1.Choice of the treating methods of neovascular age-related macular degeneration

Neovascular age-related macular degeneration (AMD) is the main cause of vision loss in AMD patient.Now,the application of anti-vascular endothelial growth factor(VEGF) has become the preferred therapy for the treatment of neovascular AMD,and it has been proved to improve the visual acuity and reduce the blindness rate.But the formation of choroidal neovascularization (CNV) in AMD is a comprehensive and integrated pathological process.It is difficult to cure the CNV with only anti-VEGF drug for all the neovascular AMD patients.Therefore,the targeting drugs toward other signal pathway associated with CNV are being in different phases of clinical trials.Resent years,with the development of science and technology,small interference RNA(siRNA) and genome technology has been applied to treat neovascular AMD and show a potential prospect,and gene therapy and stem cell therapy are the most remarkable and predominant ways for AMD.Doubtlessly,these therapies offer novel choice for the management of neovascular AMD.
2.Results of selective expansive open-door laminoplasty for the treatment of cervical spondylotic myelopathy

[Objective] To evaluate the efficacy and short-term results of selective expansive open-door laminoplasty(ELAP).[Method] From June 2005-June 2006,a total of 102 pataents with cervical spondylotic myelopathy(CSM)were enrolled in this study.Forty-four patients underwent ELAP.Fifty-eight patients undergoing conventional C3~7 ELAP served as controls.The clinical results and radiological examinations of both groups were evaluated at 12 month after surgery.[Result]There was no significant difference in recovery rate of Japanese Orthopaedic Association(JOA)scores.The incidence of axial symptoms in the selective ELAP group was significantly lower than those in the C3~7 ELAP group(P
3.The relationship between restoration and furcation involvement on molar teeth

Objective: To investigate the correlation between the presence of a crown (CR) or a proximal restoration (PR) and furcation involvement (FI) on molars. Methods: Data were collected from 237 teeth in 44 patients who had restored and non-restored molars with or without FI.The clinical examinations were conducted on the molars including CR,PR,FI, mobility(MO), endodontic treatment(EN) and probe of periodontal attachment level(PAL). Results: Molars with CR or PR had a significantly highter percentage of FI (P0.05) when compared to molars without restorations. Conclusion: The molars with crowns or restorations involving the proximal tooth surfaces have a highter prevalence of FI and more periodontal attachment lose than molars without restorations.
4.Volumetric MRI analysis of the amygdala and hippocampus in subjects with major depression.

In order to explore the MRI volume of the amygdala and hippocampus in patients with major depression, quantitative MRI of the amygdala and hippocampus were studied in 22 patients with major depression and compared with 13 age-matched controls. The results showed that both groups exhibited similar significant hippocampal asymmetry (left smaller than right). The volume of the bilateral hippocampus was significantly smaller in the major depression group than that in control group. The patients had significant asymmetry of the amygdalar volumes (right smaller than left). No correlation was found between hippocampal volume abnormalities and ill duration. It was concluded that the hippocampus and amygdala within limbic-cortical networks may play a crucial role in the pathogenesis of major depression.

5.Evaluation of early changes of femoral artery elasticity in type 2 diabetes mellitus combined with microalbuminuria patients using ultrasound radio-frequency

