1.Protective Effects of Ginsenosides Rb_1 and Re on Injury of the Neonate Rat Cardiomyocyte Induced by Aconitine Alkaloids

Objective To investigate the protective effect of ginsenoside Rb1 and Re on injury of the neonate rat cardiomyocyte induced by aconitine alkaloids.Methods The influence of aconitine and ginsenoside Rb1,Re together in neonate rat cardiomyocyte was observed respectively.The influence of these components in neonate rat cardiomyocyte was examined by myocard zymogram.The expression of Ca2+ channel gene Cav1.2 mRNA of neonate rat cardiomyocyte after treatment was observed by RT-PCR.Results The ginsenoside Rb1 and Re were able to decrease the release of AST and LDH,reverse Cav1.2 mRNA abundance induced by aconitine.Conclusion The ginsenoside Rb1 and Re which together with aconitine can alleviate the side effects and boost up the therapeutic effects of aconitine.The action mechanism may be relation with that the ginsenoside Rb1 and Re can decrease injuries of the neonate rat cardiomyocytes and abundant expression of Cav1.2 mRNA induced by aconitine.

5.The correlation between CD4+ T cell count and the routine blood tests in HIV/AIDS patients

Objective To determine the feasibility of human routine blood tests as a surrogate for CD4+ T cell count through studying the correlation of CD4+ T cell count with total lymphocyte count(TLC),hemoglobin(Hb),blood platelet(PLT),and white cell count(WBC)in HIV/AIDS patients.Methods 1 038 person-time blood tests among 778 HIV/AIDS patients were performed and Spearman correlation analysis was used.Predictive power and the cut-off for potential predictors of CD4+ T cell count were assessed through receiver operating characteristic(ROC)curves.Combination test was used to assess the capability of multipie indexes to serve as surrogate markers for CD4+ T cell counL Results Significant correlations with CD4+ T cell count were observed for TLC,Hb,PLT and WBC.The Spearman correlation coefficients were r=0.64,P=0.000;r=0.36,P=0.000;r=0.24,P=0,000;r=0.09,P=0.000,respectively.No correlation between TLC and CD4+ T cell count was found when,TLC was more than 2 000 × 106/L(r=0.12,P=0.15).The areas under ROC curve of TLC and Hb for predicting CD4+ T cell count were between 0.82 to 0.84,and 0.66 to 0.70,respectively.When CD4+ T cell count were less than 50,200,350 cells/μl respectively,the optional cut-off value was TLC<1 100 × 106/L,1 200 × 106/L and 1 400 × 106/L.When the study combined TLC<1 200 × 106/L and Hb<120 g/L for prediction of CD4+ T cell count<200/μl,the sensitivity was 45.3% and specificity was 82.8%.Conclusion There is no significant application value for combination of TLC<1 200×106/L and Hb<120 g/L as a surrogate for prediction of CD4+ T cell count<200/μl.
6.The reason and management of the sunken upper eyelid after hydroxyapatite platform implantation.

OBJECTIVETo study the reason and the management of the sunken upper eyelid after implanting a hydroxyapatite platform.
METHODSFrom 1998, we developed a method of subperiosteal implantation at the orbital floor to repair the introcession of the upper eyelid after hydroxyapatite platform implantation. 11 cases of the sunken upper eyelid were treated with this method. The implants included Medpor in 2 cases, hydroxyapatite plates in 7 cases and acellular dermal matrix in 2 cases.
RESULTSPostoperatively, all the patients obtained satisfactory results. Follow-up for 1 to 4 years showed no complications of extrusion or infection of the implants. Re-operation was needless in all of them.
CONCLUSIONSubperiosteal implantation to correct the sunken upper eyelid is a safe and effective method.

