1.Clinical and pathological features of leprous neuritis (report of 1 case)

Objective To explore the clinical and pathological features of leprous neuritis.Methods Sural nerve biopsied specimens from one patient with leprous neuritis were analyzed with histopathology and electron microscopy.Results The main clinical characteristics included injuries of sensory nerves with numbness and pain in the distal limbs, vegetative nerve functional disturbance, and the asymmetrical bulky ulnar nerve. The pathological changes were the presence of lymphocytic infiltrates in the endoneurial and epineurial spaces, a mild stenosis of small vessel with a large number of perivascular lymphocytic and mononuclear phagocytic infiltrates combined with epithelioid cells form. Anti-acid staining demonstrated the positive leprosy bacillus in nerve fibers. A nearly completely loss of myelinated fibers and obvious connective tissue proliferation were also seen in the sural nerve. The electron microscopy observed marked axonal degeneration of unmyelinated nerve fibers with many leprosy bacillus in Schwann' cells. This case was treated with Aminophenylsulfone and Dehydrocortisone for one year and his condition had improved.Conclusions Diagnosis of leprous neuritis depends on the nerve biopsy, and the early treatment may relieve disability.
3.Improvement of Forming Process of Jingning Particle

Objective:To improve the forming process of Jingning particle according to the influencing factors in its effectiveness, such as low yield, high moisture absorption and difficult dissolubility during oral administration. Methods:A new technology that im-mediate granulation after the extract was well mixed with lactose and dextrin and dried. The appearance, dissolubility, hygroscopicity and pellet formation rate ( granularity) were compared between the new technology and the old one, and the difference in critical rela-tive humidity was also studied. Results:The appearance, dissolubility and pellet formation rate of the new technology were all better than those of the old one, and the moisture absorption rate was reduced with the critical relative humidity up to 70%(25℃), which enhanced the granule stability. Conclusion:The pellet formation rate is improved by the new technology, which effectively solves the problems such as high moisture absorption and poor dissolubility, and the granule quality is improved.
4.Gemcitabine plus carboplatin regimen in treatment of advanced non-small cell lung cancer in aged patients

Objective To evaluate and the effects and safety of gemcitabine plus carboplatin (GEM+CBP) regimen and navebine plus cisplatin (NVB+DDP) regimens for aged patients with advanced non-small cell lung cancer (NSCLC). Methods The 60 cases of aged patients with NSCLC,over 65 years old and 35 cases beyond 70,were randomly and equally divided into 2 groups. Group A received 1 000 mg/m2 GEM at day 1 and 8,and 300 mg/m2 CBP at day 1. Group B underwent the therapy of 25 mg/ m2 NVB at day 1 and 8,and 80 mg/m2 DDP at day 1 and 3. Both were treated with 3 weeks as a cycle. Results Overall response rate was 43.3% in group A and 36.7% in group B without significant difference. The median survival time (MST) and disease median response time(DPT) was 9.2 and 4.4 months in A group,and 8.9 and 4.2 months in B group. MST,DPT and one year survival rate were similar in 2 groups. The Medical Healthcare Service Center,located at the Youxian District,Mianyang City of Sichuan Province,successfully rescued the people affected by the Wenchuan earthquake and the Tangjiashan barrier lake with great help from the senior hospitals.We suggest that the community healthcare service center could play an important role in emergency response system.

7.Autologous transfusion of drained blood from abdomen for conservative treatment of blunt trauma of spleen

Objective To study the safety and feasibility of auto-transfusion of blood drained from patients own abdomen in patients with blunt trauma of spleen as conservative treatment.Methods From January 1,2008 to December 31,2010,94 patients were diagnosed to be blunt trauma of spleen.After careful assessment and series lab tests,44 patients were eligible to be non-operative treated.They were randomly (random number) into two groups in equal number (n =22).The patients of auto-transfusion group were transfused the blood drained from patients own abdomen,and the control group was transfused red blood cells from donors.The comparison of demographics,ISS (injury severity scores),AAST (American Association for the Surgery of Trauma) and lab findings between two groups before and after transfusion with t test and chi-square test.Then paired-samples t test was used to analyze the hemodynamics,blood components and blood clotting indexes of two groups before and after blood transfusion.Meanwhile t test was done as well to compare the differences in the above variables between two groups after transfusion.SPSS 10.0 version was used to analyze the collected data.Results There were no significant differences in demographics and physical condition between two groups.After blood transfusion,the hemodynamics and anemia of two groups were significantly improved,and there were no differences in the changes of the above variables between two groups.Conclusions Blood drained from patient own abdomen can be auto-transfused in patients with blunt trauma of spleen treated non-operatively as safe and efficient as transfusion of blood from donors.This study proves the autologous transfusionto to be an easy,feasible and economic measures in urgent situation as expected.
8.Effect of C-erbB-2 shRNA on chemosensitivity of mouse lung adenocarcinoma cells and its mechanism

