1.Risk Factors and Drug Resistance of Nosocomial Pneumonia Caused by Imipenem-resistant Acinetobacter baumannii

OBJECTIVE To investigate the risk factors for nosocomial pneumonia(NP) caused by imipenem-resistant Acinetobacter baumannii(IRAB) and its antimicrobial susceptibility in vitro. METHODS The data of 34 cases of IRAB-NP were analyzed and 68 cases of NP caused by imipenem-susceptible A.baumannii(ISAB) were randomized as control.Antimicrobial susceptibility(MIC) was determined with the method of agar dilution. RESULTS The two independent factors associated with the development of IRAB-NP: previous fluoroquinolone(OR=5.738) and imipenem/meropenem(OR=7.129) use.The drug sensitivity test in vitro showed that these strains were multiresistant to commonly used antibiotics,and only ampicillin/sulbactam and cefoperazone/sulbactam whose resistance rate was less than 30%. CONCLUSIONS Previous imipenem/meropenem and fluoroquinolone use is independent risk factors for IRAB-NP.These strains are high drug resistant.
2.Inhibitory effects of the immunoconjugate composed of anti-type IV collagenase antibody Fab' fragment and lidamycin on tumor invasion and metastasis.

This study is to investigate the tumor invasion and metastasis inhibition effects of the immunoconjugate composed of lidamycin and anti-type IV collagenase monoclonal antibody Fab' fragment. Boyden chamber assay was used to evaluate the influence of Fab'-LDM on HT-1080 cells invasion ability, gelatinase spectrum was used to measure the change of invasion factor MMP-2 and MMP-9's secretion, and RT-PCR was adopted to determine TIMP-1 mRNA expression level. The immunoconjugate inhibition of tumor in situ metastasis was also tested in nude mice. The Fab'-LDM conjugates had dose-dependent inhibition effect on HT-1080 cells' invasion. At the concentrations of 5 and 10 nmol L(-1), the Fab'-LDM inhibited the invasion by (60 +/- 12) % and (79 +/- 11) % respectively. At the concentration of 5 and 10 nmol L(-1), the Fab'-LDM inhibited the secretion of MMP-2 by (42 +/- 8) % and (54 +/- 6) % and that of MMP-9 by (57 +/- 3) % and (87 +/- 1) %, respectively. RT-PCR indicated that conjugates increased the anti-invasion factor TIMP-1 level. The in vivo experiment showed that, compared with the control group, the tumor inhibition rate in Fab', Fab'-LDM, and LDM group equaled to (30 +/- 13) %, (86 +/- 26) %, (74 +/- 22) % respectively. In conclusion, Fab'-LDM could inhibit the invasion and metastasis of tumor and it might be a new tumor biotherapy agent.
3.Reformation and Practice of the Experimental Course of Fermentation Engineering

The traditional fermentation engineering experiment requires a reform on the experimental con-tents and teaching pattern. According to the production process of industrial enzymes, we set up a two-week comprehensive experiment. The students designed and prepared the experiment by themselves. Moreover, the pattern of self-management was used in the process and the experiment scores included the self-assess-ment and objective assessment. It was proved that the new teaching pattern increased the study interesting of students, inspired their initiative and trained their spirits of team cooperation. The teaching effect was im-proved markedly and good ideas are also put forward to solve the possible problem.
4.Polymorphisms of FcγRIIA, FcγRIIIA and FcγRIIB in patients with immune thrombocytopenia and their clinical significance.

This study was aimed to investigate the correlation of FcγR polymorphisms with the susceptibility, severity and efficacy of immunotherapy for patients with immune thrombocytopenia (ITP). PCR and DNA sequencing were used to determine the polymorphisms of FcγRIIA, FcγRIIIA and FcγRIIB in 44 ITP patients, and in 97 healthy control subjects. The results indicated that FcγRIIIA-158V/F polymorphisms between patients and controls were statistically significantly different (P = 0.015); among FcγRIIIA genotypes, the frequency of 158V/V homotype was higher in ITP (P = 0.005). However, the FcγRIIA-131H/R or FcγRIIB-232T/I polymorphisms were not significantly different between patients and controls; there were no correlation of FcγRIIA, FcγRIIIA and FcγRIIB genotype frequencies with the platelet counts or the courses of ITP; among the 38 ITP patients who received treatments, the complete response (CR) rate was 42% (16/38), and partial response (PR) rate was 34% (13/38). The therapeutic response was significantly different between FcγRIIIA-158V/V homotype and 158F/V heterotype (P = 0.034). The CR of patients with 158V/V homotype was obviously lower than that of patients with 158F/V, but the frequencies of FcγRIIA and FcγRIIB genotypes not correlated with the responsiveness to treatment. The CR rate of 6 patients treated with rituximab was 67%, and PR rate was 17%. The overall response rate was as high as 84%, the adverse reactions were not observed. It is concluded that the polymorphism of FcγRIIIA-158V/F, but not FcγRIIA-131H/R or FcγRIIB-232T/I, correlates with the patient susceptibility and therapeutic response of ITP.

