3.Therapeutic effects of fenestration in treatment of dentigerous cyst in mixed dentition stage

Objective To observe the therapeutic effects of fenestration in the treatment of dentigerous cyst in mixed dentition stage. Methods From 2004 to 2007, 8 cases of dentigerous cyst in mixed dentition stage were treated, among whom 3 were boys and 5 were girls with ages between 7 and 12 years old. The dentigerous cysts of 2 cases were in superior maxillary bone and the other 6 cases were in inferior maxillary bone. Each of 5 cases had one tooth in dentigerous cyst cavity, and each of the other 3 had 3 teeth in dentigerous cyst cavity. All patients received fenestration under local anesthesia. The patients were followed up for 18 months after fenestration, and the soft tissue healing, facial malformation, permanent teeth eruption and bone tissue healing were observed. Results It was found during follow up that cyst cavity disappeared in all the patients and all had normal facial morphology with no maxillary bone malformation. In five cases, permanent teeth erupted totally and dental occlusion kept normal. In one case, 2/3 of permanent teeth erupted and dental occlusion kept almost normal. While in the other two cases, permanent teeth were to erupt. It was revealed by X ray examinations that the shadow of maxillary bone density decrease disappeared in all the patients. Conclusion Fenestration can keep the teeth and bone in the treatment of dentigerous cyst in the mixed dentition stage.
4.Application of Plasma Exchange Therapy on Critical Diseases in Children

Objective To explore continuous renal replacement therapy(CRRT) machine for plasma exchange in critical disease in children.Methods Retrospective study of 8 patients(8 month to 14 years,mean 5.7 years) and 32 plasma exchange treatments,after(adowble) lumen catheter inserted into the subclayian venous,using the Baxter BM25 machine with commercially available plasma filters.Results Five patients(3 ABO-incompatibility in bone marrow transplantation,1 thrombotic thrombocytopaenic purpura TTP,1 sepsis) gained full recovery.One systemic lupus erythematosus(SLE) and 1 sepsis experienced moderate improvement while 1 case of acute disseminated encephalomyelitis failed PE treatment.The average total exchange volume was 80-100 mL/kg,achieved at a blood flow rate of 5-10 mL/(kg?min) and a turnover rate of 60-120 mL/(kg?h) over a 3-hours duration.Thirty-one PE treatments were finished smoothly,one of which experienced the serious complication involving plasma filter.Conclusion Plasma exchange therapy is a safe and effective procedure for severe autoimmune abnormalities and pathogen removal in children.
5.Analysis of active surveillance on the etiology of foodborne diseases in Xi'an in 2017-2019

Objective To analyze the epidemiological characteristics and the detection of pathogenic microorganisms in the cases of foodborne diseases under active surveillance in Xi'an, and to provide references for prevention and control of foodborne diseases. Methods Information on surveillance cases of foodborne diseases reported by two sentinel hospitals in Xi'an from 2017 to 2019 was collected. Data was statistically analyzed. Results From 2017 to 2019, a total of 904 cases of active surveillance of foodborne diseases were reported by two active surveillance sentinel hospitals in Xi'an. There were more cases in the age groups of 1-5 and 5-8, accounting for 35.18% and 20.02%, respectively. There was a statistically significant difference in the number of cases reported in four quarters(χ2=545.38,P<0.05). The majority of cases were reported in the third quarter, accounting for 53.76% of the total. The majority of suspicious foods were mainly mixed foods and the most suspicious dining places were families. Of the 901 samples, 251 were positive with a positive rate of 27.86%. There was a statistically significant difference in the detection ratio of different foodborne pathogens(χ2=321.92,P<0.05). The detection rates of Norovirus and salmonella were higher, accounting for 14.98% and 11.43%, respectively. There was a statistically significant difference in the detection rate of Salmonella in different seasons(χ2= 26.49,P<0.05), and the detection rates were higher in the second and third quarter. There was also a statistically significant difference in the detection rate of Norovirus(χ2=92.17,P<0.05) in different seasons, and the detection rate was higher in the first and fourth quarters. Conclusion The main pathogens of food-borne diseases are Norovirus and Salmonella in Xi'an. It is suggested that active surveillance should be strengthened. The prevention and control knowledge of foodborne diseases should be carried out according to the epidemiological characteristics.