Objective To evaluate early changes of femoral artery elasticity in type 2 diabetes mellitus (T2DM) combined with microalbuminuria (MA) patients using ultrasound radio-frequency technology,and to study the clinical significance.Methods 51 T2DM without MA patients were enrolled in Group A,and 53 T2DM with MA patients were enrolled in Group B.In order to observe the early elastic changes in the subjects,two-dimensional ultrasound was used to eliminate patients whose intima-media thickness (IMT) was >1 mm.The femoral arteries were imaged by two-dimensional ultrasonography,and then the IMT,femoral artery inner diameter (D),distensibility coefficient (DC),compliance coefficient (CC),elasticity coefficient (α and β),and pulse wave velocity (PWV) were real-time tested using quality intima-media thickness (QIMT) and quality arterial stiffness (QAS) analyzing systems.Besides,the differences and correlation analyses of these parameters between the two groups were conducted.In addition,some other parameters of sujects in both group A and B were also determined and analysed,including total cholesterol (TC),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C),creatinine (CREA),glycosylated hemoglobin (HbA1c),fasting blood glucose (FPG),2h postprandial plasma glucose (2hPG) and body mass index (BMI).Results There were no significant differences in the elastic parameters of femoral arteries between the male and female subjects (P>0.05),and between left and right sides (P>0.05) within a same group.Compared with group A,group B had lower CC and DC (P<0.05) and higher α,β and PWV (P<0.05).Pearsons correlation analysis indicateal that,CC was negatively correlated with α,β and PWV (P<0.05),and DC was also negatively correlated with α,β and PWV (P<0.05).There was no significant difference in the inner diameters of femoral arteries between the two groups (P>0.05).The levels of TG,HbA1c,FPG and 2hPG in group B were significantly higher than those in group A (P<0.05).There were no significant differences in TC,HDL-C,LDL-C,CREA and BMI between the two groups (P>0.05).Conclusions The decrease of arterial elastic function is earlier than morphological changes,that is,it may have occurred before MA in T2DM patients.The application of ultrasound radio-frequency analyzing technology has certain significance in the clinical early diagnosis and treatment.
6.Clone and Expression of Loop1 and Loop2 Gene of Hexonof Infectious Canine Hepatitis Virus

The mainly antigenic sites for the adenovirus neutraliation are present on Loop1 and Loop2 of hexon.Majority research were focus in the human adenovirus.Little was known on infectious canine hepatitis virus (ICHV), which was also called canine adenovirus typeⅠ.Here,ICHV (the isolated strain) DNA was isolated and purified from the cultured MDCK cells.The Loop1 and Loop2 fragments were amplified by polymerase chain reaction(PCR) method,and then was connected by ligase T4.The target fragment was then connected with vector pET28a.The nucleotide sequence ecoding Loop1 and Loop2 was determined.The nucleotide sequence identity of Loop1 region between the isolated strain and CLL, RI261 and Toronto A26/61 strains is 100%, 100% and 83.8%, and the nucleotide sequence identity of Loop2 region between the isolated strain and CLL, RI261 and Toronto A26/61 strains is 88.1% , 88.1% and 99.3%, and amino acid identity is 93.6%, 93.6% and 98.6%.The recombinant Loop protein was expressed in E.coli and was approximately 36kDa in size,and then was purified. Then BALB/c mice were injected subcutaneously in the back and armpit with the recombinant Loop protein.The anti-ICHV antibody titers of immunized serum was tested by indirect ELISA and the titers were up to 1:320.Western blot demonstrated that immunized sera could specifically combine with ICHV. The research laid a foundation for creating new genetic engineering products of infectious canine hepatitis virus.
7.Expression of let-7a2 in lung adenocarcinoma

Objective To explore the expression and significance of let-7a2 in lung adenocarcinoma.Methods To adapt technology of realtime PCR with reference of U6, the expression of let-7a2 in tumor tissues and normal tissues was detected in 15 cases of resected lung adenocarcinoma. The laboratorial data were analysed by measure of 2~(-△△CT) method. The analysed data were managed by statistics. Results The expression of let-7a2 in lung adenocarcinoma tissue was significantly lower than that in lung normal tissue (0.8733±0.4821 vs 2.4527±1.0111). This difference possess markedness significance(t=6.816, P<0.001).Conclusion The expression of let-7a2 in lung adenocarcinoma is obviously lower and is concerned with the course of lung cancer evolution.
8.Surgical treatment for chronic pancreatitis characterized by a pancreatic mass

