Objective To investigate the effects of short-term continuous subcutaneous insulin infusion (CSII) in newly diag-nosed type 2 diabetes mellitus(T2DM ) and to evaluate the treatment regimen conversion after CSII therapy .Methods 72 patients with newly diagnosed T2DM were treated with CSII for 2 weeks .Then they were randomly divided into two groups :the basal insu-lin group(glargine) and the oral anti-diabetic drug(OAD) group .Both groups were followed up for 2 years .Blood glucose ,insulin and HbA1c were measured before and after CSII and during the 2-year follow-up .Results CSII significantly controlled the glucose levels ,reduced the TG ,TC ,LDL levels and the homeostasis model assessment of insulin resistance ( HOMA-IR) and increased the homeostasis model assessment of insulin secretion (HOMA-IS)(all P< 0 .05) .During 1-year follow-up ,HbA1c in the glargine group and the OAD group was (6 .13 ± 0 .47)% and(6 .21 ± 0 .38)% respectively .During 2-year follow-up ,the HbA1c values in the two groups were (6 .91 ± 0 .57)% and(6 .43 ± 0 .62)% respectively .T HOMA-IR and HOAM-IS obtained the long tern improve-ment without significant body weight increase .Conclusion Short-term intensive insulin therapy can effectively control the blood glucose ,improve the sensitivity of insulin and the β-cell function ;after CSII ,adopting basic insulin and oral antidiabetic drugs can a-chieve the ideal glycemic control .
6.Physical and mechanical characteristic of bone cement mixed with antibiotics

Objective To investigate the physical and mechanical properties of home-made bone cement mixed with different antibiotics.Methods The specimens of bone cement each of 40g were divid-ed in to6groups depending on mixed antibiotics,groupⅠmixed with1g gentamicin,groupⅡwith2g gen tam icin,groupⅢwith1g cephradine,groupⅣwith1.5g cefuroxine,groupⅤwith1g ceftriaxone and groupⅥwith out any antibiotics served as control.The dough time,compressing depth,compressive strength,bending strength and bending elastic modulus were measured respectively,elution examination of antibiotics from bone ce ment was performed by chromatography.Results The dough time and compress-ing depth of the groups were better than that of ISO5833standard,and there was no significant difference a-mong the groups.The com pressive strength and bending strength of the groupⅡwere lower than that ISO5833standard,but there was no significant difference among the groups by analysis of variance.The bending elastic modulus of the groupⅡwas lower than the control group and other groups(P

8. Urine neutrophil gelatinase-associated lipocalin and MEDS score in evaluating the severity and prognosis of sepsis patients with acute kidney injury

Objective To evaluate the value of urine neutrophil gelatinase-associated lipocalin (NGAL) combined with mortality in emergency department sepsis (MEDS) score in evaluating the severity and prognosis of sepsis patients with acute kidney injury (AKD. Methods A total of 64 septic patients were selected from the intensive care unit of emergency department of Shanghai Seventh People’s Hospital, and they were treated from January 2013 to December 2015. According to the AKI diagnostic criteria of Kidney Disease; Improving Global Outcomes (KDIGO), the patients were divided into group AKI1 (21 cases), group AKI2 (24 cases) and group AKI (19 cases). The patients were also divided into death group (33 cases) and survival group (31 cases) according to their survival slate within 28 days. The urinary NGAL levels and MEDS score were compared between groups of different AKI stages and different survival slates. ROC curves were plotted to assess the predictive abilities of urine NGAL, MEDS score and the combination of both for the 28 day mortality of sepsis patients with AKI, with Youden index used to identify the optimal cut-off value. Results The urinary NGAL level and MEDS score in the three groups of different stages gradually increased with the severity of the disease, showing significant difference (P<0. 01). We also found that NGAL level and MEDS score in the death group were significantly higher than those in the survival group (P<0. 01). ROC curve analysis showed that the areas under the curve (AUC) of urinary NGAL and MEDS score were 0. 885 and 0. 841, respectively, with the optimal cut-off values being 157. 5 ng/mL and 10. 5 points, respectively. The AUC of urinary NGAL combined with MEDS score for predicting 28 day fatality rate was 0. 936, with the sensitivity being 91. 4% and specificity being 89. 2%, which were better than using urinary NGAL or MEDS score alone. Conclusion Both urine NGAL and MEDS score can be used as for predicting severity of sepsis patients with AKI. and the combination use of the two has even a greater value for the prognosis.
9. Parental haploidentical hematopoietic stem cell transplantation for hematologic diseases: A report of 45 cases