7.Effects of N-acetylcysteine on expression of transforming growth factor-β1 in diabetic nephropathy rats

Objective To investigate the effects of N-acetylcysteine (NAC) on the expression of transforming growth factor-β1 (TGF-β1) in renal cortex of diabetic nephropathy rats.Methods A rat model of DN was established.The rats were randomly divided into control group,DN group and NAC group.After 8 weeks treatment,urinary albumin excretion rate (UAER) was determined.The expression of TGF-β1 in renal cortex was detected by immunohistochemistry and RT-PCR analysis.Pathomorphological changes of renal cortex were observed.Results (1)The levels of UA ER were significantly higher in DN group and NAC group [(1268.3±297.5) μg/24 h and (315.9-±86.8) μg/24 h] than in control group [(31.2±8.9) μg/24 h,q-29.85,16.76,both P<0.01].The groups of DN and NAC versus group of control showed the increased levels of activity of TGF-β1 in renal cortex [immune-histochemistry index of glomerular mesangial area:7.35±1.17 and 3.87 ± 0.71 vs.1.95±0.34,q= 10.75,5.82,both P<0.01];immune-histochemistry index of renal tubulointerstitium [21.21± 3.78 and 10.67±1.86 vs.3.62±0.79,q=15.20,11.36,both P<0.01];the expression of mRNA in renal cortex[0.72±0.06 and 0.45±0.05 vs.0.23±0.04,q=9.13,7.45,both P<0.01].The pathomorphological changes were significant in DN group and NAC group.(2)The NAC group versus DN group showed a decreased levels of UAER (q=8.17,P<0.01),activity of TGF-β1 in renal cortex [immune-histochemistry index of glomerular mesangial area:q= 4.97,P<0.01]immune-histochemistry index of renal tubulointerstitium (q = 6.86,P < 0.01 );the expression of mRNA in renal cortex (q= 3.69,P<0.05) and showed improvement of pathomorphology in renal cortex.(3) There was a significantly positive correlation between expression quantity of TGF-β1 mRNA in renal cortex and UAER level in NAC group(r= 0.749,P<0.05).Conclusions The protective effects of NAC on the kidney of DN rats may be partly related with inhibition on the expression of TGF-β1.

To review the background and advance of a strategy of bioethnopharmaceutical analytical pharmacology(BAP).BAP is an idea for elucidating absorbed bioactive compound(ABC)in traditional Chinese medicine(TCM)formula or herbal remedy.It is also a mode of active comparison between absorbed compounds and the parent formula including determination of ABC in blood,the ABC dosage identical to its content in formula,and the active comparison of ABC and the parent formula.The ABC of Guan-Xin-Er-Hao formula elucidated by BAP strategy has been reviewed and the significance of BAP strategy will be further discussed.
9.Lateral crus graft with autologous rib cartilage for cleft lip nostril asymmetry: a report of 35 cases.

A surgical technique of lateral crus strut graft to correct the deformity of nostril contour and improve asymmetric nostril shape by autologous rib cartilage was reported. Thirty-five patients (20 males and 15 females), with a mean age of 19 years (range of 16-26 years) were included in this study. All of the patients suffered moderate to severe unilateral cleft lip nostril deformity and underwent surgical technique of lateral crus strut graft by autologous rib cartilage to correct the asymmetric nostril contour from 2010 to 2012. The views of the nostril contour on both sides were reviewed and some parameters were measured at different time points (preoperatively, and 3 months and one year postoperatively) including the long axis and short axis of the nostrils. The differences in nostrils including long axis, short axis, elliptical area and eccentricity on the both sides were compared at different time points. The differences in the long axis and eccentricity of the nostrils on the two sides were decreased significantly between the preoperative view and postoperative view (P<0.05). The surgical technique of lateral crus graft with autologous rib cartilage is effective to correct the abnormal nostril contour and improve the asymmetry for unilateral cleft clip patients.
10.Flavonoids from Selaginella uncinata.

In the current study, nine flavonoids were isolated and purified from 75% ethanol extract of Selaginella uncinata (Desv.) Spring by column chromatographic techniques over macroporous resin, polyamide, silica gel, Sephadex LH-20 and pre-HPLC. On the basis of their physico-chemical properties and spectroscopic data analyses, these compounds were elucidated as cirsimarin (1), nepitrin (2), apigenin-6-C-α-L-arabinopyranosyl-8-C-β-D-glucopyranoside (3), apigenin-6-C-β-D-glucopyranosyl-8-C-α-L-arabinopyranoside (4), apigenin-7-O-β-D-glucopyranoside (5), 2,3-dihydroamentoflavone (6), 4'-O-methylamentoflavone (7), 2,3-dihydro-4'-O-methyl-amentoflavone (8), and 2,3,2",3"-tetrahydron-4'-O-methyl-robustaflavone (9). Compounds 1-5 belong to flavonoid glycosides and were isolated from the genus Selaginella for the first time.
