1.Research Advance and Prospects of Evidence-Based Acupuncture Science in China

Acupuncture is a traditional empirical medicine, and evidence-based medicine(EBM) is a new medical method and development of the empirical medicine. Since 1990s, Chinese acupuncture workers began to absorb and use the methods and principles of EBM to guide acupuncture clinical research and practice. Now more and more domestic acupuncture clinicians use and follow the methods and principles of EBM. On the systemic review of acupuncture-associated articles in Cochrane library, previously effective acupuncture therapy lacks adequate evidences, which may be related to the low quality of randomized controlled trials(RCTs). In the future, only developing specialized team of acupuncture clinical research and clinical research center can radically elevate the levels of acupuncture clinical research and evaluation. Meanwhile, developing China's evidence-based acupuncture should base itself on the characteristics of acupuncture, such as emphasis on original research in ancient books,high-quality single RCTs and systemic review of non-randomized researches.
2.Application of cavity isolation method on left colorectal obstruction in the anastomosis isolation

ObjectiveTo investigate the value of cavity isolation method on left colorectal obstruction in the anastomosis isolation. Methods60 patients with left colorectal obstruction were randomly divided into two groups:ileostomy group and cavity isolation method group,30 patients for each. After left colon resection was completed, ileostomy anastomosis group used the traditional method for colorectal anastomosis, and protected the anastomosis by ileostomy. Cavity isolation method group protected the anastomosis by cavity isolation method. Then the indexes in the two groups,such as anastomotic height, operation time, anal exhaust time,incidence of anastomotic leakage and its complications,hospitalization time, medical cost and so on were analyzed. Results Longer postoperative hospital stay (9.3 + 4.2) d were observed in ileostomy group than those ( 15.6 + 3.3 ) d ( P < 0.05 ). Earlier time of the first flatusand oral intake(87.2 ± 6.3 ) h, ( 91.2 ± 7.2 ) h were observed in ileostomy group than those ( 101.8 + 7.5 ) h,( 117.3 ± 8.6) h in cavity isolation method group ( all P < 0.05 ). ConclusionThe anastomosis isolation was a safe and feasible new technology that could reduce the complications caused by anastomotic leakage, and it had the advantages of short hospital stay, low cost,less suffering ect.
3.Detection of ocular fundus hemodynamics before and after scleral buckling by retina flowmeter

Objective To detect the ocular fundus hemodynamic changes in patients with rhegmatogenous retinal detachment before and after the scleral buckling procedure by Heidelberg retina flowmeter,and analyse the related influential factors. Methods Heidelberg retina flowmeter was employed to measure the volume,flow and velocity in retinal vessels of the impaired eyes in 47 patients with unilateral rhegmatogenous retinal detachments 1 day before and 1 week,1 month and 3 months after scleral buckling procedure.The contralateral eyes were served as controls. 4.Clinical effect of Danshen injection iontophoresis combined with Lumbrokinase in early retinal vein occlusion patients

Abstract? AlM: To explore the effect of Danshen injection iontophoresis combined with lumbrukinase on early patients with retinal vein occlusion ( RVO ) , and observe the prognosis to provide objective basis for clinical?METHODS: Eighty-two patients ( 117 eyes ) with early RVO were randomly selected in our hospital from January, 2011 to April, 2014, 41 cases (59 eyes) of control group treated with Lumbrokinase. Forty-one cases ( 58 eyes) of the observation group, treated with Danshen injection iontophoresis combined with lumbrokinase. The main indexes were the treatment effect, visual acuity and retinal circulation time.?RESULTS: The total effective rate of the observation group was 85. 4% ( 35/41 ), more than that of control group 68. 3% ( 28/41 ), the difference was statistically significant (P<0. 05). The invalid rate of the observation group was 14. 6% (6/41), lower than that of the control group 31. 7% ( 13/41 ), the difference was statistically significant (P<0. 05). The vision of the observation group 4. 8 ~5. 0 was 53. 7% (22/41), more than that of control group 41. 5% ( 17/41 ), the difference was statistically significant ( P < 0. 05 ). The restore vision of the observation group 4. 3 ~ 4. 7 was 31. 7% (13/41), lower than that of the control group 41. 5% ( 17/41 ), the difference was statistically significant ( P<0. 05 ). After treatment, retinal circulation time of two groups were lower than before treatment, the difference was statistically significant (P<0. 05). After treatment of retinal vein circulation time of the observation group was 8. 15 ± 1. 30s, 9. 70 ± 1. 28s lower than that of the control group, the difference was statistically significant (P<0. 05).?CONCLUSlON: There is better effective on Danshen injection iontophoresis combined with Lumbrokinase for early retinal vein occlusion, and can improve the patient's visual acuity, promote the rehabilitation of patients.
5.Prevention and control of infectious diseases among primary schools in Xi’an

Objective:
To understand prevention and control of infectious disease among primary schools in Xi’an.
Methods:
A total of 93 primary schools were selected through multi-stage stratified sampling method from 14 counties in Xi’an, then these schools are surveyed to meaure the classroom ventilation.
Results:
About 28.0% (26/93) of these schools met the national standard for health staffs, with 7.4%(4/54) in urban primary schools and 56.4% in rural primary schools (22/39), the difference between urban and rural areas is statistically significant(P<0.01). Urban primary schools were better than rural primary schools in morning inspection, registration and tracking for illness, verification of vaccination certificates, surveillance and reporting of epidemic outbreaks, verification of school recovery(P<0.05). The average per capita area among the 92 primary schools was (1.22±0.34) m2, with (1.08±0.26) m2 in urban primary schools and (1.42±0.33) m2 in rural primary schools, the difference between urban and rural areas was statistically significant (P<0.01). The average per capita ventilation area of 92 primary schools was (0.15±0.07) m2 with (0.13±0.04) m2 in urban primary schools and (0.19±0.08) m2 in rural primary schools, the difference between urban and rural areas was statistically significant (P<0.01).
Conclusion
School-based infectious diseases prevention and control needs to be improved in Xi’an, especially in health personnel. Urban schools appear superior in the prevention and control of infectious diseases and sanitary conditions, while rural schools appear superior in student density and classroom ventilation.
6.Auricular acupuncture for primary insomnia: a systematic review based on GRADE system.

