Objective To explore the clinical comparative value of little abdominal incision and laparoscopic treatment of large ovarian cysts.Methods A total of 31 consecutive cases of large ovarian cysts were analyzed.Two groups were made with the random number table method.Eighteen women with little abdominal incision were served as study group.The corresponding nineteen women with laparoscopic treatment were treated as control group.All the people were treated with placket suctionand helicism suture.The operation time,amount of bleeding,hospitali-zation time,cost of hospitalization and postoperative complications of the two groups were compared,and the levels of C -reactive protein and leukocyte count were also compared.Results The operation time,amount of bleeding,hospi-talization time and cost of hospitalization in the study group were (52.17 ±3.02)min,(30.09 ±1.91)mL,(5.75 ± 0.98)d and (5 272.85 ±355.21)yuan respectively,which in the control group were (72.40 ±4.45)min,(29.71 ± 0.56)mL,(5.15 ±0.31 )d and (11 825.59 ±857.71 )yuan respectively;There were no statistically significant differences in amount of bleeding and hospitalization time between the two groups(t =1.57,1.19,all P >0.05),but the operation time in the study group was less than that in the control group(t =4.51,P <0.05),and the same result of the cost of hospitalization was detected in the two groups(t =5.24,P <0.05).Conclusion In the basis of placket suctionand helicism suture,little abdominal incision surgery method is worthy of the clinical promotion with shorter operation time,more rapidly postoperative recovery and less hospitalization expense.

Objective Through application of clinical pathway in hospitalization patients undergoing uterectomy, we aimed to study the efficacy of clinical pathway in uterectomy patients. Methods 120 patients randomly sampled into the experimental group and the control group with 60 in each. The experimental group received diagnosis, treatment, nursing and rehabilitation according to the clinical pathway table, patients in the control group received routine nursing interventions. The average hospitalization days, medical costs, patient satisfaction degree, disease cognition, first exercise time after operation and medical disputes were compared between the two groups. Results The average hospitalization days and hospitalization costs of the experimental group were obviously lower than those in the control group, patients' satisfaction degree, knowledge of health and first functional exercise time in the experimental group were better than those of the control group, while the medical disputes in the experimental group were obviously de -creased in the experimental group. Conclusions Application of clinical pathway in nursing care for uterectomy patients can effectively decrease hospitalization days, medical costs and disputes. It is also helpful for patients to cooperate with doctors in diagnosis, treatment, nursing, thus increase the satisfaction degree of patients, ensure the reasonable use of medical resources and promote the sustainable improvement of nursing quality.

Objective To explore the value of applying personalized medicine in the clinical teach-ing of glioma surgery. Methods Totally 80 7-year program undergraduates were divided into experimental class and control class randomly, with 40 cases in each group. The experimental class received personalized medical teaching through introducing idea and method of personalized medicine. The control class received conventional teaching. The effect of teaching was evaluated by questionnaire and examination. The data were analyzed through Chi-square and t-test test. Result Questionnaire survey of satisfaction showed that 37 students of experimental class (92.5%) were interested in the new course. Questionnaire survey of teaching effect showed that in experimental class 36 students (90%) felt it could increase learning interest, 37 students (92%) believed it improved glioma diagnosis and treatment, 34 students (85%) felt their medical view ex-panded, 38 students had consolidated clinical thinking, and 32 students (80%) improved negotiation with patients. All issues were significantly better than control class (P<0.05). Meanwhile, statistical difference was found in average score between experiment class and control class [(88.71 ±6.54) vs. (76.49 ±7.32)] (P=0.000). Conclusion The introduction of personalized medicine concept helps medical students to make personalized diagnosis and treatment plan for glioma patients, and is conducive to the cultivation of their clinical thinking ability and evidence based medicine concept, which is worthy of promotion.

Objective To determine the value of forced expiratory volume in 6 second (FEV6) and forced expiratory volume in 1 second (FEV1)/FEV6 in diagnosis of obstructive and restrictive lung ventilation dysfunction.Methods A total of 470 cases receiving spirometric examinations were analyzed retrospectively.A subject was considered to have obstruction if FEV1/forced vital capacity (FVC) was ＜ 70％.The restriction was defined as FVC ＜ 80％ in the absence of obstruction.The best cut-off of FEV1/FEV6 and FEV6 were determined through receiver-operating characteristics curve,and the sensitivity,specificity,accuracy and Kappa of FEV1/FEV6 and FEV6 were calculated.Results It showed that the current cut-off points used to detect obstruction and restriction could be replaced by FEV1/FEV6 was 71％ and FEV6 was 82％,respectively.FEV1/FEV6 had sensitivity of 97.5％ (154/158),specificity of 98.7％ (308/312),accuracy of 98.3％ (462/470) and Kappa of 0.962 (P=0.000).For restrictive pattern,FEV6 had sensitivity of 96.1％(73/76),specificity of 95.7％ (222/232),accuracy of 95.8％ (295/308) and Kappa of 0.890 (P =0.000).Conclusions FEV6 can be a valid alternative for FVC in the diagnosis of obstructive and restrictive lung ventilation dysfunction.
