OBJECTIVE: To analyze the clinical features of severe acute pancreatitis (SAP) in aged patients, and to explore the measures of its diagnosis and treatment. METHODS: The clinical data of 100 aged patients (55-85 years old) with SAP admitted from January 2003 to December 2005 were reviewed and compared with those of 221 non-aged SAP patients (11-54 years old) admitted at the same period. RESULTS: Totally 112 times of onset occurred in 100 aged patients, and the main causes for senile SAP were biliary diseases and hyperlipemia. Both acute physiology and chronic health evaluation II (APACHE II) and Ranson scores of the aged patients with SAP were higher than those of the control group (P<0.05); while there was no significant difference in Balthazar score between the two groups(P>0.05). The incidences of low serum calcium, low albumin and high aspartate aminotransferase (P<0.05), liver, kidney or brain impairment as well as the mortality were higher in the senile SAP group than those in the control group (P<0.05). CONCLUSIONS: Biliary disease and hyperlipemia are the main causes of senile SAP, which lacks characteristic clinical symptoms and is often associated with multiple complications such as infection and lung, gastrointestinal tract, liver, kidney, and brain impairment. Individualized differential treatment with integrated traditional Chinese and Western medicine based on stage classification is an effective approach.

Objective To explore the moisture absorption and related components of aqueous extract of Salviae Miltiorrhizae Radix et Rhizoma. Methods The hygroscopicity of aqueous extract of Salviae Miltiorrhizae Radix et Rhizoma and its water and alcohol elution parts separated by macroporous resin was measured, and the contained low molecular sugars (monosaccharides, oligosaccharide), polysaccharide, protein, amino acid, tannins and salvianolic acid B, etc. were analyzed. Results D101 macroporous resin was used for separation and purification of aqueous extract of Salviae Miltiorrhizae Radix et Rhizoma. The hygroscopicity of water elution parts was enhanced;carbohydrate, protein and other hydrophilic substances content increased; the content of salvianolic acid B was reduced to 1.76 mg/g. While the hygroscopicity and hydrophilic substances of alcohol elution parts were greatly reduced; the content of salvianolic acid B increased to 146.57 mg/g. Conclusion The hygroscopicity of aqueous extract of Salviae Miltiorrhizae Radix et Rhizomais closely related to the contained strong hydrophilic components, such as low molecular sugars, etc. Using D101 macroporous resin to purify aqueous extract of Salviae Miltiorrhizae Radix et Rhizoma can not only effectively gather the contain phenolic acids active ingredients, but also sharply decrease the extract yield and extract moisture absorption.

Objective To evaluate the occurrence and management of complications following ultrasound-guided minimally invasive percutaneous nephrolithotomy (MPCNL). MethodsFrom November 2003 to January 2011,2300 cases of ultrasound - guided MPCNL were performed for upper urinary tract stones in our department.Of these cases, renal calculi were found in 1305 cases, upper ureteral calculi in 322,renal and coexisting ureteral stones in 673. Results Among the 2300 cases of MPCNL,a total of 756 (32.9%) patients encountered complications.Of these cases,peel-away sheath placement failure occurred in 184 cases( 8.0% ),in which six cases needed secondary surgery.Collecting system perforation occurred in 308 cases ( 13.4% ),fever in 303 cases ( 13.2% ),including septicemia in 20 cases (0.87%).The 20 septicemia patients received intensive antibiotic treatment and were successfully cured.Thrity-six patients required transfussions due to severe hemorrhaging ( 1.57% ).Renal vein injury occurred in three cases (0.13%),for which these patients received intensive care therapy to provide haemostasis with a second procedure months later. There was extensive hemorrhage in 16 cases (0.70%) post-MPCNL,super-selective renal artery embolisation was performed in 12 cases and nephrectomy in 1 case.Pleural injury occurred in one case (0.04%) and pleural effusion in two cases (0.09%),all of which were cured with conservative therapy. There were no cases of abdominal organ injury.ConclusionsThe rate of ultrasound guided complications in MPCNL was lower than that of X-ray guided MPCNL in adjacent organ injury, but higher in complications related to the access ( such as:peel-away sheath placement failure, collecting system perforation),parenchymal bleeding and fever. Most complications (i.e.,bleeding,fever) could be managed conservatively or with minimally invasive procedures ( i.e.superselective renal embolisation, antibiotics treatment) when the complications were recognized early.Renal severe hemorrhage in operation, delayed hemorrhage and infection after MPCNL were several of the severe complications that required active prevention and cure measurement.

