1.Treatment of perimenbranous ventricular septal defect-transcatheter occlution vs surgery

Objective To evaluate the therapeutic effect of transcatheter closure of perimenbranous ventricular septal defect (PMVSD) with membranous VSD occluder device and compare the effect of transcatheter closure of VSD with surgery. Methods The therapeutic effect of transcatheter occlusion in 48 children with VSD is compared with that of other 73 cases who had received surgical treatment. Age, height, weight, pulmmary artery pressure, size of the defect, time for hospitalization, success rate as well as complications after operations are compared between the 2 groups. All the data are proccessed either by means of t test or ? 2 test statistically. Results There are no statistical differences between the two groups in age, height, weight, pulmonary systolic pressure and the rate of success. But the size of VSD is smaller and the time for operation and hospitalization is shorter in the group of transcatheter occlusion. There are no significant differences between the two groups in the rate of complications including the second surgery, residual shunt, bleeding, nerve injury, hemolysis, Ⅲ atrioventricular block, pneumothorax, hydrothorax, hydropericardium, aortic valve insufficiency and infection. But left anterior bundle branch block and complete left bundle branch block occur more often in the group of the catheter interventions during or after the occlusion. Conclusion Transcatheter occlusion shows a promising therapeutic effect on VSD. To prevent complications, therapeutic rules and technical criterias should be obeyed and close monitoring after the closure should be carried out.
2.Transcatheter closure of congenital vascular malformations with cook coils and aGA amplatzer duct occluder in children

Objective To explore the method and therapeutic principle of transcatheter closure of coronary artery fistula, aortopulmonary collaterals and aortopulmonary window. Methods Interventional occlusion of 4 cases of coronary artery fistulas and 1 case of aortopulmonary collaterals with Cook coils, 1 case of coronary artery fistulas and 1 case of aortopulmonary window with AGA Amplatzer duct occluder. The mean age of 7 children were 8.5 years. Results Transcatheter closure of 7 cases of vascular malformations were successful. During the follow-up period of 1 month to 5 years, no residual shunt and other complications appeared. Conclusion The curative effect of transcatheter closure of vascular malformations was good. But for the occlusions′ success and prevention from complications occurrence, the indications of closure should be obeyed, the time of X-ray exposure should be less than 60 minutes.
4.Effects of Goserelin on Vascular Growth Factor and Immune Function of Rats with Prostatic Hyperplasia

OBJECTIVE:To study the effects of goserelin on vascular growth factor and immune function of rats with prostatic hyperplasia. METHODS:Rats were selected to establish prostatic hyperplasia model and randomly divided into model group,gose-relin low-dose,medium-dose and high-dose groups (0.4,0.8,1.2 mg/kg);normal rats were selected as normal control group, with 10 rats in each group. Normal control group and model group were given normal saline intragastrically,and goserelin groups were given relevant dose of drugs intragastrically,once a day,for consecutive 25 days. The prostate volume,wet weight,prostatic index of rats were detected as well as positive cell area of VEGF,TGF-β1,FGF,CD4 and CD8 in prostate tissue. RESULTS:Compared with normal control group,prostate volume,wet weight,prostatic index,positive cell area of VEGF,TGF-β1,FGF, CD4 and CD8 were all increased in model group (P<0.05). Compared with model group,the above indexes of goserelin groups were all improved (P<0.05),especially those in medium-dose and high-dose groups were better than in low-dose group (P<0.05);there was no statistical significance between medium-dose group and high-dose group(P>0.05). CONCLUSIONS:Gosere-lin can relieve prostatic hyperplasia of rats,reduce the expression of VEGF in prostate tissue and regulate immune function.

7.Spatial and temporal variation characteristics of air pollutants in Xi'an during 2017-2020

Objective To analyze the temporal and spatial variation characteristics of six common air pollutants in Xi'an, and to provide a scientific basis for the environmental protection departments to conduct targeted prevention and control. Methods Based on air pollution monitoring data with high temporal resolution, the temporal and spatial distribution characteristics of six common air pollutants in Xi'an were analyzed, and the correlation between the pollutants was studied. Results Except for O3_8h, the annual average concentrations of the other five air pollutants decreased year by year. By 2020, the annual average concentration of PM2.5 and PM10 still exceeded the second-level concentration limit. The annual average concentration of NO2 was slightly higher than the first-level concentration limit, and the number of unqualified days of O3_8h accounted for one-tenth of the entire year. The concentrations of PM2.5, PM10, CO, SO2, and NO2 all were the highest in winter, equivalent in spring and autumn, and the lowest in summer. The concentration of O3_8h generally presented the characteristics of summer > spring > autumn > winter. In 2020, the annual average concentrations of PM2.5 and PM10 in 9 urban areas exceeded the second-level concentration limit. The annual average concentrations of NO2 in Xincheng District, Lianhu District, Beilin District, and Yanta District all exceeded the second-level concentration limit. The annual average concentration of O3_8h in Beilin District, Chang'an District, and Yanliang District all exceeded the average level of Xi'an. PM2.5, PM10, CO, SO2, and NO2 in general showed a positive correlation in varying degrees. In summer, O3_8h showed a positive correlation with other air pollutants, but a negative correlation in other seasons. Conclusion The air pollution situation in Xi'an has been improving year by year, but the pollution of PM2.5, PM10, and O3_8h still exceeds the standard, which will be the focus of long-term prevention and control of air pollution in Xi'an.
8.Triangular flaps and fascia-tissue flaps rotation for correction of severe inverted nipple in shallow and deep areola of breast

