7.Improved RP-HPLC method for determination of ferulic acid concentration in human serum

Object To develop an improved RP-HPLC method for determining ferulic acid in the human serum. Methods The determination was carried on RP-HPLC, using Kromasil-C 18column, methanol-water-acetic acid (36.4∶63∶0.6) as mobile phase with a flow rate of 1.0 mL/min and at the detection wavelength of 322 nm, and p-hydroxybenzaldehyde was used as internal standard. The serum sample was deproteinized with acetonitrile to extract ferulic acid. Results The calibration curve showed good linearity over the range of 9.94-159.04 ng/mL (r=0.992 5). RSD was less than 10% within day and day-to-day, the average recovery was 99.77% and the minimal concentration in serum was 5 ng/mL. Conclusion This method, which is simple, rapid, sensitive, reproducible and low toxic, is appropriate for the quantitative determination of ferulic acid in the human serum.
8.Trends of demographic and primary disease in new end-stage renal disease dialysis patients of Zhejiang province from 2008 to 2013

Objective To analyze the changes of primary disease and demography of patients with end-stage renal disease (ESRD) who started dialysis between 2008 and 2013 in Zhejiang province, in order to provide statistical support for the improvement of dialysis quality.Methods The study retrospectively reviewed all the incident ESRD patients on dialysis registered in Zhejiang Dialysis Quality Control Center from 2008 to 2013.Their demographics and primary cause were investigated.Stratified analysis based on dialysis modality, duration and age were conducted.Results A total of 26 310 incident ESRD patients who started dialysis between 2008 and 2013 in Zhejiang were selected, among which 14 886 cases were male and 11 424 cases were female (male: female: 1.30: 1).The study gave priority to patients aged 45 and above (73.9%), with an average age of (55.7± 16.1) years.The top primary disease was chronic glomerulonephritis (51.3%), followed by diabetic nephropathy (17.3%) and hypertensive nephrosclerosis (6.4%).From 2008 to 2013, the incidence of dialysis was 46.3, 60.1, 71.2, 85.8, 104.1, and 119.9 per million population respectively.The average age of patients was increased year by year, from 52.4, 53.3, 54.8, 56.0, 56.9, to 57.6 years.The share of peritoneal dialysis, as well as the promotion of diabetic nephropathy in primary disease was on the rise.With the increase of age, the proportion of primary diseases such as diabetic nephropathy and hypertension nephrnsclerosis was increased.Conclusions In Zhejiang province, incident dialysis patients increase annually.More males than females are on dialysis.The average age and proportion of peritoneal dialysis are rising.The leading cause is chronic glomerulonephritis, but the incidence of diabetic nephropathy is on the increase, which requires attention.
9.The MRI findings of mitochondrial encephalomyopathy,lactic acidosis and stroke-like episodes

Objective To analyze retrospectively the MR features of MELAS patients,in order to improve the early diagnosis of MELAS.Methods MR data of 1 6 MELAS patients confirmed by clinical diagnose and muscle biopsy were retrospectively analyzed. MR features of plain scan(n=1 6),ASL(n=3),MRA(n=1 5),DWI(n=14)and MRS(n=3)were analyzed.Results MRI data demonstrated brain lesions mainly distributed in posterior cerebral hemisphere extending to subcortical area,which was not consistent with the vascular supply territories.The lesions were observed as low signal on Spin-echo sequence of T1 WI,while high signal on T2 WI, FLAIR and DWI.The focal lesions showed increasing Lac peak on MRS.Fifteen of 16 patients underwent MRA and one showed increased arterial branches in lesion zones.Three patients had ASL scanning demonstrating high irrigation in lesion zones which would wander once relapsed.Meanwhile,old lesions showed encephalatrophy and cerebromalacia.All patients’muscle biopsy pathology showed ragged red muscle fibers.Under electron-microscopic,bioblasts were bigger and more than average level and muscle fibers atrophied.Mitochondrial DNA sequence of 3 patients showed mtDNA A3243G transgenation.Conclusion The DWI,MRS and ASL sequences show good ability in MELAS diagnosis and differential diagnosis.
10.Clinical efficacy of bariatric surgery for elderly and nonelderly obese patients: a Meta analysis

Objective To systematically evaluate the clinical efficacy of bariatric surgery for elderly (age ≥ 60 years) or nonelderly (age <60 years) obese patients.Methods Literatures were researched using PubMed,Embase,Cochrane Library from January 2006 to June 2016 with the key words including bariatric surgery,metabolic surgery,gastric bypass,gastric band,sleeve gastrectomy,biliopancreatic diversion,elderly,aged,over 60 years.The cohort study about clinical efficacy of bariatric surgery for elderly or nonelderly obese patients were received and enrolled,and surgical procedures were unrestricted.Patients with age ≥ 60 years and with age <60 years were respectively allocated into the elderly group and nonelderly group.Two reviewers independently screened literatures,extracted data and assessed the risk of bias.Count data were described as odds ratio (OR) and 95% confidence interval (CI).The heterogeneity of the studies was analyzed using the I2 test.Results Eleven retrospective studies were enrolled in the Meta analysis,and the total sample size was 9 913 patients,including 792 in the elderly group and 9121 in the nonelderly group.The results of Meta analysis showed that there were no statistically significant differences in the early mortality,incidence of postoperative early complication,remission rates of postoperative diabetes,postoperative hypertension,dyslipidemia and obstructive sleep apnea syndrome between elderly group and nonelderly group (OR =3.31,1.94,1.00,0.61,0.99,1.40,95 % CI:0.86-12.77,1.01-3.74,0.66-1.50,0.34-1.10,0.42-2.29,0.72-2.72,P>0.05).Conclusion The safety and clinical efficacy of bariatric surgery in the elderly obese patients are equivalent to that of nonelderly patients.