Objective To summarize the diagnostic and therapeutic experience on chronic pancreatitis characterized by a pancreatic mass. Methods The clinical data of 28 cases of chronic pancreatitis with mass undergoing surgical operations were retrospectively analyzed in our hospital from June1999 to June 2009. Results Among the 28 cases, 19 were diagnosed as carcinoma, 9 cases were diagnosed as chronic pancreatitis respectively before operation. Needle aspiration biopsy and/or postoperative pathology identified chronic pancreatitis in all cases. The symptom included abdominal pain (22 cases),jaundice (15 cases), and obstruction of duodenum (4 cases). Pancreaticoduodenectomy was performed in 17 cases, choledochojejunostomy performed in 3 cases, pancreatojejunostomy performed in 1 case.Duodenum-preserving resection was performed in 4 cases, and resection of body and tail of the pancreas were performed in 3 cases. There was no operative death. Postoperative complications included pancreatic leakage (2 cases), severe gastroplegia (2 cases) and stress peptic ulcer with massive bleeding ( 1 case). All patients got follow-up ranging from 6 months to 5 years. Recurrence of abdominal pain developed in 7 cases after 2 years. Canceration of pancreatic mass was found respectively in 8 months, 1 year after operation in one each cases. Conclusion Preoperative differential diagnosis of chronic pancreas and pancreatic tumor was difficult. Although needle aspiration biopsy is the effective method for diagnosis, there may be still a possibility of missed diagnosis/misdiagnosis.
9.Free-hand cervical pedicle screw fixation for upper cervical fracture and instability

Objective To evaluate the clinical effect of the free-hand cervical pedicle screw fixation in treatment of the upper cervical fracture and instability.Methods A retrospective review was performed on 15 patients with upper cervical fracture and instability treated with cervical pedicle screw fixation and fusion from September 2006 to January 2009.There were 11 males and 4 females,at average age of 41.2 years(range,18-60 years).Of all,there were five patients with atlas fracture and dislocation(including three simple anterior arch fractures and two Jefferson fractures),three with axis fracture and dislocation,one with dens fractures plus nonunion,two with C2,3 fracture and dislocation and four with atlantoaxial instability without fracture.The main clinical complaints included local neck pain and/or tetraplegia.Halo traction was recommended to restore the cervical sequence preoperatively in all patients.All 15 patients were treated by cervical pedicle screw-rods internal fixation and bone graft fusion.During the operation,the point and angle of the implanted pedicle screws were determined by preoperative X-ray and CT scan and the bony channel drilled with free-hand before implantation of the Summit or Vertex pedicle screws(22-26 mm long)and posterior interlaminar autologous or allogeneic bone fusion.Patients could get out of bed with neck collar at days 1-2 after operation.Results A total of 64 cervical pedicle screws were implanted in all 15 patients,with no vertebral artery injury,spinal cord injury or cerebrospinal fluid leakage.Postoperative X-ray and CT scan confirmed satisfactory internal fixation.The clinical symptoms were improved significantly.Fourteen patients were followed up for 12-36 months,which showed bony fusion,with no looseness or breakage of the screws.Neurologic impairment was improved in all patients,with no complications associated with the cervical pedicle screw.Conclusions Cervical pedicle screw internal fixation can reestablish the upper cervical vertebrae stability and help to recover the spinal cord and nerve function and hence is a reliable method for upper cervical fracture and/or instability.
10.The effect of receptor tyrosine kinase inhibitor genistein on phenotype expression of collagen and proteoglycan of chondrocyte

Objective To explore the protective role of the receptor tyrosine kinase inhibitor in chondrocyte in vitro.MethodsReverse transcription-polymerase chain reaction (RT-PCR), immunocytochemistry (ICC) and toluidine blue staining were used to understand the changes of phenotype expression of collagenⅠ, Ⅱ and proteoglycan in chondrocyte in vitro. Results Genistein of 20 μg/mL and 25 μg/mL concentrations decreased the expression of collagen Ⅰ and increased the expression of collagen Ⅱ and proteoglycan (P< 0. 05). Conclusions This study showes that genistein can effectively inhibit the expression of collagen Ⅰ above certain concentration, and it can improve the expression of collagen Ⅱ and increase the synthesis of proteoglycan.
