) acupoints for fifteen days consecutively. Rats in the blank control group and untreated group were not interfered. Body weight was measured every 3 days during treatment. After treatment, obestatin in serum and hypothalamus was detected by radioimmunoassay or immunohistochemiscal method respectively. Visceral fat weight was also detected. Results: Body weight and visceral fat weight of rats in the untreated group were significantly higher than those in the blank control group (P<0.01). Body weight and increase of body weight in the electro-acupuncture group were significantly lower than those in the untreated group (P<0.01). Visceral fat weight in the electro-acupuncture group was significantly lower than that in the untreated group (P<0.01), but there was no significant difference as compared with the blank control group. The obestatin expression in hypothalamus was higher in the untreated group than in the blank control group (P<0.05), while it was lower in the untreated group than in the electro-acupuncture group (P<0.01). Conclusion: Electro-acupuncture has an effect against obesity in rats, which may be related to up-regulating obestatin expression in hypothalamus.
