4.Investigation on the primary healthcare capabilities of village healthcare clinics in the four provinces in Chinas west

Objective Probing into the primary healthcare capabilities of healthcare clinics in the four provinces in China's west,in order to recommend the government in making relevant policies.Results It is found that among those village doctOrs under 45 years of age,76.0%have senior high school diploma/secondary technical school education,89.6%have rural doctor certificates;those above 45 years old,59.5%have middle school education,96.9%have rural doctor certificates,and 1.6%have licensed assistant doctor certificates.x2check is statistically significant,as the village doctors born in different eras have different composition of education and different percentage of qualification certificates.44.44% of the village clinics have less than three rooms,and 41.11%occupy less than 40 square meters.Only 19.44%of the clinics receive funding for their medieal facilities from higher authorities.Conclusion It is imperative to enhance resources input tO the staff and facilities of village clinics in the west,to cater to the primary healthcare needs of the local people.
5.Clinical application of continuity care model in stroke patients

ObjectiveTo probe into the clinical application effect of continuity care model in stroke patients. Methods58 patients with stroke in our hospital during June 2008 to June 2011 were selected.According to block randomization method,the selected patients were divided into the study group and the control group with 29 patients in each group.The control group received routine care interventions,while patients in the study group was given routine care interventions plus continuity care model.Activities of daily living,compliance behavior,quality of life,nursing satisfaction degree and other related indicators of patients in two groups were comparatively analyzed. ResultsCompared with the control group,the activities of daily living of the study group at discharge did not show significant differences,while 1 month,3 months,6 months after discharge,the activities of daily living was significantly increased.Regular medication,exercise time,timely return visit,proper diet and other indicators of compliance behaviors were significantly improved.The quality of life score did not show significant differences at discharge,while 3 months after discharge,the quality of life score were significantly higher in the study group than that of the control group.The satisfaction degree of patients was significantly higher in the study group than that of the control group. ConclusionsApplication of continuity care model in stroke patients can improve activities of daily living,compliance behavior,quality of life,satisfaction degree with nurses.It's worthy of further clinical application and promotion.

Objective To compare the myocardial protection effects of cold blood cardioplegia and cold crystalloid cardioplegia in patients undergoing valve replacement surgery.Methods One hundred adult patients with cardiac function Ⅱ~Ⅲ class undergoing primary elective valve replacement surgery due to rheumatic heart disease were retrospectively analyzed.For patients with cardiac arrest,cold blood cardioplegia was applied to 50 cases(Group B),and cold crystalloid cardioplegia to the others(group C).The process of heart resuscitation,the postoperative need of dopamine,the blood pressure and heart rate,the postoperative level of serum myocardial enzyme,the intensive care unit(ICU) stay after operation were collected and statistically analyzed.All the cardioplegic solution was perfused in a similar manner and topical cooling was employed simultaneously.Results There were less cases in group B than in group C who needed isoprenaline for cardiac resuscitation after clamp removal(P0.05).Conclusion The cold blood cardioplegia and cold crystalloid cardioplegia have similar myocardial protection effects on rheumatic heart disease patients with cardiac function Ⅱ~Ⅲ class undergoing valve replacement surgery.
10.Analysis of the relationship between the postgastrectomy cholelithiasis and gastrectomy

Objective To explore the factors for the development of gallstones after gastrectomy. Methods 52 cases of patients with postgastrectumy,were retrospectively analyzed according to their diseases for gastrectomy and operation form. Results Incidence of gallstone in patients of postgastrectomy was higher than in general population. Among which, Billroth Ⅱ type of gastrectomy for carcinoma was the highest (35.5%), then the total gastrectomy (31.5%) and proximate gastrectomy(13.5%). Billroth Ⅰ type operation was 10.3%. Incidence of gallstone in pa-tients of selective vagotomy was lowest (2.5%). Condusion Billroth Ⅱ type of gastrectomy and total gastrectomy were the risk factors of postgastrectomy cholelithiasis. The causes for gallstone formation after Billroth Ⅱ type of gas-trectomy were the restitution of digestive canal and metabolic disorder of bile acid.