Objective The present report describes the clinical effects of parental haploidentical hematopoietic stem cell transplantation (hi-allo-HSCT) in the treatment of hematologic diseases. Methods A total of 45 patients received parental hiallo- HSCT from July 2007 to January 2011. The therapeutic effects and complications were observed. Results Engraftment was successful in a total of 43 patients. Implantation failed in 2 patients. The incidence of the graft versus host disease (GVHD) was 62.2%. The incidence of acute GVHD was 40.0%, and chronic GVHD occurred in 22.2% of the patients. The incidence of GVHD was lower when the father was the donor compared with the mother was the donor. The incidence of GVHD was related to the age of the donor and the number of HLA matching sites. In addition, infections observed in the present study were mainly blood-borne with cytomegalovirus as the invader and lung infections. During the follow-up period of 6 months to 4 years, six patients died in the 43 patients with successful implantation. The major cause of death was infection and a relapse of their original disease. The disease free survival (DFS) rate was 86.7%. Seven patients additionally received umbilical cord blood, their efficacy in the transplantation seemed better than those who received parental stem cells only, as hematopoietic reconstruction was faster and the incidence of GVHD accounted for only 7.14% of the total incidence rate. Conclusions Parental hi-allo-HSCT is an effective treatment for hematologic diseases. A young male donor with more HLA matching sites is recommended to prevent GVHD and infection. The combination of parental hi-allo-HSCT and umbilical cord blood transplantation could result in positive effects with faster hematopoietic reconstruction and a lower incidence of GVHD.
10.The Role and Mechanism of MiR-130b-5p in Down-regulating IGF-1 and Inhibiting the Migration and Invasion of Human Chorionic Trophoblast Cells

ObjectiveTo explore the effects of microRNA 130b-5p (miR-130b-5p) on the migration and invasion of human chorionic trophoblast cells (HTR8/SVneo) by targeting insulin-like growth factor-1 (IGF-1). MethodsHTR8/SVneo cells were divided into control group, miR-NC group, miR-130b-5p mimics group, miR-130b-5p mimics+pcDNA3.1 group and miR-130b-5p mimics+pcDNA3.1-IGF-1 group. The dual luciferase reporter gene was used to detect the targeting relationship between miR-130b-5p and IGF-1; real-time fluorescent quantitative PCR was used to detect the expression level of miR-130b-5p and IGF-1in HTR8/SVneo cell samples; the CCK-8 method was used to detect the proliferation of HTR8/SVneo cells; holographic cytometer and Transwell method was used to analyze the migration and invasion abilities of HTR8/SVneo cells; Western blot method was used to detect the expression of IGF-1, invasion, and epithelial-mesenchymal transition (EMT) proteins in HTR8/SVneo cell samples. ResultsThe results of the dual luciferase reporter gene showed that IGF-1 was a potential target gene of miR-130b-5p; compared with the miR-NC group, the expression level of miR-130b-5p, the cell proliferation inhibition rate, and the expression level of E-cadherin increased significantly in the miR-130b-5p mimics group. The expression levels of IGF-1, c-Myc, Cyclin D1, migration distance, number of invaded cells, the expression levels of matrix metalloproteinase 9 (MMP9), matrix metalloproteinase 2 (MMP2), neural cadherin (N-cadherin) and Vimentin were significantly reduced (P<0.05); compared with the miR-130b-5pmimics+pcDNA3.1 group, the cell proliferation inhibition rate and the expression level of E-cadherin in the miR-130b-5p mimics+pcDNA3.1-IGF-1 group were significantly reduced, while the expression levels of c-Myc and CyclinD1, migration distance, number of invasive cells, the expression levels of MMP9, MMP2, Vimentin and N-cadherin were significantly increased (P<0.05). ConclusionThe overexpression of miR-130b-5p may inhibit the migration and invasion of HTR8/SVneo cells by inhibiting the expression of IGF-1.