The clinical efficacy and safety of auricular acupuncture (AA) for treatment of primary insomnia was evaluated. After a comprehensive retrieval in domestic and foreign databases, literatures were strictly screened and Revman 5.2 software was applied to perform a Meta-analysis on eligible randomized controlled trials (RCTs). The evidence quality was assessed with GRADE profiler 3.6 software. As a result, 8 articles were included involving 894 patients. Compared among AA and sham AA, placebo AA, blank control, there was significant difference in Pittsburgh sleep quality index (PSQI) [WMD = -3.48, 95% CI (-3.96, -3.00)], sleep latency LWMD = -10.14, 95% CI (-17.16, -3.12)] and sleep awakening times [WMD = -9.98, 95% CI (-1.10,-0.48)]. Compared between AA and western medication, there was significant difference in PSQI [WMD = -3.62, 95% CI (-4.59, -2.65)]. The evidence quality was moderate in AA vs. sham AA, placebo AA or blank control, while that of the rest was extremely low. No reports of adverse events were described in all studies. In conclusion, for the treatment of primary insomnia, AA could effectively improve sleep quality, but due to the low evidence quality, cautious attitude should be taken on this conclusion, and clinical trials with large sample and high quality were needed in the further.

The rising of evidence-based medicine requires high quality for clinical research and randomized controlled trial is considered as clinical research of the most level of evidence degree and contidence activity.This article illustrates several important and instructive designs of randomized controlled trials,which can give some reference for clinical research on acupuncture.And the developing direction of future clinical research on acupuncture is discussed.
8.Inhibition of α-crystallin on proliferation of rat retinal microglia following optic nerve injury

Objective To investigate the effect of α-crystallin on the proliferation of rat retinal microglia after optic nerve injury. Methods The effect of α-crystallin on number and proliferation of microglia were analyzed by MTT assay.After the rat model with optic nerve injury was established,α-crystallin was iniected into vitreous cavity and the microglia cell number were counted and compared by retinal fiat counting and immunofluorescence labeling in different groups. Results The proliferation and activation of microglia cells could be stimulated by LPS at 10-6g/L to 10-2g/L.α-crystallin at 10-4g/L and 10-6g/L could inhibit proliferation and activation of microglia cells.Compared to BSA iniection group,α-crystallin could inhibit more significantly the number of microglia cells 1-3 weeks after injury (P<0.05). Conclusions α-crystallin can inhibit proliferation and activation of retinal microglia and alleviate overphagocytosis and secondary damage of retinal microglia to retinal ganglion cells(RGCs),which may be another mechanism that α-crystallin contributes to indirect protection of RGCs.
9.Epidemiology and risk factors of invasive fungal infections in old patients for non-respiratory tract

Objective To investigate the epidemiology and relevant risk factors of invasive fungal infection (IFI) in hospital old patients for non-respiratory tract. Methods Seventy-eight patients of IFI in non-respiratory tract were enrolled in this investigation. The incidence and risk factors of IFI were analyzed by prospective case-control study. Results In 78 old patients, 84 strains were isolated from different parts, and the most was Candida spp 82 strains (97.62%,82/84), followed by Candida albicans 55 strains (67.07%,55/82), Candida glabrata 13 strains ( 15.85%, 13/82), Candida krusei 6 strains (7.32%, 6/82), Candida tropicalis 4 strains (4.88% ,4/82), Candida parapsilosis 3 strains (3.66% ,3/82), Candida lusitaniae 1 strain ( 1.22%, 1/82). Aspergillus 2 strains (2.38%,2/84). Multivariate Logistic regression analysis showed that age, pathogen detection time, underlaying disease,glucocorticoids, immunosuppressants were the risk factors for IFI in non-respiratory tract. Conclusions Candida albicans is the main pathogens of Candida infections in old patients. To efficiently control the risk factors should be emphasized in old patients, including early diagnosis and treatment underlying diseases, appropriate use drugs, right to shorten hospital stay.
10.Human umbilical vein endothelial cells support hematopoiesis and expansion of hematopoietic stem/progenitor cells in vitro

Objective: To investigate the role of human umbilical vein endothelial cells (HUVEC) in supporting hematopoiesis and the expansion of hematopoietic stem/progenitor cells in vitro. Methods: According to the fact that HUVEC supernatant has colony stimulating activity shown by methylcellulose colony-forming assay and HUVEC can maintain the survival of mononuclear cells for at least four weeks in vitro, CD34+ cells from umbilical cord blood were seeded with (HUVEC group) or without (control group) HUVEC monolayer. Every week cells were collected and counted, the frequency of CFU-GM was measured by using methylcellulose colony-forming assay, and the percentage of CD34+ and CD41a+ cells was measured by flow cytometry. Results: In control group,all the CD34+ cells died in two weeks. However, in HUVEC group,most nucleated cells and CD34+ cells were expanded by 68.1±14.8 fold and 6.6±1.4 fold,respectively at the third week while CFU-GM expansion reached its peak (5.7±2.1 fold) at the week 2. Moreover, the percentage of CD41a+ cells was enhanced significantly, reaching a maximum (15.6%) at the week 3. Conclusions:HUVEC can support hematopoiesis in vitro and expand the hematopoietic progenitor cells and CD41a+ cells in direct contact coculture.