Objective To investigate the effect of C-erbB-2 shRNA on chemosensitivity of mouse lung adenocarcinoma cells and its mechanism,and to find new therapy method for non-small cell lung cancer,especial lung adenocarcinoma.Methods The mouse lung adenocarcinoma Lewis cells were cultured regularly and divided into non-transfected group, pGPU6/RFP/Neo-shNC group and pGPU6/RFP/Neo-erbB-2 group. The plasmids were synthesized and transfected into Lewis cells in each group by Lipofectamine 2000.The expression levels of C-erbB-2 mRNA and protein in the cells in various groups were tested by RT-PCR and Western blotting method, respectively. The Lewis cells were divided into non- transfected group, pGPU6/RFP/Neo-shNC group, carboplatin group, pGPU6/RFP/Neo-erbB-2 group, pGPU6/RFP/Neo-shNC+carboplatin group and pGPU6/RFP/Neo-erbB-2+ carboplatin group. The apoptotic rates of the cells in each group were detected by flow cytometry;the expression levels of Bcl-2 and Bax proteins in each group were determined by Western blotting method.Results The expression levels of C-erbB-2 mRNA and protein in pGPU6/RFP/Neo-erbB-2 group were lower than those in non-transfected group and pGPU6/RFP/Neo-shNC.The apoptotic rate of the cells in pGPU6/RFP/Neo-erbB-2+carboplatin group was the highest in all of the groups (P<0.01);compared with the others, the expression of Bax protein in pGPU6/RFP/Neo-erbB-2+carboplatin group was increased, while the expression level of Bcl-2 protein was decreased.Conclusion C-erbB-2 shRNA can increase the Lewis cells’sensitivity to carboplatin.The mechanism may be that it can enhance the Lewis cells’apoptosis induced by carboplatin through increasing the expression of Bax protein and decreasing the expression of Bcl-2 protein.
9.Study on the perioperative changes of electrogastrogram of thoracic (tube) stomach in patients with esophageal cancer