7.Prevalence and associated risk factors for obesity and nonalcoholic fatty liver disease in adolescents in Xi′an, Shaanxi province, China

ObjectiveTo provide a scientific basis for the prevention and early intervention of obesity and nonalcoholic fatty liver disease (NAFLD) in adolescents. MethodsAnthropometric measurements, liver function test, and abdominal ultrasonography were performed in adolescents who received physical examination in our hospital from March to April, 2012. Between-group comparison of continuous data was made by t test, and between-group comparison of categorical data was made by χ2 test. The further multivariate analysis was performed by logistic regression analysis. ResultsA total of 4141 adolescents, including 2061 girls and 2080 boys, were enrolled in the study with an average age of 18.62±0.66 years. Boys had significantly higher overweight and obesity rates than girls (χ2=49.5, P<0.01; χ2=20.4, P<0.01). The overall incidence of NAFLD in subjects was 8.1% (335/4141), while the incidence of NAFLD in boys was significantly higher than that in girls (13.4% vs 2.8%, χ2=156.4, P<0.01). The detection rate of NAFLD was the highest in obese subjects, followed by subjects with high body mass index (BMI) and subjects with normal BMI. Moreover, obese boys had a significantly higher detection rate of NAFLD than obese girls (71.6% vs 29.0%, χ2=56.5, P<0.01). Among both boys and girls, BMI, body mass, alanine aminotransferase level, and aspartate aminotransferase level were significantly higher in the NAFLD group than in the non-NAFLD group (all P<0.05). Logistic regression analysis revealed that body mass and BMI were likely to be independent risk factors for NAFLD. ConclusionThe prevalence of obesity and NAFLD in adolescents in Xi′an is not optimistic. For obese adolescents, healthy lifestyle, body mass control, and BMI reduction are important approaches for prevention of NAFLD.

AIMTo explore the effects of interleukin-1beta (IL-1beta) on inducible nitric oxide synthase (iNOS)-nitric oxide (NO) system activity in arginine vasopressin (AVP)-induced rat cardiac fibroblasts (CFs).
METHODSCFs were isolated by trypsin digestion method. Nitric acid reductase method, spectrophotometry and reverse transcription-polymerase chain reaction (RT-PCR) were used to detect NO contents, NOS activity and iNOS mRNA expression.
RESULTSAVP significantly increased iNOS mRNA expressions, NOS activity and NO contents (P < 0.05) in CFs. IL-1beta enhanced the effects of AVP on iNOS-NO system activity in a concentration-dependent manner, moreover the iNOS mRNA expressions, NOS activity and NO contents of AVP + 3 ng/ml, AVP + 5 ng/ml IL-1beta group were both significantly higher than those of AVP group (P < 0.05). But when IL-1beta concentration increased to 5 ng/ml, the iNOS mRNA expressions, NOS activity and NO contents did not increase accordingly, slightly decreased instead.
CONCLUSIONWithin certain range of concentrations IL-1beta cooperates with AVP to increase iNOS-NO system activity in CFs.

10.The value of procalcitonin for diagnosing infection in critically ill patients receiving long-term immunosuppressive therapy

Objective To assess the value of procalcitonin(PCT)measurement to differentiate infection from non-infection in critically ill patients requiring long-term immunosuppressive therapy.Methods A prospective study was conducted in patients with underlying diseases requiring corticosteroids or chemotherapy in ICU from January 2008 to December 2009.Patients were divided into the infection group and the non-infection group and their PCT levels were compared.Results A total of 103 patients (65 women)were enrolled in this prospective study[aged(47.9 ± 21.9)years old]with 84 in the infection group and 19 in the non-infection group.The baseline level of PCT was significantly higher in infection than in non-infection patients[2.58(0.08-44.65)pg/L vs 0.62(0.15-6.00)pg/L,P =0.002].Different levels of PCT were manifested in different pathogen groups with 3.41(0.45-44.65)pg/L in bacteria infection,0.99(0.28-6.67)pg/L in fungus infection,0.11(0.08-0.20)pg/L in virus infection group(P =0.018).The AUCROC of PCT was 0.867 for diagnostic bacterial infection.By multivariate analysis,the factors associated with the level of PCT were bacteria infection(OR 5.1,P =0.031)and septic shock(OR 7.5,P =0.027),while the factors not associated with the level of PCT were age,renal function,infection site and prognosis(P > 0.05).Conclusions The level of PCT is increased in the critically ill patients requiring immunosuppressive therapy with infection and it can be used for diagnosis for bacterial infection.