6.Effect of calcaneocuboid joint arthrodesis on weight-bearing area of subtalar joint and its clinical significance:a cadaveric study

Objective To discuss the effect of the calcaneocuboid joint arthrodesis on the weight- bearing area of subtalar joint and its clinical significance.Methods Twelve fresh-frozen cadaver foot specimens were used for determination of weight-bearing area of the subtalar joint on foot and ankle neutral position,dorsiflexion,plantoflexion,adduction,abduction,inversion and eversion motion by means of pressure sensitive film before and after calcaneocuboid joint arthrodesis under weight loading.Results Weight-bearing area of the subtalar joint averagely increased for (32.54?7.45)% in all positions after calcaneocuboid joint arthrodesis,with statistical significance (P<0.05).Conclusion Weight-bear- ing area of the subtalar joint increases after calcaneocuboid joint arthrodesis,which contributes to decrea- sing the pressure and increasing the stability of the subtalar joint.
7.Factors Related to Ventricular Size and Valvular Regurgitation in Healthy Tibetans in Lhasa

Background: Lhasa is the main residence of Tibetans and one of the highest cities in the world. Its unique geography and ethnic population provide the chance to investigate the interactions among high altitude, ethnicity, and cardiac adaptation. Meanwhile, echocardiographic data about healthy Tibetans on a large scale are not available. This study aimed to analyze physiological factors related to ventricular size and valvular function in healthy Tibetans in Lhasa. Methods: A representative sample of residents in Tibet was recruited using a multistage cluster random sampling method. Two?dimensional echocardiographic measurements and Doppler evaluation for valvular function were performed. Healthy Tibetans in Lhasa constituted the study population. Associations between physiological parameters and ventricular dimensions in healthy Tibetans were analyzed by canonical correlation analysis. Factors related to valvular regurgitations were determined by logistic regression analysis. Results: The 454 healthy Tibetans (340 females and 114 male) in Lhasa were included in the final analysis. Canonical correlation analysis revealed that weight was positively correlated with the proximal right ventricular outflow diameter and the basal left ventricular linear dimension in both genders. Weight and pulse were negatively related to mild tricuspid regurgitation. Age was a positive factor for pulmonary and aortic regurgitations. The same was found between systolic blood pressure and mitral regurgitation. Conclusions: Weight is associated with ventricular size and valvular regurgitation in healthy Tibetans. It should be of more concern in research of high altitude population.
8.Silver-ion release and particle distribution of denture base resin containing nanometer-sized silver supposed antimicrobial agent

Objective To investigate the distribution of antimicrobial agent STR-1 of nanometer level which was incorporated with ball-grinding method in the polymethylmethacrylate(PMMA)denture base,and to study the release mode of silver ions from the base. Methods The distribution of the antimierobial agent in the PMMA denture base containing STR-1 at concentrations of 0 g/L,5 g/L,and 10 g/L Was examined with scanning electronic microscopy. Then,PMMA resin bases containing STR-1 at the three concentrations were respectively immersed in artificial saliva at 37 ℃ f0r 54 days. The release of silver ions from the resin bases was surveyed with inductively coupled plasma-mass spectroscopy(ICP-MS)every 24 hours.Results The antimicrobial agent incorporated by ball-grinding method was even-distributed with individual particles of nanometer level in the PMMA resin base.The release of silver ions from the PMMA resin with antimicrobial agent was extremely slow during the test,a very small fraction of the silver ions released.At the beginning of the test,the release speed was extremely slow,the speed increased rapidly in tlle middle of the test,and at the end of the test,the speed returned to slow and steady. The cumulative release curve of silver ions was of"S" type.Conclusions STR-1 can be even-distributed in the denture base,and the silver ions release from the base with extremely slow speed.It also indicates that biological safety and long-term antimicrobial efficacy of denture base containing silver-supported antimicrobial agents of nanometer level are possibly obtained based on their slow release of silver ions.
9.Mutation of tet2 gene and malignant blood disease.

Tet2 (the 2nd member of tet oncogene family) is a newly discovered antioncogene on the chromosome 4q24 of the patient with malignant myeloma, which has a potential for functional deletion. Recent studies demonstrated that tet2 mutation was found in polycythemia vera (PV), essential thrombocythemia (ET), myelofibrosis, systematic mastocytosis (SM), and myelodysplastic syndrome (MDS). However, a great number of perspective researches are still needed for exploring the role of tet2 in the pathogenesis of malignant blood diseases. In this review, the relation of tet2 mutation with myeloproliferative neoplasm, systemic mastocytosis, myelodysplastic syndrome, acute myeloid leukemia and other malignant blood diseases are summarized.

10.Comparison between Gd-DTPA-enhanced MRI and USPIO-enhanced MRI in diagnosing benign and metastatic cervical lymph nodes in rabbit models.

OBJECTIVETo compare the diagnostic value of gadolinium diethylenetriaminepenta-acetic acid (Gd-DTPA)-enhanced MRI with ultrasmall superparamagnetic iron oxide (USPIO)-enhanced MRI in differentiating reactive hyperplastic lymph nodes from metastatic lymph nodes in rabbit models.
METHODSReactive hyperplastic cervical lymph node model was established in 18 rabbits as hyperplasia group, and tumor-bearing lymph node model was established in another 18 rabbits as tumor group. For Gd-DTPA-enhanced MRI, T1WI and T2WI were performed on 9 animals of each model, and T1WI was acquired 80 seconds after administration of Gd-DTPA. For USPIO-enhanced MRI, T1WI, T2WI and T2*WI were performed on another 9 animals of each model before and 24 hours after administration of USPIO. MRI images were analyzed and correlated with surgical specimens and pathological results.
RESULTSIn the tumor group, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of Gd-DTPA-enhanced MRI were 62.5%, 91.3%, 88.2%, 70.0% and 76.6%, respectively. The corresponding values of USPIO-enhanced MRI were 95.0%, 90.9%, 90.5%, 95.2% and 92.9%, respectively. The sensitivity and accuracy of USPIO-enhanced MRI differ significantly from those of Gd-DTPA-enhanced MRI. In the hyperplasia group, the accuracy of Gd-DTPA-enhanced MRI was 74.2%, while 87.1% for USPIO-enhanced MRI.
CONCLUSIONUSPIO-enhanced MRI has higher accuracy in diagnosing metastatic lymph nodes than Gd-DTPA-enhanced MRI.