Objective To investigate the perioperative characteristics and changing trends of gastric electrical activity of thoracic (tube) stomach in patients with esophageal cancer.Methods The clinical data of 30 patients with esophageal cancer who were admitted to the Sichuan Cancer Hospital between March 2013 and November 2013 were prospectively analyzed.All the eligible patients underwent esophageal cancer resection by Ivor-Lewis according to the inclusion criteria.The electrogastrograms of patients were recorded at preoperative day 1 and at postoperative day 3,7, 11 and 30.The electrogastrograms of patients at preoperative day 1 were used as the control.The parameters of electrogastrogram were analyzed including main frequency, coefficient of dominant frequency instablility, main power, postprandial/preprandial power ratio, percentage of normal gastric slow wave,percentage of slow gastric slow wave and percentage of tachycardia gastric slow wave.Measurement data with normal distritution were presented as x ± s, and measurement data with skew distritution were presented as M (Qn).The postoperative time and pre-and post-prandial electrogastrograms were compared by the repeated measures two-way ANOVA.The comparison between groups were evaluated with the LSD test and analysis of variance.Results Thirty patients were screened for eligibility with a mean age of 62 years (range, 49-75 years), including 26 males and 4 females.The pre-and post-prandial main frequencies were changed from 2.83 ± 0.13 and 3.01 ± 0.17 before operation to 2.66 ± 0.10 and 2.82 ± 0.10 at postoperative day 30 with coherent changing trend.The main frequencies at postoperative each time points were significantly lower than those before operation while postprandial above indicators were higher than preprandial those, showing a significant increasing trend with the passage of postoperative time (F =285.62, P < 0.05).There was no interaction between the time and meal (F =0.22, P > 0.05).The pre-and post-prandial coefficients of dominant frequency instablility were changed from 0.133 ±0.031 and 0.045 ±0.019 before operation to 0.150 ±0.043 and 0.115 ±0.010 at postoperative day 30 with coherent changing trend, and coefficients of dominant frequency instablility at postoperative each time points were significantly higher than those before operation while postprandial above indicators were lower than preprandial those, showing a significant reducing trend with the passage of postoperative time (F =16.51, P < 0.05).The pre-and post-prandial main powers were changed from (85 ± 15) μV and (149 ± 23) μV before operation to (74 ± 9) μμV and (98 ± 10) μV at postoperative day 30, and main powers at postoperative each time points were significantly lower than those before operation, showing a significant increasing trend with the passage of postoperative time (F =48.45, P < 0.05).There was interaction between the time and meal (F =7.39, P < 0.05).The postprandial/preprandial power ratio was changed from 3.00 ± 0.35 before operation to 2.52 ± 0.25 at postoperative day 30, and postprandial/preprandial power ratios at postoperative each time points were significantly lower than those before operation, showing a significant increasing trend with the passage of postoperative time (F =26.66, P < 0.05).The pre-and post-prandial percentages of normal gastric slow wave were changed from 81% ± 6% and 94% ± 5% before operation to 57% ± 5% and 70% ± 5% at postoperative day 30 with coherent changing trend, and percentages of normal gastric slow wave at postoperative each time points were significantly lower than those before operation while postprandial above indicators was lower than preprandial those, showing a significant increasing trend with the passage of postoperative time (F =49.36,P <0.05).There was no interaction between the time and meal (F =0.24, P > 0.05).The pre-and postprandial percentages of slow gastric slow wave were changed from 17% ± 7% and 4% ± 4% before operation to 32%±4% and 21%±4% at postoperative day 30 with coherent changing trend, and percentages of slow gastric slow wave at postoperative each time points were significantly higher than those before operation while preprandial above indicators were higher than postprandial those, showing a significant reducing trend with the passage of postoperative time (F =46.54, P < 0.05).There was interaction between the time and meal (F =18.12, P < 0.05).The pre-and post-prandial tachycardia gastric slow wave percentages were changed from 1.55% (1.04%,2.21%) and 1.95% (1.74%, 4.22%) before operation to 8.97% (5.76%, 12.02%) and 12.41% (8.04%,16.85%) at postoperative day 30 without completely coherent changing trend, and percentages of tachycardia gastric slow wave at postoperative each time points were significantly higher than those before operation while postprandial above indicators were higher than preprandial those, showing a significant difference between before operation and postoperative day 3 (Z =11.47, 13.28, P < 0.05) and no significant difference among the postoperative day 7, 11, 30 (Z =1.88, 0.31, 0.03, P > 0.05).There was no interaction between the time and meal (F=0.85, P<0.05).Conclusions After the esophagectomy, gastric electrical activity of thoracic (tube) stomach is also retained before and after the meal.There are significant differences among the main frequency, main power, coefficients of dominant frequency instablility, postprandial/preprandial power ratio,percentage of normal gastric slow wave, percentage of slow gastric slow wave, percentage of tachycardia gastric slow wave of thoracis (tube) stomach, they have changed dynamically in the perioperative period.
10.Clinical Study on Effect of Baohe Granules on Chronic Heart Failure Patients with TCM Syndrome and Gastrointestinal Hormone Secretion

This article was aimed to study the effect of Baohe granules for patients with chronic heart failure (CHF) of traditional Chinese medicine (TCM) syndrome and the effect of gastrointestinal hormone secretion. Patients with CHF were used as the object of study. A total of 80 selected patients were divided into 2 groups. The control group was treated with western medicine anti-heart failure therapy. The treatment group was treated with the combination of Baohe granules. All patients were compared in aspects of TCM efficacy, symptom score and serum gastrin, motilin secretion after two-week medication. The results showed that both treatments can significantly improve the TCM syn-drome and symptom total score of CHF patients. And the effect of the treatment group with Baohe granules was bet-ter. The TCM single symptom integral was also obviously improved in the treatment group combined with Baohe granules, which included heart palpitations, shortness of breath, abdominal distension, and loss of appetite. The GAS and MTL levels of CHF patients were significantly increased in the treatment group. It was concluded that the treat-ment combined with Baohe granules can improve CHF patients with TCM syndromes, symptoms and gastrointestinal hormone secretion. Thus, it contributed to the stability of the disease condition. It can slow the disease progression and improve prognosis. So it is worth using widely in the clinical practice.
