Objective To investigate the bacterial distribution and resistance of clinical isolates ,so as to provide basis for the clinical medication .Methods The drug susceptibility testing was performed by Kirby‐Bauer method or automated system .The re‐sults were analyzed by WHONET 5 .6 referring to the breakpoints of CLSI 2013 .Results Of 1 240 clinical iso1ates ,the gram‐posi‐tive cocci and gram‐negative bacteria accounted for 32 .7% and 67 .3% respectively .The top five of isolated strains were Escherichia coli ,Klebsiella spp ,coagulase negative staphylococci ,Staphylococcus aureus ,Streptococcus pneumonia;the first five of isolated spec‐imens constitute followed by sputum ,urine ,blood ,secretions and stool .The average prevalence of methicillin‐resistant strains in S . aureus (MRSA) and coagulase‐negative Staphylococcus (MRCNS)was 17 .1% and 74 .7% ,respectively ;no vancomycin ,teicoplanin or linezolid‐resistant strains were found;no vancomycin‐resistant strains were found in Enterococcus spp;Enterobacteriaceae was still highly susceptible to imipenem and meropenem (> 85% );extended spectrum beta‐lactamases(ESBLs) producing strains ac‐counted for 56 .7% of Escherichia coli ,and accounted for 18 .6% of Klebsiella pneumonia;the drug resistance rates of Pseudomonas aeruginosa to imipenem was 11 .9% .The drug resistance rates of Acinetobacter spp to imipenem was 19 .0% .Conclusion The main infection site of patients in this hospital is respiratory tract and attention should be paid to docimastic sample .The top five clinical i‐solates and drug resistance are roughly the same with the results of Chongqing drug resistance monitoring net .

Objective To determine the median effective target effect-site concentration (EC50) of sufentanil when intraoperative wake-up test was successful in patients undergoing scoliosis surgery.Methods Ninety ASA Ⅰ or Ⅱ patients,aged 18-64 yr,with body mass index ＜ 30 kg/m2,scheduled for scoliosis surgery under sevoflurane and sufentanil anesthesia,were randomly divided into Ⅰ-Ⅵ groups (n =15 each).Anesthesia was induced with sufentanil,etomidate and cisatracurium,and maintained with sevoflurane,sufentanil and cisatracurium.The patients were tracheal intubated and mechanically ventilated.Before the wake-up test,infusion of cisatracurium was stopped and the end-tidal concentration of sevoflurane was adjusted to 0.The EC50 was determined by the K(a)rber method.The target effect-site concentration of sufentanil was set at 0.19 ng/ml in group Ⅰ,0.18 ng/ml in group Ⅱ and gradually decreased in decrements of 0.01 ng/ml.The wake-up test was performed 5 min later.The EC50 and 95 ％ confidence interval of sufentanil were calculated by the K(a)rber method.Results EC50 of sufentanil obtained was 0.164 ng/ml and 95％ confidence interval of sufentanil obtained was 0.157-0.172 ng/ml when the wake-up test was successful.Conclusion The EC50 of sufentanil is 0.164 ng/ml when the intraoperative wake-up test is successful in the patients undergoing scoliosis surgery.

Objective To discuss the techniques of ventricular endoscopy for obstructive hydrocephalus.Methods A total of 33 patients with obstructive hydrocephalus were treated by ventriculostomy using ventricular endoscopy from June 2003 to December 2006.The patients had received CT,MRI,and 99mTc-DTPA cistern SPECT before the surgery.Results Endoscopic third ventriculostomy was performed successfully in 32 patients,the other patient died of hemorrhage under the third ventricle fistula.Postoperative fever occurred in 13 patients.One patient developed bilateral chronic subdural hematoma 6 months after the operation,and was cured by drill and drainage.Three patients developed subdural hydroma 6 months postoperation without showing symptoms.They were monitored without any treatment.The 32 patients were followed up for 2-42 months(mean,16.7 months),all of them achieved clinical relieve.In the patients,the intracranial pressure(ICP) was 26(16-39) cm H2O before the operation,and was significantly decreased to 22(12-28) cm H2O 7 days after the operation(t=2.903,0.005

Objectives To analyze a rare autosomal recessive disease, aggrecan type spondylometaphyseal dysplasia (SEMD), which was caused by ACAN gene mutations. Methods A 7 years old girl was diagnosed with short stature after excluding growth hormone deficiency, idiopathic short stature, and hypothyroidism. Combining family history and clinical features, SEMD were suspected and genetic tests were performed. Results The patient was found with homozygous mutations of c.512C > T inACAN gene, and was diagnosed with aggrecan type SEMD. Her parents were found to be heterozygous mutation carrier. Conclusions In patients with high suspection of a special type of short stature, early genetic tests should be carried out for a clear diagnosis.