Objective To investigate the value of ultrasound in tethered cord syndrome (TCS) in infants. Methods From December 2005 to July 2013, 25 cases TCS were confirmed by surgery in Shenzhen children's hospital. The ultrasonogram and MRI of 25 infants were analysed retrospectively. The diagnostic accuracy of ultra-sound and its clinical significance were evaluated. Results In 25 cases of TCS confirmed by surgery, the coincidence rate of conus level detected by ultrasound [96%(23/24)] was lower than that by MRI (100%(25/25)). Ultrasound showed reduced spinal cord vibration in 4 cases, disappeared spinal cord vibration in 21 cases. Two cases of reduced spinal cord vibration detected by ultrasound were confirmed as disappeared spinal cord vibration by surgery. The coincidence rate of disappeared spinal cord vibration detected by ultrasound was 91%(21/23). The sacral spinal seg-ments form showed by ultrasound were entirely consistent with those of MRI, including 4 cases of enlarged spinal cord, 13 cases of spinal cord without enlargement and 8 cases of spinal cord ended with rat caudate. Twenty-five cases of TCS had malformations:7 cases meningocele (3 cases combined lipoma), 14 cases myelomeningocele (5 cases combined lipoma, 1 case combined hydromyelia), 3 cases spinal canal-epidermis fistula (all combined lipoma) and 1 case solitary lipoma. Compared with the operation findings, ultrasound misdiagnosed 2 cases of myelomeningo-cele as meningocele, missed 1 of case lipoma which combined with meningocele. MRI missed two cases of spinal canal-epidermis fistula. Conclusions Infantile spinal ultrasound examination can accurately locate the position of conus, accurately display the spinal cord. Compared with MRI, ultrasound examination can real-time visually display spinal cord vibration and help to diagnose tethered cord. Ultrasound examination are convenient, repeatable opera-tion with low cost, therefore it can be used as the preferred screening method to diagnose of tethered spinal cord.

ObjectiveTo investigate the effects of short-term enriched environment on the hippocampal formation and the myelinated fibers in the hippocampal formation of mid-aged female rats. Methods Twenty 14-month female SD rats were randomly divided into 10 enriched environment (EE) rats and 10 standard environment (SE) rats. EE rats were reared in enriched environment and SE rats were reared in standard environment for 4 months. Then, five rats were randomly selected from each group. The spatial learning capacity was assessed with Morris water maze. The hippocampal formation and the myelinated fibers in the rat hippocampal formation were quantitatively investigated with transmission electronic microscopy technique and stereological methods. Results Short-term enriched environment enhanced the spatial learning capacity of the mid-aged female rats. The total length and total volume of the myelinated fibers in the hippocampal formation of the EE rats was significantly increased by 43.3% and 47.4%, respectively, when compared to the SE rats. There was no significant difference in the hippocampal volume and the mean diameter of the myelinated fibers in the hippocampal formation between two groups. The increase of the total length of the myelinated nerve fibers in the hippocampal formation was mainly due to the increase of the myelinated fibers with small diameter. Conclusion Short-term enriched environment had significant effects on the spatial learning capacity and the myelinated fibers in the hippocampal formation of middle-aged female rats.

ObjectiveTo investigate the effects of short-term enriched environment on the hippocampal formation and the myelinated fibers in the hippocampal formation of mid-aged male rats. MethodsTwenty 14-month old male SD rats were randomly divided into enriched group and standard group. Enriched rats were reared in enriched environment and standard rats were reared in standard environment for 4 months. Then, the spatial learning capacity of enriched rats and standard rats was tested with the Morris water maze. After the Morris water maze test, the total volume of the hippocampal formation and the myelinated nerve fibers in the hippocampal formation were quantitatively estimated with transmission electronic microscopy technique and stereological methods. Results There was not significant difference in the spatial learning capacity between enriched group and standard group. The total volume of the hippocampal formation of enriched rats was not significantly increased by 4.6% when compared with that of standard rats. The total volume, total length and mean diameter of the myelinated nerve fibers in the hippocampal formation of enriched rats were significantly increased when compared with those of standard rats. Conclusions Four-months enriched environment significantly affected the myelinated fibers in the hippocampal formation of mid-aged male SD rats. The present results might provide an important theoretical basis for searching the ethology strategy to delay the progress of brain aging in the future.

Acta Anatomica Sinica 2010;41(2):169-174
ObjectiveTo investigate the effects of exercise on the hippocampal formation and the myelinated nerve fibers in the hippocampal formation of middle-aged rats. MethodsTen 14-month female SD rats were randomly divided into exercise group and sedentary group.Rats in the exercise group were forced to run on a treadmill for 4 months. After 4 months, spatial learning capacity of two group rats was tested using the Morris water maze. Then, the hippocampal formation and the myelinated nerve fibers in the hippocampal formation were quantitatively estimated using transmission electronic microscopy and stereological techniques. Results Treadmill running enhanced the spatial learning capacity of the rats. The volume of hippocampal formation and the total length of the myelinated nerve fibers in the hippocampal formation were significantly increased after 4 months exercise. However, there was no significant difference in the total volume of the myelinated fibers in the hippocampal formation between the two groups. The absolute distributions of the total length of the myelinated fibers in the hippocampal formation of two groups indicated that the exercise-induced increase of the total length of the myelinated nerve fibers in the hippocampal formation was mainly due to the increase of the myelinated fibers with small diameter. Conclusions Four months running exercise remarkably influence the spatial learning capacity, hippocampal formation and the myelinated fibers in the hippocampal formation of the middle-aged famale SD rats. The present results reveal a potential mechanism for the fact that exercise might improve brain function.