Objective To explore a new methodology for surgical treatment of severe retracted nipple in women. Methods 20 patients with inverted nipple and 4 recurred patients were involved in this study. Firstly, the site of neonipple tip was marked in the central part of the inverted nipple and its mean diameter usually was 1.2-1.5 cm. Then two shallow and deep triangular pedicled flaps were designed, respectively, in both superior and inferior areas near areola. With temporary traction of the nipple apex provided by a stay suture, the fibrotie bands underneath the nipple base might be cautious-ly released. Moreover, the shallow skin flaps should be about 0.5 cm in thickness and their blood sup-ply was from the subdermal arterial rete of the areola, which were used to cover and reconstruct the neck area of neonipple after a clockwise rotation and advancement simultaneously. While the deep fas-cia tissue flaps were revolved and advanced either horizontally to the opposite pedicle or upward to the inner tip through the tunnel underneath the nipple base in order to improve the height or width of the neonipple neck and prevent flattening as the supporting tissue and their blood supply was from some small perforating branch arteries in the deep part of mammary gland. Finally, purse-string suture was necessary in the base of neonipple which played a key role in avoiding recurrence of nipple inversion. Four vertical diamond-shaped excision-suturation treatment in neck area could make improvemts on the height of those stout and short nipples. Results In all 24 cases corrected by shallow and deep triangu-lar flaps rotation, after 3-6 months' follow-up, there were no complications related to surgery such as infection, hematoma, permanent sensory disturbance, or nipple necrosis, and postoperative recovery was rapid and uneventful. Especially, follow-up data revealed no evidence of recurrence of inversion and all patients were satisfied with their results. Conclusions Triangular flaps and fascia-tissue flaps in shallow and deep areola rotation is effective and easy to be popularized in correction of inverted nip-ple. This technique can improve both the diameter and height of the nipple, and certainly lower the re-currence rate of nipple inversion and achieve good aesthetic results.
9.Effects of Different Surgical Methods for Treating Femoral Intertrochanteric Fracture in the Elderly and Biomechanical Study

Objective To analyze the efficacy and biomechanical properties of locking proximal femoral plate (LPFP) and proximal femoral nail anti-rotation (PFNA) for treating intertrochanteric fracture in elderly patients. Methods One hundred and six elderly patients with intertrochanteric fracture of femur were randomly divided into LPFP group (53 cases) and PFNA group (53 cases). After treatment intervention, the operation time, intraoperative bleeding volume, weight-bearing time, fracture healing time and Harris hip function score of 9 months after operation in two groups were recorded. The complications after operation in two groups were analyzed. Ten elderly fresh femoral specimens were selected to prepare the model of intertrochanteric femoral fracture in the elderly. They were randomly divided into PFNA group and LPFP group with 5 models in each group. After treatment and intervention, axial compression test, destructive load test and torsional stiffness test were conducted by mechanical testing machine, and biomechanical properties were recorded. Results The average operation time, weight-bearing time and fracture healing time in PFNA group were shorter than those in LPFP group (P<0.05), and the average intraoperative bleeding volume in PFNA group was less than that in LPFP group (P<0.05), and the average Harris score was higher than that in LPFP group (P<0.05). The total incidence of postoperative complications in PFNA group and LPFP group was 7.56% and 18.87%, respectively, indicating a significant difference between the two groups (P<0.05). After intervention treatment, the average axial compression, damage load and torsional stiffness in PFNA group were higher than those in LPFP group (P<0.05). ConclusionsPFNA caused minimal trauma for treating intertrochanteric femoral fractures in the elderly. With its good biomechanical properties, PFNA could effectively promote fracture healing and hip function recovery, and significantly reduce the incidence of hip varus, screw loosening and cutting complications.

"Mistreatment by warming therapy" refers to various symptoms/syndromes caused by improper use of fire methods mentioned in Treatise on Febrile Diseases, which are similar to damaged yin induced fire-heat syndrome (injury of yin, consumption of qi, loss of yin, consumption of body fluid) manifested after radiotherapy and/or chemotherapy in modern clinics. Better efficacy was obtained in treating various symptoms/syndromes by using nourishing yin purging fire method.