OBJECTIVE:To establish a method for the simultaneous determination of chlorogenic acid,paeoniflorin,ferulic ac-id,berberine hydrochloride and baicalin in Lianqiao baidu pill. METHODS:HPLC was performed on the column of Agilent Eclipse XDB-C18 with mobile phase of 0.5% phosphoric acid(triethylamine adjusted to pH 3.0)-methanol(gradient elution)at a flow rate of 1.0 ml/min,the column temperature was 30 ℃,the detection wavelength was 230 nm(for paeoniflorin and berberine hydrochlo-ride),324 nm(for chlorogenic acid and ferulic acid)and 278 nm(for baicalin),and the injection volume was 5 μl. RESULTS:The linear range was 1.884-30.144 μg/ml for chlorogenic acid(r=0.999 9),0.392-6.272 μg/ml for paeoniflorin(r=0.999 7), 0.102-1.632 μg/ml for ferulic acid(r=0.999 7),1.326-21.216 μg/ml for berberine hydrochloride(r=0.999 8) and 1.95-31.2 μg/ml for baicalin(r=0.999 9),respectively;RSDs of precision,stability and reproducibility tests were lower than 2.0%;recoveries were 99.58%-102.47%(RSD=0.93%,n=9),99.21%-102.18%(RSD=0.90%,n=9),98.28%-101.23%(RSD=0.86%,n=9), 99.66%-101.84%(RSD=0.82%,n=9)and 99.18%-101.05%(RSD=0.62%,n=9). CONCLUSIONS:The method is accurate,re-liable,simple and fast,and can be used for the simultaneous determination of chlorogenic acid,paeoniflorin,ferulic acid,berber-ine hydrochloride and baicalin Lianqiao baidu pill.

Nowadays,the study of tumor stem cells has become the heatpoint in cancer research.Many experiments have successfully demonstrated the existence of tumor stem cells,which have been isolated from some solid tumors.As the research on the origin of tumor stem cell is developing,the knowledge of the occurrence and development of tumor has become more clear,which will influence the diagnosis and treatment for tumor significantly.Moreover it will bring benefit to the following-up after surgical operation and giving hopes to cancer sufferers.

Objective To detect the correlations between vascular endothelial growth factor (VEGF) expression, microvessel density ( MVD) and time intensity parameters of contrast enhanced ultrasound in hepatocellular carcinoma (HCC). Methods Seventy-three patients of HCC were analyzed with time intensity curve of contrast enhanced ultrasound. VEGF and CD34 in different degree of differentiated HCC specimens were detected by immunohistochemistry. The correlations between VEGF, MVD and time intensity parameters were analyzed. Results Arrival time had no significant difference among different differentiated groups. However, the area under the curve, enhanced time and washout time had significant difference among different differentiated groups respectively ( P <0. 05). There was a positive correlation between VEGF expression and MVD in HCC. MVD was associated with enhanced time and the area under the curve ( r = - 0. 615,0. 563,respectively). The area under the curve,enhanced time and washout time of VEGF positive expression group had significant difference comparing to those of VEGF negative expression group ( P < 0. 05). Conclusions It was helpful to assess HCC differentiated degree with time intensity parameters of contrast enhanced ultrasound.

Objective To investigate the incidence and risk factors of adverse drug reactions in hematological system induced by linezolid.Methods In this retrospective study, 124 inpatients treated with linezolid (n=62) or vancomycin (n=62) for anti-infective therapy between January 2012 and December 2015 in clinical departments of the Second Affiliated Hospital of Anhui Medical University were included.The incidence hematological adverse drug reactions were observed, and the single factor and the multiple factor Logistic regression methods were used to analyze the risk factors of developing thrombocytopenia and decline of hemoglobin.Results Among the 62 inpatients treated with linezolid, thrombocytopenia occurred in 21 patients(33.87%), and decline of hemoglobin occurred in 17 patients (27.42%).No patient discontinued the use of linezolid for the reason of thrombocytopenia or decline of hemoglobin.In multiple stepwise regression analysis, linezolid use[OR=7.699,95%CI (1.408,42.090),P=0.019], treatment duration>14 d[OR=7.639,95%CI(1.162,50.226),P=0.034], baseline eGFR<80 mL·min-1[OR=6.150,95%CI(1.604,23.577),P=0.008], baseline ALB<25 g·L-1[OR=4.078,95%CI(1.017,16.351),P=0.047] and baseline platelet count<200×109·L-1[OR=6.148,95%CI(1.705,22.172),P=0.006] were independent risk factors for thrombocytopenia;linezolid use [OR=4.335,95%CI(1.308,14.365),P=0.016] and baseline ALB<25 g·L-1[OR=5.424,95%CI(1.824,16.129),P=0.002] were independent risk factors for the decline of hemoglobin.Conclusion The incidences of thrombocytopenia and anemia induced by linezolid are not rare, and most of them can be returned to normal.The risk factors of thrombocytopenia and anemia should be concerned and the routine blood test should be monitored during use.
