ObjectiveTo probe into the clinical application effect of continuity care model in stroke patients. Methods58 patients with stroke in our hospital during June 2008 to June 2011 were selected.According to block randomization method,the selected patients were divided into the study group and the control group with 29 patients in each group.The control group received routine care interventions,while patients in the study group was given routine care interventions plus continuity care model.Activities of daily living,compliance behavior,quality of life,nursing satisfaction degree and other related indicators of patients in two groups were comparatively analyzed. ResultsCompared with the control group,the activities of daily living of the study group at discharge did not show significant differences,while 1 month,3 months,6 months after discharge,the activities of daily living was significantly increased.Regular medication,exercise time,timely return visit,proper diet and other indicators of compliance behaviors were significantly improved.The quality of life score did not show significant differences at discharge,while 3 months after discharge,the quality of life score were significantly higher in the study group than that of the control group.The satisfaction degree of patients was significantly higher in the study group than that of the control group. ConclusionsApplication of continuity care model in stroke patients can improve activities of daily living,compliance behavior,quality of life,satisfaction degree with nurses.It's worthy of further clinical application and promotion.

Objective Probing into the primary healthcare capabilities of healthcare clinics in the four provinces in China's west,in order to recommend the government in making relevant policies.Results It is found that among those village doctOrs under 45 years of age,76.0%have senior high school diploma/secondary technical school education,89.6%have rural doctor certificates;those above 45 years old,59.5%have middle school education,96.9%have rural doctor certificates,and 1.6%have licensed assistant doctor certificates.x2check is statistically significant,as the village doctors born in different eras have different composition of education and different percentage of qualification certificates.44.44% of the village clinics have less than three rooms,and 41.11%occupy less than 40 square meters.Only 19.44%of the clinics receive funding for their medieal facilities from higher authorities.Conclusion It is imperative to enhance resources input tO the staff and facilities of village clinics in the west,to cater to the primary healthcare needs of the local people.

Garcinol, a benzenetriol compound extracted from Garcinia cambogia, has antitumor activity, however, its antitumor mechanism remains unclear. The aim of this study was to investigate the role and mechanism of garcinol as a novel potential proteasome inhibitor. We applied the drug affinity responsive target stability (DARTS) method coupled to mass spectrometry to determine the binding protein of garcinol; the proteasome activity assay was used to determine the effect of garcinol on its hydrolase activity; immunofluorescence and proximity ligation assay (PLA) were used to detect the effects of garcinol on ubiquitin and RPN6; and flow cytometry were used to determine the effects of garcinol on cell apoptosis; and the anti-cancer effect was studied in organoid models. The results showed that RPN6 was a direct binding protein of garcinol; garcinol inhibited the hydrolase activity of proteasome, and induced the accumulation and aggregation of ubiquitin protein, and its proteasomal inhibitory effect was dependent on RPN6; further studies showed that garcinol induced oligomerization of RPN6 and formation of granules in the nucleus; finally, it was verified that garcinol induced apoptosis of tumor cells, and inhibited the growth of organoids of Apc^min/+ small intestine mice. These results suggest that garcinol is a potential proteasome inhibitor, which inhibits proteasome activity by directly targeting RPN6 on proteasome 19S, which in turn induces cell apoptosis and inhibits tumor growth.

Objective To detect the ocular fundus hemodynamic changes in patients with rhegmatogenous retinal detachment before and after the scleral buckling procedure by Heidelberg retina flowmeter,and analyse the related influential factors. Methods Heidelberg retina flowmeter was employed to measure the volume,flow and velocity in retinal vessels of the impaired eyes in 47 patients with unilateral rhegmatogenous retinal detachments 1 day before and 1 week,1 month and 3 months after scleral buckling procedure.The contralateral eyes were served as controls. Objective To explore the factors for the development of gallstones after gastrectomy. Methods 52 cases of patients with postgastrectumy,were retrospectively analyzed according to their diseases for gastrectomy and operation form. Results Incidence of gallstone in patients of postgastrectomy was higher than in general population. Among which, Billroth Ⅱ type of gastrectomy for carcinoma was the highest (35.5%), then the total gastrectomy (31.5%) and proximate gastrectomy(13.5%). Billroth Ⅰ type operation was 10.3%. Incidence of gallstone in pa-tients of selective vagotomy was lowest (2.5%). Condusion Billroth Ⅱ type of gastrectomy and total gastrectomy were the risk factors of postgastrectomy cholelithiasis. The causes for gallstone formation after Billroth Ⅱ type of gas-trectomy were the restitution of digestive canal and metabolic disorder of bile acid.

Objective To investigate the toxic effect of nona-ferroso-ferric oxide(Nano-Fe_3O_4),nano-silicon dioxide(Nano- SiO_2)and single walled carbon nanotubes(SWCNTs)on the lung and mechanism in rats.Methods Fourty-nine Wistar rats were randomly divided into 7 groups,the control group,low and high dose groups of three nanomaterials.The rats were exposed by intratracheal instillation once two days for 5 weeks,and then killed by abdominal aorta bloodletting.The pathology of lung,lactate dehydrogenase(LDH),total antioxidant capacity(T-AOC),superoxide dismutase(SOD),malondialdehyde(MDA),interleukin-1(IL- l),interleukin-6(IL-6)and tumor necrosis factor-?(TNF-?)in bronchoalveolar lavage fluid(BALF)were determined.Results The fibrous tubercle and the matrix inflammation of lung tissue was found in the experimental groups.The activities of T-AOC and SOD decreased while IL-6 concentration increased significantly in the experimental groups(P
